Tearing out a mine fan, the top half of the shroud had been unbolted from the lower half with several bolts left in the conndu without nuts. As one piece was being lifted by a 15ton crane ee stepped onto the top of the shroud closest to the blade. The shroud moved ee jumped off and the shroud rolled onto him pinning him under it.

Employee was in process of picking up a timber and turned to place on a stack of timbers and felt pain in lower back. Employee had MRI and was diagnosed with lumbar sprain/strain on 10/21/2008.

An unintentional roof fall occurred in the # 1 entry of 6-Mains starting at SS# 3907 extending to SS# 3847 and continuing inby SS# 3847 into the #4 entry of 6-Left panel for approximately 80'. The fall dimensions were approximately 18' w x 6' h x 260' long.

An unintentional roof fall occurred at ss# 4215 in the mains bleeder #4 entry. The fall dimensions were approx. 35;L x 18;W x 6'H.

EE was in process of setting turn timbers in the #13 entry of Mains section for the right barrier lift, when two sections of roof broke loose from the roof, measuring from 0-2" thick and 45" X 28" and 45" X 27". The roof fell out from around the roof bolts, striking him on back of the head, middle of back, top left hand and left elbow.

Employee was n the process of extending line curtain when a section of roof fell measuring 7.5'L x 6'W x 0-14" thick from the unbolted face and extending approximately 16" past last row of bolts to where he was standing under a permanently supported area striking him across both legs resulting in fracture to left big toe and tailbone.

He was shoveling snow out of truck bed. Fell down in the truck bed.

An unintentional roof fall occurred approx one crosscut inby SS# 3779 in the #4 entry of the Mains section. The fall dimensions were approx 20'W x 45'L x 6'H.

Employee was operating unihauler while pushing disabled scoop on the surface, when the hauler bucket was raised up approximately one and one half feet and dropped back down to the ground causing him to be shaken in deck. He complained of pain in back, neck and right shoulder. He started missing work on 12/27/07 and medical diagnosis not received until 1/8/08.

A methane ignition occurred on Mains section in the face of #3 entry while mining with Joy 14cm15 continuous miner approximately 136' inby SS #4101.

EE was in process of tramming miner out of #2 entry on 6-Left when a bucket started to fall off miner. He stopped miner and reached to grab bucket and inadvertenly contacted tram controls on remote box, causing the miner to move partially, catching his right hip between the cable stand off on miner and coal rib.

Methane ignition occurred in the face of #3 entry of 6-Left section while mining with Joy 14cm15 continuous miner approximately 30' inby SS #4084.

Employee was in the process of turning on water hose at #6 belt drive and as he turned valve on from the hard line going to the hose he was using it jerked his right arm. Employee was diagnosed with a torn bicep tendon and repaired on 10/11/07.

Employee stated that while pulling shuttle car cable he struck the roof with his hard hat causing irritation to his neck while on the Mains section. Employee was diagnosed with neck spasms and started missing work on 10/23/07.

Notified by an MSHA employee an unintentional roof fall had occurred approximately 100' inby SS# 3817 in the #1 entry of the abandoned and not active workings 4-Left panel. The fall dimensions were approximately 5' high by 20' wide. Length could not be determined due to area not being accessible.

Employee was attempting to move a rock off of the #6 belt tailpiece when he felt a strain in his groin area.

An unintentional roof fall occurred approximately 300' inby SS# 1304 in left return. Fall dimensions were approximately 4'H x 18'W x 60' long.

Methane inundation occurred at SS#3906 in #2 entry of 5-Left.

An unintentional roof fall occurred in the intersection at spad #3840 in the #5 entry at number 24 break on number 6 belt. The dimensions of the fall were approximately 6-10'h x 20'w x 60' l.

An unintentional roof fall occurred at SS# 3460. The fall dimensions were approximately 30' L X 60' W X 8' high.

An unintentional roof fall occurred approximately 100 feet outby SS# 3839 on 4-Left panel. The fall dimensions were approximately 6' H X 120' L X 20' wide.

