EE was removing "bolts" from the Meva forms and EE was using a hammer to tighten down a pancake nut. When EE wrenched the nut, EE felt a sharp pain in EE's shoulder. EE treated it with OTC medicine and ice at the time of the incident. As of December 11, EE has now seeked medical attention.

While removing shovel cab windshield, EE pinched tip of right thumb between window frame and 9/16" wrench during removal of bolt resulting in pinch/shear injury to right thumb.

EE was removing lower pin from production truck dump box, left hoist jack using a slide hammer. EE's thumb wrapped around the handle while hammering, putting the thumb in the pinch point causing an avulsion at the end of the right thumb.

EE swung sledgehammer to cleanout material B mixer drum. Hit left pinky finger on frame of B mixer drum. EE sustained closed fracture. Surgical repair to nailbed.

EE repeatedly swung sledge hammer to release dozer tooth pin, causing discomfort in rt shoulder. Rt shoulder impingement syndrome & tendinopathy.

EE using 3ft pry bar on fan shroud to align bolt holes. Bar slipped striking EE in left ear.

Employee was removing the lip shrouds off the BE spare bucket in Etterville using a 12 pound soft blow hammer. While striking the rear of the lip shroud with the hammer, a piece of shrapnel came off either the hammer of the lip shroud and struck the employee in the right shin area causing a laceration.

EE was changing shovel tooth using pin puller bar. Felt discomfort in right shoulder.

Employee was swinging sledge hammer to put plug in mill shell. The employee reported general soreness in right shoulder the next day. On January 22, 2019, the employer learned that employee commenced physical therapy.

Employee was using a high torque wrench to tighten right hand segment bolts on Unit 326. The wrench slipped off the nut, driving the employee's left hand into the side frame causing a laceration to the left ring finger.

Employee used a lifting bar with co-worker to lift a truck box pin. The employee felt a strain in lower back.

On April 17, 2018, employee utilized a sledge hammer to drive out pins on drill side frame. On May 2, 2018, the employee reported pain in left shoulder. On May 11, 2018, the incident review attributed the shoulder pain to the repetitive motion of repeatedly swinging (~75 times) the 8lb sledge hammer.

Employee was changing lip shrouds on the 50 shovel bucket. Employee hit shroud with an 8 lb sledge hammer. A small piece of metal broke off and entered employee's right shin. The employee received stables to close the laceration.

Employee was cutting a hydraulic hose protective wrap with a knife. The knife slipped and the employee cut left hand, resulting in a laceration and sutures.

Employee was cutting an air line hose with a utility knife. Employee sliced right thigh and the laceration required sutures.

Employee was trimming the corners of a conveyor belt. While attempting to remove the utility knife, the knife blade sliced into the employee's boot and caused a laceration on EE's left foot.

Employee attempted to place a nylon sling under pipe while it was being lifted with a pry bar. The pry bar slipped and employee's left index finger was pinched between the floor grating and pipe.

Employee was installing allen bolts. Employee attempted to free allen socket from bolt with a breaker bar. The breaker bar slipped and struck the employee in the mouth resulting in fractured tooth.

Employee used a 11.5 lb dead blow hammer to loosen a stuck wheel off the 686 truck. Employee swung the dead blow hammer about 5 or 6 times and felt discomfort in his right shoulder and arm.

Employee was cutting off a section of 1/2 inch air hose. While making the last cut, the knife blade cut through the Employee's leather glove resulting in a laceration to the left index finger.

Employee was using a bolt cutter to remove a Kirk padlock. As he was cutting the lock, the bolt cutter head slipped and bounced off his left elbow.

Removing bushings from a shovel dipper. Reported to supervisor on October 6, 2013. Reported to clinic for examination of right shoulder pain on Thursday, October 10th.

Green Feed Attendant was cleaning out H mixer Chute from H Balling Drum. He was bending over scraping the filter cake from the sides of the chute. Felt a strain in lower back. Was sent to Urgent care and was taken off work.

Electrician was disconnecting the power cable from the 61 shovel swing motor. The connections had black tape on them and the Employee was using a utility knife to remove the tape. He knife slipped and cut his left forearm. required sutures. Was wearing gloves at time

Auto Mechanic was removing a front tire on 648 pickup. After removing lug nuts he used a 10 Lb urethane hammer to break the tire free. Following the task he felt soreness in his left shoulder. No restrictions until surgery on 06/13/2013

Welder was changing out liners in apron feeder and lifting the worn liner on the belt. The liner slipped and his finger was pinched. Right Index Finger

EE was changing out a filter bag on Ph-1 Filter Floor and felt a strain in his lower back. The bolt on the sector was tight and was using a wrenching to take the nut off. Pt was required on the clinic run.

EE was attempting to change out a tooth on the loader. The operator struck a tooth with a metal hammer in order to loosen the tooth. He hit the side of the tooth and a piece of metal came flying off and hit him the abdomen

EE was performing maintenance on a pallet car. While using a chisel to tighten the lube clamp to the wheel in order to tighten a bolt. When he went to hammer the chisel he missed and struck his left thumb.

While bolting up structural steel the employee got his finger pinched between lining bar and beam.

