Employee tripped/got foot tangled in airhose and fell resulting in pain in lower back.

Operator was dismounting a D-10 dozer and when making the transition to get down off of the push arm, EE grabbed the lower handrail which had a metal shard that cut EE's left middle finger. EE was taken to medical care and received 6 stitches.

Steamers were identified in pit area and were addressed as soon as we got equipment running as it was -30 degrees.

A 793 haul truck was dumping and when pulled forward the front end raised and when it came down it jarred the driver.

While grinding on a haul truck bed a piece of metal got into employees eye. I.E. was taken to hospital where foreign particle was removed. Employee was wearing safety glasses and face shield.

IE was removing strut bolts on a haul truck. While dismounting the carrier, transitioning to a 6' ladder, EE fell approx. 44'. EE's right shoulder/head impacted the concrete pad. IE was conscious and talking and transported for medical care diagnosis was a skull fracture. IE was released on 9/6/23. At no time did medical professionals describe this injury to be life threatening

While removing a filter on a haul truck with a strap wrench, mechanics hand slipped off wrench and the handle hit EE in the mouth resulting in a chipped tooth.

End dump hit slick spot in road and was laid over on its side. Operator had a laceration on head that required 2 staples.

While walking an employee slipped and caught themselves on flat ground, which resulted in a tear to some muscles in rotator cuff. Employee had surgery on 1-13-23 making this a reportable.

While employee was pulling on an access ladder, EE strained shoulder. EE had surgery on 12/9/22 making this a reportable incident.

Employee fell while walking and strained shoulder. Employee had surgery on 10/13/22 making this a reportable injury. Found out about surgery on 11/17/2022.

During a rain/fog event an employee slid off the edge of a roadway in a pick-up truck. The employee remained in the cab until the vehicle was recovered. There was no injury or vehicle damage.

IE was performing fabrication duties. the IE hit a thumb with a hammer causing a laceration requiring stitches.

While moving crushing unit twisted back. EE entered restricted duty on November 4th 2020.

Spontaneous combustion smoker was found in Area 1. Had to take a drill over and shoot dirt over the top of it. The area was buried at 16:30 pm 0721/2020

While removing cutting edge on a 637 CAT scrapper, the rad gun IE was using, slipped off nut and contacted IE's mouth resulting in a broken #7 lateral incisor tooth.

At 1:30 pm MST, a spontaneous combustion of coal fire was reported in Area 6. Due to the location, it will take longer than 30 minutes to cover with dirt. Track dozer is actively working on road into affected area to mitigate smoking hazard.

IE was operating crew van, contacted berm with left side tires, the impact caused a compression fracture in the lower back

Spontaneous Combustion Fire (smoker) was found in Area 0 at 9:00 am. Called in at 9:16 am. The smoker will take more than 30 minutes to bury with dirt. D-11 actively working to bury smoker.

A smoker was found between the area 1 and two pit at 10:25 am. Due to remote location, it will take more than thirty minutes to bury.

Spontaneous combustion coal smoker in Area 0 at Caballo. Remediation time will exceed 30 min. Dozer working to cover it up with dirt.

I.e. was struck in the forehead by a lock ring while it was being installed on a 797 haul truck.

Smoker/coal fire in area 7 caused spontaneous combustion. No personnel or equipment were/are at risk of harm/damage.

Deceased Individual suffered a fatal event due to Hypercarbic Respiratory Failure, this cause of death is according to Coroner's report. Time of Death was ruled at the Hospital at 1554 hours, 8/25/2019.

While lowering radiator cowling down from pump, the weight of the cowling shifted causing IE to lose grip and right hand to get entangled in the radiator cap access hole that is cut into the panel. This motion and weight pulled down and twisted on the right arm resulting the bicep tendon to be slightly torn.

Spontaneous Combustion of coal in an inactive area of the pit that was going to take more than 30 minutes to put out. No damage or threat to people. Have to haul a dozer over and bury the smoker.

Minor amount of smoke was observed from spontaneous combustion of coal occurred in area 6.

IE stepped off step stool to the ground, stool slipped causing IE to fall to the ground injuring left shoulder. IE examined by the Coalition on 4/16/18 and referred to orthopedic specialist. MRI results were read 6/12/18 and indicated torn rotator cuff. Surgery was performed 6/18/18 to repair injured rotator cuff. IE worked normal schedule through 6/17/18.

Employee was loading EE's personal back pack into the back of our company pick-up truck the rock (ballast) slid and EE injured EE's right knee. The ballast was on a slight incline, and lose which caused EE to slip.

Injured employee (IE) was installing a track pin using a track pin press; a metal fragment dislodged from the pin and was impaled in IE's left lower leg. Metal fragment was removed by doctor.

Injured Employee was walking in from shop line to change house at the end of shift, slipped on frozen surface, resulted in torn medial patella femoral ligament.

