Struck against stationary object

While moving a piece of belt structure EE hit tip of finger on another piece of structure mashing and cutting same.

Employee was using a lifting jack while working on a shuttle car and the jack handle broke, striking his arm/wrist and causing him to fall on his arm/wrist.

Over-exertion in lifting objects

Employee was lifting rock off of the continuous miner to gain access to a broken conveyor chain to be repaired when injured his back.

Caught in, under or between a moving and a stationary object

Employee was backing the roof bolter up to reposition and roof bolt machine came into contact with the employee when it slid to the side, pushing him into the rib.

Roof fall off section in the #5 entry on the 1 north main between break #12 & 13, will not be cleaned up

Rock Fall in the #4 Entry of the 1st West Main by the 19th crosscut in old works, top was laminated and moist.

Section Foreman walked over to the power center to check on the mechanic, the mechanic's buggy rolled pinning foreman between power center and buggy, breaking his leg.

Struck by falling object

Reporting as a part of Part 50 Audit. Employee was removing the bolts to replace rear end, a piece of debris fell and hit his left eye. Employee received medical treatment, foreign body was removed and employee received prescription medication for the left eye.

Leaving Roof Bolter stepped up on some muck and struck left side of head and left ear on roof mat (strap) cutting head and ear.

Employee was installing roof bolt and steel mat. While spinning bolt the mat turned pushing left arm back hurting back on left side.

Employee was changing the tire on the Water Truck using a 1"" air wrench, wrench got caught and twisted his hand and wrist.

Employee slipped & fell hitting Johnson Buggy.

Reporting due to Part 50 Audit. Employee was walking down a steep hill and lost footing falling down twisting left leg. Employee sought medical treatment, returned to work the following day, missed 5 total days and eventually had Orthoscopic Knee Surgery on 6/4/2010. Returned to work 6/7/2010

Over-exertion in lifting objects

The continuous miner was stuck, was placing 4' x 8"" crib block under the tracks of the continuous miner for traction, EE picked up a crib block and pulled his back.

MSHA Inspector was in worked out area of mine with Mine Foreman, collecting dust samples when a roof fall occurred measuring 80' x 20' x 6' at Break #16 in the 002 South Mains off the 1st West Mains of the 002 Section.

Employee bent over to pick up a bolt and a pin fell and hit employee on the back of the head.

Making the weekly exam of worked out areas, mine foreman and MSHA Inspector discovered a fall was encountered at Break #41 of the 1st East Mains 20' x 10' x 4'

While making the weekly exam of worked out areas Mine Foreman and an MSHA Inspector encountered a roof fall at break #43 measuring app. 20' x 20' x 4' on 1st East Mains. It was phoned outside and the 800# was notified at app. 2:10 pm. The fall has been dangered off.

Struck by falling object

Moving the blue station up, a piece of loose draw rock measuring app. 8' x 2 1/2' x 8"" fell from between rows of roof bolts knocking employee down & injuring his left arm, leg, & ribs (no broken bones).

Tramming the continuous miner and placing himself too close to the miner and the coal rib, pinching himself between the body of the miner and the coal rib, breaking his left leg and bruising the right leg.

Roof bolt operator was letting head of machine down and his right knee slipped underneath causing energy to his right thigh & right knee.

Walking beside 002 sec head drive, tripped and fell into the main line belt hurting his lower back and right arm and elbow.

Doesn't know how he got hurt. Said his back started hurting after bringing mantrip outside.

Section foreman was lifting oxygen bottle from buggy to hook up torches to work on mine equipment. When he turned with bottle his lower back popped and injured his lower back.

EE was operating a mantrip buggy transporting workers to their U.G. work location. The front wheel of the buggy caught in a rut and the steering wheel pulled out of ee's hand and hit the ee on the right hand.

At approximately 7:00am an employee called out to another and said he had some top trying to fall in and he was cribbing it off. The top fell in at about 7:30 am. We finished setting cribs and began to move section.

Running high voltage cable down belt, The cable knoted up and pulled off belt and pinned victim agains the coal rib bruised lower back.

Drilling for roof bolt. Victim got glove caught in drill steel, twisting left arm and breaking it.

Employee was rolling a shuttle car tire out behind a pallet of dust when he got off balance and the tire fell on his legs bending him over backwards across the end of the pallet causing him to injure his back by extending him backwards over the pallet.

Victim was pulling the miner cable ( cutting slack) when he lost his footing and slipped and fell. He hit on his left side hip or back causing a bruise or hurting of some type.

The victim was drilling top to install roof bolts and was using a 12"" wrench to finish the hole and the wrench broke and struck his hand and thumb, (right hand) broke thumb.