EE was changing out the starter on the lube van. The Mechanic was pushing with right hand pulling with left hand and started to feel discomfort in left shoulder and upper left bicep. Surgery was 10/25/2011

EE was using a pry bar on a conveyor belt and the bar slipped on hit his left hand on a metal object. He cut his left middle finger

EE was torquing sideframe roller bolts on the backhoe and reported soreness in his left shoulder. Under further investigation and physical examiniation that it was tendinosis. The incident was November 18, 2008 and was working full duty until his scheduled surgery for 04/28/2009

Employee was assigned to work with the mechanics to remove the trommel rock box in order to remove and replace the conical rock deflector. Employee was cutting the welds and was using a pry bar to remove the structure. Using a 5 ft bar and prying up and the bar slipped and hit himself in the jar. The skin was not broken or bruised.

Employee was doing his routine shop clean up and while sweeping his area with a hand broom, his left pinky finger came in contact with a piece of metal in the steel cutoff rack.

Employee was changing Hydraulic hoses in a confined space and installing new ones, approximately 8 lines, and when he finished the job felt a pain and tingling sensation in his right arm and right hand

Employee was removing flights from an apron feeder and was using a hook and jarred his shoulder. He visted the clinic ten days later.

Employee was installing a pin on a production truck using a 5lb porta-power jack when the jack slipped and hit his mouth and tooth.

Employee was installing a coupling on electric motor and was using a allen wrench to tighten the set screw down. While tightening the screws, the wrench slipped and he hit his first knucke on the coupling teeth

Employee was boring a conveyor belt pulley using a 4'' portable boring bar.

Changing rougher dist. elbows on #2 mill, used torch to cut bolts from the elbow clamp, hammer used to knock off the clamp that was holding elbow pipe. Hammering on the clamp, pipe broke free, hitting ee in the face. Went to clinic, cut received on upper lip did not require stitches, ee was told stiches could be administered for cosmetic purposes (not leave scar)

Replacing pipe unions & nipples on a HAV unit, taking out the gas unit and tightening an elbow using a 3" x 3' cheater pipe over a 36" aluminum pipe wrench. The cheater pipe slipped off the pipe wrench, bounced off the bridge of his nose and then sliced into the left nasal alae area. He had a 3.5 cm curvilinear laceration that was tangential and down to the deep cartilage.

WHILE REPAIRING OVERHEAD DOOR SEAL, EE BUMPED HIS TOOTH WITH A PRY BAR. THIS MAY HAVE CAUSED HIS TOOTH TO BREAK. EE SAW A DENTIST AND IT WAS DETERMINED THE REMAINING PART OF THE TOOTH SHOULD BE REMOVED AND A BRIDGE WAS ADVISED. MEDICAL TREATMENT.

Assisting a mechanic move a shank and pound in a pin. He was not sure what caused the soreness. Was put on restriction of 8 hours a day and 20# lifting which did not limit him from operating a shovel. In Oct. went back to doctor & said he was interested in surgical intervention but not until after deer season. Received info 01-10-06 that he was having surgery on 01-12.

POUNDING ON A CUTTER BAR, GOT A PIECE OF METAL IN EYE SOMETIME DURING HER SHIFT.

THREE EMPLOYEES WERE REMOVING (CUTTING) HL-5 CONVEYOR BELT WITH A STANLEY UTILITY KNIFE. WHILE CUTTING THE BELT THE KNIFE SLIPPED, CUTTING EE'S LEFT FOREARM CAUSING A 1/4" DEEP BY 1 3/4" LONG LACERATION.

EE WAS USING A WRENCH TO LOOSEN A SEVERLY RUSTED NUT AND BOLT WHEN HE SUFFERED A RECURRENCE OF A VENTRAL HERINA. EE CONTINUED TO WORK WITHOUT RESTRICTIONS UNTIL REPAIRED 07.27/04. REPORT BEING FILED BECAUSE OF CITATION 6159917. WE BELIEVE THAT THIS IS NOT REPORTABLE

EE WAS LOWERING PRIMARY ORE CONVEYOR BELT WITH CHAIN JACKS WHEN A SUPPORTING CABLE ON A BEAM MOVED AND SHOWERED THE EE IN DUST. EE WAS WEARING SAFETY GLASSES. EE WENT TO CLINIC THE NEXT DAY A PARTICLE WAS REMOVED AND REFERED TO DR. THE FOLLOWING DAY WHERE 6 PARTICLES OF DUST WERE REMOVED.

AN AUTO MECHANIC WAS USING A TORQUE WRENCH TO TIGHTEN THE LEFT FRONT BRAKE ASSEMBLY ON A CATERPILLAR 980G LOADER WHEN THE WRENCH SLIPPED OFF THE STUD CAUSING HIM TO PULL HIS GROIN MUSCLE. HE W ENT TO THE DR ON 2/5/03 WHERE HE WAS DIAGNOSED WITH A BILATERAL INGUINAL HERNIA.

EE WAS USING A 5' METAL PRY BAR TO REMOVE A PALLET CAR AXLE. THE BAR SLIPPED CAUSING THE EE TO HYPEREXTEND HIS LEFT SHOULDER. HE WAS DIANGNOSED WITH A ROTATOR CUFF TEAR ON 1/28/03. SURGERY WAS CONDUCTED 4/23/03