EE was mounting a haul truck when they felt discomfort in their left shoulder. The EE reported the incident at the end of their shift. EE elected not to see a physician that evening. The following morning pain was still present. After three subsequent physician visits an MRI was ordered. Results were received 3/14/16, diagnosis torn rotator cuff.

Employee fell and received medical treatment. Employee slipped while descending a ladder. Employee's shoulder struck the ground. Treated for pain and contusion.

Grass fire located approximately 1 mile away from pit 1, no mine personnel, equipment or facilities were at risk at any time. Suspected cause of fire was fireworks ignited by someone outside mine property. Fire was extinguished approximately 50 minutes after it was detected. Personnel posted for fire watch for one hour after extinguished and spot checked throughout night shift.

Employee was walking on level ground around the fuel island when he slipped on some ice that was covered by fresh snow causing him to fall. The fall resulted in a simple spiral fracture to the 3rd metacarpal in his right hand.

The employee was jarred operating a haul truck and refused medical care. On 8/8/2014 the employee stated that she was having neck pain. The employee was seen by physicians on three occasions, receiving a release to return to work each time. The employee was diagnosed with degenerative disk disease and received a cortisone injection on 10/21/2014 making this a reportable illness.

Employee was attempting to attach a hot start hose to a dozer and experienced pain in his wrist. The employee was evaluated and received a full return to work release. A follow up visit was conducted and the employee received a release with restrictions for his next scheduled shift on 05/28/2014.

Employee was performing a walk around inspection and slipped on a slick surface. This injury resulted in a sprained ankle.

Technician was changing a swing cylinder on a backhoe. They screwed a bolt in the end of pin to protect the keeper threads from getting damaged while driving out the pin. While driving out the pin, the bolt sheared off and struck the technician above the right eye causing a puncture wound of about 3/8" wide.

The IE suffered a hairline fracture of his left fibula as he stepped over a piece of poly pipe on to rough ground during the pre-op inspection of a light duty vehicle.

Employee felt his left knee "pop" while stepping into pickup. Employee states there was no outside force of any kind to the leg. The doctor notes confirm the existence of "degenerative changes within the meniscus both medially and laterally" (see attached document). Therefore, this incident will be classified as an illness.

The employee pinched the end of the left index finger between a piece of plywood and the wall onto the plywood was being fastened.

Employee was air arcing on a Letourneau Loader bucket bores. Employee flipped his welding hood up and felt something hit his eye. Employee thought that the object was not in his eye and went home. The following night the employee's eye started to hurt and informed his supervisor. Employee was taken to Eye clinic where metal chips were removed.

Employee stumbled on a frozen chunk and dislocated the right thumb. The skin was also broken which required sutures.

The employee right hand was injected with hydraulic oil after he removed the pressure sensor from the steering the brake manifold. The employee was transported to the hospital and underwent surgery to remove the oil from the tissue.

EE felt pain in the right shoulder while holding a pressured hose in a bore hole. The EE continued to had discomfort over the next 7 weeks, had a MIR which revealed a tear. The EE worked and missed no time till surgery was performed on 5/30/12.

EE reported having neck & back pain for the prior 7 weeks. The EE has had neck & back pain in the past which got worse after being jarred when a chunk of dirt fell off the shovel bucket while being loaded on 12-11-2011. The Doctor ordered an MRI which revealed arthritis in the neck which was aggravated by operating equipment. Thereby classifying this as an illness.

The injured employee was riding in a pickup when the pickup bounced over a rock. The employee complained of back pain and was transported to the hospital where he was diagnosed with a T12 compression fracture. Seatbelts were in use.

Area was wet and somewhat muddy making vac hose slippery. Cutter/hose slipped and cut employee's finger

Blade operator backed into the water truck after pulling a rock out of the roadway. The operator could not return to his next scheduled shift after seeing an orthopedic doctor.

EE was helping pull and strip wire on unit 547. While stripping wire he slipped, cut thumb on left hand.

Employee pinched his finger while installing a shim pack into the front wheel of a haul truck lacerating his finger. The wound required five stitches to close the wound.

A shovel door latch bar fell over onto the employee's foot. Consulted an orthopedic doctor on 9-8-11 who confirmed a fracture of the second metatarsal on the right foot. The employee was given a restricted work release.

EE was operating scraper #249. He was traveling too fast for conditions and attempted to pass a slower moving scraper. In his attempt to pass the slower scraper, EE hit a hole causing his scraper to bounce hard three times. EE requested that his sup. take him to the E.R. with back pain. ER diag. indicate a compression fracture to the lower back. 1 injured this occurrence.

Employee twisted the left knee while lifting engine cover. The employee was checked out and released back to work. The knee continued to hurt, an MRI indicated a meniscus tear which was repaired 12-28-2010 making the incident reportable.

EE was in "buddy seat" while training cheek. While truck was in motion, ee reached down to stabilize himself, jamming his finger. While extrication his hand, ee cut his finger.

EE slipped and struck the side of his head with an electrical part he was carrying, causing a small laceration requiring 5 staples.

