Employee was bolting when a piece of the rib slid off the ventilation tube and struck EE's neck and left shoulder. Employee was diagnosed with neck contusions and a possible concussion. EE was taken off of work the same day 5/19/26.
Employee was standing next to the loader straighten out the rock dust to rock dust at the end of the shift, when a piece of the rib rolled out from the top of the binder striking EE in the right ankle. Employee was treated with a fracture to EE's right ankle.
Employee was side bolting when a rock fell from above the rib protector striking EE causing lacerations. EE was treated with sutures to close the lacerations. EE was released to work EE's next scheduled shift.
IE slipped while descending some steps in the prep plant. IE did not maintain three points of contact while on the stairs. Injury was not the result of a training or compliance issue.
Employee was spraying with a hose under tail drive on belt. EE stood up and a chain swinging from overhead structure accidentally hit EE in mouth and chipped a couple teeth.
While walking out of the Aleppo portal locker room, employee slipped and felt a pain in EE's right knee. Employee was diagnosed with a torn Meniscus and subsequently taken off of work.
While employee was center bolting in the #3 entry of the 15L Tailgate Section, the center bolter lost suction to the drill head. While employee was feel for suction with EE's left hand, EE inadvertently drop a drill steel on EE's left index finger. This caused a laceration and a fracture to EE's finger. The employee was subsequently taken off of work.
Employee was using the fuel truck to grease the 815F compactor. EE was attempting to reel back up the grease hose when EE took a step backwards and fell. When EE fell EE's tailbone/back landed on the front blade of the compactor.
Employee was removing a piece of Monorail from the 14L Longwall when EE felt a pain in EE's left shoulder. The employee finished EE's shift and worked all scheduled shifts until being evaluated and diagnosed with a torn rotator cuff and scheduled for surgery to repair
The Crabapple elevator was inoperable for an extended period of time due to a short in the trailing cable. The short occurred in the area of the cable that continues to flex, the cable did not rub against anything. An extra pair of wires that was already ran was used to correct the issue. No entrapment, injured or equipment damage as a result
While the employee was rigging & lifting a gang box/metal crate, the box/crate began to swing oddly and ended up pinching the employee's right hand & wrist between the wall & the box/crate. Employee ended up needing seven stitches and was put on restrictions.
Employee was using a slap hammer to remove a staple on LW shield and accidentally hit hand against hammer and shield causing injury.
Employee was a passenger in a locomotive, EE had EE's hand placed on the wind-shield frame when the windshield inadvertently closed with EE's fingers in the track. Employee suffered 2 fractured fingers as a result.
Employee was using a bar to adjust a coupler on a trash car to secure it to a mantrip. As they were prying the couple up with the bar they felt a pain in groin area. The employee was seen by doctor on 3/20 and was diagnosed with hernia. Employee has currently missed no work and has worked without restrictions. Surgery is scheduled for 4/17 at which point it will be a LT.
An unintentional roof fall occurred the front end of the previously mined 12L section. This fall was in the #1 crosscut between #6 and #7 entry. The roof in this area previously identified as an area of concern and was supported and dangered off. It has been noted that a stream valley runs though this area and may have contributed to the fault. approx 30'by 13.5' by 7' high.
EE was cutting a discarded piece of belt with a belt utility knife. EE was cutting away from self and snapped the blade off. A small piece of the broken blade was lodged in the belt and as EE's momentum and EE's right linear distal medium forearm was struck against causing a 2CM laceration.
Employee was cutting a belt when EE's hand slipped and EE cut EE's left pinky
The Employee was attempting to lift a cribbing block out of the back of a pickup truck while fully extended over the tailgate. In the process of lifting EE felt a pop in EE's right shoulder and noticed a lump. EE later noticed a bruise on EE's shoulder
A Bailey Prep Plant grader operator was exiting the grader when the wind blew the door open and struck the employee. They fell onto the blade of the grader and hit their tailbone. They felt pain in the tailbone area. They finished their shift with the crew.
Employee got EE's foot caught and twisted EE's knee while repairing a sump jack on a Continuous Miner. EE did not feel immediate pain but began to experience pain when EE exited EE's mantrip at the end of EE's shift. EE worked all shifts until being taken off and scheduled for surgery to repair a torn meniscus on 5/5/2026
IE was assisting placing new floor section into a vessel. The crew was in the process of lowering it in with an air hoist. The floor plate shifted and the employee's ankle/foot was wedged between the floor section and the feed impact plate. Injury was not the result of a training or compliance issue.
IE was shoveling the tunnel belt. As EE was shoveling under the stairs, EE bumped EE's head into the stairs. EE continued shoveling until EE bumped EE's head into the stairs again. Injury was not the result of a training or compliance issue.
