Employee had a gloved hand on a lift chain when employee gave the command to raise the load. Employee's glove was pinched in the chain pulling employee's finger tip in the pulley before employee pulled hand out of the glove. Resulting in a fractured finger tip.

Employee was assisting with an elevator chain replacement. Employee picked up a section of chain that was lying on the edge of a concrete slab and got left index finger pinched between the chain and the concrete causing a split in the end of index finger.

Employee was walking down stairs and felt a pain in EE's lower abdomen.

Employee was walking down stairs without using the handrails when she slipped and fell. She had a sore back and initially there was no medical treatment. The pain was anticipated to subside within a few days/weeks. This week she called her doctor who ordered her a steroid shot and she was scheduled to attend a few physical therapy sessions because the pain has not subsided.
Employee was in the baghouse cleaning the rust scale buildup on the walls of a baghouse. He was using a scraper mounted on an 8' pole. The scraper was leaning up against the wall and was inadvertently bumped, causing it to fall down. The blade struck him causing laceration to right calf. He went to emergency room where he received stitches.

EE stepped off of a ladder and felt pain in his ankle on June 3rd and reported it to his supervisor on June 4th. After going to the doctor, the doctor determined it was arthritis, old age or weak tendons (not work related). A second opinion after pain continued determined this was a ligamentous injury to ankle. This is now considered work related.

Removing filter bags from cages on baghouse, repetitive motion. REPORTED INJURY TO SUPERVISOR ON 12/8/05. Employee thought it was just sore muscles, but pain has not diminished. Pete Lien & Sons employees went to Wyoming Lime Producers to help with annual maint. issues. Employee normally works at the Pete Lien & Sons Rapid City Quarry and Mill, 39-00020.

EE WAS ADJUSTING THE BELT TENSION BETWEEN THE CRUSHER AND THE MOTOR, WHEN HE GRABBED THE ADJUSTING HANDLE AND ROTATED IT. "AT THAT TIME I HEARD OR FELT A 'POP' IN MY LEFT BICEP AREA."

EE WAS ATTEMPTING TO FREE JAMMED AUGER. WHILE REACHING FOR A SHOVEL HE PUT HIS HAND ON A V-BELT DRIVE. THE DRIVE WAS STARTED REMOTELY AND HIS FINGER GOT CAUGHT IN THE DRIVE.

THE EMPLOYEE SLIPPED WHILE UP A CAT WALK. WAS HANGING ONTO THE HANDRAIL WITH HIS LEFT HAND AND GOT CAUGHT IN AN AWKWARD POSITION. THIS RESULTED IN LIGAMENT AND TENDON DAMAGE TO HIS LEFT BICEP.
HUMID CONDITIONS WITH WATER CLOSE BY. LIME FINES WERE HYDRATING DUE TO CONDITIONS. HE STEPPED INTO THE LIME FINES. THE HEAD FROM THE HYDRATED FINES BURNED HIS LEFT LEG IMMEDIATELY.
