While using a sledgehammer to clear for a drill to move forward, a piece of the stone broke apart and flew back into the left shin of the worker. The impact caused a minor puncture but imbedded a rock under the skin.
While using an air nail gun to make a crate for a job, the nail deflected off the side of the board and punctured EE's pointer finger on EE's left hand. The employee was seen at Rutland Regional Medial Center for a puncture wound. The employee returned to work on 01/06/23 at full duty with no restrictions.
Employee was deenergizing power cable to machine, when the cable got entangled on a rock. When they tried to pull harder the cable broke loose making them loose balance falling on the edge. They were conscious at all times and complained of ankle pain. Was taken to nearest hospital after the x-rays, they were recommended to go the chiropractic. *EE had surgery on heel on 9/23/22.
EE was walking down the paved roadway at entrance of the quarry when EE rolled an ankle on loose gravel. EE went to the Doctor and was diagnosed with a sprained ankle.
the wire saw operator was trimming a granite block when the diamond wire snapped, hitting at the safe guard and ricochet from there with one of the beads hitting EE on the leg, EE was 39 feet away from the machine at the moment of the accident, EE was conscious at every moment and EE was transported to the nearest hospital, EE got 3 stitches
EE was working with the wire saw gamma 875, and around lunch break (12:30 PM) EE reported a back pain after setting the machine in place prior to begin to cut. EE was taken to the doctor, finding that EE have mild posterior disc bulge, recommending to be off the rest of the week.
The miner was using a wrench to disassemble a DTH drill while a colleague was on the control of the machine. The machine was set in motion while the miner had hand close to the wrench (but not on it). The wrench struck the miner on the right hand thumb, pinching it between the tool and the shaft. Miner was taking to hospital for stitches and x-ray. Thumb is broken.
The employee was trying to position the track of the wire saw using a steel bar. As employee was lifting/pushing the bar, employee's foot slipped and employee hurt knee.
Employee was dragging a coiled up air when they felt a pop in the left shoulder. Employee went to the doctors and had an MRI and will eventually need surgery on the shoulder.
Failed hearing conservation test.
Employee was carrying a dth hammer on their shoulder. EE slipped and dropped the hammer on their ankle.
EE was moving gang saw blades for gang #2, when EE bent over and picked up a box and felt a pop along with a sharp pain from the groin to the knee.
EE was disposing wet dynamite caps. The caps were put into a designated area, lit on fire, and EE left the barricaded area. EE returned to the barricaded area, leaned down to relight the fire and at that point the caps detonated. EE was wearing safety goggles but EE still received minor burns to the face and a small fragment of steel in right shoulder.
Employee was adjusting saw wire on stone block EE was setting up to saw. In the process EE slipped and fell breaking EE's left wrist. EE was taken to the hospital for treatment. EE was sent home the morning of the accident. EE returned to the hospital on 1/22/2016 for out patient surgery to install pins in EE's wrist. EE returned home the same day.
The injured employee was standing behind the wire saw when the saw wire broke and a piece of metal from the wire struck his hard hat, penetrated the hard hat, and hit him in the back left side of his head. The employee suffered a possible skull fracture. He was admitted to the hospital for two days for observation and is now at home recuperating.
While driving a wedge w/a sledge hammer, left foot slipped which caused him to twist his back.
Employee was trying to clear ice from a 3 airline. When the ice blew out of the end of the line it caused the employee to lose control of the line. The line moved backwards out of his hands. The coupling on the end of the line caught the thumb on his left hand, breaking his thumb, and causing contusions to his chest wall and ribs.
At around 6:05am IEE was walking down the ramp heading to his work area when Co-EE and he started horse playing. Co-EE was pulling IEE's head down and IEE was moving back causing him to sprang his neck and shoulder. He walked back to the lunch room and took one Aleave and continued to work till the end of the shift.
While the employee and his supervisor were discussing the job, a corner piece of rock fell out of the block they were standing by and hit the employee in the right foot.
Removing marble slabs - Pelvis fracture
Operator was moving a horizontal lift drill to another location in pit area when an unsecured drill steel on machine fell striking a miner in the forehead (miner was wearing hard hat) and approximately 15 ft. from suspended load.
While lifting corner of marble block with excavator so ee could place block of wood under the block of marble 4 piece of marble fell off. Striking ee in the crest and lower neck ee had wind knock out of him, and sustained bruise to his chest and neck area.
Hooking chain on turntable of saw, operator told loader operator to lower the boom on loader. The chain pinched end of finger with chain. Replaced short chain with long chain.
DRILLING HOLE, DRILL CAUGHT ON METAL, DRAGGING HAND ACROSS RIVET, CAUSING A CUT ON LEFT HAND (5 STITCHES).