Employee was operating an excavator on a hillside. The employee stepped out of the equipment and when they stepped onto the ground they slipped on ice and fell on left shoulder. Incident occurred on 01-06-2026, however, initial doctors assessment did not show any injuries or restrictions. Follow up assessment was conducted today 02-03-2026 and a scan showed a torn muscle.
Employee was stepping onto a general car when they slipped. This caused them to strike their left leg on the general car. The laceration was treated with 1 stitch. The employee returned to work with no lost work days.
An unintentional roof fall occurred at 6 South 101 x-cut between 7 & 8 entry. The fall measured 14ft wide and 16ft long. Initial height measurements did not indicate that the fall was above the primary support. Additional material fell causing this to become a reportable incident. The fall was caused by adverse roof condition in the form of deteriorated laminated shale.
Employee was driving a mantrip out of the 6K section. There was a broken fish plate on one side of the rail that caused the rails to be uneven. EE's mantrip hit this rail causing EE to come to a abrupt stop. EE fell out of the operator's seat and struck EE's head on the equipment frame causing a laceration.
Employee bent over to pick up a cable sling for PVC pipes and felt a pop in the center of EE's lower back.
Employee was installing 20ft center bolts in 1M headgate section. As employee was changing out drill steels, they lost their grip on the steels in the roof, causing them to fall striking the top of their left hand. Employee was diagnosed with a fracture to the third and forth metacarpal.
Employee was unloading props from a flat car. When EE cut the band, the props fell from the car. As the employee stepped out of the way, EE tripped and was struck in the hand by a falling prop. Injury was not the result of a training or compliance issue.
An employee was emptying a muck bucket that was filled with water to prepare it for a man trip. During this activity, the bucket started to rotate and dump the water towards the employee. The employee stepped in to stop the rotating of the bucket with EE's right hand and caught EE's middle finger between the bail stop and bail. The employees received 5 sutures, and small fractures.
Employee tripped over loose material in a crosscut. Injury was not the result of a training or compliance issue.
Employee was carrying a beam with another employee to put on top of bolter. The employee in the back tripped on rough bottom and dropped the beam. The beam accidentally slid, and inadvertently, the injured employee's finger got pinched between the rib and the beam.
Injured was prepping to drop roller and the grinder came out of their grip from kickback when cutting.
Employee was putting a high line hook of extra bits in the parts car at the load center. When EE let go of the hook, the end of the hook caused a laceration to EE's left index finger. Employee received sutures so close the laceration and treated and released to return to work. No time was lost.
Employee struck by screen deck during deck replacement. Employee struck by deck between deck and chute of screen in the hop area. Initial treatment sought on 11/16/25 with a follow up to specialist for final diagnosis on 12/1/25. Final diagnosis of pelvic and tailbone fracture. Employee was not pinned.
Employee was removing a piece of rigging from a screen deck. Employee positioned themselves between the screen deck and a piece of metal chute. Deck shifted forward striking the employee. Deck did not pin employee. Deck was supported by two additional pieces of rigging at the time. Pelvis fracture
Employee was picking up K-doors with crew to put on the hinges. A piece of metal skirting came loose from the door and slid and accidentally caused employee's thumb to get caught between two pieces of metal.
An unintentional roof fall occurred in the 4 West area of the mine at 8 xcut. This event occurred in the track entry and extended through the crosscut to the belt. The fall measured approx 80ft wide by 40ft long by 20ft high. The fall was caused by adverse roof conditions in the form of deteriorated laminated shale rock.
Employee was traveling walking through a low area and caught right ear on a piece of metal roof mesh causing a laceration. Employee was treated with 4 stitches and cleared to return to work.
Walking down stairs from the 5th floor in 1A to the 4th floor 1A. Felt the knee twist as walking down the steps.
The employee's pant leg got caught up in a fallen down branch while walking a stream for environmental monitoring. Upon trying to free EE's pant leg from the branch, EE lost EE's balance, fell and cut EE's left palm on a rock, requiring five sutures.
Employee was bolting on the side of the miner in 7 South left section. As EE was pushing the bolt into the hole, the expander caught on the hole causing the bolt to bend. EE's hand was pinched between the bolt and the strap jack. Employee received 4 stitches and was treated and released, no lost time.
Employee was getting out of the shuttle car and stepped into a tire rut. EE twisted EE's right leg as EE fell to the ground. EE felt a pain in EE's right knee. EE continued to work all scheduled shift until being taken off of work to repair a torn ACL ligament in EE's right knee on 10/13/2025
Employee was assisting in loading a piece of rail onto a car. EE's right hand was caught between the rail and the bed of the car as another employee shoved the rail into position. Injury was not the result of a training or compliance issue.
