Employee was in the process of removing a hydraulic hand pump and jack from the top of the tub base and hung EE's foot on a hose and fell forward and EE's thumb on the left hand was caught between pump and ram which caused a fracture of the first joint of the left thumb.
Employee was using a pry bar to unplug a clogged bit on a blast hole drill. Once the clogged bit was free of material, the employee dropped the pry bar onto EE's foot which contacted EE's foot in between the safety toe and the met guard of the boot causing a superficial fracture to EE's big toe.
Technician pinched EE's hand while using an air impact gun causing a laceration that required stitches.
A train started a grass fire on the outskirts of the mine and was not extinguished within 30 minutes of discovery.
Employee pinched a finger while changing a return idler on the belt line causing a laceration requiring stitches.
Employee experienced a medical emergency while at the mine site. The employee then fell to the floor from a leaning position. The employee was transported via county EMS for further care.
IE experienced left hip and back pain after loading drill on hauler. After loading drill, fell from desk of heavy haul trailer. IE had follow up visit on 4-22-26 and was diagnosed with a possible pinched nerve and was prescribed physical therapy and a steroid. IE was never restricted to do EE's job.
IE tripped and fell on shoulder, after Orthopedic appointment on 4-21-26 was diagnosed with rotator cuff injury that will require surgery
Employee twisted while changing hydraulic filters and began to feel a pain in their knee. later they asked to be checked out and was seen at the ER. An ORTHO appointment on 03/26/2026 revealed a torn ACL and they were given a restriction.
Employee began to experience pain in EE's shoulder after dismounting a haul truck. EE was seen by an ER doctor and referred to an ORTHO. An MRI reading on 03-11-2026 revealed significant chronic arthritis and was restricted for no work.
Employee cut finger while grinding resulting in a laceration that required stitches.
Upon receiving an audiometric test result, the administrating assistant has reportable hearing loss.
Employee forgot water bottle in the cab and when they reached back into the cab, they closed a finger in the cab door causing a laceration. Employee apparently was given a splint and restriction.

Employee felt back pain after hitting bump in a light duty vehicle. After emergency room visit employee was given restrictions and missed the next shift.
Employee was exposed to blasting emissions from a neighboring mine. They sought medical evaluation at the hospital, was released with no restrictions but missed their next scheduled shift.
Employee stated that they pinched a finger while changing a brake caliper. After returning to work after 3 days off, the employee reported the event to their supervisor via phone call and missed their next scheduled shift.
Employee twisted knee while dismounting a mobile crane. Employee was seen at an ORTHO on 01/07/2026 and was where an MRI revealed a torn ACL that will need surgery.
Employee was cleaning dirt, oil, and grease from a retainer cap. After removing all the debris, they ran their hand over the cap's surface. As the employee placed their hand on the cap, a small metal burr protruded from a bolt hole. The bur poked the employee in the hand, and they jerked it back, causing a laceration to their palm requiring three sutures to close the wound.
Technician pinched finger while handling metal object causing a fracture to the end of right index finger.
Employee pinched the right index finger while moving railroad ties resulting in a laceration requiring stitches.
Technician felt pain in left bicep while moving a pallet in the warehouse. Upon an ORTHO appointment on 11-10-2025, technician was given a restricted release.
Employee was walking and rolled EE's ankle. After an ORTHO visit on 10/09/2025, EE was restricted with no work for 4 weeks.
Spontaneous combustion in the upper rider seam of coal in the Middle pit. The pit is currently inactive.
Employee began to feel discomfort in neck and upper back while operating a haul truck. EE was taken to the clinic for evaluation and given a full release with no restrictions or follow up appointment needed. EE did not return to their next shift upon their own decision.

While walking towards crew van employee twisted and began to feel back pain. On 4-17-24 the employee was seen by an Orthopedic and prescribed physical therapy.
A fire was noticed near the North Narm lay down yard. This area is located in a remote part of the mine so at no time were mine personnel or mine facilities threatened. The fire was extinguished at 2:59AM.

IE stated that EE stepped down from a haul truck ladder and felt discomfort in EE's right leg. On 10/18/2024 EE went to the Dr for swelling in EE's right ankle. EE was diagnosed with a strain and given a 1 week restriction with no treatment.

IE was operating haul truck and began to complain of back discomfort after hitting a bump. The employee was seen again at the ORTHO on 01-03-2024 and was restricted no work and prescribed PT for work hardening.
IE had thumb momentarily caught in a pinch point while replacing a bearing on a conveyor. They received stitches to close the wound to the tip of their right thumb.

Employee tripped same level and caught self with right arm causing pain. An MRI reading at the ORTHO on October 20th revealed a tear to the bicep requiring surgery.

