The miner was picking up a log that was ~6" x 8" and ~3' length. They tripped and fell. When they were falling, they put their hands down to catch the fall. The log landed on their left hand and broke 3 fingers. The miner was walking on a slope. The walk way was dry. There was nothing obstructing their pathway. Sunny day.

EE was standing between a primary rock screen and a crusher jaw when a rock hit EE in the head rendering EE unconscious. No witness saw the incident and it is unsure if the rock came off of the primary screen or out of the jaw. When EE came to, EE began to throw up. EE was taken to the hospital for a CT scan. Conclusion of injury is a concussion.
