While removing a track link from a D8R, employee hit the old link with a hammer. A sliver of metal came off the link and penetrated his right arm.
The employee alleges he was thrown/knocked from the tracks of the dozer, while he was relieving himself, when a 777 truck bed came into contact with the dozer blade at the dump site. The company has reason to believe that this was a staged accident and will contact the workers comp claim.
Employee was using a lifting jack while working on a shuttle car and the jack handle broke, striking his arm/wrist and causing him to fall on his arm/wrist.
Employee was lifting rock off of the continuous miner to gain access to a broken conveyor chain to be repaired when injured his back.
A roof fall occurred in the #4 crosscut intersection of the #4 entry. The roof fall was 50' long x 18-20' wide x 5-7' thick. The fall was cleaned up and the area was re-supported. There were no injuries or property damage.
Employee was backing the roof bolter up to reposition and roof bolt machine came into contact with the employee when it slid to the side, pushing him into the rib.
Employee received an injury as a result of handling cribs blocks. Employee received a pulled/strained muscle injury while turning with a crib block to place it off the belt to be installed.
Employee was bent over walking toward the #1 entry and was struck on the back by a piece of falling draw rock which measured 3'x1'x3".
EE alleges an injury occurred which resulted in stomach and chest pain while bending and or installing roof bolts and cable bolts. Note: This accident may be updated EE indicated to supervisor that he is suffering from a collapsed lung and pneumonia. May not be work related.

An unintentional roof fall occurred on the 3 east north mains between crosscuts 4 & 5 in the No5 entry. The fall was approximately 60'long X 18' wide X 8'-10' high ( estimate). No injuries or property damage.
An unplanned roof fall occurred in the #3 belt entry one cross cut in by SS 361 n the intersection. The fall was approximately 10' high - 28' wide (including area in adjacent crosscuts) and 3.5' long. These are approx. measurements. There were no injuries. The belt drive and tailpiece were under the fall and will not be recovered. Area old.
An unplanned roof fall occurred in the intersection of the 12th crosscut of the #4 entry of the north mains. The fall was approximately 18'wide X 20'long and 8' to 10'high. There was no property damage or injuries. The fall will be cleaned up. This is an out by area along the main travel way.
While attempting to connect 3 north mains to 2 east panel 4 for the purposes of pumping water from worked out panel an inundation occurred. The amount of water encountered at cut through was greater than expected & the result of the inflow typically was from a feathers edge to 6"-12" deep with the max. being approx 30" in a swag in belt entry. No injuries or entrapment occurred.
An unplanned roof fall was found the exact date/time of fall is not known found at approx. 11:30 a.m.The fall is located in #5 entry of the #3 East Mains at c.c. #35 in the intersection. Fall approx. 8' X 10' high and 18' wide. The length of the fall could not be determined. There was no property damage or injuries.
EE was adding water to the radiator of the dozer and hot water was expelled from the radiator, which contacted the EE's arm and upper right quadrant of torso, resulting in burns to the arm.
While dismounting the haulback truck the EE put his foot on the ground twisting his ankle causing him to fall backwards to the ground.
An unplanned roof fall 20' x 60'-70' long x approx 8' high occurred in the No. 4 entry between xcuts 34 and 35 and extended into the xcut (No. 35) between No. 3 & 4 entries. There was no property damage or injuries. Proximity of soft shale roof, slips & joints and water in roof contributed to the occurrence.
An unplanned roof fall, 20' wide x 40' long x 5' thick occurred in the 2 L crosscut at crosscut No. 35 of the No. 4 Right Mains. There were no property damage or injuries. Proximity to the intersection, shale roof (soft) and slips/joints contributed to the occurrence.
Roof fall off section in the #5 entry on the 1 north main between break #12 & 13, will not be cleaned up
Rock fall off of section, in primary escape way in #4 entry between break #13 & 14, catching high voltage cable, tracking and communication, 2nd phone line and life line. Ventilation still good.
Loading side post jack fo Fletcher Bolter onto a two man personal buggy caught right hand cutting middle finger requiring 6 stitches.
Injured was using hammer to strike a metal pin - part of hammer head broke away and struck employee in face.
Rock wheel on grinder busted sending a chip of rock into ee chin, cutting a small gash in the bottom.
A piece of rock 23X17X3 fell striking victim on right side of face and continuing down his right arm.
EE was throwing steel in a high lift bucket. A piece broke off and hit his right forearm.
Rock fall. Soft laminated top.
Splicing belt and struck hand (right) with hammer.
Slipped on low structure.
Changing out bad production car. Man cleaning coal off the front car and got in the way of pusher jack. He did not realize pusher jack was coming in contact with his leg.
Weather conditions were snowy & icy. Employee stepped out of lube trk, slipped on ice/snow resulting in twisting his left ankle.
Terex 33-09 back-up shorted at radio and caused fire in cab, operator stopped truck and tried to extinguish in an excited state he turned wrong and twisted knee (left).
Wind blew dust in EE's eye while on the ground.

TAKING PIPE DOWN, SLIPPED OUT OF HAND AND MASHED RT. 4TH FINGER.
HE WAS HELPING DO MAINT ON EQUIP., SQUATTED DOWN AND THEN COMPLAINED OF KNEE HURTING.
EE FELL ON ICE & SNOW BREAKING RIGHT WRIST.
TWO PLANT EMPLOYEES WERE REPLACING THE REFUSE SIEVE SCREEN. THEY HAD BEEN WORKING FOR APPROX 1 HOUR WHEN A SECTION OF FLOOR SUPPORT GAVE WAY WITHOUT WARNING CAUSING EMPLOYEE TO FALL AND RESULT ED IN AN ANKLE INJURY.
A CO-WORKER WAS WORKING WITH A HAMMER/WRENCH AND A PIECE OF METAL FLEW INTO HIS RIGHT EYE CAUSING A CUT ON THE EYEBALL.