Employee felt something in his eye after a gust of wind came through the open shop door. The person was taken to the hospital where a metal flake was removed. He was prescribed eye drops and recieved a full release. Safety glsses were being worn at the time.

The EE stated his back started to tighten 2 or 3 days prior and steadily got worse to the point of seeing a doctor on 5/28/10. The EE was referred to an orthopedic doctor, who was the same one who treated his back in the past, who comfirmed this was a pre-existing issue of a tear in the L5/S1 disc. See attached release signed by the doctor.

A fire occurred on a Letourneau 2350 loader. The fire was not extinguished within 30 minutes and was called into MSHA. No injuries associated with the fire.

Employee was walking to the parking lot in an enclosed breezeway, when he felt pain in his right leg after stepping along side a co-worker. The leg began to swell and be painful. He was seen by a dr and diagnosed with a partial tear in the achilles tendon. There was no event to cause the tear and as such is classified as an occupational illness.

While dismounting a mobile crane, maintenance technician stepped down hard off of the boarding ladder and fractured his lower left leg.

While removing the driveline from a 24H blade, the driveline slipped and pinched the employee's finger between the shaft and the concrete floor. The incident resulted in a laceration and a tuft fracture.

The employee's foot slipped off a rung while climbing a ladder and slid/fell landing on an outstretched left hand. The fall resulted in a fractured left wrist and dislocated shoulder.

The lid on a metal container closed on the employees left thumb. The thumb was fractured and required sutures.

Employee was moving a piece of steel when it slipped and mashed his finger.

The employee was in a squatting position going to a standing position when his knee locked. The employee was not lifting anything at the time. He has a history of knee problems, and was diagnosed in 2006 with a partial ACL and meniscus tear, neither was repaired. The orthopedic doctor agreed this is more of a chronic illness than an acute injury.

Acetic Acid line was being taken apart, when the coupler came loose, some residual acid sprayed the employee in the face. The line was under pressure unbeknownst to the employee.

While performing maintenance on service truck, EE had an aersol spray splash back over the top of his safety glasses into his left eye. Dr said he had suffered a mild chemical burn and prescribed him medicine.

The employee was helping hold the drill steel in line with the guide roller on a DMM2 blasthole drill. As the carousel moved into position, the steel kicked to the side striking the left gloved hand resulting in a simple fracture of the ring and middle finger. The was seen at the hospital, the fingers were splinted and the employee returned to work.

Employee was using a small grinder to dress mushroomed metal on an R.R. machine. The grinding wheel separated and a shard from the wheel struck the employee in the neck, puncturing the skin.

Employee sprained ankle while walking from Port-o-potty to truck. Walking on even dry surface - road base.

Employee was cutting a plastic wire tie with a utility knife. The blade struck the left hand after cutting the tie. Four sutures closed the laceration and the employee returned to work, full duty.

Wash down in crushers, 2" water hose hit him in the knee after the fitting came off the hose. At time of initial exam, employee was released to return to work. In a follow-up appointment it was determined there was a tear to the ligaments in his knee. Surgery was performed on 7/15/2008. The employee worked up until the time of surgery.

The operator of the DMM2 blasthole drill finished drilling the drill pattern around midnight, shut off the machine and left the area around 12:15am. The dragline oiler noticed the fire around 12:50am and called a MayDay. The fire was under control about 45 minutes after the call. Maintenance supervisor notified MSHA around 1:30am.

Right rear wheel mounting bolts sheared off while approaching the dump area at a low speed. The right wheel motor and tires came off the haul truck.The employee declined a medical examination, stated she was not injured, continued to work remainder of shift and rotation (1/11-12-13).On 1/15 the employee called to say she was seeing a doctor due to neck and back strain.

Mechanic was working in an elevated position on the rear of a haultruck. He turned to come down the ladder and felt pain in his lower back.

While walking down an incline on a concrete floor the employee's foot slipped and pain developed in the right knee. He reported the incident and continued to work but the discomfort did not go away and he saw a doctor. A MRI showed a meniscus cyst on the outside of the right knee and orthoscopic surgery is scheduled. Post surgery will determine any restricted or lost work days.

The employee was in the process of positioning the center cutting edge on a 637 Cat scraper when the cutting edge caught under the can. The employee pulled back on the nylon sling and the welded on washer broke loose, swung up, striking the individual above the left eye. Sutures were required to close the laceration.

Aligning transverse shaft bearing to bore, employee was holding 1" plate to outer race and using a sledge hammer to straighten bearing. When employee adjusted plate, the other employee was in mid-swing and hit finger of first employee.

The ee was removing the rear brake caliber on BD270 coal truck when the caliber slipped and pinched. The left ring finger against the wheel rim. X-ray revieled a fracture on the finger tip. The mechanic was wearing gloves at the time of the incident.

The accident/illness was reported on 01/02/08. The accident/illness actually occurred on 11/20/07. Surgery to remove a blood clot was done on 12/14/07.

