While the employee was rigging & lifting a gang box/metal crate, the box/crate began to swing oddly and ended up pinching the employee's right hand & wrist between the wall & the box/crate. Employee ended up needing seven stitches and was put on restrictions.

Employee was using a slap hammer to remove a staple on LW shield and accidentally hit hand against hammer and shield causing injury.

Employee was using a bar to adjust a coupler on a trash car to secure it to a mantrip. As they were prying the couple up with the bar they felt a pain in groin area. The employee was seen by doctor on 3/20 and was diagnosed with hernia. Employee has currently missed no work and has worked without restrictions. Surgery is scheduled for 4/17 at which point it will be a LT.

EE was cutting a discarded piece of belt with a belt utility knife. EE was cutting away from self and snapped the blade off. A small piece of the broken blade was lodged in the belt and as EE's momentum and EE's right linear distal medium forearm was struck against causing a 2CM laceration.

Employee was cutting a belt when EE's hand slipped and EE cut EE's left pinky

Employee was cutting a plank for blocking rail when the bucksaw became stuck in a knot. Employee cut their hand after pulling the saw loose. Injury was not the result of a training or compliance issue.

EE was assisting with a motor change out in G5 longwall section, the EE was using a small pry bar hold back hose and cables through an access panel when right hand was caught between the shearer frame and bell housing once it broke free. The EE suffered a laceration which required 10 sutures and broken index finger.

Employee was cutting belt to take out slack to get thru door, belt shifted and knife slipped and accidentally struck employee in the leg area causing injury.

Employee was attempting to set a jack in the roof to secure a tailpiece during a belt move. The jack needed lowered for adjustment. When they set the jack down, the employee's hand was caught between the jack and the tailpiece. Injury was not the result of a training or compliance issue.

Employee was cutting up old belt to depose of it. While cutting the knife blade broke and slipped and cut into their left upper thigh. Employee received two stiches.

Employee was cutting a rope off of a tarp off of a handrail. They knelt down and on right knee and started to cut the ripe free and the wind was blowing and they slipped with the knife and cut into their left knee on the inner side above their knee. They received four stiches.

Employee was trimming excess belt from a splice with a utility knife. The employees utility knife slipped from the belt and cut the employee on the right thigh. Employee received three sutures.

Employee was swinging a sledgehammer to break up a rock on top of the miner in the 9 South Mains- Right section, while swinging the hammer EE's left ring finger was caught between the hammer handle and the miner frame. This event resulted in a cut requiring 16 sutures and a fracture to the tip of finger.

Employee was setting a vertical pin keeper on longwall face and using a horizontal pin to hit it in place. Employee accidentally hit left finger with the pin causing injury.

Employee was looping shield hoses and walking by another employee that was removing a staple using two hammers to pry out the staple. When striking hammer, a piece of metal from the hammer accidentally struck employee's arm causing injury.

Employee was working with a 30mil chain on the Crabapple slope bottom while hooking up supply cars. EE pinched right middle finger between the chain and the general car causing a laceration. EE was treated with 6 stitches and released for work. EE returned on next scheduled shift.

Employee was assembling a miner bolter station when they were using their right hand to hold a pin in place and striking the pin with a hammer in their left hand. Employee missed the pin striking their right index finger. Employee is right hand dominate, employee received 6 sutures.

The employee was cutting a crib bag fill hose with a cutting tool and in the process cut left index finger, causing a laceration. This incident did not involve any aspect of compliance with rules and regulations; mine equipment or mining system; job skills and miner proficiency, training and attitude; or protective items relating to clothing, or protective devices on equipment.

Employee was cutting green dust hose for a sump using a knife. They had one hand on the hose to secure it and was cutting with the other hand. The knife accidentally slipped and struck finger causing injury.

Employee was removing excess jabco cable from the 7 South Belt, when the employee struck left hand with a hammer causing a laceration that was medically treated with 6 sutures to close. Employee did not miss any scheduled time as a result of the injury.

Employee was putting in a bed rail in the structure and employee's hand was too close to pin when another employee hit the employee's thumb with a hammer when trying to set the pin on the bed rail.

Swinging 12 lb sledgehammer at a block wall. Missed wall and hyperextended Rt arm and falling to the ground. Employee said EE felt like something popped in Rt shoulder and in the same shoulder 3 weeks prior with another contractor employee started EE never filled out a previous accident report.

