While an employee was walking underground, they tripped on a rock and fell on their knee. Employee received first aid at the time of the incident and was later taken to the hospital for worsened pain resulting in time away from work.

While an employee was walking to their vehicle at the end of shift, they slipped and fell from the ice and snow. Employee was evaluated for continued pain and was given time away from work.

While an employee was exiting a bolter, their tool belt caught on the grab bar railing resulting in the employee twisting their back in an awkward motion. Employee was evaluated the following day for worsened pain resulting in days away from work.

While a miner was scaling the face, a rock 1'x6"x6" came loose and struck the miner's hardhat. The miner was taken to the hospital and was prescribed medication and time away from work.

While employee was attempting to open a stuck valve, they felt a pop in their neck. Employee did not seek treatment until the following morning after having worsened pain. They were prescribed medication and time away from work.

While preforming an inspection on the Alimak, the cage surpassed a limit switch that caused a fault resulting in the Alimak to be inoperable.

While miner was disposing of hydraulic buckets, the handle broke off leaving a laceration on the miner's finger. The miner was transported to the hospital and received 3 stitches.

While employee was attempting to pick up wheel chocks, they felt a pop in their wrist. Employee was given first aid by security and was seen at occupational medicine days later for continued pain. Employee was prescribed a splint and work restrictions resulting in days away from work.

While cleaning cement out of haul truck bed, employee was struck on the eye by an air hose that came away from the air chipper. Employee received first aid by security and was taken to the hospital for further treatment; employee was prescribed medication and days away from work.

While attempting to change the blade on a reciprocating saw, the employee's left index finger and thumb were pinched. The resulting laceration required stitches.

While performing the daily inspection of the Alimak (secondary escapeway elevator) it was found to be faulted and inoperable due to overheated VFD.

Miner was handling a bundle of ground support bolts and stepped on a rock awkwardly, resulting in them falling to the ground. Miner was evaluated at medical center, lost one day due to pain in their knee, no restricted duty.

Employee pulled a case out of back of truck with one hand, set it down on ground and felt pain in shoulder. The employee didn't pursue treatment until a week later and was evaluated at medical center where lifting restrictions were applied starting 12/3/24.

While placing their tool bag in the back of the pickup truck, electrician twisted and experienced pain in their lower back. Employee was evaluated at occupational medicine clinic, prescribed time off and then restrictions.

A section of ventilation tube shifted while removing it, the employee's finger slipped into one of the mounting D-rings which pinched it as the tube slipped. This resulted in the loss of the tip of their right-hand index finger.

While assisting in handling a VFD module the employee felt discomfort in their back after the module was tilted.

While performing electrical switching underground during maintenance activities, a circuit breaker failed causing an arc flash. As a result of the arc flash, an employee received third degree burns on the back of their left hand. The employee was evaluated at the local hospital and will remain away from work to receive further treatment.

Electrical components of the Alimak secondary egress were found to be faulty and needed replacing. Underground work ceased until the electrical component was fixed and the Alimak was operable again.

An employee was helping lift a junction box in muck bay 22 when they felt pain in their right shoulder. The employee felt okay at the time, but then later in the day sought additional medical treatment. After being evaluated and given ice by security, they were sent to the local hospital for further evaluation.

While walking underground, the employee tripped and fell, landing on their arm. The employee was evaluated at a Medical Center and diagnosed with a forearm contusion. Lifting/gripping work restrictions were applied.

A fall of ground occurred at the intersection of muck bay 24 and the main ramp in the south decline impeding passage. No miners were in the area the time of the fall.

While performing an inspection on the Alimak, the lower call button panel was found to be inoperable.

While doing a drop test on the alimak, the cage surpassed a limit switch that was faulty causing the Alimak to be inoperable.

Employee was removing rock from travelway and experienced shoulder pain. Employee was diagnosed with a shoulder strain, prescribed medication and provided with a shoulder sling.