Operator was trying to adjust the driver's side window, when it came down suddenly, causing a hairline fracture to his left little finger and a laceration requiring sutures to close the wound.

EE was removing a rock from the Jaw Crusher, when a piece of rock broke off, hitting EE on the chin. EE was taken to Champbell County Hospital and received stitches. The rock was being removed with a chain Front-end Loader.

The brakes grabbed while crossing the crusher with a 270 ton tractor-trailer haul truck which "jarred" the operator causing neck/shoulder pain. The employee stated he was bounced up and his right shoulder/neck hit the roof of the cab. He was seen at the hospital and later by an orthopedic doctor who diagnosed a shoulder strain. Employee states he was wearing a seatbelt.

While dismounting from the water truck the employee experienced a sharp burning pain in the left ankle. The employee states her ankle was not twisted or rolled and that she has had ankle surgery in the past. Upon seeing an orthopedic doctor, the doctor agreed this is a chronic or a relapse of the prior injury.

The ee was removing a broken left lower suspension bearing when a fractured piece of the bearing broke loose hitting ht ehaul truck tire, bounced back, striking the ee on the left hand. the ee received a laceration to the left ring and middle finger which required sutures to close.

The brakes grabbed while crossing the crusher with a 270 ton tractor-trailer haul truck, which "jarred" the operator, causing neck pain. The orthopedic doctor diagnosed a neck strain and restricted duty.

EE was dropping hoist ropes at side of shovel with cable tugger. The cable hung up, pulled on tugger line to release cable, the weight & tension of tugger rope caused the tugger rope to pinch his hand between rope & tugger.

The employee had just finished replacing toilet supplies. The employee stumbled when turning to leave and extended the right hand to catch himself, jamming three fingers. The doctor diagnosed right hand metacarpal dislocation.

While walking down the belt line the employee struck the right knee on a hydraulic reservoir for the return idler belt trainer. He was first diagnosed with a bruise-strain MRI conducted 3/28 revealed a menigus tear.Surger is scheduled on April 19th (out patient). He return to work the next schelued.

Operator attached a wiggins fitting to fuel the equipment. When the nozzle was turned on, the fitting came off, spraying fuel for split-second. Some fuel was sprayed on operator with some getting in the eyes. Eyes were flushed & the EE returned to work. The next morning the EE went to see the doctor where anti-biotic medication was prescribed. Safety glasses were worn.

Employee slipped on some dust covered ice while walking around the blast hole drill. The right leg buckled underneath with the rest of the body weight on top. The employee received a simple fracture of the fibula bone in the lower leg. The employee was released on restricted duty.

At the time of the incident employee was removing the left muffler from a 690 Tiger. As he was holding the muffler with his right hand and pressing on a prybar with his left hand the muffler suddenly broke free from the turbocharger coupler, causing his hand to be pinched between the muffler and frame and lacerating his finger.

Welder/mechanic climbed ladder to assist mine mechanic in pulling the padlock into place to be pinned. He was wearing a full body harness with landyard, but failed to tie off. He slipped and fell off the dipper.

At the time of the incident, the employee was loosening flange bolts on a transmission line. As he was pulling on the wrench, the bolt suddenly broke free, causing his elbow to swing back and strike the torque converter fill tube cap.

Tripped on uneven floor grate & fell on knee.

The employee was backing up a 270 ton, 5th wheel type coal truck. When the person applied the brakes, the tractor "bucked" bouncing her in the cab. See attached letter of explanation for delay in reporting this illness.

While driving a wooden lath into the ground, the lath splintered inti his hand causing a cut between the thumb and index finger which required stitches.

Wildland grass fire caused by a spark of a train burned about 90 acres. The fire was contained around 7:00 pm. Local fire department assisted. All units were released at 10:20 pm (mop-up completed). MSHA was called - message was left - was never called back.

The employee notified us on 3/7/06 concerning his hands breaking out in a rash over the recent months. Unkown the date the rash started. The employee is seeing a doctor and is receiving treatment for the dermatitis. The employee has worked as a mechanic for 15 to 20 years.

WHILE DISMOUNTING FROM A 24-H BLADE, THE EMPLOYEE LOST HIS BALANCE AND LOST THE RIGHT HAND GRIP ON THE GRAB BAR. HE FELL APPROX. 7 FEET TO THE GROUND, WITH THE RIGHT LEG TAKING THE MAJORITY OF THE IMPACT. DIAGNOSED WITH KNEE SPRAIN WITH POSSIBLE ACL (ANTERIOR CRUCIATE LIGAMENT) TEAR. RELEASED TO WORK WITH RESTRICTIVE DUTY.

The technician pushed spillage into the track area after loading a train. As she walked back across the silo floor, she stepped onto a piece of coal and rolled her ankle. She was diagnosed with an ankle sprain. Her ankle was immobilized and she was limited to light duty.