While bolting off of the miner in 5K CM section, a piece of rock fell while the employee was switching out the drill steels, pinching EE's hand between the drill steel and the rock. This caused a laceration that required medical attention. Employee missed scheduled days worked as a result.
The Aleppo Elevator as taken out of service for an extended period of time. The elevator was showing a fault on the depth sensor. The system was not able to identify where the elevator was in the shaft. It was identified that the ETS Switch needed replaced. The elevator was able to run on inspection mode. No persons hurt or entrapped due to this condition
Employee tripped while walking on the 4K longwall face. EE tried to catch self as EE fell and felt a pain in EE's left arm. EE continued to work all scheduled shifts until seeking medical treatment on 10/14/25 to treat a torn bicep muscle. EE began missing time on 10-14-2025
While employee was walking to the end of the track. They tripped and felt a pain in their right ankle. They had a follow up appointment on 9/9/25 and was diagnosed with a sprained ankle and subsequently taken off work at that time. Lost workdays began 9/9/25.
Employees were pulling a CMI door closed with a come-along after changing a screen. The hinge of the door broke, and the door fell, striking the IE on the collarbone. Injury was not the result of a training or compliance issue.
Employee was cutting belt to take out slack to get thru door, belt shifted and knife slipped and accidentally struck employee in the leg area causing injury.
Employee was a bolter helper that was installing roof bolts, Operator lowered the bolter head and struck the employees hand between a drill steel and the bolter head. This caused a laceration that stitches were used to close the laceration.
An unintentional roof fall occurred at 105 xcut in 6 south area of the mine. This fall occurred in a belt line intersection. The fall measured approximately 50ft long by40ft wide by 20ft high. The fall was caused by adverse roof conditions created by two stream valley intersecting at this area. Depth of cover in the area was 750 ft and was originally mined in March of 2022
A unintentional roof fall occurred in the #1 entry of 6 south @ 61 crosscut. The fall measured approx. 50ft long by 8ft high by 16ft wide. The fall occurred due to unconsolidated laminated shale above the primary anchorage zone. A shift in the adverse roof caused is to fall. Depth of cover 1020
Employee was standing on the pan-line chain when a rock fell from the roof, at the tip of the shields, and flopped toward the employee. The rock struck the employee and pinned them against the panline structure. The rock was approx. 9 ft long by 5ft wide by 16inch thick. This incident resulted in fractured legs.
An unintentional roof fall occurred in the 14L miner section. This was located in the #2 belt entry between #1 and the #2 crosscut. The fall measured approximately 65ft long 16ft wide and 20ft high. The fall occurred as a result of faulted area due to horizontal stress that occurred causing the strata to separate. No injury or equipment damage as a result of this incident.
While the employee was hanging cables, EE slipped and fell backwards twisting EE's right ankle. EE worked all scheduled shifts until EE was treated for a sprained ankle on 8/21/25 and subsequently taken of work. Lost work days began 8/21.
A Bailey Prep Plant employee was in the process of installing new CMI belts. As EE stood on a working platform, the anchorage points of the platform failed allowing the employee to fall down the A1, A2 lean coal chute. In the process of falling down the employee struck EE's right knee against the opposing platform, causing a laceration below EE's right knee cap.
An unintentional roof fall occurred between 71 and 72 x-cut in the number 2 entry of 4 South. This area is not an active area and is the travelway for a weekly exam. This area was previously supported with floor to roof supports in the form of turbo props. Adverse geological conditions due to horizontal stress and age of this previously mined area contributed to the deterioration.
EE was cutting a 2'x6'x10' piece of wood in half. EE was cutting with a circular saw. EE's co-worker was holding onto one side of board while EE was holding other side. EE had almost cut completely through the wood. The board dropped and EE's left hand slid forward into the running saw blade. EE has lacerations middle and ring finger and EE severed EE's pinky finger.
Employee was operating the headgate side of the longwall shearer. As they were cutting to the tailgate, the employee tripped. They struck their right calf off of the toes of the shields when they landed on the ground.
Employee was cutting up old belt to depose of it. While cutting the knife blade broke and slipped and cut into their left upper thigh. Employee received two stiches.
Employee was attempting to get out of the skid steer. When EE stepped out EE's right boot got caught on the bucket causing EE to fall to the ground and land on EE's ankle
Employee was cutting a rope off of a tarp off of a handrail. They knelt down and on right knee and started to cut the ripe free and the wind was blowing and they slipped with the knife and cut into their left knee on the inner side above their knee. They received four stiches.
Employee was attempting to lift a spool of telephone wire from a storage trailer on the refuse pile. Employee felt a pain behind EE's right shoulder.
