Employee reached over the catwalk railing and inside the side rails of the Jaw Crushing Plant Screen Box while the plant was in production, and crushed EE's left ring finger and middle finger in-between the rocks.

Conveyor stopped running & mat'l was piling up on the conveyor. LOTO was applied. The employee started cleaning the area where the belt wraps around the pulley with a screwdriver as we tried cleaning underneath it on the opposite side. Once the material was removed the conveyor rolled backwards & rolled his hand with the screwdriver around the pulley.

EMPLOYEE WAS STEPPING OFF OF MECHANICS TRUCK AND MISSED THE STEP FALLING ON HIS RIGHT ARM. FRACTURE RADIAL HEAD

EMPLOYEE WAS HELPING INSTALL CUTTING EDGES ON BULLDOZER, BOLT SPUN IN HIS HAND AND LACERATED THE PALM OF HIS RIGHT HAND.

CONVEYOR STOPPED ALLOWING TAIL PULLEY TO GET BURIED BY OVER FLOW MATERIAL. THE EMPLOYEE LOCK OUT THE PANEL, REMOVED THE GUARD TO ACCESS AREA AND PROCEEDED TO CLEAR MATERIAL. ENOUGH MATERIAL WAS CLEARED OUT WHICH ALLOWED THE BELT TO SHIFT CAUSING HIS ARM TO GET PULLED INTO TAIL PULLEY.

EMPLOYEE WAS ADJUSTING STACKER BELT, A ROCK BOUNCED OFF OF CATCH BOX AND FELL ON TOP OF EMPLOYEE'S FOOT

REMOVING BUCKET TEETH FROM 988 LOADER 023-809 FOR REPLACEMENT. EE WAS USING A SLEDGE HAMMER TO REMOVE ONE OF THE TEETH. UPON STRIKING ONE OF THE TEETH A PIECE OF METAL CAME OFF AND LODGED INTO HIS SKIN JUST UNDERNEATH HIS CHEEK BONE. FULL RELEASE.

HEAT - THE EMPLOYEE WAS ONLY OUT OF THE VEHICLE FOR 15 - 20 MINUTES FELT HOT. INCIDENT IS UNDER INVESTIGATION UNTIL MEDICAL REPORTS ARE OBTAINED TO COMPLETE OUR INVESTIGATION.

WHILE CHANGING BUCKET ON THE SKID STEER THE BUCKET DROPPED ON THE EMPLOYEES RIGHT ANKLE.

1ST NOTIFICATION OF INCIDENT JULY 30, 09 BY THE INDUSTRIAL COMMISSION. THE EMPLOYEE GAVE THREE DIFFERENT DATES OF INJURY. THE EMPLOYEE WAS NOT WORKING ON THOSE DATES. THE EMPLOYEE IS REPRESENTED. WE APPEARED FOR MEDIATION WITH LEGAL COUNCIL. THE DATE REPORTED BY EE & HIS ATTORNEY IS JUNE 4, 2009 (?) FOR THE DATE OF INJURY. CURRENTLY THIS INCIDENT IS UNDER INVESTIGATION.

EMPLOYEE WAS REMOVING DEBRIS OFF OF A CONVEYOR, THE DEBRIS WAS HEAVIER THAN ORIGINALLY THOUGHT, THE EMPLOYEE GRABBED THE DEBRIS, LACERATING HIS HAND.

Employees were positioning conveyor to load onto a flatbed trailer. EE walked between the two pieces of equipment when the conveyor shifted position at crest, striking him.

EE was welding on conveyor for 10-15 minutes, while another EE was welding on the opposite side of conveyor. Then injured EE put his tools away.

While working on a pet conveyor employee attached a comealong to the head pulley of conveyor. The comealong slipped off hitting the employee in the face. Employee is scheduled for surgery 8/25/06.

Employee stepped in a dip in the ground walking from the conveyor to another conveyor. Employee refused medical attention. First doctor's visit 03/17/06.

Changing screens. Employee was removing screens and walked forward. His lanyard got caught on one of the screens. Employee stepped back lanyard became loose employee pulled forward and fell forward on his hands.

Working in tight area ee states he strained/sprained muscles while sliding out. Employee continued working October 5 and October 6. Employee reported injury 10/7/05

EMPLOYEE DRANK TOO MUCH WATER, FLUSHING HIS SYSTEM. MEDICAL ATTENTION AT CASA GRANDE REGIONAL MEDICAL CENTER.

Knocking wedge through eyelet. Wedge popped out causing eyelet to hit his fingers.

Employee was moving stacker with the assistance of the loader. A clump of dirt fell on the employee hitting his hard hat and left shoulder.

PLACING NEW CORE WEIGHTS IN WHILE TIGHTENING BOLTS BRAKER BAR WRENCH BROKE HITTING HIS LEFT RIB CAGE

EMPLOYEE WAS CLEANING SAND OFF STACKER W/SHOVEL WHILE STANDING ON A TIRE ADHERED W/SAND. SHOVELING & STANDING ON TIRE CONTRIBUTED TO EE LOOSING HIS BALANCE AND FALLING.

Using pry bar to line up gear box, bar slipped cutting his finger.

EMPLOYEE NUMBER ONE WAS CLEANING SCREEN WITH A 20# HAMMER. HE WAS HITTING THE SCREEN WITH THE HAMMER THEN WAS GOING TO HIT THE SIDE OF THE BOX WHEN EMPLOYEE NUMBER TWO PLACED HIS HAND ON THE SIDE OF THE BOX.

LIFTING AND CHANGING SCREENS WITH A PARTNER AT CRUSHER. EMPLOYEE UNABLE TO PINPOINT EXACTLY WHEN INJURY OCCURRED.

PLACING A TRANSMISSION IN 2100 CRUSHER TRANSMISSION SLIPPED SMASHING FINGERS

RIGHT HAND KNUCKLES WERE HIT BY FALLING ROCK

EE FELL OFF OF CONVEYOR BELT WHILE CLEANING.

EE WAS CLEANING AROUND TAIL PULLY CATCHING MIDDLE FINGER.