WHILE THE HAUL TRUCK DRIVER WAS BEING LOADED WITH COAL, DUST BLEW INTO THE DRIVER'S ROLLED DOWN WINDOW & INTO THE TECHNICIAN'S LEFT EYE CAUSING A CORNEAL ABRASION.

The ee felt a sharp pain while pulling on a chain wrench taking apart a steering cylinder con a haul truck pulled muscle under right shoulder blade.

WHILE INSTALLING A PIECE OF PLEXIGLASS ON A SHOVEL CROWD MOTOR WINDOW, EE (AN ELECTRICIAN) FELT SOMETHING ENTER HIS EYE. A PHYSICIAN FOUND A CORNEAL ABRASION AND REMOVED A FOREIGN BODY FROM EE'S LEFT EYE.

EE HIT HIS FINGER WITH SLEDGE HAMMER WHILE DRIVING CONCRETE STAKES.

WHILE HOISTING THE RIGGING OUT OF THE SHOVEL HOUSE, A CLEVIS ATTACHED TO THE END OF THE SLING CAUGHT ON A BEAM ENOUGH TO "WHIP" THE SLING AND THE CLEVIS STRUCK EE ON THE LEFT EYE OF THE SAFETY GLASSES. THE IMPACT CAUSED THE GLASSES TO CUT THE SKIN ABOVE THE EYE. 4 STITCHES WERE REQUIRED.

EMPLOYEE WAS LOADING HIS TRUCK BACK WITH PRODUCTION FUEL. HE WAS TRYING TO UNHOOK FUEL DISCONNECT NOZZLE FROM UNIT FUEL TANK AND HE FELT A POP AND A SHARP PAIN IN HIS RIGHT ELBOW

Went to pull chocks from tires on crew van, left foot slipped, left knee popped then gave out and then right knee gave out.

While removing duels from a Komatsu haul truck employee got his right ring finger caught between hub and Rad gun, causing an open tuft fracture requiring 9 stitches. Employee was wearing gloves due to the cold temperature while working outside, therefore he did not notice his finger being in wrong place between the hub and Rad gun.

WHILE BEING LOADED UNDER THE SHOVEL A LARGE CHUNK DROPPED JARRING HAUL TRK. 20 MINS LATER EE REPORTED THAT HIS BACK MUSCLES WERE GETTING TIGHT. HE SAID HE HAD BEEN IN SEVERAL AUTO ACCIDENTS HIS BACK WAS BROKEN 2-3 YRS AGO & REINJURED INA HEAD ON 5/04. DR DIAGNOSED MUSCLE STRAIN & RX'D PHYSICAL THERAPY. NO VISIBLE DIFFICULTY CLIMBING DOWN LADDER OR WALKING

At the time of the incident, employee was washing a CAT 797 off-highway truck. To reach a certain area on the underside of the truck he knelt down such that he was nearly sitting on his right heel with his left knee bent at a 90 degree angle. As he went to get up, he felt his left knee "lock up".

WHILE OPERATING AN AIR-CONDITIONED BLADE, THE TECHNICIAN BEGAN TO FEEL NAUSEOUS AND DIZZY. THE TECHINCIAN WAS DIAGNOSED WITH HEAT EXHAUSTION.

Got foreign body in eye while cleaning equipment. Later developed conjunctivitis.

WHILE OPERATING A SCRAPER, THE RIGHT REAR BOUNCED OVER A CHUNK OF MATERIAL WHICH CAUSED A MUSCLE SPASM IN EE'S LOWER BACK. HE WAS SEEN AT THE ER & DIAGNOSED WITH A LOWER BACK STRAIN. HIS CONDITION DID NOT IMPROVE SO HE VISITED A SPECIALIST 7/19/04. THE SPECIALIST DIAGNOSED HIM WITH A L4 HNP (HERNIATED NUCLEUS PULPOSUS) & PRESCRIBED THERAPY, ANTI-INFLAMATORY & MUSCLE RELAXERS.

An electrician was stripping the insulation off a power cable with a utility knife. The knife slipped off the cable striking and lacerating the top of his left hand. He received 5 sutures.

While torqueing the left rear wheels on a Cat 797 the rad gun slipped off the lug nut striking employee on the right side of the face. The impact caused a fracture of the lower right cheek bone.

BUMPED ELBOW ON ARM REST WHEN HAUL TRUCK HIT BUMP.

WHILE WALKING BEHIND A HAND SHOVEL PUSHING SOME COAL ACROSS A FINISHED CONCRETE FLOOR THE EMPLOYEE'S LEFT LEG "GAVE OUT" WHILE TAKING A STEP. THE DOCTOR DIAGNOSIS WAS A QUADRICEPS TENDON DETACHMENT FROM THE BONE FROM A DEGENERATIVE PROCESS OVER A LONG PERIOD OF TIME.