A methane gas inundation occurred on the active pillar line of the 4-left section due to a pillar fall inby the pillar line in the active gob.

An unintentional roof fall occurred on top of a Joy 14-15 continuous miner in the face of the #7 entry of 4-Left approximately 30' inby SS# 3813. The fall dimensions were approximately 20' L x 20' W x 6' high.

Employee was attempting to straighten a pallet of cinder block by using the bucket of scoop and as he backed up the bucket of scoop came off pallet causing the scoop to bounce. His hard hat came off and he struck the left side of forehead on canopy of scoop resulting in a laceration.

An unintentional roof fall occurred on top of an Eimco 2810 continuous miner in the unbolted face of #7 entry approximately 35' inby SS3 3640 on 3-Left section. The roof material dimensions were approximately 20'L x 20'W x 3'H.

An unintentional roof fall occurred approximately one crosscut inby SS# 1294 between the #6 and #7 entries. Fall dimensions were approximately 40'L x 20'W x 5' high.

Received letter on 11/16/06 that this part 90 miner has exercised his option.

An unintentional roof fall occurred at SS# 3255 in the #4 entry. The fall dimensions were approximately 90'L x 20'W x 5' high.

Employee was aligning pump motor into position for installation into continuous miner. Pump motor became lodged and as he repositioned motor it shifted catching his right ring finger between motor and frame, fracturing it.

Employee was pushing a wheel barrow loaded with coal material at #2 belt drive, when wheel barrow shifted he tried to correct it and strained his right knee. He underwent an MRI on 9/28/06. The preliminary results comunicated on 10/2/06 that he has torn meniscus to right knee and surgery necessary for repair.

An unintentional roof fall occurred at SS#3505 in the #8 entry of 2-right mains. The fall dimensions were approx 40'l x 20'w x 7' high.

An unintentional roof fall occurred approximately 100' inby SS# 3405 in the #5 entry of the 003 mmu retreat mining section. The dimensions of the fall were approxiamtely 7'h x 15'w x 20' L.

An unintentional roof fall occurred one crosscut outby SS# 3257 in the #5 entry. The dimensions were approximately 20'L x 20'W x 4'high.

An unintentional roof fall occurred two crosscuts inby SS# 34460 between number 8 and 9 entries of the 001 mmu. The fall dimensions were approximately 40'L x 18'W x 7'H.

An unintentional roof fall occurred at survey station # 3058 on the 001 section. The fall dimensions were approximately 60'L x 18'W x 6'H.

An unintentional roof fall occurred approximately 20 feet inby SS# 1211 in the #1 entry. The fall dimensions were approximately 40' L x 14' W x 4' high.

An unintentional roof fall occurred approx. 100 feet inby ssn# 1187 in the #1 entry. The fall diamensions were approx. 80'L x 18'W x 4'H.

Employee was taking a scoop off charge in the #5 entry on the Mains Section and stated that he twisted his right knee in the mud at the scoop charger station.

An unintentional roof fall was discovered at approximately 9:10 am in the intersection of the 3 entry about 45' inby survey station NO. 1213. The fall was approx. 30' long x 19' wide x 4' to 8' high.

A methane ignition occurred at the face of 5 right crosscut approx. 90 ft to the right of survey station number 2931 on the 001 unit. There were no injuries or property damage as a result of the ignition and the flame was self extinguished.

AN UNINTENTIONAL ROOF FALL IN TWO DIFFERENT LOCATIONS: 1. ROOF FALL DISCOVERED ONE BREAK INBY SURVEY STATION 1302 INTO LEFT CROSSCUT, 60'X20'X42". 2. ROOF FALL WAS DISCOVERED ONE BREAK INBY SURVEY STATION 1290 INTO THE RIGHT CROSSCUT AND CONTINUED UP THE ENTRY ONE BREAK AND INTO RIGHT CROSSCUT. 90'X20'X42"

An unintentional roof fall occurred int he number 3 entry, 2 breaks inby survey station 1604 in the belt entry. the fall was approx. 60 ft long x 20 ft widex 5 ft high.

