installed brake shoe, it slipped causing it to smack finger resulting in a cut

working in shop (wearing safety glasses) doing own work, another person grinding some distance away and foreign material got in eye.

Lost balance on a flat surface and fell on hand

Tracking cable on to yarder spool. Clamp came loose causing cable to come out and hit employee on arm and leg.

Employee stated has worked for multiple companies air arcing and other noisy work over EE's work experience and hasn't always worn protection. Protection is provided in the shop and EE has had numerous training on hearing conservation. EE stated to MSHA that 80-90% of EE's work is fixing over the road trucks and construction equipment.

Cut to left hand by base end of bolt

Climbing down a ladder of loader, slipped and caught self when right arm slammed into steps and railing
