Employee was working down by ball mill 6 and 7 in the cyclone feed pump build up area. Employee was using the bolt for a hammer and smashed a finger.

While changing a hose fitting on a mine shovel, a worker was loosening a fitting using a wrench. The fitting was looser than expected and the workers hand struck a ball valve causing a 2.5 cm avulsion to the right, mid-third index finger and a minor fracture to the second phalanx on the right index finger.

Craftsmen were working on rebuilding a hammer mill. While in the process of rotating the mill, the craftsmen inserted a small bar into a hole in the counterweight to help rotate the mill to check for clearances. When the mill began to spin the craftsmen lost control of the counterweight pinching EE's finger between the bearing and pry bar.
While working on TK-1A overflow pump valve, the employee was loosening bolts when employee caught thumb between the wrench and the valve body.
Employee cut two fingers while cutting a zip tie off of a hydraulic motor on a drill with a utility knife. Employee received 14 sutures in a medical facility.
Technician was stripping cable sheathing utilizing safety cutter to cut it to expose the internal wires. During the operation technician's hand slipped causing a laceration to the top of the right index finger. The injury required 6 stitches
Tightening a fitting in a small space under truck, only could turn fitting 1/4 turn at time, didn't get wrench all the way on and it slipped hitting EE in the tooth chipping the tooth.

A mechanic was using a Porta Power to push a bearing on a drive pulley to achieve the final alignment. A bolt was used to extend the length of the porta power cylinder. The bolt ejected during the process and struck the mechanic in the chin.

A millwright was working on a linkage. While using a "tug-it" the millwright was applying pressure when the handle gave way. The momentum carried EE into the piece of the equipment which caused a minor laceration that required 3 stitches
While removing a tooth from an excavator bucket craftsperson used a 3lb hammer to hit the tooth after the third strike EE felt a sting to EE's left forearm and a blood spot appeared. at the hospital a metal shard was removed and EE's forearm received 5 stitches
Mechanic was removing a haul truck rim assembly utilizing a port-a-power tool that was being held by a sling. The port-a-power tool slid out of position, twisted and hit the mechanic just below the brow area. The worker was wearing safety glasses. The impact force of the port-a-power tool was deemed low energy.
A Mechanic was using a pry bar to free a binded part prior to using a crane. The pry bar slipped causing the mechanic to impact the hand on the frame of a grader
Employee smashed pinky finger of right hand between pry bar and steel structure holding the donut inlet tab above ball mill feed.
An Operator was clearing jammed product from a locked out screw. The Operator was using a ball peen hammer. The Operator came into contact with the screw flight and sustained a laceration that required stitches

Craftsman was working to replace a wear plate on a shovel dipper door when the cable supporting the plate caught on the door creating stored energy in the system. The craftsman tried to free the plate with a pry bar and as the cable suddenly released; the craftsman sustained a crushing injury to his middle and ring finger(s), by contact with the wear plate and dipper door.
AN EMPLOYEE WAS REPOSITIONING A GET (GROUND ENGAGING TOOL) TOOTH IN A SERVICE TRUCK WHEN A SECOND TOOTH SHIFTED AND PINCHED THE EMPLOYEES FINGER BETWEEN THE TEETH.
A worker was struck on his lower lip by the pry bar tool he was using to release a lever binder. The impact caused a 1/2" laceration to his lower lip and the loss of one of his lower incisor teeth. He reported that the release of energy was more powerful than he had anticipated and he could not keep the bar from striking his face.

According to employee statement: while conducting repair work a piece of angle iron suspended between two I-beams fell. The come-along handle struck the employee first followed by the angle iron. The angle iron struck the employee in the neck shoulder region.

Geologist using machete to clear brush about 40? uphill on a steep slope, in order to step forward up the hill, Geo stuck the machete in the ground freeing his hands to grasp onto trees and assist him up the hill. He began to step forward and up the hill; right knee contacted machete, resulting in a ~1" long laceration on the outside of right knee.
