Carpenter installing taper ties; hardhat brushed against planks, causing debris to fall onto EE's hardhat and forehead. EE removed EE's safety glasses to brush the debris from EE's face at which time a small piece landed in EE's left eye. After flushing the eye with no relief, EE was transported to medical facility where a doctor flushed and wiped the eye clean.

Employee was welding and grinding on the 5230 shovel. At the end of the shift the employee noticed he had something in his eye. He thought it was only dirt and assumed it would work it's way out. The next morning he reported an irritation to his eye and was taken to an eye doctor who removed a small piece of metal from his right eye.

EE WAS HELPING TO MOVE 4" IRRIGATION PIPE. WHEN THIS WAS COMPLETED HE COMPLAINED OF BEING VERY TIRED AND WEAK. HE CALLED IN SICK THE FOLLOWING TWO DAYS. ON 4/18 HE WAS ADMITTED TO THE HOSPITA L. HE WAS DIAGNOSED WITH KIDNEY FAILURE. HIS DR.CONCLUDED THAT THIS WAS DUE TO OVER-EXERTION OF HIS MUSCLES ON 4/15/02.

GETTING OUT OF TRUCK AFTER MOVING EQUIPMENT AND SLIPPED ON TRUCK STEP AND SLIPPED ON MUDDY GROUND AND FELL BACKWARDS.
MOVING DRILL BIT HAMMER ON RIG WHEN LEFT FOOT SLIPPED OFF CAT-WALK AND HE FELL WITH HAMMER IN LEFT HAND PINNING LEFT ARM BETWEEN CAT-WALK AND HAMMER. FRACTURED LEFT ARM, FIVE-INCHES ABOVE WRIS T. REQUIRED SURGERY.
