A fatal accident occurred on December 30, 2017. The employee was driving a Dodge pick-up in a stockpile area and pulled in behind a Caterpillar 992G loader. As the loader reversed, it struck the pick-up which then caught fire. The resulting injuries were fatal.

Two bulldozers pushing in the same area, operators talking on CB. D9 operator is injured worker. D-11 is colleague. D9 stopped talking. D11 found that D9 had gone over highwall. Injured worker passed away from injuries sustained.

Haul truck struck man van resulting in two fatalities. A passenger van carrying nine miners was run over by a 340 ton haul truck.

Investigation indicates that the employee was attempting to cross a moving belt or cross a belt that had just started, when employee was pulled down the belt line and later recovered at the raw coal stockpile at the plant.

Operator was up on hill pushing overburden down to excavator for feeding crusher. Operator went down a small incline on Cat Dozer and was thrown from the cab.

On September 28, 2017, employee was helping in the roof bolting activities on the longwall face. During this activity, coal came from the longwall face fatally injuring the employee.

Rope access technician was performing inspection of downcomer duct. Around 11:30a.m.the technician became unresponsive by radio. Upon rescue and removal from downcomer duct, technician was alert and responsive with apparent head and leg injuries. Technician was transported to the hospital where they passed away on 9/21/2017.

The victim was the Kolberg Plant Operator. Employee may have been trying to perform maintenance on the tail pulley or conveyor belt, when employee became entangled in the conveyor system, causing fatal injuries. The operator is continuing to investigate the root cause(s) of this accident.

The employee was found unresponsive at approximately 2:05 PM on the offside of the #1 belt at the portal entrance of the surface mine. Preliminary findings has determined the employee received fatal injuries when coming in contact with the belt conveyor.

On August 3, 2017 a miner was fatally crushed while ee was dismantling a metal structure used to supply water to tech fine coal filter at the preparation plant.

Employee in flatbed service truck was parked on a waste dump at the Mission Mine. A 320 ton haul truck ran over flatbed service truck resulting in fatal accident.

A Cat D9L bulldozer ee had been operating ran over ee. The bulldozer ran over the edge of the refuse bank and came to rest at the bottom of the embankment. The victim was pronounce dead at the scene. The reason why the victim was outside the bulldozer when the accident occurred is under investigation.

Cracked stone fell and crushed employee. Employee didn't follow procedure. Employee's lanyard wasn't tied off and the employee was using a foot wedge and jackhammer with 1 1/4" bit. Employee should have been using 3/4" plug drill and 5" tether and wedge. The employee was drilling from the base and should have been drilling at the top.

Temporary contracted employee contacted the 12500 volt overhead power line with an irrigation pipe

Injured employee sustained fatal injuries resulting from head trauma after jumping or being thrown from the lead locomotive of a supply trip at crosscut 16 on east mains track.

On 6/13/2017 a continuous miner operator was moving the continuous miner out of the #3 entry on the #1 Section. As EE backed the miner out of the entry EE was caught between the head of the continuous miner and the coal rib. This resulted in fatal crushing injuries.

The pile gave away.

EMPLOYEE WAS OPERATING #43 DC MOTOR ALONG 8 HAULAGE EXTENSION TRACK WHEN EMPLOYEE STRUCK EMPLOYEE'S HEAD AGAINST A ROOF SUPPORT BEAM. INCIDENT INVESTIGATION ONGOING

On May 6, 2017 at approximately 16:55 a haul truck operator backed up to a berm to dump, the berm breeched and the haul truck and berm slid down the highwall approximately 150 ft. There were no witness to the incident.

While changing work cutter head bits on the coal auger machine, ee was struck by a rock approx 4 feet by 5 feet by 30 inches that fell from the bottom section of the highwall. EE was pronounced deceased at the scene.

Approx. 8-8:30 pm after shift, employee drove a vehicle onto mine site and stopped close to generator, left vehicle running, gear selector in drive position, did not set emergency brake or chalk wheels. Got out and started generator. EE appeared to have come out of generator and stopped in front of vehicle to urinate, back to vehicle. Vehicle rolled, pinned EE against generator.

The decedent, an o/operator, was located behind the truck, behind the raised dump trailer when a load of sand dumped from the trailer. The decedent was buried in the sand. Mgmt became aware of the missing driver at approx. 10:45 am and promptly began a search. EEs began digging in the pile of sand behind the decedent's truck and the decedent was discovered at approx 11:43 am.

Employee was last observed as employee began to ascend an extension ladder. There is no eye witness, it is apparent the employee fell approximately 20ft. onto a moving conveyor belt and was fatally injured.

A piece of draw slate approximately (3 foot by 2 foot by 3 inches thick) broke across the strap and fell striking ee. Injured miner was transported to a medical center. Due to medical complications from the injuries ee sustained ee died on 4/6/17

Truck bed turned over while dumping after load failed to clear from bed. This caused the truck to tip up on passenger side tires-truck did not turn over or sustain any damage. Material was wet and damp and temp. was cold. Driver chose not to use bed treatment station provided by company. Trailer appeared to be loaded toward the front. Driver chose to jump from truck.

Death

EE was reported missing and was later discovered under a rib failure several hours later. The miner was found in an abandoned section of the mine beyond a barricade berm, along the top of a 20 foot high, waste material spoil pile. The victim was recovered on 1/26/17 at approximately 3:30am. and officially pronounce dead by the local medical examiner.
