Employee was using a bit extractor fork and a 4lb hammer to remove bits from the miner in 2 South left. A piece of the extractor tool chipped off and struck the employee in the right arm. Employee was treated at hospital with 3 stitches and returned to work the next schedule shift.

Located at 6 South 100 Wall the employee was in the process of grabbing a rock prop, one rolled off the pile onto their hand.

While advancing shields and Pan in G-6 longwall section, a large portion of the roof fell filling up the pan-line. A rock approx 6ft by 2 ft by 6 inches came out of the pan-line and struck the employee on the back of the right leg. The employee suffered a fractured tib/fib as a result.

The BMX Portal Elevator hoist was out of service due to a door switch fault at 10:50am. No persons were injured and 2 employees were entrapped due to this event. The Hoist elevator began operation again at 11:18am and the employees exited the elevator.

IE was positioning themselves in the driver seat of a mantrip and their knee was driven into the operators box as they sat down. Injury was not the result of a training or compliance issue.

Employee was replacing a blown hose on the angle bolter. When they were getting down from the bolter, they placed their weight on to left leg and left knee gave out. Employee finished their shift. Was seen on the 22nd of July and diagnosed and taken off work with a possible tear to their ACL in their left knee.

IE was on a crew changing rollers on the beltline. While crossing a designated belt cross-over, IE bumped head against a roof bolt, resulting in a headache. Injury was not the result of a training or compliance issue.

Employee was side bolting off of the miner in the 15L Tailgate section. EE was stepping down from the miner after grabbing a strap from the strap tray. While stepping down EE felt a pain in EE's left knee. Employee was taken off of work until EE receives a MRI on August 4 to examine EE's left knee.

An unintentional roof call occurred at previously mined area at 101 crosscut 3-4 entry. This was a continuation from a previous fall that occurred at 101 crosscut 5-6 entry. The approximate measurement of the fall is 110' by 16' by 10'. This fall was caused by adverse roof conditions in the form of deteriorating laminated shale.

At roughly 12:30 am on Saturday 7/11/26 employee was overcome by clean coal while shoveling spillage out from bottom of Clean Coal Silo. Employee is currently in hospital. Vitals are stable but final outcome still TBD. ***EE passed away 07/20/2026***

Employee was in the process of picking up a bundle of 6 foot roof bolts from the scoop bucket. While picking them up to place them on their right shoulder, they felt a pain in their right shoulder. Employee worked all scheduled shift until they were taken off on the 22nd for July after being diagnosed with torn rotator cuff and scheduled for surgery to repair.

EE was standing on guarding rail 3 ft in air when rail broke; they extended left hand to break fall & crashed into the nearby wall w/that hand. EE ended up w/ scratch on L forearm, pain in L shoulder/R lower back. Worked rest of shift. Stiff/sore next day so went to Dr, was put on restrictions we can't accommodate, so lost time eff. 7/4.

An unintentional roof fall occurred along the 4 South track. The fall was in the 70th crosscut from the #6 to #7 entry. The roof in this area previously identified as an area concern and was supported and dangered off. The area proceeded to fall within the supports previously set. The fall was approx 60' by 16' by 8' high.

A shaft employee was closing the concrete chute on an empty bucket and missed the latch. By missing the latch it caused the chute to fall catching EE's right middle finger in a pinch point.

While changing bits on the miner. EE struck the bit tool, a piece broke off and struck EE in the abdomen. This resulted in 3 sutures. EE returned to work for EE's next scheduled shift.

While using a grinder to cut a piece of metal guarding, the grinder kicked back and struck EE's left index finger causing a laceration. The lacerations was medically closed with 4 stitches. Employee did not miss any scheduled work days

EE was standing, when they went to turn and start walking they tripped. EE sustained a laceration to left pinky finger when EE caught themself on a pie pan resulting in 3 stitches.

An ignition occurred in the 3N miner section, while mining through a sandstone intrusion.

Mechanic stepped off a work scaffold and rolled/twisted EE's right ankle.

While drilling a hole to install a 20ft roof bolt, the top steel got stuck in the hole, while retrieving the steel out of the roof, it inadvertently fell pinching their right pinky finger between two steels, causing a laceration and fracture of the tip of their finger

An unintentional roof fall occurred in the 2 South Right Mains between 108 and 109 x-cut, in the No.10 entry. The fall was approximately 7' high x 12' wide and 40 ' long on the left side of the entry. The immediate roof consisted of laminated shale type material. Adverse geological conditions contributed to the fall. No injuries occurred due to this accident.

Employee was handling crib block when they smashed their right index finger between 2 cribs blocks, causing a laceration requiring 4 stitches.

IE was a passenger in a mantrip headed outby on the inby side of the mantrip, headed toward the elevator bottom. The mantrip entered a switch that had been left for the turn by an unknown individual. IE was thrown from the mantrip and suffered injury to EE's right knee. Injury was not the result of a training or compliance issue.

One miner was entrapped due to a faulty car buffer switch located on Archer elevator. The employee was entrapped for approximately 45 minutes. No employees injured and no other part 50 criteria applies.

Fractured Cap Rock fell from the open space between Long Wall Shield Canopy Tips and the Coal Face. The rock fell at an angle across the Shearer Ranging Arm and forced the Joy Global LCM assisting with technical service into an encumbered position by pinning their foot.

