IE was working on raw coal screen doing some lifting and twisting motions and started feeling pain in shoulders and back. IE went to Sparta Hospital and is off work till released by EE's doctor.

The injured employee was assisting with dragging a scrap truck frame when EE reached under the front bumper area to guide the Dyneema rope. A sharp metal shard protruding from the underside of the bumper contacted EE's right forearm.

Employee pinched a finger while changing a return idler on the belt line causing a laceration requiring stitches.

IE experienced left hip and back pain after loading drill on hauler. After loading drill, fell from desk of heavy haul trailer. IE had follow up visit on 4-22-26 and was diagnosed with a possible pinched nerve and was prescribed physical therapy and a steroid. IE was never restricted to do EE's job.

Employee forgot water bottle in the cab and when they reached back into the cab, they closed a finger in the cab door causing a laceration. Employee apparently was given a splint and restriction.

Employee was carrying a box of detonators while distributing them on blast pattern when they stepped into a low spot in the ground, employee felt discomfort in their abdomen. Employee was diagnosed with a hernia on 1-29-2026 making this a reportable incident.

Employee stated that they pinched a finger while changing a brake caliper. After returning to work after 3 days off, the employee reported the event to their supervisor via phone call and missed their next scheduled shift.

Employee was cleaning dirt, oil, and grease from a retainer cap. After removing all the debris, they ran their hand over the cap's surface. As the employee placed their hand on the cap, a small metal burr protruded from a bolt hole. The bur poked the employee in the hand, and they jerked it back, causing a laceration to their palm requiring three sutures to close the wound.

Technician pinched finger while handling metal object causing a fracture to the end of right index finger.

Employee pinched the right index finger while moving railroad ties resulting in a laceration requiring stitches.

Employee was loading a training roller into the back of a mantrip. When roller got stuck IE lifted roller heard and felt a pop in their right arm

IE was helping repair a 4' heavy-duty hose that feeds the dewatering system's #7 shaker. IE was holding the hose when it shifted, striking and pinching IE's right pinky finger. IE was taken to emergency room where it took 4 sutures to close the wound.

Technician felt pain in left bicep while moving a pallet in the warehouse. Upon an ORTHO appointment on 11-10-2025, technician was given a restricted release.

IE was helping set feeder in place and had EE's foot underneath load end of feeder. IE's foot was smashed when feeder was set on the ground. IE was taken to emergency room where EE suffered two fractures on EE's left foot.

IE caught their left arm between a set of equipment doors while trying to enter the intake aircourse causing a fracture to the ulnar bone between their wrist and elbow.

IE was doing normal work duties and felt pain in both shoulders. IE continued to work full duty up until December 5th and missed their first scheduled shift.

While lubricating haul truck wheel hardware, metal protruding from hardware lacerated left pinky finger

IE was greasing components on screen bowl. While pulling on drive belt to turn components, EE's pinky finger was pinched between belt and pulley requiring 5 stitches to close cut.

IE was moving a cable spool and felt a pain in lower left leg. IE was transported to emergency room and diagnosed with an Achilles heel tear to left leg.

On 06/12/2025 employee reported that a coworker kicked the back of the front seat from the back seat of a quad cab pickup on 06/11/2025. On August 4th the employee was treated at an urgent care facility and released with restrictions.

On 6-8-2025 IE felt knee pain while changing cutting edge on a scraper. On 6-16-2025 IE was seen at orthopedic and given a restriction after MRI reading.

IE hanging high volt cable and hit right index finger on a roof bolt plate causing a laceration. IE received two stitches due to the laceration on their finger.

Employee pinched EE's finger while removing an access panel in the plant causing a laceration that required stitches.

IE was installing a roof bolt. While holding the bolt in the drilled hole, the bolt inadvertently moved, bending the bolt causing EE's hand to get caught between the drill chuck and the drill guide. This caused a laceration that required sutures.

Employee's left heel was caught between the relay bar clevis pin and the headgate debris plate, causing an avulsion to the heel requiring surgery.

IE went under parked haul truck to check it out & got hydraulic oil into both eyes. IE received Rx eye drops and was not released to return to work

Employee bumped their lip while changing a mudflap causing a laceration that resulted in 3 stitches.