Employee was cleaning rock off of the top of the miner when a piece of rock slid off of the miner head and struck them on the right foot. Employee was diagnosed with a fracture to their little toe (metatarsal)and subsequently taken off of work
Employee was walking between a piece of equipment and the coal rib, when they struck their right forearm on an installed rib strap causing a laceration. The laceration was treated with 5 stitches, employee returned to work with no lost work days
Employee began experiencing shoulder pain on May 5th 2025. Overhead strain handling a rock dust hose caused the pain to increase. It has been diagnosed that tendinopathy with a partial articular surface tear is causing the pain. Majority of damage caused by repetitive movement/strain. Employee worked all scheduled shift until taken off on 11/5/2025.
An unintentional roof fall occurred in the 13L longwall at 43x-cut. The fall occurred between the 2-3 entry and extended into the #3 entry. The fall measure approx. 30ft long by 16ft wide by 20 ft high. The fall occurred in a fault line area due to adverse geological condition in the form of laminated shale that separated above primary support.
Employee retrieving a piece of steel plate from plate rack. While bending over another piece of punch plate contacted employee causing laceration to employees left cheek. No Lost time or restrictions. 4 Stitches administered.
Employee was setting a vertical pin keeper on longwall face and using a horizontal pin to hit it in place. Employee accidentally hit left finger with the pin causing injury.
Employee was looping shield hoses and walking by another employee that was removing a staple using two hammers to pry out the staple. When striking hammer, a piece of metal from the hammer accidentally struck employee's arm causing injury.
Contractor bent over a generator panel to change an alternator belt. This caused a back injury.
Bailey Prep Plant employee was climbing onto a screenbowl to install the spray bar when they felt a pop and after some time felt a burning sensation. The employee began to miss work on 03-24-2025.
Employee was working with a 30mil chain on the Crabapple slope bottom while hooking up supply cars. EE pinched right middle finger between the chain and the general car causing a laceration. EE was treated with 6 stitches and released for work. EE returned on next scheduled shift.
An unintentional roof fall occurred in 6 south area of the mine at 102 xcut in the #5 entry (Track). This fall measured 18ft long by 8ft high by 50 ft wide. The fall was caused by adverse roof conditions in the form of laminated shale that separated above the anchorage point.
At #6 EE was parked beside the refuse shop. EE exited EE's pickup truck to take supplies to a truck driver that was in line at the bin. When walking back to EE's truck EE slipped on the ice and fell backward onto back. When EE fell EE's hardhat came off and ended up under EE's top of back/neck area. Employee did not miss work until 2/23/2025.
Employee was assisting another employee push a 16 ft cable bolt into a drilled hole and felt a pain in EE's left bicep. EE continued to work all scheduled shifts but was taken off to repair a torn bicep muscle on 2/7/2025.
Employee has been experiencing left shoulder pain that continues to get worse while preforming normal job duties, After seeking medical attention the employee was diagnosed with glenohumeral degeneration that has occurred over an extended period of time. Employee has worked all scheduled shifts until EE was taken off for treatment on 6-25-25.
An unintentional roof fall occurred in the 4 west track entry at 5 x-cut. This fall occurred due to adverse geological roof conditions that caused the roof strata to separate above the primary bolts. This condition occurred in a 4 way intersection and measured approximately 20 ft wide 25 ft long and 12 feet high. No injury or equipment damage as a result of this incident
Employee operating hoist had their right ring and pinky finger smashed after running the hoist block onto their fingers. Injury was not a result of lack of training.
Employee lost control of a high pressure wash down hose while hosing the right side of the stage loader in 12L. The hose struck EE in the face causing fractures to EE's face.
IE was using a tag line for a suspended load when EE stumbled over a traffic cone causing EE to fall to the ground injuring left ribs and elbow. After medical evaluation, IE had no broken bones but did receive one suture in the left elbow. 2 days later IE was having issues with pain, went to ER and confirmed 3 broken ribs.
Employee was grabbing the hook of a top roller in which was halfway off of the bottom belt. Afterwards, employee felt damage to their right hand.
Employee was a passenger in a mantrip that picked a switch and derailed. Employee struck head and right shoulder on the inside of the passenger compartment of the mantrip. Injury was not the result of a training or compliance issue.
An unintentional roof fall occurred in the 12L section at 14 Crosscut between the intake and the track entries. This fall fell from the center bolt of the xcut to the out by rib. Size approx. 25ft long by 8ft wide by 12-15ft high. Area was dangered off and will not be cleaned.
An Unintentional roof fall occurred in the 6 South area of the mine. This fall occurred at 101 X-cut in the #7 track entry. The fall measured approximately 31 ft long by 16ft high by 16ft wide.
Employee was operating a locomotive. The equipment came off of the track (D-railed) causing it to hit the ground. They braced to hit the ground by putting a hand on the frame of the windshield. The windshield bounced up and pinched their fingers causing a fractured middle finger.