A geologist was inspecting core samples and when they moved one stack of boxes off a pallet they felt an abdominal pull. The geologist received lifting/pulling/pushing restrictions.

The operator fell on a patch of ice covered by a dusting of snow while performing a walk around inspection of a skid steer. Employee reported right shoulder pain. EE was evaluated by Mine Security who suggested EE go to the ER. Diagnosis was strain of the right shoulder rotator cuff muscle and tendons. ER prescribed non-OTC medication for inflammation.

Employee lifted a bag of ore weighing less than 5 pounds. While putting this bag in a box located on the outside of the truck, EE felt EE's neck and/or shoulder pop. The employee refused further evaluation at that time. At 11:15 p.m. EE informed EE's supervisor it was getting worse. When employee returned to site at 1 a.m., the supervisor took EE to ER for further evaluation.

On Nov 29th EE slipped when getting out of a pickup and caught themself on the pickup door with their left arm. No treatment desired at the time. On Jan 8th EE reported continual soreness and weakness in their shoulder/arm/hand. They were evaluated by Occ Med and prescribed physical therapy and lifting restrictions. No time was lost and able to perform their normal job.

An injury occurred when the employee was struck by material that fell from the face while tying in detonators to the E-cord. The employee was hit in the hardhat and right shoulder causing several minor lacerations and a shoulder sprain/strain.

Loader operator drove over a 9"x9"x6", 75 lb. piece of ore and it shot out from the tire and hit the employee 28 feet away in the upper left thigh. Mine Security determined it was not a medical emergency. The supervisor took EE to the emergency room where it was determined the employee was bruised. No prescription medicine was provided.

Driller Assistant was pulling the wireline so it could be cut and re-crimped. EE could not pull any more slack so driller pushed forward on controls to let out more slack and the wireline abruptly retracted catching assistant's left hand between the stuffing box and the sheave wheel of the drill.

The miner's tool slipped causing the right elbow to receive a 3/4" long cut by the loader bucket. The Miner and co-worker cleaned and dressed the injury. On 6/20/23 a doctor diagnosed cellulitis in the elbow, drained fluid, and applied a dressing.

The Grader operator was pushing material to the right rib at roughly 3mph when a rock measuring 1'X 1.5'X 1.5' rolled over top of the blade and then under right rear tires of the Grader. When this occurred the seat-belted operator's right elbow hit the armrest forcefully, causing a sprain to the employee's right shoulder. The full extent of the injury is currently unknown.

An underground surveyor was setting up survey equipment on the 352 level in the 1485 heading. As the employee was attempting to use the eyepiece of the survey tool, a foreign object fell into their eye when their safety glasses made contact with the eyepiece.

Employee had a pre-existing non work eye condition that became irritated while working as drill assistant on a UG core drill. The salt or salinity content of the drill water may have contributed to this aggravation. Employee was taken for medical review and given prescription eye drops which resolved the eye irritation.

Technician felt muscular discomfort in their back while moving drill core sample boxes from pallets located on the floor to tables 40 inches high. Boxes range in weight from approximately 35-50 lbs. Technician was diagnosed with a thoracolumbar strain and placed on restricted duty. No lifting, pushing, pulling > 10 lbs and no reaching (keeping elbows near torso).

The miner was loading blast holes in the face of the 352 west footwall drift. Material fell from above the miner's location and struck the miner on the Hardhat causing a temporary loss of consciousness. This incident was immediately reported to the onsite MSHA inspector, and an investigation initiated.

The operator was removing a plastic coated insulated cable hook (CAB), used to suspend a 480V trailing cable to the wire on the rib. The miner received a laceration to the base of EE's right index finger when the hanger sprung back and hit EE with the metal attachment hook.

A roof/back fall was discovered in a previously barricaded heading (352-1425), at approximately 4:09 PM. The heading remained barricaded and MSHA was notified of the roof fall.

