Struck by falling object

A ""HORSE BACK"" (ROOF FAULT) MEASURING 2' X 14' X 38' DISLODGED FROM THE MINE ROOF PULLING 2 ROOF BOLTS FROM THE LAST FULL ROW OF BOLTS, FALLING ON THE VICTIM WHOSE LOCATION WAS NEAR THE 2ND ROW OF BOLTS, CAUSING CRUSHING BLUNT FORCE TRAUMA, RESULTING IN FATAL INJURIES BEING SUSTAINED.

Over-exertion in pulling or pushing objects

The employee was bolting in the #1 entry in the first cut. EE was operating the offside of the bolter and mining height was approximately 40 inches. EE was bending a bolt when EE felt a pop in the right shoulder. EE immediately felt severe pain. EE was brought to the surface and first aid was administered. EE was taken to the medical center and was diagnosed with muscle strain.

Struck against stationary object

Employee was exiting EE's shuttle car when EE stepped on coal and twisted EE's ankle. An MET applied a cold pack and an Ace Bandage. EE went to the doctor the following day and was given a full release to return to work.

Over-exertion in lifting objects

EE was helping build a brattice and as EE picked up a block, EE twisted around and felt severe pain in the back. EE was taken to the hospital and after x-rays, EE was diagnosed with sciatica. *EE went back to the doctor on 9/7/22 and was taken off work until 9/10/22.

Contact with electrical current

On Friday during an extremely bad storm, lightening struck on the surface and went underground. Five people received an electrical shock. One person went to the hospital to be examined. EE was released the same day as EE's vitals were normal. MSHA was called and a (K) was issued and completely abated on 6/21/2022.

Fall onto or against objects

EE was helping make a belt splice when EE tripped over a belt stand and fell on a roller that was lying on the mine floor.

Struck by falling object

EE was bolting the inside bolt when the drill pot caught on the ATRS cable. When EE repositioned the drill pot the pot dropped striking EE on the mid right thigh causing a deep tissue bruise. The doctor gave EE 3 days off and EE was able to return to work on Tuesday. However, EE decided to wait until Monday 4/11/2022 to come back to work.

Struck by flying object

EE was using a 2 ton chain ratchet to remove the bottom head jack pin off a miner. As the bolt came out of the pin pulley the ratchet flew back striking EE on the lower left thigh. EE sustained a laceration requiring several stitches.

EE was installing roof bolts in top, when roof bolt plate slid down roof bolt, hitting operator on left arm, cutting his left forearm.

Was pinning top. Drilled hole in top for pin. While installing bolt, he was spinning bolt to mix glue. While positioning plate against top plate, he caught his glove, pulling finger off of his left hand.