A bolt broke on a chute and the chute landed on employee's right middle finger, causing a laceration. Injury was not the result of a training or compliance issue.
Employee was installing a roof bolt off of the miner when a piece of rock fell from the roof (apex) striking the employee. The rock measured approximately 2ft long by 2-3ft wide by 6 to 10inches thick. The rock fell striking the employee on the lower back and right ankle. Diagnosed with a fractured bone in right ankle
Employee attempted to uncouple a mantrip by hand. They reached for a coupler and attempted to disconnect. At the same time, another employee hit the hydraulic uncoupler on mantrip and then injured employee accidently got a finger caught between two couplers causing a laceration.
Employee was roof bolting off of the side of the miner when a piece of rock fell from the roof at the edge of the roof mesh and struck EE on the side/back of the neck head and shoulder. Piece of rock was approx. 16inches long by 6 inch wide by 2 inches thick.
While rib bolting off of the continuous miner in 7 South mains between 4 and 5 entry at 10.5 xcut, a piece of the rib fell from above the rib bolt striking the employee on the right leg causing a fracture.
Employee was operating the continuous miner in 7 South mains. A piece of rib fell out while mining between 4-5 entry at 10.5 X-cut and struck the employee pushing employee to the ground. The employee suffered 2 fractured ribs as a result of the incident.
Employee was rib pinning in the 14L Section, when a rock fell from the mesh and struck EE in the left ear. Employee finished EE's shift and was treated to close the laceration with 6 stitches. Employee did not miss any scheduled time as a result of the injury.
Employee was installing intersection bolts in the 7 South Left Section, when EE was removing EE's drill steels from the roof, it slipped and struck EE in the bottom lip. Employee finished EE's shift and was treated for a laceration with 3 stitches to close. Employee did not miss any schedule time as a result of EE's injury.
Employee was removing debris when a piece of rock fell between two shields and struck EE in left hand. EE was wearing EE's metacarpal gloves at the time. Injury was not the result of a training or compliance issue.
The employee was cutting a crib bag fill hose with a cutting tool and in the process cut left index finger, causing a laceration. This incident did not involve any aspect of compliance with rules and regulations; mine equipment or mining system; job skills and miner proficiency, training and attitude; or protective items relating to clothing, or protective devices on equipment.
Employee was resting left hand on the frame of the Headgate drive when the shields began to push the drive forward, EE's fingers were pinched between the HG drive frame and a cover. This caused a fracture to EE's ring and middle finger on the left hand. Employee did not miss any scheduled work days as a result of this incident.
Employee passed out while looking for tool on floor of transfer tower at crabapple slope. Cause of due to pre-existing medical conditions with BP & heart. Bruised ribs were result of falling over. First aid, no treatment. EE fell over due to light head. Immediately conscious. EE could immediately return. Having diagnostics for pre-existing medical condition.
Employee was assisting changing bits on the tailgate shearer drum. When a rock fell from the tip of the shields and struck the employee on the back of the head and neck causing a laceration. The employee received staples to close the laceration and is being treated for lower back issues.
EE was walking on tight side of belt at 7 wall 2K long wall and went to step over coal accumulation and a rock that was about 2ft. wide and as EE was slipping EE caught EE's left foot under the rock and twisted EE's left knee.
Employee was removing excess jabco cable from the 7 South Belt, when the employee struck left hand with a hammer causing a laceration that was medically treated with 6 sutures to close. Employee did not miss any scheduled time as a result of the injury.
Employee was carrying a middle roller weighing about 4.5 lbs. Employee slipped and roller came down on EE's left 1st finger.
The Crabapple brakeman car was taking out of service for an extended period of time due to a fault that occurred with the panel view touch screen. The panel view was replaced by another programmed unit and the car was again operational.
No injuries as a result of this incident. Plant operators were doing minor plant maintenance in preparation for beginning operations for the day. At Approximately 0540 hours plant personnel noticed a fire near the process chemical tanks, there was nobody working in the area before, after or during that time. MSHA, Fire Marshall nor we were able to determine what initiated the fire
Employee stepped through a man door and twisted EE's right knee. Employee was diagnosed with a fracture to EE's patella and was taken off work for rehab.
Employee began feeling pain in right knee while working in the Crabapple shop. Employee continued to work for several months until taken off of work on 8/15 and diagnosed with Degeneration damage to the right knee that resulted with surgery to repair
Swinging 12 lb sledgehammer at a block wall. Missed wall and hyperextended Rt arm and falling to the ground. Employee said EE felt like something popped in Rt shoulder and in the same shoulder 3 weeks prior with another contractor employee started EE never filled out a previous accident report.
Employee was cutting a piece of 1/4 tubing with a belt knife. The knife went through the tubing and inadvertently struck EE's left index finger causing a laceration that was medically treated with 6 sutures to close. Employee did not miss any scheduled time as a result of the injury.
