During normal mining, the continuous miner cut into an existing auger hole. There was no flooding of water or gas, therefore no persons were affected by the occurrence, other than the cessation of production, until the hole was sealed off from the mine. The mine resumed production on 5/02/2016.

EE and another ee were changing a wheel unit on 21SC shuttle car - while they were taking out the last bolts - wheel unit fell from blocking-catching ee tight finger.

Subject was attempting to crush a piece of rock that was lodged in the Belt Feeder. He said, he was busting the rock with a sledge hammer and he went to move it with his left hand, the rock broke crushing his left hand.

EE was positioning a gear for the continuous miner ripper when he felt a sharp pain in his lower back area and a burning sensation in his left leg.

EE had loaded a coal truck when he went to exit the end loader he lost his footage ended up swinging on hand rail - hitting his back & head on side of end loader.

Crew were setting timbers one guy threw a timber to be set and hit EE's right hand.

EE roof bolter operator had started to bolt in the #6 entry when he attempted to bend G bolt he hurt the lower part of his back.

Employee was bolting a corner when a piece of draw rock fell and hit him on the left hand.

Scoop operator was scooping rock and know out a stopping and a piece of rock and a block hit right hand of operator.

Bolting top rock fell striking him on Rt. Forearm, as he was drilling.

Bolting top got finger caught between drillpot and drill steel, mashed little finger and cut it.

EE was attempting to move rock out of the feeder when the rock fell over & caught his thumb between the rock & metal.

EE was taking drill steel apart when his right ring finger was mashed between 2 pieces of drill steel.

A roof fall occurred in the #4 right crosscut into the #5 heading. The fall was approximately 25'long x 18'wide x 8' high. The roof fall did not affect workers,ventilation nor equipment. The fall also continued in the #5 heading on 4-22-10 appx. 10' in length x 18, wide x 8' high.

HE WAS GETTING READY TO SCOOP THE #1 ENTRY WHEN A PIECE OF DRAW ROCK ABOUT 1"THICK X 6"LONG FELL BETWEEN ROOF BOLTS & CUT HIS MIDDLE FINGER.

SUBJECT WAS ATTEMPTING TO LIFT A BATTERY OUT WHEN STRAP BROKE, CAUSING HIM TO BE OFF BALANCE, HITTING HIS HEAD AGAINST ROOF OF MINE.

EE WAS HELPING TO MOVE MINER FROM #3 HEADING TO #2. HE WAS LAYING CABLE UP ON SIDE OF MINER AND THE OPERATOR LET HEAD DOWN ONTO EE'S HAND. THIS BROKE EE'S THUMB ON RIGHT HAND.

SCATTERING TIMBERS AND TWISTED TO THROW TIMBER OUT AND TWISTED HIS LOWER BACK.

CABLE WHEEL HANGING SPLIDED OUT OF HAND HIT IN FACE KNOCKING INJURES PERSON TO GROUND.

LIFTING ON WHEEL UNIT, EE SAID HE HURT HIS BACK.

LIFTING WHEEL OR TIRE FOR A 21 SHUTTLE CAR, HE PULLED SOMETHING IN HIS CHEST. DID NOT BECOME REPORTABLE UNTIL 2/15/04; SUBJECT IS NOW MISSING TIME.

BACKING THE MINER UP AND CAUGHT HIS HEAD BETWEEN THE MINER AND THE TOP.

LOADING THE ROCK DUSTER AND IT SPLAHED IN HIS EYE.

IT WAS 60 FT LONG AND 20 FT WIDE, 4 FT HIGH.

HE TRIPPED ON A BOARD AND FELL CUTTING HIS FACE AND NOSE. STITCHES WERE REQUIRED.

EE WAS OPERATING ROOF BOLTER DRILLING HOLE TO INSTALL ROOF BOLT, STEELE VIBRATED ROCK LOOSE STRIKING HIM ON RIGHT ARM.

GOING DOWN INTAKE & A ROCK FELL OUT BETWEEN BOLTS AND STRUCK HIM IN THE CHIN SLICING IT OPEN.

HE WAS HANGING MINER CABLE AND HURT BACK.

EMPLOYEE WAS WORKING ON PIECE OF EQUIPMENT AND WAS USING RATCHET TO TIGHTEN BOLT AND OVER EXTENDED HIS ARM.

EMPLOYEE WAS LOADING STRUCTURE IN A LOADER AND PULLED SOMETHING IN HIS BACK. ACCIDENT DID NOT BECOME REPORTABLE UNTIL 08-06-03.

EE WAS HELPING ON COAL DRILL AND WAS TAKING DRILL STEEL APART WITH PIPE WRENCH. HE HAD THE WRENCHIN HIS HAND WHEN ROTATION KICKED ON AND PINNED HIS FINGER BETWEEN THE WRENCH AND BOOM OF DRILL.

HE WAS PUTTING UP HIS TOOLS AND HURT HIS BACK.

EE WAS TAKING A RUBBER HOSE TYPE WATERLINE DOWN FROM BERN HUNG AND HOSE FELL DOWN CATCHING HIM ON THE BACK OF HIS NECK KNOCKING HIM OFF HIS FEET ON TO HIS BACK.