Mobile maintenance greaser/oiler reported on 01/13/25 that they noticed a noise in their shoulder, void of any pain or discomfort. During consultation it was learned the the EE had a previous right shoulder surgical repair. Diagnostics revealed degenerative changes to the same shoulder from years of repetitive motion and overhead work. Surgical repair was performed on 07/11/2025.

Employee had a piece of debris in left eye and was given a prescription from the optometrist.

While changing out a left front tire on a haul truck, I.E. was trying to spread the lock ring and EE's hand slipped and was pinched in lock ring causing a laceration to EE's right wrist.

IE was changing scoop battery, and the nip was tight, so IE claims they twisted harder and felt pain in their right thumb. Incident happened around 12:15 AM on the 5th. IE was on midnight shift. IE worked at full duty until 11/25/24 when at a follow up appointment, they were given restrictions and a thumb splint. IE is still working but light duty.

IE was hanging tie wire for bolter cable across entry #3 in unit 2. The ends of the tie wire were bent and when IE let go of the wire it flexed and hooked under their safety glasses, striking them in the left eye. IE received a prescription eye drop and returned to work.

Employee pinched a thumb while installing a ripper cylinder pin causing a tuft fracture.

IE caught fingertip between brass bushing and shovel saddle block while setting the bushing in place. Fingertip was avulsed and required medical treatment.

Employee stated that EE reached for EE's falling lunchbox and felt a pop in EE's shoulder. Upon an ORTHO appointment on 10/15/2-24, EE was issued a restriction for a shoulder sprain and scheduled an MRI.

IE lifted cable & reported back pain and was evaluated with no acute injuries and a back strain. IE was given a full release and could perform all job duties. IE was moved to an alternate role for training for upcoming vacancies. Despite being released, an MSHA inspector believed this met the criteria of RWC. We disagree but are reporting the case.

IE was bolting on left side of unit 1 in entry 5 installing hog panel. When IE lifted the sheet of hog panel over the canopy it hung up causing them to strain their right shoulder. IE continued to work full duty until 11/26/24, when they were seen by a doctor and given restrictions until MRI could be performed.

Carrying sheet of hog panel over the roof bolter to put on ATRS and IE hurt right shoulder. On 12/31/2024 IE went to doctor and received restrictions that we could not accommodate with EE's normal work duties.

Employee got a drop of transmission fluid in their eye and was prescribed eyedrops from the optometrist.

Employee felt debris in EE's eyes and after an optometrist appointment, EE was prescribed an antibiotic. There was nothing embedded.

IE was loading pinner when a roof bolt plate slipped in IE's arms and Kevlar sleeve rolled up and the plate cut IE on left forearm. Unit 3 room #4, 1+80.

Walking into the front of a Locomotive cab, hand was in the doorway while the door was closing. The gloved right hand was pinched in the door momentarily, causing right index finger to be lacerated when the door closed on it.

Employee was unthreading a water hose from the wet dust truck while the hose was stretched. When the hose was undone the water hose sprung back striking the employee in the cheekbone causing a contusion just under the right eye. After further evaluation from an ophthalmology specialist, IE was diagnosed with a closed fracture of the right orbital floor on 7/9/2024.

Employee lifted a coal sample and felt a pain in their back. Upon an ORTHO appointment on 06/14/2024 employee was prescribed physical therapy for a back strain.

IE was removing a line boring bar by pulling it with right hand. While pulling, the far end of the bar slipped off what was holding it. This caused near end to fall and strike right thumb. The thumb was fractured and was set in a cast at the ER.

Contract temporary employee pinched a finger while helping to change a belt idler resulting in a laceration to the finger requiring stitches.

Lifting a 30lb sample into the floor crusher and felt pain EE's right shoulder. After an appointment with ortho on 5/23/2024, the employee was prescribed physical therapy.

IE was walking in uncleaned x-cut between entry #4 and #5 in Unit 2. IE stated there was no incident EE just heard a pop in EE's left foot.

IE was loading 2 x 4 on to the battery scoop. As IE was pulling on board their right ring finger was caught in between the end of the board and scoop battery.