Employee was repairing a pan jack hose fitting on the miner in 3K when a piece of rock fell from the roof between two roof straps striking EE. Employee suffered a laceration to the right ear as a result of this incident. Stitches were used to close the laceration. Employee did not miss any scheduled work as a result of this incident.
Employee was installing a "tilt out" bolt off of the miner in 13L section. A small piece of roof rock vibrated loose from the roof corner, while employee was drilling the hole. The rock fell striking the employee in the nose causing a laceration that was treated with 9 stitches. Employee did not miss any scheduled work as a result of this incident.
While swinging a sledgehammer to remove the pin on a ram car battery jack, employee felt a sudden pain in the left knee. Employee missed scheduled work due to knee immobilization.
The Crabapple hoist car was taken out of service for an extended period of time. This was due to a crack in the coupler support beam that was found during a routine examination. There was nothing on the slope track that indicates it would cause the damage. This does not affect the hoisting of personnel because it was the inby coupler which only affects the hoisting of supplies.
Employee was pulling a 2-inch discharge line and felt a pain in right shoulder. Employee was diagnosed with a torn labrum in right shoulder that needs surgical repair. Worked all scheduled shifts until being taken off of work on 4/10 for shoulder surgery.
A Bailey Prep Plant warehouse employee was operating a forklift when the employee inadvertently hit a center strap that was located on a pallet. The center strap kicked up and struck employee's right-hand, ring finger. Employee finished shift.
Employee was involved in the recovering of track rails in the 12L longwall section. The scoop that was being used during the process, struck a piece of the track causing the other end of the rail to move striking the employee on the left ankle.
A fire was detected on the 5 South #1 Belt starter located on Dry Ridge bottom. A breakdown of the insulation on the windings caused the secondary side of the transformer to go phase to phase causing the fire. The fire was contained within the fireproof enclosure and was extinguished with the use of water and fire extinguishers.
Employee was making a volcanize splice. EE was using a set of electrical shears to cut the rubber belt. While EE was using the tool, EE had put down pressure on the tool in which it cut up through the rubber and the metal tip came up and struck EE in the left thumb area below the last knuckle on the palm side.
Employee was working on belts and was cutting belt while on knees. The knife accidentally slipped and EE cut self above the knee causing injury.

Hoist operator was coming out of the shaft with an empty hook and came to an abrupt stop which triggered the slack rope switch. This caused a backlash in the load line cable causing the derailment of the load line from the point sheave.
Employee was using a chisel and hammer to free a dog bone connecting two pieces of panline. The hammer glanced off the head of the chisel, and struck the employee in the left thumb. Injury was not the result of a training or compliance issue.
The Crabapple Hoist car was inoperable for an extended period of time due to a communication fault between the surface computer and the underground computer. An additional processor switch was added with the same IP address causing the computers to lose communication.
The Dry Ride elevator inadvertently stopped while transporting an employee into the mine. This employee was stuck on the elevator for an extended period of time until maintenance personal could reset the frozen door switch on the top of the elevator. No injury or equipment damage as a result of this incident.
The Crabapple slope hoist car was taken out of service for an extended period of time due to a damaged hoist car wheel phalange. The phalange looked to be broken due to excessive wear over time. The wheel and axle were replaced by mine personal and the hoist was put back into service.
Employee tripped on uneven bottom and fell to the ground catching self with left hand on a sharp rock. This caused a laceration on EE's left hand requiring sutures to close. Employee did not miss scheduled work time due to this injury.
The Crabapple elevator was out of service for an extended period of time due to a faulty breaker. This breaker is for the backup battery that controls the charger for the dynamic breaks. After charging the elevator was put back into operation.
Employee was carrying and spotting J-Crib bags in the intake, when EE lost their balance and struck the right knee off of the ground. This incident did not involve any aspect of compliance with rules and regulations; mine equipment or mining system; job skills and miner proficiency, training and attitude; or protective items relating to clothing, or protective devices on equipment.
The Crabapple portal elevator was out of service for an extended period of time. The elevator was not operating at normal speeds and was taken out of service to troubleshoot. The VFD Drive PC was changed along with the Secondary slowdown switch. The corrected the issues with the elevator. It was tested and returned to service.
The Crabapple elevator was out of service for an extended period of time due to a faulty leveling switch on the upper landing. The switch was replaced, and the elevator was tested and was put back into operation.

An unintentional roof fall occurred in the 6 South area of the mine at 104 x-cut between the #7 to 8 entry. The fall measured approx. 40 ft long by 16 ft wide 7-10 ft high. It is believed that horizontal stresses caused by a stream valley affected the roof strata causing a separation that ultimately fell. No injury or equipment damage as a result of this incident.