An employee allegedly was exposed to blasting fumes and went home sick. The following day, the employee also went home sick and was seen by a Doctor, where the employee was given a full release with no restrictions or treatment. The employee did not return to work upon the employee's own decision.
Mine was struck by a Tornado. Tornado flipped our vehicle on its side , while our driver was in it. Driver was taken to the hospital , treated and released. Driver was out of service due to DOT policy. Driver allegedly claimed had PTSD from the event which placed EE out of service.
Manpower temporary employee was injured during a weather event. Upon an Orthopedic follow up visit, it was determined that the employee had a hairline fracture of the femur.
Employee began complaining of wrist pain and after several Orthopedic visits was diagnosed with carpel tunnel syndrome and underwent surgery.
Standing on the adjustment springs for the lower curtain liner, EE went to step aside to adjust self when EE's foot slipped on the adjustment screw and fell to the deck of the inertia, landing on left leg and at that point went down onto the deck of the inertia holding knee.
A mine fire was not extinguished within thirty minutes.

Person slipped in the parking lot when exiting a personal vehicle before shift resulting in a fracture to EE's ankle.
Supervisor lost balance while observing conditions after a weather event resulting in a fractured leg.
While repairing a loader in the field, our service technician was struck in the face with a tool resulting in a fractured cheek bone.
Shovel faulted making contact with truck on 09-29-2022. The employee did not return to work.
Replacing a dragline wear liner the employee was unable to see a developing crack on the opposite side of the plate due to a welding hood. A preexisting weld failed to hold plate due to age causing the plate to fall & injure the employee. Employee was found to have a fractured toe and a laceration resulting in sutures. Employee returned to work for employee's next scheduled shift.

Hot coals were noticed on a coal shovel bag house. A couple of bag house filters combusted and created a coal smoker that was not extinguished within 30 minutes. There were no injuries.
Employee bumped face on a roller idler frame while changing a roller causing a small laceration on the bridge of EE's nose. It appears that the laceration was from EE's safety glasses rather than the frame or tools. EE's glasses were correct and approved safety glasses.

Employee began to feel discomfort in left eye. Employee was taken to the optometrist where a small rust ring was removed. No object was imbedded. A prescription for a steroid was given.

IE was helping to install feed chute for a crusher. The feed chute moved unintentionally, catching IE between the feed chute and the structure. The IE was taken to the hospital and diagnosed with a pelvic fracture.
IE was reeling a grease hose when IE felt pain in the RH abdomen. IE was taken to the ER for evaluation and given a full release with a follow up appointment in 10 days. On 08/18/2020 IE was diagnosed with a small hernia.

IE was rotating a hoist drum coupler by hand to align the halves and pinched thumb between them causing an avulsion and tuft fracture that required stitches.
IE stated that EE was descending ladder, missed the last rung and slipped causing a laceration to EE's left hand requiring stitches.
While changing haul truck air filters, the IE cut finger on a sharp burr that was on the outside mesh of the filter requiring sutures.
While performing welding duties employee received a laceration from an electrical grinder requiring stitches
Employee was removing a chunk of dirt from the push-arm of a dozer and pinched a finger between the chunk and the push arm. EE received 2 stitches to hold the rear of the finger nail in place.
Employee lost footing in shovel bucket while changing bushings causing a laceration to left arm that required sutures.
During high winds, the haul truck operator opened the cab door, the wind caught it and pinched a finger between the door and the handrail. Operator's ring caused a laceration that required stitches. Gloves were being worn.
Medical issue - Employee was experiencing severe head pain, non-chargeable
IE was installing a pin on a drag line bucket and pinched thumb causing a laceration requiring stitches.
Contract employee was arc gouging wear plate from a dragline bucket and as employee finished the cut, the piece of material dropped down contacting employee's lower left arm resulting in a laceration requiring stitches.
Employee was walking to haul truck and slipped and fell on the freshly bladed ground causing a fracture to EE's left femur.
Hot coal started a fire in solid waste. It was determined the best way to put the fire out was to bury the material. The fire was extinguished by burying the pit.
Employee was using a high pressure wash bay hose to clean mine equipment. The hose twisted causing the pressure to drop. The hose freed resulting in a surge of pressure causing the employee to be knocked off balance, the hard hat to fall off and EE struck the back of head on a low hanging piece of the equipment that was being cleaned. Resulted in a small head wound.
IE was descending a ladder, slipped and struck a guard on beltline with back, then fell to ground.
Employee was performing cleaning tasks and lost footing. EE caught self with EE's right arm and began to feel pain in EE's wrist. EE was diagnosed with a wrist fracture.
Found a smoker about 1'x1'. Was in an area that was better accessed in the day. Day shift is working on burying it now.

