Miner was dislodging accumulated material in a log washer with a ½" steel rod measuring approximately 8ft in length, which was inserted through an opening in the expanded metal cover of the feed head box on the log washer. When the material broke free the rod went further down the chute and caused the employees' left hand to strike the metal cover.

Mostrando los 46The employee went to start the pump on the water truck with the pull rope and the handle broke cutting EE's right index finger, which required 6 stitches.
Employee was shoveling/moving wet raw material under a conveyor belt to the walkway just to the employee's right in the wash plant surge tunnel, when employee felt some discomfort in left shoulder blade. This was a first aid until May 14th when employee had initial visit at the clinic.
Employee was replacing shoes in the VSI crusher with new ones. During this task their back started to feel tight. Later that day this employee went to install a scraper on a conveyor and as they stepped over a beam they felt a twinge in their back.
Employee was rinsing the mud off of loader and was walking backwards pulling the water hose towards the back of the machine when they tripped over a wheel hub assembly that was on the ground. They reached backwards to brace themselves for the fall, and in doing so their hand contacted the top of the hub assembly causing a cut to the palm of the left hand.
Employee was moving a roll of skirt board rubber that was on a pallet into the lube shed. EE squatted down and pulled the top roll to the edge of the bottom roll and pulled it half way off so it tilted to the ground. As EE was standing the roll up on it's side it slipped from EE's grip. As EE reached out to grab the roll EE felt a pull in lower left side back.
Employee was helping complete a belt splice and was removing a clamp from the opposite end of the belt section that was installed, when the belt clamp slipped and traveled over a deck roller and struck him in the chin.
Two employees were performing maintenance on a hazmag crusher and while flipping a blow bar, it fell off of the stud on the lifting mechanism towards ee who was standing on the belt facing the rotor; he was able to keep his hands on it as it fell, however at approx 600 lbs it did hit his left leg which pushed him back up against the back wall of the crusher.
Employee stated he had been experiencing some soreness in his right knee for a few days. He could not identify any one specific incident or act that may have cause an injury to his knee. This was diagnosed as a right knee strain.
Employee was attempting to remove a bearing housing from an AC screen when his left thumb got pinched between the lifting chain and the bearing housing. The edge of the housing caused a cut on the inside of his thumb requiring 14 stitches and a cut on the outside requiring 4 stitches.
Employee was pulling on electrical cable for the hot sand plant when he felt pain in his groin area. Employee did not report this incident until 11/4/2009 three weeks after injury and after going to his own doctor. Employee was taken to the clinic on 11/9/2009. Currently employee is on restricted duty on his regular job and will see surgeon in two weeks.
Employee was shoveling in tunnel and felt pain in his right side. He was given 2 prescriptions from the clinic (15 tablets of Cyclobenzaprine 10mg and 30 tablets of Ibuprofen 600 mg) and work limitations of a sitting job/minimum walking.
Installing belt scraper on stacker head pulley. Pipe of scraper barely in mounting fork, went to slide collar into place in gear box, caused pipe to fall off fork. Trying to keep scraper from falling, grabbed pipe too close & the force of it falling pinched his hand between the pipe & mounting fork.
Employee was trying to "stand up" a rear tire from a 1987 Ford service truck, which was lying flat on the ground. Employee knelt down and took hold of tire, then attempted to stand tire up and as he did, he claims to have heard a "snap" in his back.
Employee picked up a 1-1/2" water hose with a spray nozzle attached to it and began pulling it up some stairs. As he started pulling, his finger came in contact with the end of the hose clamp. As his finger passed over it, the clamp caused a 1" laceration of his left index finger. Employee was taken to POH Medical Ctr where he was given a tetanus shot and 4 stitches.
