IE was in process of building a stopping and tripped over a pallet of blocks and twisted back.

Employee hand was contacted by a ladder as it was raised. Employee was treated at the hospital for a finger injury requiring medical treatment to reset a dislocated joint and closure of the wound.

Employee did not notify the company of the injury until 2 days later. EE was using a 4 ft ladder and was 2 or 3 rungs up the ladder. EE leaned on the tire and the ladder kicked out. EE thinks it happened around 12:30-1 pm on 7-18-26. Dr. Diagnosed it as "Left radial head fracture" Has a follow up appointment in 2 weeks on Aug 4th. restriction no lifting over 2 lbs.

Employee was cutting rubber belting with a self retracting box cutter type knife and cut EE's left pointer finger causing a laceration that required 1 stitch to close.

IE was pushing rock into tail of miner. When rock started going into tail the rock cantilevered and pinched IE's right middle finger tip between rock and roof. IE received a fracture on tip of finger.

Employee was dismounting a track dozer and felt pain in the left middle finger. After and X-ray at the ER, it was determined that the employee had a fracture.

IE was walking in battery barn when IE felt pain in their back. IE sat down to try and eat lunch but the pain intensified. IE was transported to Hospital by a company official.

Employee was re-sealing a swing gearbox cover on a dragline and pinched left ring finger causing a laceration and avulsion that required stitches.

The employee was line boring with an electromagnetic drill. EE's glove got snagged on the rotating parts, and EE's right arm became entangled. EE had a broken arm and 2 lacerations resulting in 6 stitches each.

Employee began to feel pain in EE's lower back after hitting a rough spot while operating a track dozer. Upon an ORTHO visit on 06-10-2026 EE was given a restriction for no work. There was no treatment or PT prescribed.

Several contract employees were reclaiming discharge pipe that was hung against mine roof. While separating the pipe sections, the pipe began to fall in a chain reaction. Discharge pipe fell from the mine roof, springing upwards, striking employee in the face

Employee was loading a new excavator bucket tooth into the back of EE's service pickup and pinched EE's pinky finger causing a small avulsion that required 3 sutures.

Employee was moving an oxygen bottle from working skid to the storage skid when another bottle moved pinching left index finger between the two bottles causing a fracture of tip of the left index finger.

The driller shooter was carrying boxes into the blasting magazine. EE bumped EE's hard hat on the door frame, and when EE went to grab it, EE slipped on the magazine floor (raining and muddy). When EE's weight re-distributed to EE's right leg, EE felt it pop at the knee, which was a ruptured quadricep tendon and right thigh muscle. The knee was professionally immobilized.

Employee was in the process of removing a hydraulic hand pump and jack from the top of the tub base and hung EE's foot on a hose and fell forward and EE's thumb on the left hand was caught between pump and ram which caused a fracture of the first joint of the left thumb.

Years of pushing, pulling and lifting. No specific incident.

IE was working on raw coal screen doing some lifting and twisting motions and started feeling pain in shoulders and back. IE went to Sparta Hospital and is off work till released by EE's doctor.

Employee was using a pry bar to unplug a clogged bit on a blast hole drill. Once the clogged bit was free of material, the employee dropped the pry bar onto EE's foot which contacted EE's foot in between the safety toe and the met guard of the boot causing a superficial fracture to EE's big toe.

Technician pinched EE's hand while using an air impact gun causing a laceration that required stitches.

The injured employee was assisting with dragging a scrap truck frame when EE reached under the front bumper area to guide the Dyneema rope. A sharp metal shard protruding from the underside of the bumper contacted EE's right forearm.

The employee was using a grinder when it kicked back into EE's hand, causing a laceration. EE received 6 sutures.

Employee was chipping tiles out of a TEMA in the prep plant. Employee was wearing safety glasses and a full-face shield. Employee felt something in the right eye and went a mirror to find a foreign debris in the right eye. Employee asked for assistance from another employee to flush eye. The employee was taken to a local optometrist and prescribed antibiotics

IE was bitten by some sort of spider/insect, and EE's right leg became infected. IE reported it on April 24th and did not miss work until 04/29/2026.