IE was driving a loaded haul truck when ee hit a pot hole and felt tightness in neck. IE was seen by an ortho and an MRI was ordered. On 5/9/19 the MRI was read and it was determined the employee had a bulging disk that would require surgery.
IE was lifting an electrical component and felt a pain in IE's right arm. An MRI revealed a tear to the bicep tendon requiring surgery to repair.
IE twisted EE's ankle when walking down stairs. EE was taken in for evaluation revealing a fracture. The landing area of the stairs was a clean, clear and flat concreted area.
While moving trailing cable, the IE slipped on the freshly bladed ground and felt a pop in the right arm. EE was taken to the ER for evaluation and referred to an Orthopedic Dr. The IE was seen by an ORTHO and scheduled for an MRI. The MRI reading revealed a tricep tear that required surgery to repair.

While setting up to lower power cable from the cables tree the brake handle contacted the employee's face resulting in a laceration and a fracture.

The IE felt irritation in left eye after getting a drop of water in it. The IE was taken in for evaluation and was prescribed an antibiotic for a scratch to the cornea.
IE was dismounting a Cat 834 Rubber tired dozer, stumbled or tripped on feet, slipping causing a laceration to right knee and pain in left shoulder.
IE was lifting a spring assist ramp of a utility trailer and felt some discomfort in shoulder. IE was evaluated by an ortho where an MRI was ordered. The MRI was read on 9-28-2018 where a rotator cuff tear was diagnosed with corrective surgery for Oct 23.
A lighting strike started a small grass fire. The fire was extinguished by rain 35 minutes later. There was no danger to people or equipment
IE was removing a hydraulic hose. The IE was using an end wrench to remove the clamps due to the size of the area and when the last bolt broke loose IE's hand came in contact with the support bracket of the valve. IE was then taken to the hospital where an x-ray revealed a fracture to a 5th metacarpal bone on right hand. IE was wearing mechanics gloves.
The IE began to experience pain in shoulder after catching self with left hand on the handrail. The employee was seen by a doctor and eventually had a MRI that revealed a small tear in the labrum. The IE continued to work until 8-28-18 when the doctor took ee off work to focus on physical therapy.
IE was dismounting the ladder on a D10N. When the IE stepped to the ground, foot slipped causing IE to pull on the hand rail. IE felt a pop in shoulder. On 6.25.18 the IE was seen by an ORTHO and it was determined that IE pulled the biceps tendon off the shoulder. IE was given a work as tolerated release until surgery on 7.19.18 at which point it became an LTA.
While installing a pull down chain on a blast hole drill, the pry bar slipped pinching the IE's finger on the structure. The IE received a laceration requiring stiches.
A contractor vehicle caught fire while in a blasting pattern and was not extinguished within 30 minutes of discovery.
while preparing to unload diesel fuel i was pulling fuel hoses into place and fell in a pot hole causing me to fall and hit mouth on a piece pipe coming out of the tank trailer and broke one tooth in half. The area does not have lighting for after dark site.
Employee was stepping off the last step of a 3' ladder and stepped onto a large metal object rolling EE's foot slightly causing a fracture to EE's little toe.
Employee was power washing a piece of the mine owners equipment in an open air wash bay. The overspray was accumulating on the equipment and glazed the equipment almost like black ice and was very difficult to see. As the employee stepped down off the equipment employee stepped on the dozer tracks, slipped and fell approximately 1 foot on left elbow.
The victim was assisting another employee to reposition an idler trough frame to allow room to make a belt splice. While rotating the frame onto it's side, the frame unexpectedly tipped over onto the conveyor frame striking and catching the victim's hand between the idler and conveyor frames. The victim suffered a lacerated finger and a fractured finger.
IE was opening a small manual slide gate on a coal chute that is used to divert coal fines from the dribble chute to a belt line. When the IE pulled the gate, it came all the way out and landed on IE's right foot causing a laceration to little toe requiring stitches. The IE was wearing all proper PPE.