AT THE TIME OF THE INCIDENT, EE WAS CHANGING HEADS ON THE ENGINE OF A CAT 793C OFF-HIGHWAY TRUCK. AFTER UNBOLTING THE HEAD FROM THE ENGINE, HE PLACED THE INPACT WRENCH ON THE FRAME OF THE TRUC K. AS HE AND A CO-WORKER WERE LOADING THE HEAD ON A CART, THE IMPACT WRENCH FELL OFF THE TRUCK FRAME AND STRUCK EE ON HIS LEFT FINGER.

WHILE HELPING POSITION A HANDRAIL (FOR INSTALLATION) THE PIPE SLIPPED OFF THE POSTS AND THE END OF THE PIPE CUT THE LEFT THUMB. THE EMPLOYEE RECEIVED FIVE STITCHES.

AFTER WELDING ON A REPLACEMENT LINER, THROUGHOUT THE ENTIRE SHIFT THE EE STATED HIS LEFT EYE WASSTARTING TO BOTHER HIM. THE FOLLOWING DAY THE EESAW THE EYE DOCTOR AND HAD FLASH BURN TO THE LEF T EYE. THERE WAS NOT AN INSTANTANEOUE EXPOSURE.

While removing cutting edges off a 24H blade, a 4' section of cutting edge fell, striking the ee's left hand, fracturing the left index finger and thumb. The thumb also received stitches.

EMPLOYEE WAS OPERATING 40-4460 HAUL TRUCK. AS HE WAS PULLING A RAMP LOADED THE TRUCK POWERED OUT AND STALLED. THE EMPLOYEE PANICKED AND JUMPED OUT OF THE TRUCK BRUISING HIS HEEL.

WHEN AN ICE BLOCKAGE DISLODGED FROM A PIECE OF PIPE, THE PRESSURE KICKED THE PIPE BACKWARDS, SPRAYING ICE CHUNKS, ONE OF WHICH IS BELIEVED TO HAVE STRUCK THE EE'S LEFT LOWER LEG, FRACTURING THE FIBULA BONE.

WHILE OPERATING A BD270 TON COAL TRUCK EE WAS MAKING THE CIRCLE TO POSITION UNDER THE SHOVEL AND CROSSED OVER A BLADE WINDOW, JARRING THE OPERATOR WHICH CAUSED THE BACK PAIN. DR PRESCRIBED RES TRICTED DUTY FOR SHORT TIME, ALONG WITH SOME PHYSCIAL THERAPY.

EMPLOYEE WAS REMOVING THE LEFT REAR OUTSIDE TIRE OF UNIT 622 TO BE ABLE TO WORK ON THE INSIDE DUEL TIRE. WHEN REMOVING THE LUG NUTS HE WAS MOVING THE RAD TOOL BETWEEN THE LUG NUTS USING HIS LE FT HAND TO GUIDE THE TOOL ONTO THE NEXT NUT HE ACCIDENTALLY HIT THE TRIGGER PINCHING HIS LEFT INDEX FINGER & HAND BETWEEN THE TOOL & THE WHEEL. THE TOOL CAUSED A LACERATION OF THE LEFT FINGER

EE WAS WALKING ON THE GROUND, WHICH TURNED OUT TO BE A CRUST OVER THE TOP OF MUD, WHEN THE RIGHT LEG BROKE THROUGH THE CRUST AND SANK ABOUT 2', RESULTING IN A PARTIAL TEAR IN THE RT.CALF MUSCL E. DR. RELEASED HIM ON RESTRICTED DUTY.

TECHNICIAN BENT OVER TO MARK A PIECE OF METAL WITH A MARKER AND FELT SHARP PAIN IN HIS LOWER BACK, HE RETURNED TO WORK WITH RESTRICTIONS.

WHILE CHANGING OUT HOIST ROPES ON SHOVEL, IN WET/MUDDY CONDITIONS, TECHNICIAN SLIPPED. HIS RIGHT LEG SLIPPED IN MUD, CAUSING HIS LOWER BACK TO TIGHTED UP. HE IS RELEASED ON RESTRICTED DUTY.

EMPLOYEE WAS CLIMBING DOWN FROM THE TOP OF HIS TRUCK. HE SLIPPED OFF THE BUMPER AND HURT HIS KNEE.

AT THE TIME OF THE INCIDENT, EE WAS INSTALLING A WASTE GATE UPDATE ON THE AIR TANK OF A CAT 793 OFF-HIGHWAY TRUCK. TO RECONNECT THE AIR LINES ONT HE BOTTOM OF THE AIR TANK, EE HAD TO LEAN OVER THE FIRE SUPRESSION SYSTEM BOTTLES AND REACH DOWN BETWEEN THE BOTTLES AND THE AIR TANK. AS HE DID THIS HIS FEET SLIPPED OUT FROM UNDER HIM, CAUSING HIM TO FALL AND TWIST BACK.

THE EMPLOYEE WAS STANDING ON A LADDER INSPECTING THE SWING SHAFT ON A P & H SHOVEL, SOME OIL OFF OF THE BOOM BEGAN DRIPPING ON HIM AND THE EMPLOYEE JUMPED OFF OF THE LADDER AND FRACTURED A BON E IN HIS LEFT FOOT WHEN HE CONTACTED THE GROUND.