Employee was cutting a piece of 1/4 tubing with a belt knife. The knife went through the tubing and inadvertently struck EE's left index finger causing a laceration that was medically treated with 6 sutures to close. Employee did not miss any scheduled time as a result of the injury.

While swinging a sledgehammer to remove the pin on a ram car battery jack, employee felt a sudden pain in the left knee. Employee missed scheduled work due to knee immobilization.

Employee was using a sledgehammer to remove blocks from a K-door when the employee got a finger caught in between the sledgehammer and the frame causing injury.

Employee was working on belts and was cutting belt while on knees. The knife accidentally slipped and EE cut self above the knee causing injury.

Employee was using a chisel and hammer to free a dog bone connecting two pieces of panline. The hammer glanced off the head of the chisel, and struck the employee in the left thumb. Injury was not the result of a training or compliance issue.

Employee was conducting maintenance and loosening a bolt on the miner. The wrench slipped causing employee's finger to be struck against a panel on the miner causing injury.

Employee was installing vertical pin on the H3 longwall face. Employee used a pick hammer to set the pin fully in place. When the employee was striking the pin the employee struck right middle finger between the pin and hammer causing a laceration. The employee received 7 sutures.

Employee was helping complete a belt splice and was trimming off some belt edges. EE was on EE's knees on the left side of the belt cutting off an edge with a belt knife. The knife accidentally slipped and cut EE's left forearm causing an injury.

Employee struck in face by a Shive Puller while trying to remove shive from a Raw Coal Screen on 4th floor plant 2B.

Employee was cutting the tabs on a bucket of rivets to get the lid open. EE was cutting the tabs left handed and was holding the bottom of the bucket with right hand. The bucket is plastic. So being it was cold outside the plastic was cutting harder than usual and EE was exerting more pressure to cut. When they cut through they cut their right hand.

Employee was attempting to remove a keeper pin from a dog bone using a pick hammer. Employee hit the dog bone with the pick hammer and a piece(s) of metal broke off causing injury to EE's eye.

Employee was pulling bits from shearer using a hammer and bit puller and accidentally was hit by a piece of metal in the arm causing injury.

Employee was using a pry bar to move a roller. The bar slipped out from under the roller causing EE's hand/finger to be caught between the bar and the belt structure causing injury.

Employee was cutting a flapper on the belt and the knife accidentally slipped out of their hand and came in contact with their forearm causing injury.

Employee was removing tangled up belt when the knife slipped causing a laceration to right leg. They received 2 stitches.

Employee was hanging a crib bag with a pogo stick and cut EE's right pinky finger on a metal roof strap. Received 2 sutures. This incident did not involve any aspect of compliance with rules and regulations; mine equipment or mining system; job skills and miner proficiency, training and attitude; or protective items relating to clothing, or protective devices on equipment.

Employee pinched right thumb between a slate bar and a belt crossover while installing an arch on #5 mother belt. Employee received ten stitches to the thumb.

Employee was attempting to cut belt with knife and lost balance and inadvertently cut right thigh causing injury.

Employee was using a razor knife to clean off the end of a slick line and the knife slipped causing injury to forearm.

Employee was an the process of replacing the cover on the walk side rear jack of the cat mounted tailpiece, when the tool twisted in their right hand and they received a laceration.

Employee was using a come-along to lift an ATRS jack over the rib door on the miner when EE pinched EE's left hand between the jack and door. EE received 15 stitches.

Employees were cutting bolts off belt structure. When employee hit the bolt with a hammer, the bolt flew off hitting another employee in the face causing injury.

Employee was cutting a window opening in between the cables of a conveyor belt to put piano wire in to be able to strip the belt to do a vulcanize splice. While cutting the belt window opening employee was leaning on left hand and knife slipped resulting in cutting left wrist. Employee had to get nine stiches.

Employee was using a claw hammer while putting together hose on the monorail. EE hit the hammer with a pick hammer to line up holes for the staple and a sliver of metal came off causing injury to EE's abdomen area.

Employee was rolling a rail when the rail rolled back causing the slate bar to hit employee in the chest/ear.

Employee pinched finger while helping change out a belt roller. Received 5 stitches, and x-rays revealed a small fracture.

The employee was cutting rib mesh. The knife slipped and caused a laceration on EE's left forearm. EE received 13 stitches.

Employee was leveling a belt line at nine and half block. EE was using a Hi-Lift jack. EE was releasing the jack and EE's hand slipped off of the handle and hitting EE on the right side of face, resulting in some fractures. All employee's are being retrained on this jack. No other part 50 criteria applies.
