EMPLOYEE WAS HELPING INSTALL CUTTING EDGES ON BULLDOZER, BOLT SPUN IN HIS HAND AND LACERATED THE PALM OF HIS RIGHT HAND.

EMPLOYEE WAS REMOVING DEBRIS OFF OF A CONVEYOR, THE DEBRIS WAS HEAVIER THAN ORIGINALLY THOUGHT, THE EMPLOYEE GRABBED THE DEBRIS, LACERATING HIS HAND.
EE was welding on conveyor for 10-15 minutes, while another EE was welding on the opposite side of conveyor. Then injured EE put his tools away.
Employee stepped in a dip in the ground walking from the conveyor to another conveyor. Employee refused medical attention. First doctor's visit 03/17/06.
Employee was moving stacker with the assistance of the loader. A clump of dirt fell on the employee hitting his hard hat and left shoulder.