While working in tight quarters on the crusher, employee hit his right elbow on the frame of the crusher causing it to bruise. Employee was taken to St. Joseph Mercy Maple Health Building where his right arm was put in a sling and he was given a prescription for Motrin.
Employee struck his right elbow on a 3/8" spray nozzle in the spray bar while installing a steel liner on the feed end of sand screen. X-rays at clinic revealed no positive results. He was placed on restrictions by doctor but was able to perform normal job duties within restrictions. Visit to doctor on 1/10/05 said no work until 2/14/05.
A customer truck was overloaded and driver had to dump off some of the load. The truck driver was holding his tailgate partially open and did not see loader operator come over and grab on to the other end to help. The truck driver went to slam the tailgate shut and employee's left hand was smashed in the latching mechanism of the trailer.
Employee was cleaning crush plant floor. He picked up the 41/4 crusher feedbox support post to move out of way to clean. Post is approximately 5' long, 3" diameter steel, 120 lbs. While dragging water hose to clean area, he felt a sharp pain in his right groin area. He states that he initially felt pain when he lifted up post.
EMPLOYEE WAS CLIMBING INTO HIS LOADER AND FELT A POP IN HIS LEFT SHOULDER. DOCTOR DID NOT GIVE A DIAGNOSIS BUT PUT HIM ON RESTRICTED DUTY WITH NO USE OF LEFT ARM. HE HAD RETURNED TO WORK AT A DIFFERENT JOB TO ACCOMODATE HIS RESTRICTIONS.
WHILE OPERATING A 988 CAT LOADER, EE OPENED RIGHT SIDE CAB WINDOW SO HE COULD "SPIT". WINDOW WAS NOT SECURED IN OPEN POSITION WHEN EE REACHED FOR WINDOW HANDLE, HIS HAND SLIPPED OFF & WINDOW S LAMMED SHUT, PINCHING HIS RIGHT INDEX FINGER. HE PULLED HIS FINGER FROM WINDOW CUTTING THE SKIN. ALL THIS TOOK PLACE WHILE MACHINE WAS IN FORWARD MOTION. 11 STITCHES REQUIRED.
ON 5/23/03, EE REPORTED THAT 2 DAYS PRIOR HE HAD PICKED UP A BEARING FROM THE BACK OF A BUCKET TRUCK & CARRIED IT TO BUCKET & LIFTED IT APPROX 15' ABOVE TO THE PULLEY SHAFT. HE PLACED BEARING ON BOOM OF BUCKET WHILE HE CLIMBED INSIDE. HE CLAIMS TO HAVE FELT A STRAIN IN LOWER ABDOMEN REGION. ON 6/5/03, DOCTOR CONFIRMED THAT EE HAD A HERNIA WHICH WILL REQUIRE SURGERY.
EE WAS TRYING TO BEND A RAKE CABLE INTO A LOOP SO THAT A CABLE CLAMP COULD BE PUT ON. A SMALL FRAY IN THE CABLE POKED A HOLE IN HIS RIGHT MIDDLEFINGER AND BECAME A SLIVER. HAND BEGAN TO HURT A ND SWELL LATE IN THE DAY ON 5-16-03 OR EARLY 5-17-03. INCIDENT WAS NOT REPORTED TO SUPERVISORUNTIL 5-16-03. EE WENT CLINIC ON 5-19 AND WAS RELEASED WITH NO RESTRICTIONS. FOLLOW UP APPT ON
WHILE EMPLOYEE WAS PULLING ON A FLAT TIRE ON THE MAINTENANCE TRUCK, HE FELT A PULL IN HIS LEFT SHOULDER. WENT TO THE CLINIC ON 1/30/03 WHERE HE WAS PUT ON RESTRICTED DUTY.
WHILE PULLING A BLOWN HYDRAULIC HOSE TO REMOVE IT FROM THE PLANT, THE HOSE CAUGHT ON THE FRAME OF THE PLANT WHICH IN TURN CAUSED PAIN TO RIGHT HAND AND FOREARM. INJURY WAS REPORTED TO THE SUPE RVISOR ON 10/18/02 AT WHICH TIME THE EE WENT TO WIXOM OCCUPATIONAL HEALTH CLINIC WHERE THEY PUT HIM ON RESTRICTED DUTY.
EE WAS USING A 1-1/2 TON HOIST TO LIFT A 5-1/2 STD CRUSHER DISTRIBUTION PLATE FROM THE CRUSHER DECK FLOOR & BACK UP INTO THE CRUSHER. AS THE PLATE TRAVELED UP THE SIDE OF CRUSHER, IT GOT CAUGH T ON A LOCKING POST. EE & CO-WORKER GRABBED A SIDE OF THE PLATE & PUSHED IT AWAY FROM POST. EE FELT PAIN IN LOWER BACK & LEFT SIDE. REPORTED INJURY TO SUPERVISOR 10/18/02. WENT TO CLINIC.
EE FELT A PAIN IN THE CENTER OF HIS CHEST AS HE WAS LIFTING A LARGE ROCK OUT OF THE SCREEN CHUTE. EE DID NOT GO TO THE CLINIC UNTIL 6-7-02 AT WHICH TIME HE WAS GIVEN RESTRICTIONS AND A PRESCRI PTION FOR 400MG. OF ETODOLAC.
WHILE EE WAS WASHING MATERIAL BUILD UP AWAY FROMCONVEYORS, A ROCK FELL FROM SOMEWHERE ABOVE STRIKING EE ON LEFT FOREARM.
WHILE MOVING PERSONAL WELDER TO THE BACK OF HIS SERVICE TRUCK, EE FELT A PAIN IN HIS LOWER BACK. HE HAD AN APPT. AT 1400 ON 10-2 TO RE-EVALUATE AN INJURY FROM 7-13-2001.

EE SOUGHT MEDICAL TREATMENT ON HIS OWN AT A LATER DATE & IT NOW REQUIRES SURGERY, SCHEDULED FOR 3-16-0

EMPLOYEE DROPPED A SIEVE CLEANING BRUSH ON THE FLOOR OF LAB. BRUSH LANDED BEHIND COOLING RACK. SHE GOT ON HER HANDS AND KNEES TO TRY TO RETRIEVE BRUSH. AFTER RETRIEVING BRUSH, SHE STOOD UP AND WHEN SHE DID, SHE FELT SEVERE PAIN IN HER BACK AND WAS UNABLE TO STAND UP STRAIGHT.
