Employee was fuelling up a pump used to de-water pond. Whilst doing this he slipped due to wet ground and twisted his right knee.

The employee (Plant Operator) was carrying out routine plant walk around when he slipped and fell against sand classifier.

Employee helping pull a new conveyor belt on to a mobile crushing/screening unit. He was pulling the new belt with a rope on ground level, when he stumbled and fell over loose ground. His right ankle was injured.

Whilst carrying out greasing on 980G Loader the right rear wheel fender would not latch. The operator was resting his left hand on the battery box and pushing fender with his right hand. When the latch engaged the operator's middle finger on right hand was jammed and sliced the end of the finger tip.

Employee opened a door to clean out screw and piched his fingers when he swung it out. He got 5 stitches on his finger where it was cut. He is expected to get full recovery.

Operator had previous back injury 15 years ago. Operator was driving CAT 350 haul truck on rough road and complains of back pain. Operator is out pending examination and treatment for pain.

slipped in muddy conditions and hit back on tracks of parked excavator.

Operator was driving rock truck on 11-8-07 for about 2 hours. On 11-9-07 at noon he reported to his supervisor that he hurt his neck on 11-8-07 while driving rock truck.

Cut to little finger on right hand while putting a hydraulic pump back on an engine and it caught his finger between pump and engine. Engine was not running. Pump was not running. Unit was shut down and locked out.

Operator was declining the ramp to pit when he hit a pothole, jarring his neck.

Wrench slipped while working on valve and hit employee over the bridge of the nose.

Employee was stepping off loader ladder and claims he twisted right knee. Released to full duty 7/9/07.

EE was using length of pipe to hold tension on screen motor belts with prying motion on mounting frame pulling muscle near rib area.

Changing bearing on conv. belt. While taking old bearing off (515/16) bearing slipped & dropped onto EE's finger pinching it between bearing & frame work. Bearing as approx 15 lb.

EE was working on impact crusher trying to dislodge a piece of steel which was stuck with a pry bar. When the steel broke loose, he hit himself in the forehead.

Employee was splashed with diesel in eye when refueling. Employee has no damage or impairment, and has returned to duty.

Was loosening bolts, trying to take off roller to be replaced with a new one.

EE was changing roller and felt something bite him on right forearm.

While servicing the Cat D400 haul truck, employee fell to ground from the fender of the truck.

While repairing Powerscreen 1400 employee slipped & lost footing. He then grabbed machine to hold his balance & caused injury to his right arm.

Tripple and over water hose and fell on concrete landing on left knee.

While working on grizzly he was bitten by a spider on left leg calf muscle.

AFTER BEING LOADED WITH SAND, DRIVER WAS CLEANING THE TAILGATE WHEN THE TAILGATE FELL CATCHING HIS RIGHT INDEX FINGER BETWEEN THE GATE AND THE TRUCK BODY.

While stepping down off D6 dozer he slipped and fell.

TRYING TO ADJUST FLASHING ON HOPPER BELT, PRYBAR SLIPPED & HIT HAND ON FRAME OF CONVEYOR.

WASHING OUT CHUTE, BIG ROCK BOUNCED OUT OF SHAKER HIT RIGHT HAND, PALM REQUIRING STITCHES.