A single pass drill was drilling on top of coal when the drill hole began to smoke then later caught on fire and was not extinguished within 30 minutes.
Employee got a flat tire on pickup and called the mechanic to help change it. While pulling the jack off of the mechanics truck the jack slipped out of hand and fell to the ground. Then the jack struck the ground the handle on the jack bounced up and hit the employee in the cheek which caused a laceration which required stitches to close.
The IE twisted ankle while working on a haul truck. The individual felt pain in ankle but did not go in for medical treatment. The IE reported still having pain on the 14th at which point ee was scheduled with an orthopedic doctor. IE was seen on the 16th and an MRI was scheduled. On the 30th the employee's MRI was read and ee was diagnosed with neurapraxia.
Upon receipt of audiometric testing results 08/07/2017, the employee has incurred a 25db of hearing loss in left ear from the initial base line in 2010.
IE was working on a belt scraper when the wrench IE was using kicked back pinching IE's finger between the wrench and a piece of metal resulting in a laceration and fracture to the tip of IE's right ring finger. IE was seen at the emergency room where the decision was made to remove the tip of the finger.
On 8-1-17 IE was shoveling and felt a pop in shoulder. IE thought it was just sore and did not seek treatment. On 8-22-17 IE decided to seek treatment and an MRI was ordered for 8-28-17. The MRI reading on 9-5-17 reveled a rotator cuff tear requiring surgery to repair.
The IE felt pain in the lower back when they bent over to lift shovel trail cable. The IE went home at the end of shift assuming it was an aggravation to a previous back injury. On 07-13-17,the IE called wanting to see a doctor. Medication was prescribed and was referred to an ORTHO. On 7-14-17 the ORTHO evaluated the IE and ordered a MRI for 7-18-17 revealing a herniated disk.
IE hand placement was not adequate. 4th Fingertip RH, Laceration. Crush, Open Fracture, Subungual Hematoma.
IE was performing welding repairs to a shovel car body on Wednesday and went on days off. Employee began to feel discomfort on Thursday evening then woke Friday morning to a red irritated eye. Employee was accompanied to the optometrist where a small FOB and rust ring were removed from the left cornea.
IE was removing the rear hoist brake pressure plate on a P&H 4100 shovel to replace Orings. When air was applied the brake casting broke causing the pressure plate to strike the IE below the left knee. IE was referred to an Ortho for an MRI to determine if there was ligament damage. On 5-4-17 IE attended a follow up appointment & was issued two weeks restricted duty of no work.
The IE was removing mud from the transmission linkage area under a F-250 pickup when a second employee, attempting to test the motion inadvertently put it into reverse and the front left tire made contact with the IE's chest. The IE was diagnosed with simple thoracic spinous process fractures.
The IE was assisting on installing hoist ropes on a P&H 4100 shovel. The IE felt a pop in arm while making a simple lift to uncross the cables. The employee was diagnosed with a tendon strain and referred to an orthopedic doctor. The MRI determined bicep tendon rupture. The employee continued to work until 3-23-17 when surgery to repair the tendon was performed.
The IE was opening the access door to the pump drive area on a loader when EE lost grip on the door panel and it fell the rest of the way open, causing a laceration to middle finger on the left hand requiring 4 sutures.

Haul truck operator backed through the berm on a 25 foot dump. The operator was told to stay in the unit while the truck was secured. Bucket truck was used to retrieve the operator. Operator was restricted in the cab longer than 30 minutes.
The IE slipped and fell to the ground when stepping out of the vehicle. The IE experienced pain in the right knee and was treated at first aid, then transported to the hospital for further evaluation. The IE was referred to an orthopedic doctor. An MRI was ordered and was read on 12-19. The IE was diagnosed with a meniscus tear and will undergo orthoscopic surgery.
Employee was stripping the insulation from a 480V power cable when the safety knife slipped cutting employee on the left palm of the hand. Employee was wearing gloves at the time of the injury. Employee received three sutures to close the wound and issued a full return to work release.
IE felt a "pop" while entering the crew van at the end of shift. IE was examined at the ER, referred to an Orthopedic doctor, MRI was ordered which revealed a small meniscus tear and was prescribed physical therapy. The IE had surgery on the same knee for a meniscus tear in the past. Given this along with the ee denying any slip or trip the event will be classified as an illness
Contract employee received a small laceration that required three stitches on left hand.

EE was walking down a stairway when EE slipped and fell resulting in a deep bruise to EE's back. EE was not released to return to work due to pain management.
Employee was walking across a frozen rutted area when he tripped and fell on his right shoulder resulting in a simple fracture to the upper humerus bone.
Employee was placing a tow hook onto the back of a service truck when the tip of his left ring finger was pinched between the bed rail and the tow hook resulting in a laceration and fracture.
The injured employee received a laceration to the left arm after the box knife slipped when opening a mylar bag. The laceration required stitches.
Employee felt an irritation in his eye after taking his glasses off during lunch break. He was treated at an optometrist for a mild corneal abrasion and given a prescription for an antibiotic making this a reportable event.
While operating a motor grader, the moldboard caught a rock jarring the operator side to side. The IE felt a pop in between his shoulder blades. The IE was transported to the hospital for evaluation, diagnosed with a back strain, released to work as tolerated, and referred to an orthopedic doctor. The IE worked until 11-8 when the discomfort would not let him finish the shift.