AN UNINTENTIONAL ROOF FALL OCCURRED APPROXIMATELY ONE BREAK INBY SURVEY STATION #1498 ONE CROSSCUT TO THE RIGHT PLUS 20 FEET.

AN UNINTENTIONAL ROOF FALL OCCURRED APPROXIMATELY ONE BREAK OUTBY SURVEY STATION #1297. THE FALL WAS APPROXIMATELY 4' HIGH X 25' WIDE X 20' LONG.

AN UNINTENTIONAL ROOF FALL OCCURRED ON THE #2 BELT APPROXIMATELY 40 INBY SURVEY STATION #1213. THE FALL WAS APPROXIMATELY 5 FT HIGH X 22 FT WIDE X 30 FT LONG.

EE WAS HELPING TO PUT THE COUNTERWEIGHT ON THE CRUSHER SPEED REDUCER FOR A STAMLER FEEDER AND WAS TRYING TO ALIGN IT TO GO OVER THE KEY STEEL. WHEN IT DID LINE UP IT CAME DOWN SUDDENLY, CATCHING EE'S RIGHT INDEX FINGER AND THUMB. EE WAS DIAGNOSED WITH A FRACTURED RIGHT INDEX FINGER AND THUMB ON 9/20/04. (FRACTURE/CHIP/DISLOCATION)

An unintentional roof fall was discovered at approx 9:30 pm in the intersection of the #6 entry to the right of survey station no. 1289. The fall was approx 4 ft high by 25 ft wide by 25 ft long.

An unintentional roof fall was discovered at about 5:30 AM in the intersection of the #3 entry and into the right crosscut at survey station no. 1311. The fall was approximately 4 ft high x 19 ft wide x 60 ft long.

Employee was loading 6 foot timbers into a scoop bucket and felt pain in his lower back.

AN UNINTENTIONAL ROOF FALL OCCURRED IN THE NO.1 BLEEDER ENTRY OF THE 1ST RIGHT PILLAR SECTION, ONE CROSSCUT OUTBY SURVEY STATION NO. 1664. THE FALL WAS APPROX. 70'LONG X 20'WIDE X 5'+ HIGH.

AN UNINTENTIONAL ROOF FALL OCCURRED IN THE NO.2 BLEEDER ENTRY OF THE 1ST RIGHT PILLAR SECTION, ONE CROSSCUT INBY SURVEY NO. 1666. THE FALL WAS APPROX. 40'LONG X 18'WIDE X 4'HIGH.

THE INJURED EMPLOYEE WAS BACKING A FLETCHER ROOF RANGER II ROOF BOLTER UP THE #4 ENTRY TOWARD THE FACE ONE BREAK INBY SPAD #1434. THE EE WAS OPERATING THE BOLTER FROM INSIDE THE OPERATOR DECK ON HIS KNEES IN AN UPRIGHT POSITION FACING THE REAR OF MACHINE USING LEFT HAND TO OPERATE TRAM LEVERS. TRAMMED THE MACHINE CRUSHING HAND BETWEEN MACHINE & RIB, SURGICAL REMOVAL RT RING FINGER

The continuous miner backed into shuttle car after it had been loaded and the operator was about to turn around. His feet were caught between shuttle car and rib.

EE WAS WORKING ON THE HEAD OF A JOY 14CM-10. MINER STEPPED DOWN ONTO A HYDRAULIC HOSE LOCATED ON THE EDGE OF THE MINER PAN, SLIPPED OFF THE HYDRAULIC HOSE ONTO THE GROUND & CUT THE BACK OF HIS MIDDLE FINER ON HIS RIGHT HAND REQUIRING 4 SUTURES TO CLOSE, EE STARTED MISSING WORK 1/24/04

PUTTING UP BELT HANGERS DRILLED HOLE, INSERTED BOLT, TIGHTEN BOLT AND A SMALL PIECE OF ROCK FELL 13 FT HITTING DRILL HEAD ON ROOF BOLTER THEN BOUNCED AND HIT OPERATOR IN THE MOUTH RESULTING IN HIM NEEDING SEVEN STITCHES.