A train started a grass fire on the outskirts of the mine and was not extinguished within 30 minutes of discovery.

Employee pinched a finger while changing a return idler on the belt line causing a laceration requiring stitches.

Employee experienced a medical emergency while at the mine site. The employee then fell to the floor from a leaning position. The employee was transported via county EMS for further care.

IE experienced left hip and back pain after loading drill on hauler. After loading drill, fell from desk of heavy haul trailer. IE had follow up visit on 4-22-26 and was diagnosed with a possible pinched nerve and was prescribed physical therapy and a steroid. IE was never restricted to do EE's job.

IE tripped and fell on shoulder, after Orthopedic appointment on 4-21-26 was diagnosed with rotator cuff injury that will require surgery

Employee complains of shoulder pain. No recent event contributing to the symptoms.

Employee twisted while changing hydraulic filters and began to feel a pain in their knee. later they asked to be checked out and was seen at the ER. An ORTHO appointment on 03/26/2026 revealed a torn ACL and they were given a restriction.

IE was working on a skid steer's hydraulic system and when they stepped backwards, they tripped over a bucket they placed by them. IE hit their neck area on the metal plate of the skid steer directly behind them.

Employee began to experience pain in EE's shoulder after dismounting a haul truck. EE was seen by an ER doctor and referred to an ORTHO. An MRI reading on 03-11-2026 revealed significant chronic arthritis and was restricted for no work.

IE was stepping over belt structure and when employee went to step over structure lost footing and felt a pop in right ankle. IE was treated conservatively and didn't miss any scheduled shift until 03/31/2026 when it became an LTI.

Employee cut finger while grinding resulting in a laceration that required stitches.

Upon receiving an audiometric test result, the administrating assistant has reportable hearing loss.

Employee forgot water bottle in the cab and when they reached back into the cab, they closed a finger in the cab door causing a laceration. Employee apparently was given a splint and restriction.

IE claims of severe cramping, weakness, loss of grip and numbness in hands and elbows.

The mobile equipment operator began experiencing nighttime shoulder pain, attributed to years of repetitive work and movements required to mount, dismount, and operate equipment. After consultation, diagnostics revealed degenerative changes in the shoulder, which required surgery on 02/06/26

There is no specific incident. Repetitive use of hands, wrist and elbows.

IE was walking across the supply yard toward the warehouse storage building. While walking across the snow covered ground, the IE turned EE's body to speak with another employee located at the fuel pad. During this time, the IE stated EE took a step and slipped, falling to the ground on EE's left side, fracturing EE's left hip.

Smoker was identified in area 6. No people or resources are at risk.

IE was changing a Lee Tram tire in the underground shop. IE was balancing tire in order to get the skid steer underneath the tire, and the operator bumped the tire causing it to fall on IE's right leg causing a hematoma and fractured fibula.

Employee felt back pain after hitting bump in a light duty vehicle. After emergency room visit employee was given restrictions and missed the next shift.

Employee was carrying a box of detonators while distributing them on blast pattern when they stepped into a low spot in the ground, employee felt discomfort in their abdomen. Employee was diagnosed with a hernia on 1-29-2026 making this a reportable incident.

Employee was exposed to blasting emissions from a neighboring mine. They sought medical evaluation at the hospital, was released with no restrictions but missed their next scheduled shift.

Employee stated that they pinched a finger while changing a brake caliper. After returning to work after 3 days off, the employee reported the event to their supervisor via phone call and missed their next scheduled shift.

Employee twisted knee while dismounting a mobile crane. Employee was seen at an ORTHO on 01/07/2026 and was where an MRI revealed a torn ACL that will need surgery.

Employee was cleaning dirt, oil, and grease from a retainer cap. After removing all the debris, they ran their hand over the cap's surface. As the employee placed their hand on the cap, a small metal burr protruded from a bolt hole. The bur poked the employee in the hand, and they jerked it back, causing a laceration to their palm requiring three sutures to close the wound.

Technician pinched finger while handling metal object causing a fracture to the end of right index finger.