Employees were attempting to get the air lock door off the hinge to be moved. As they lifted it up, the door moved upward and the injured employee's hand accidentally got caught between the roof and the door causing injury to finger.

Employee was helping load belt structure from the scoop bucket onto a flatcar. The scoop rammed out, the structure slid off the flatcar and one of the chairs accidentally struck employee in EE's foot (fracture). Employee has not missed work. Also, EE was not diagnosed with a fracture to EE's toes until June 15, 2026 after the review of an MRI.

An unintentional roof fall occurred on the crabapple slope bottom in the #9 switch. The fall was caused by a slip fault running through the area above the primary anchorage. The fall measured 40ft long x 8-10ft high by 16ft wide. No injury or equipment damage due to this incident.

Employee was taking out a roller on the slope and handing it to another employee. While swinging it to the other employee, EE accidentally got EE's finger caught between the roller and the frame causing injury. Employee did not miss work; EE was scheduled off Monday, May 25 for Memorial Day.

Employee was setting a post at 24xcut in 7 South #6 track entry, to barricade off an area of concern. While tightening up a post a piece of rock dislodged from the upper portion of the rib and fell striking the employee. The employee suffered fatal injuries as a result of this event.

IE was a part of a crew setting posts at right track, 24 wall in the mains. The crew set 2 posts and was in the process of setting a third when a large rock dislodged from the rib. IE dove out of the way of the incoming rock, EE's hard hat fell off and EE struck EE's head on a scoop parked in the crosscut. Injury was not the result of a training issue.

Employee was bolting when a piece of the rib slid off the ventilation tube and struck EE's neck and left shoulder. Employee was diagnosed with neck contusions and a possible concussion. EE was taken off of work the same day 5/19/26.

Employee was standing next to the loader straighten out the rock dust to rock dust at the end of the shift, when a piece of the rib rolled out from the top of the binder striking EE in the right ankle. Employee was treated with a fracture to EE's right ankle.

EE was operating the headgate side of the shearer, while snaking back into the coal block employee was standing 5 shields away when EE was struck by a piece of coal measuring 3"x4"x1" thick resulting in a fracture of the Maxillary sinus.

An unintentional roof fall occurred in the H7 Tailgate section, #2 entry, inby 36 wall. The fall was approximately 7' high x 16' wide x 30' long. The immediate roof consisted of laminated shale type material. Adverse geological conditions contributed to the fall. No injuries occurred because of this accident.

EE was pulling shields in, one advanced striking employees feet resulting in 2 broken toes and 2 broken metatarsals.

Employee was side bolting when a rock fell from above the rib protector striking EE causing lacerations. EE was treated with sutures to close the lacerations. EE was released to work EE's next scheduled shift.

IE slipped while descending some steps in the prep plant. IE did not maintain three points of contact while on the stairs. Injury was not the result of a training or compliance issue.

Employee was spraying with a hose under tail drive on belt. EE stood up and a chain swinging from overhead structure accidentally hit EE in mouth and chipped a couple teeth.

Employee was in the process of removing a stuck bolter wrench from the drill pot on a continuous miner when the wrench suddenly became free while being pulled on, striking the employee in the face. The employee sustained a fractured nose

EE was operating the headgate shearer when a rock came off the drum, hit the handrail then hit EE's leg. This caused the EE to fall on EE's side into the base lift jack. The EE sustained a cut to back of EE's left leg that required 6 sutures and the fall resulted in 2 fractured ribs. Rock was 1'x1'x6"thick.

While walking out of the Aleppo portal locker room, employee slipped and felt a pain in EE's right knee. Employee was diagnosed with a torn Meniscus and subsequently taken off of work.

EE was loading ventilation tubes into battery scoop bucket while it was de-energized. EE asked the operator to lower the bucket on float while dragging the tube into the bucket, not realizing EE's foot was under the bucket. The bucket was lowered onto EE's right foot. All X rays were negative, the EE is off due to swelling.

While employee was center bolting in the #3 entry of the 15L Tailgate Section, the center bolter lost suction to the drill head. While employee was feel for suction with EE's left hand, EE inadvertently drop a drill steel on EE's left index finger. This caused a laceration and a fracture to EE's finger. The employee was subsequently taken off of work.

Employee was using the fuel truck to grease the 815F compactor. EE was attempting to reel back up the grease hose when EE took a step backwards and fell. When EE fell EE's tailbone/back landed on the front blade of the compactor.

Employee was removing a piece of Monorail from the 14L Longwall when EE felt a pain in EE's left shoulder. The employee finished EE's shift and worked all scheduled shifts until being evaluated and diagnosed with a torn rotator cuff and scheduled for surgery to repair

The Crabapple elevator was inoperable for an extended period of time due to a short in the trailing cable. The short occurred in the area of the cable that continues to flex, the cable did not rub against anything. An extra pair of wires that was already ran was used to correct the issue. No entrapment, injured or equipment damage as a result

While the employee was rigging & lifting a gang box/metal crate, the box/crate began to swing oddly and ended up pinching the employee's right hand & wrist between the wall & the box/crate. Employee ended up needing seven stitches and was put on restrictions.

Employee was using a slap hammer to remove a staple on LW shield and accidentally hit hand against hammer and shield causing injury.

EE was performing maintenance on the TG Drive when a piece of stone fell from between the shields striking top of EE left hand. This caused a laceration that required 9 sutures.