WHILE WASHING SCRAPER, MECHANIC SLIPPED AND FELLCUTTING HIS LEFT ELBOW AND HURTING HIS LEFT HIP.

OPERATOR WAS DRIVING A BD-270 HAUL TRUCK; HE HIT A CHUCK HOLE ON BOTTOM OF COAL PIT; TRUCK JARRED THE OPERATOR INJURING HIS BACK.

EE WAS OPERATING A D-11 DOZER WHEN A PIECE OF DIRT GOT INTO HIS EYE; HE TRIED TO FLUSH IT OUT WITH VISINE FOR TWO HOURS BEFORE REPORTING TO HIS SUPERVISOR. AFTER FLUSHING EYE AT THE FIRST AID ROOM, IT WAS DECIDED TO TAKE HIM TO THE E.R. TO GET IT CHECKED OUT.

TECHNICIAN WAS WORKING ON AIR LOCK IN PREP PLANT PULLING ON A SPROCKET, SLIPPED WITH PLIERS AND CUT HIS LITTLE RIGHT HAND FINGER ON GUARD. FINGER REQUIRED STITCHES.

EE SAID HE WAS DRIVING ONTO DUMP AREA & HAD HIS ARM RESTING ON WINDOW SEAL (LEFT), HE HIT A SMALL BUMP IN ROADWAY CAUSING HIS ARM TO BOUNCE UP. WHEN HIS ARM CAME BACK DOWN HE HIT HIS ELBOW ON SIDE OF DOOR FRAME.

EE DROVE HAUL TRUCK OFF ROAD & INTO DITCH; WHEN HE WENT TO PULL TRUCK BACK ONTO ROAD HE RAN OVER SIGN STAND MADE OF TIRE FILLED WITH CONCRETE; JARRING FROM TRUCK RUNNING OVER TIRE THREW HIM AG AINST STEERING WHEEL, EVEN THOUGH HE WAS WEARINGSAFETY BELT.

TECHNICIAN WAS ATTEMPTING TO MOVE GREASE BARREL,AND HE CAUGHT 2 FINGERS OF HIS LT. HAND ON SHARPEDGE OF DRUM.

WHILE SWINGING A WOODEN SLEDGE HAMMER TO BREAK SUB LOOSE FROM KELLY, HAMMER HANDLE BROKE CAUSING SHARP VIBRATION TO HIS RIGHT SHOULDER.

EE WAS INSTALLING LIFTING BRACKET ONTO MAST OF OVERBURDEN DRILL. BRACKET (150 LB) WAS MADE OF 3" COLD ROLLED STEEL; CRANE WAS HOLDING ONE END OF BRACKET; WHILE POSITIONING OTHER END, EE'S HAND WAS CAUGHT BETWEEN BRACKET & MAST, INJURING HISFINGER.

TECHNICIAN WAS HELPING MECHANICS WITH CHANGE OUT OF SHOVEL BUCKET TEETH, WHILE HOLDING PIN DRIVER IN PLACE, HE WAS STRUCK BY A VERY SMALL PIECE OF METAL WHICH CHIPPED OFF PIN DRIVER WHEN IT WAS STRUCK BY SLEDGE HAMMER, PIECE OF METAL ENTERED RIGHT WRIST, PENETRATED ARTERY, AND LODGED AGAINST BONE IN LOWER ARM.

AT THE TIME OF THE INCIDENT, EE WAS INSTALLING CYLINDER PACKS IN THE ENGINE OF A OFF-HWY.TRUCK.AS EE WAS ASCENDING A LADDER TO GAIN ACCESS TO THE ENGINE COMPARTMENT, THE LADDER MOVED, CAUSIN G HIM TO LOOSE HIS BALANCE AND STRIKE HIS RT. ELBOW ON THE FRAME OF THE TRUCK.

SHOVEL BUCKET OPENED EARLY, HITTING SIDE OF TRUCK & THEN DROPPING LOAD ON END OF TRUCK CAUSING THE BED TO LIFT UP AND THEN SLAM BACK DOWN. TRUCK DRIVER BUMPED AGAINST THE INSIDE OF THE TRUCK I NJURING HIS SHOULDER.

EMPLOYEE WAS REPAIRING A LEAKING FITTING ON THE STEERING MODULE OF #634 HAUL TRUCK WHEN HE ATTMEPTED TO TIGHTEN IT UNDER PRESSURE, THE THREADS STRIPPED, FORCING THE WRENCH BACK AND STRIKING HIM IN THE FACE.

HIT HER HEAD ON THE CEILING ON THE HAUL TRUCK.

STEPPED OUT OF TRUCK AND NEW TOWER SITE ON MINE PROPERTY. STEPPED DOWN ON AN OVERSIZED PIECE OF SCORIA.

TAPPING OUT A 1/4" HOLE. THE TAP BROKE AND EE ARM STRUCK BROKEN TAP AND CUT HIS ARM.

