Employee was helping to rebuild crusher. In an attempt to line up bolt holes employee used a crusher line up pin as a hammer to knock the bonnet leg into alignment. While swinging the line up pin, employee smashed right pinky finger. 9 stitches were needed.

Employee was trying to pry a rock out of the tail pulley when he felt something in his back "pop". Employee came to the office at 3:18pm to tell the plant manager that his back was hurt. *** Reportable Lost Time because employee did not come to work August 30 & 31.
While installing rubber in the second deck of #1 screen, we lifted it up onto the catwalk. It was during the process that I felt my back get hot. * Employee has a strained back. This became a reportable on May 23 when it was determined through doctor's reports that his injury was work related.

EE was exiting screen and fell approx 36 inches and struck his arm on a small stone.320042820011-update returned to work full duty with no restrictions.(EE was inside the screen doing maintenance whent he job was finished other instructed EE to get out of the screen. On his way out of the screen EE lacerated his right arm just above the elbow on a sharp rock.

THE NE XT DAY HE WENT TO A CHIROPRACTOR WHO SAID HE HADCOMPRESSED 2 OF THE LOWER VERTEBRA & WOULD NEED SEVERAL TREATMENTS.

EMPLOYEE WAS GETTING READY TO CLIMB UP INTO HIS LOADER, WHEN THE KNEE HOLDING HIS WEIGHT GAVE WAY AND HE FELL TO THE GROUND.
EMPLOYEE WAS PUTTING BELT LAZZERS ON A BELT WHEN HIS HAND SLIPPED ON THE BELT AND SLID ACROSS THE LAZZERS.
INJURED WAS USING THE ONE INCH AIR GUN TO TIGHTEN THE BOLTS OF THE SWING DIE, WHEN THE SPACER THAT WAS IN THE JACK, HOLDING THE SWING DIE FORWARD, DROPPED OUT, APPARENTLY FROM THE VIBRAT ION OF THE AIR GUN. WHEN THE SPACER DROPPED OUT,THE SWING DIE CAME BACK APPROX 4 " TRAPPING HIS FINGERS BETWEEN THE AIR GUN AND THE TOGGLE BLOCK
EE WAS WORKING ON THE BELT WIPER TO #9 CONVEYOR. THE TURNBUCKLE ON ONE SIDE WAS STUCK, SO HE YELLED UP TOCOWORKER TO THROW HIM A WRENCH TO GET THE TURNBUCKLE UNSTUCK. WHEN COWORKER DROPPED THE WRENCH DOWN, EE HAD REACHED OUT TO CATCH IT WHEN THE HANDLE STRUCK THE SIDE OF THE WIPER FRAME CAUSING THE WRENCH TO SPIN AROUND STRIKING HIM IN THE FACE, CAUSING A CUT BETWEEN NOSE & LIP.

THE INJURED THE DRILLER WAS INJURED ON THE JOB AT NO GIVEN TIME WEDNESDAY DEC,7 1988,DURING THE WORKING HOURS WHILE DRILLING.THERE WAS ANOTHER DRILLER ON THE SAME SITE,BUT THE ACCIDENT WAS NOT SEEN BY HIM OR ANYONE ELSE.HE REPORTED THIS ACCIDENT AROUND 9:30 AM ON THURSDAY MORNING DEC 881988,CLAIMING HE HAD INJURED HIMSELF BY SLIPPING AND FALLING.
AT 9 01 AM I NAME AND I NAME WERE IN THE PROCESS OF FILLING WATER TANK ON JOY DRILL. IN CONDUCTING THIS PROCEDURE, THEY WERE USING A FORD 445 TRACTOR AND 55 GAL DRUM. WHILE DOING THIS THE DR DRUM FELL OUT OF THE BUCKET CAUSING I NAME TO FALL FROM MACHINE. IN DOING SO HE HIT HISLEFT ARM AGAINST THE TRACK OF THE DRILL MACHINE