An Unplanned event occurred on the 12L belt in which a broken belt roller caused a piece of the conveyor belt to wrap around it causing significant friction. This caused an area of the belt to melt in half. During this event a hot smoldering area with possible flames was extinguished with the use of water and rockdust.
While walking to re-hook a chain onto a track rail that was being removed from 11L longwall, the employee tripped and fell onto the track rail. EE's knee landed on the on the track rail. Employee worked all required shifts until being taken off of work for a fractured kneecap on 9/11/2023.
Employee was helping complete a belt splice and was trimming off some belt edges. EE was on EE's knees on the left side of the belt cutting off an edge with a belt knife. The knife accidentally slipped and cut EE's left forearm causing an injury.
Employee was installing a tilt out bolt off of the miner when a piece of rock fell and pinched EE's left index finger between the rock and the rib door that EE had their hand on. X-rays revealed a small fracture to the tip of the left index finger. No lost time as a result of this incident.
An unintentional roof fall occurred in the #1 entry of 6 South between 108 and 109 x-cut. Adverse geological conditions in the form of a "cutter" due to horizontal stress, ran up the left rib causing the roof strata to separate above the primary bolts. In later days this fall extended inby to approximately 80ft out by the 110 x-cut. No injury or equipment damage to report.
Employee was removing an obstruction from the walkway when employee felt a pain in left shoulder. Employee continued to work all required shifts until being taken off of work to repair a damaged rotator cuff on 11/2/2023.
Employee was removing a piece of metal from a machine. The last two bolts holding the metal to the machine broke and the metal fell onto the employee's right thumb causing a laceration.

An unintentional roof fall occurred in the #1 entry of 6 south @116 crosscut. The fall occurred due to adverse geological roof condition in the form of a stress cutter running up the left side of the entry that affected the roof support. The fall measured approx 35ft long by 17ft high by 16ft wide.
Employees were welding plastic pipe when the jaw of the pipe welder broke at the hydraulic cylinder. As the employees were attempting to remove the pipe from the jaws to remove and repair the damaged jaw, the chain holding the jaw up slipped off and struck the employee in the face. The employee recieved a laceration to the nose/cheek area and a broken nose.
Employee was a passenger in a mantrip that was traveling out of the mine after EE's shift. This mantrip struck another mantrip that was traveling in the opposite direction at 12 x-cut on the 2-east haulage. The injured employee sustained a fracture to the transverse process in EE's lower back.

While shipping old ram jacks off of the longwall face, the ram jack came out of the pan line and struck the injured employee causing EE to fall. As a result, EE received a laceration to left leg that received medical treatment. 9 Stitches. This employee returned to work the next scheduled shift, no lost time.
The Crabapple hoist car was out of service for an extended period of time due to a fault on the systems PLC card. The PLC card was eventually changed to correct the fault.
Employee struck in face by a Shive Puller while trying to remove shive from a Raw Coal Screen on 4th floor plant 2B.
While walking beside the loading machine, the employees hand struck a Pie pan rib support causing a laceration on left index finger knuckle. Employee received 4 sutures to close laceration. Employee returned as scheduled, no lost time.
While dropping the hoist car into the mine, damage occurred to the hoist car axle that caused the car to de-rail. The hoist car was taken out of service while the slope track was examined for derailment issues and the brake car's axle was changed. No track issues were identified.
Employee was helping spool out hoses to the bolters on the longwall. There was a lift of cable bolts along the way and the hose got caught on one of the cable bolts. The employee went to release the hose from the cable bolt when the cable bolt accidentally sprang back hitting the employee in the shin area causing injury.
Employee was working on the surface area of an underground mine when employee tripped over a 2" hose and fell. This incident did not involve any aspect of compliance with rules and regulations; mine equipment or mining system; job skills and miner proficiency, training and attitude; or protective items relating to clothing, or protective devices on equipment.
While bolting on the side of the miner a small piece of rib scaled off of the rib and struck the employee on the left index finger on 3/29. Employee continued to work all required shifts, EE continued to have discomfort in the index finger and went for an x-ray 4/25/23 and a fracture was discovered in EE's knuckle. Employee continues to work without restrictions.
The Crabapple elevator was out of service for an extended period of time due to two blown fused in the elevator control panel. It is unknown what caused the fuses to blow, Fuse 11 and fuse 12 was replaced and the elevator was again operational.
While hoisting a mining machine out of the mine, the slope track became damaged which took the CA slope hoist out of service for an extended period of time. There was no damage to the hoist or any other components except the track rail. The rail was replaced in this area.