CUT LEFT THUMB WHILE STRIPPING CABLE. REQUIRED STITCHES.

HE GOT HIS LEFT THUMB CAUGHT WHILE CHANGING SHIMS IN SHOVEL BUCKET.

WORKING OUT AT THE MINE AND WENT TO STWP ONTO ONE OF THE SHOVELS AND HIS FOOT SLIPPED AND TWISTED HIS RIGHT ANKLE. THERE WAS SNOW ON THE GROUND AND ICE ON HIS BOOT.

USING HYDRAULIC BEAD BREAKER TO DISMOUNT TIRE ON HAUL TRUCK. RT HAND PLACED BEHIND TOOL. TOOL DISLODGED UNDER PRESSURE CAUSING RIGHT HAND TO BE SMASHED ON INDEX FINGER CAUSING SMALL CUT AND PO SSIBLE FRACTURE OF INDEX FINGER.

TRUCK 3200 PARKED AT TIRE PAD FOR REMOVAL OF RRI. AS HE BEGAN TO REMOVE WHEEL NUTS WITH TORQUE WRENCH, WRENCH BINDED AND SLIPPED CAUSING HANDLE TO PIVOT AND HIT HIM IN THE HEAD.

HE WAS DRIVING A TRACTOR-TRAILER BEWLLY-DUMP COAL TRUCK DOWN A 6% RAMP. TRUCK HIT A SLICK SPOT, AND TRAILER BEGAN TO SKID. WHEN OPERATOR TRIED TO CORRECT, THE TRUCK HIT A BERM, AND THE DRIVER' S HEAD HIT THE WINDOW. HIS GLASSES CUT HIS CHEEK--THE CUT REQUIRED 2 STITCHES. THE OPERATOR WAS WEARING HIS SEAT BELT.

USING HYTORQUE WRENCH TO REMOVE BOLTS FROM WHEEL. BOLT CAME LOOSE AND PINNED FINGER AGAINST WHEEL.

LIGHTNING STARTED A GRASS FIRE ON MINE PROPERTY AWAY FROM ANY BUILDINGS OR PITS. THE FIRE BURNEDAPPROX. 1.5 ACRES OF GRASSLAND. THE FIRE WAS UNDER CONTRO WITHIN 2 HOURS. THE MINE IS INACTIVE THEREFORE THE COUNTY FIRE DEPT. DID NOT RESPOND.

EMPLOYEE AGGRAVATED AN ONGOING BACK CONDITION WHILE OPERATING A HAUL TRUCK.

AT THE TIME OF THE INCIDENT EMPLOYEE WAS CHANGING OUT HOSES ASSOCIATED WITH THE ENGINE OF A CATERPILLAR 793 OFF HIGHWAY TRUCK. AS HE WAS CLIMBING UP THE ENGINE COMPARTMENT OF THE TRUCK HE FEL T HIS KNEE CAP POP OUT OF PLACE.

EMPLOYEE SPRAINED ANKLE ON 4/27/00; CONTINUED TOWORK UNTIL 5/27/00. AT THAT TIME, PAIN AND SWELLING CAUSED HIM TO MISS WORK.

EE WAS MOUNTING THE REAR ENGINE OF A CAT SCRAPER. HIS RIGHT LEG SLIPPED CAUSING HIS RIGHT KNEE TO TWIST.

WHILE BACKING INTO SHOVEL, HAUL TRUCK HIT BUCKET BRINGING TRUCK TO A SUDDEN STOP CAUSING WHIPLASH TYPE ACTION RESULTING IN BACK PAIN.

STEPPED IN DEPRESSION WHEN STEPPING FROM BLACK TOP TO GRAVEL ON THE WAY TO EE'S BUS. HE FELL AND CRACKED THE END OF HIS TIBIA AT THE ANKLE.

EE WAS CLIMBING AROUND THE BACK OF A HAUL TRUCK, STEPPED DOWN OFF A BATTERY BOX, ABOUT 18", ONTO A HINGE ON THE ENGINE COMPARTMENT, TWISTING HIS ANKLE.

EMPLOYEE REPORTED AN IRRITATION IN HIS EYE. HE WAS TAKEN TO HOSPITAL EMERGENCY ROOM, WHERE A SMALL SLIVER OF METAL WAS REMOVED, ALONG WITH A RUST RING. PRESCRIPTION MEDICATION WAS GIVEN TO PRE VENT INFECTION. NO SPECIFIC EVENT OR CAUSE WAS ATTRIBUTED TO THE INJURY.

EE WAS MOVING TRAILING CABLE AFTER A BLAST. A CLOUD OF DUST CAME UP. EE RUBBED HIS EYE. EYE BOTHERED HIM THAT DAY AND HE WENT TO DR. THE NEXT MORNING. DR. REMOVED A SMALL PIECE OF METAL FROM T HE CORNEA AND EMPLOYEE RECEIVED SOME PRESCRIPTION MEDICATION.
