Four employees were installing a steel plate. The plate was 20' wide x 8' high x 1" thick. A small loader put the forks against the plate 5 feet up. The operator re-position the forks 3 feet lower so employee 1 could stand on forks and get a better reach. The plate became unstable and fell on top of employee 1, pinning the employee.

Supervisor pick-up was struck by haul truck, resulting in a fatality.
Per MSHA Preliminary Report: The screen plant was being attached to a front-end loader in preparation of being moved into the pit. The victim was proceeding to hook them together when the equipment moved, pinning the victim. The victim was later discovered injured and leaning against the loader bucket. The victim died of their injuries the following day.
16 yard Rohr Dredge located in lake on N Hummer Lake Rd in Oxford flipped over (cause unknown at this time). Employee drowned. Employee's body was recovered at 5:35 pm. Incident is under investigation by MSHA inspector Steve Field.
Employee fell from stack cap level to preheat level suffering fatal injuries.
Employee crawled 16 feet under primary and was entangled in tailpulley.
EE and Another EE responded to a stuck pallet alarm on Unit IV cooler. He became trapped when the cooler pallet released on its own causing the dump arm and wheel to swing up and strike him in the head, pinning him between the dump arm wheel and the rail.
CRANE TIPPED OVER INTO WATER WITH OPERATOR IN MACHINE.
EE's were attempting to raise the crusher discharge conveyer when a support bracket broke, causing the end of conveyer to raise upward striking the victim. The conveyer was being raised with a hoisting boom mounted to the bucket of the front end loader.
ANOTHER EE WAS DRIVING A KAWASAKI K2110-II LOADER AND WAS BACKING UP WHEN INJ EE DROVE HIS S10 PICKUP BEHIND THE LOADER. THE IMPACT CAUSE IMMEDIATE DEATH TO THE EMPLOYEE. DRIVER OF THE L OADER WAS NOT INJURED.
EMPLOYEE WAS REMOVING A SECTION OF CATWALK. HE CRAWLED UNDER THE CATWALK AND CUT THE SUPPORT BOLTS, ALLOWING THE SECTION TO FALL ON HIM.
EE CLIMBED UP AND STOOD ON THE EDGE OF THE GRAVEL, LOG WASHER. WHILE PULLING ON A PIPE IT GAVE WAY LETTING HIM TUMBLE INTO THE LOG WASHER. WASHER WAS RUNNING AT THAT TIME.