The Supervisor completed a post blast inspection of the 294-1425 stope top cut or 323-1425 and discovered the back had broken up to an undefined height above the ground support, due to the stope blast. The stope access was barricaded with appropriate warning signs.

The miner completed bolting a partial slab round in the -515 EMLD heading. The ground support was installed as per the GCMP. The miner and the shift supervisor were standing approximately 10' behind the bolter when fine material was observed trickling out of the supported area. Fractured rock material then fell from the area and came to rest against the right side of the bolter.

The UG miner was hit on the head and shoulder by rock that fell from the face while performing scaling activities and working off of a scissor deck. The miner sustained abrasions to the head, and a shoulder laceration that required sutures to close.

The employee was attempting to loosen a bolt on a Jumbo drifter to change a striker bar when the wrench slipped off the bolt. The wrench hit the employee in the lip causing a laceration requiring sutures to close.

During an inspection it was found that the doors of the Alimak would not open when the unit came down to the raise access drift underground.

During a daily inspection it was discovered the elevator was not responding to the call button commands from the station.

The ground failed to a height of approximately 8 meters above the sill. The stope was blasted on 11/26/2019 and scheduled to be mucked out within the next few shifts. The stope was supported by 8' swellex and 20' grouted cable.

The ground failed to a height of approximately 5 meters in a 26 meter stope. The area was previously barricaded off with no active mining taking place. Future plans were only to blast and remote muck with no personnel ever entering that area. The stope was supported by using 8' bolts and 20' grouted cable bolts at the time of the failure.

During the daily alimak inspection, the car door failed causing the alimak to become inoperable.

Employee was scaling work area. Rock was scaled out of mesh striking employee in the hand, cutting glove and causing a laceration to the top of the hand

The employee pinched left ring finger between the forks and the mast on a 938H surface loader, while attempting to adjust the forks. The injury resulted in a small fracture to the tip of the finger.

Electrical components of the secondary egress were below operable temperature due to heaters not functioning. Underground work ceased until the elevator is operable and ice conditions in the raise allow for safe access.

At the end of shift, while leaving the dry to exit the mine, the employee slipped and fell due to the snow that had accumulated on the stairs. The employee landed on the stairs contacting rib cage, resulting in several fractured ribs.

The drill helper was holding an 8' drill steel while attempting to install it into the chuck of the Jackleg drill. The operator applied air to the leg to assist with the connection and the employees left hand was pinched between the steel and the machine.

Employee was pulling a 10' NQ tube with 2.5' of core out of the rods. EE felt a pop in EE's back.

The employee was using an Angle grinder when the material ee was working on came loose from the clamp and moved. The employee reached to grab the clamp and at the same time the grinder kicked contacting left thumb. The employees left thumb received a small laceration due to the contact.

Employee was standing up from a kneeling position when they felt their left knee pop and immediately experienced pain.

Employee was moving a 4x4 piece of wood out of the way when EE felt a pop in the shoulder.

Electrical components of the secondary egress were below operable temperatures due to the heaters not igniting after running out of propane gas.

Miner was walking and encountered a slippery surface. Miner slipped and broke both bones in lower leg.

The employee was walking to assist with a cable pull when a small rock fell through the wire and bounced off the Ventilation duct and struck the employee on the upper lip. The employee received a small laceration requiring sutures.

An after market switch gate sensor failed causing the secondary escape (alimak) to become inoperable.

The employee just finished attaching the water line to the water header while setting up a Jumbo drill. Prior to turning on the water line the employee looked toward the Jumbo and then opened a valve on an adjacent air header that was connected to an air hose, which resulted in a laceration and fracture to the nose.

The employee pinched right little finger in the door latching mechanism of a Mechanized-bolting machine resulting in a fracture to the tip of the finger as well as a laceration.

While performing the annual alimak over speed break test the lower limit switch tripped out causing the alimak car to settle in the pit.

State elevator inspector wanted the pit door switch moved to the striking side. Once the switch was moved to the striking side, it took on water causing it to fail. During maintenance the alimak car traveled down the shaft 50ft without anyone in it and tripped a circuit breaker.

This was a personal medical condition. The employee had a stroke while walking in the water treatment plant.

Upon inspection it was discovered the breaker which feeds the Alimak was tripped. After resetting was unsuccessful, testing was performed on the feeders to the car. It was discovered that the cable from surface to the car was shorted out.

Two miners were working together to install utilities when they loosened the bolts on the Victaulic clamp on the air header prior to relieving the pressure. One miner received a fracture to right forearm, the second miner was treated for eye injuries when glasses were blown off, EE also received soft tissue injuries to the left forearm.

While completing a thorough inspection of the face (face prep) the miner was hit on the top of the hard hat by loose material. The material came off of the face above the location of the miner. The injured miner required staples to close a small laceration located on the top of EE's head. Post treatment, the injured received a full duty release.

During a post blast inspection of the 215 level, a fall of ground was identified in the 1485 stope. The ground failed to an unidentifiable contact in excess of 20 feet above the back of the stope. The area was supported in accordance to the ground control management plan.

In injured miner fell while dismounting the man basket used for scaling. The injured miner returned to work light duty post incident.

Employee removed a chain-linked fence and during clean up of work area the top strand of wire from the fence sprung up and struck employee in the tooth. This caused a fracture in the bottom third of their tooth.

Employee was laying down on grating on a conveyor catwalk to weld a new midrail along the conveyor. When he got up, he felt a poke in his left elbow. This had happened on 9-11-18 and was reported to the contractor on 9-18-14. He had medical attention which required and anti-biotic.

Employee was carrying a sheet of metal siding, strong wind caused him to lose his grip. When he grabbed it, the sharp edge cut through his glove into his hand resulting in stitches.

Injured employee was reaching for a 5' metal wire reel to assist another employee. The wire reel struck the injured employee in the right hand causing a fracture to his hand.

The employee was dismounting the Getman scissor lift truck using the ladder when his partner raised the stabilizer jacks causing the machine to shift abruptly. The injured employee was thrown or fell off the ladder and landed on the ground on both feet. The employee complained of sore left Knee and right Ankle. The employee was initially released to full duty.

Employee was working out of Genie boom basket on corner of the building. He reached out to grind and strained a muscle in his left shoulder. No medical attention at time of injury. He is now scheduled for rotator cuff surgery on 4/16/14

While unloading structural steel, a bee stung him in the back of his neck

EE opened a desk drawer in the Civil Trailer. A bat flew out and latched onto his forearm and bit him. The bat was not found so employee will have to have the rabies vaccine series.

Employee was repairing a damaged wall; while swinging a sledge hammer his hand slipped down the handle and left palm made contact with an improperly seated nail head causing a laceration. Employee received six stitches and was released back to work the same day.

Employee carrying 10ft rod across deck, slipped and fell with tube in hand. Landed on rock resulting in lacerations on index, middle, and ring finger.

A mechanic/electrican apprentice was using a Milwaukee utility knife to strip the outer jacket off a 410 cable; he was pulling the outer jacket with his left hand and cutting around the jacket with his right hand; the blade slipped and cut his left palm. Employee was wearing cut resistant gloves at the time. Employee received 5 stitches on his left palm.

Two men were working out of the man basket in main decline loading collars left from previous blast. Rock fell from right side of basket and struck right hand ring finger of employee. Result of fracture and laceration to right ring finger.

While patrolling the south fence the security guard was making a loop to turn back onto the trail. There is a slight lip where the trail is lower than the snow the guard came off. The snowmobile was going very slow at the time. IT tipped on its left side and the guard rolled off as it tipped over on its side. No damage to trail/snowmobile. Guard reminded to use extra caution.