While pre-shifting 11L miner section, a piece of rock fell from between two roof straps and struck the employee on the back/head and left leg. Employee was initially treated and released with no restrictions, but has since been taken off work for treatment. Employee began missing work days as a result of this incident on 3/6/2023
Removing a rear ram pin from shield when pin came out, the relay bar moved pinching right index finger between the relay bar and dog box resulting in laceration and needing 6 stitches
Employee has been experiencing shoulder pain for multiple years. The pain has been credited to repetitive over exertion as a roof bolter. Employee began feeling severe pain on 1/31 after preforming normal duties and began to seek medical attention. They worked all schedule work days until they were taken off for shoulder surgery on 4/10/2024
Employee was bolting on the 6 South Left Miner. While installing a 12-foot cable bolt, the expander became stuck. The cable bolt bent and pinched EE's right index finger between the bolt and the bolter mast. This caused a laceration that stitches were used to close. Employee did not miss any scheduled work as a result of the injury.
Employee was cleaning rock out of the cable trough on the longwall panline/ A piece of rock fell from between the shields and struck their left hand. Employee went to the hospital after the shift and was diagnosed with a fractured bone in the hand.
While preforming the required exams of the Crabapple Hoist rope, an area of the rope showed signs of deformation. The Hoist was taken out of service for the cable to be re-terminated.
The Crabapple Hoist car derailed as a result of ice build up on the slope track. The car was hoisted up the slope which caused damage to the Contactor Panel. The car was taken out of service to repair the damage.
Employee tripped and fell while walking past the loadcenter in 2K section. Employee used right arm to catch self and felt a pain in right elbow. Employee was diagnosed with a fractured Distal Humorous (small bone in elbow)
Employee was lifting a Barrel Claw to attach it to a 55 gal drum of oil to unload the barrel from a supply car when they felt a pain in left shoulder. Employee continued to work all scheduled shift until they were taken off of work on 1/19/2023 for shoulder treatment.
Employee was operating a locomotive and struck a broken rail which caused EE to d-rail. When EE came to a sudden stop EE struck EE's mouth on the inside of the operator's compartment causing a laceration.
While walking outby from the face past the loading machine, employee tripped on the cables and fell to the ground, landing on right knee and shoulder.
Employee was cutting the tabs on a bucket of rivets to get the lid open. EE was cutting the tabs left handed and was holding the bottom of the bucket with right hand. The bucket is plastic. So being it was cold outside the plastic was cutting harder than usual and EE was exerting more pressure to cut. When they cut through they cut their right hand.
Employee operating the outrigger on a leased truck and inadvertently had EE's r. hand in the hinged area of the outrigger when putting it up. Pinched EE's r. ring finger in the hinge of the outrigger.
Employee was lying on the ground on the walk side of the belt changing bottom rollers on 6 South belt. An employee was placing a roller on the top of the top belt when the roller fell of the top belt and struck the injured employee in the left hip.
The Crabapple Slope Hoist was taken out of service for an extended period of time due to deformation in the hoist rope. Possible cause for rope deformation - slack was put into the rope at the bottom of the slope causing the rope to double back on itself. The hoist rope was re-terminated to repair the deformation.
Employee working on impact channel. Channel rolled smashing employees left hand. Hairline fracture of wrist resulted.
During an MSHA inspection of the 6 South #1 escape capsule, on the 3rd trip of the capsule, the capsule got caught up on the guide cables at the bottom on the south side of the shaft. One of the cables was broke. The capsule was retrieved and the hoist was taken out of service to repair the cables on the bottom of the shaft.
An unplanned roof fall occurred in the #1 entry of 6 South area. The fall occurred at 102 crosscut in a return entry. The fall was caused by adverse roof conditions that affected the anchoring zone of the primary roof bolts. The fall was approx. 120 ft long by 16 ft wide by 12 ft high. No equipment damage or injury as a result of this fall.
Employee was pulling bits from shearer using a hammer and bit puller and accidentally was hit by a piece of metal in the arm causing injury.
Employee was using a pry bar to move a roller. The bar slipped out from under the roller causing EE's hand/finger to be caught between the bar and the belt structure causing injury.
The crabapple hoist was out of service an extended period of time. It was found that 2 safety central relays needed changed to repair the hoist. A possible cause was a power outage.
There was an unplanned roof fall in 4 west outby area at 22 crosscut between 2 and 3 entry. The belt nor the track entry were affected. The fall is estimated to be 40' long 16' wide and an unknown height.
There was an unplanned roof fall in 6 South section #8 entry 104 crosscut. Adverse geological conditions caused the roof to separate and break. It was measured 12' high x 60' long x 16' wide. "The conditions contributing to the fall in 6 South mains area was Adverse Geological conditions in the form of a fault line possibly caused by a stream valley"
The crabapple elevator was taken out of service due to faults from the leveling arm system. The leveling arm had seized up and needed to be lubricated to return to normal operation. The elevator returned to service at 7:50PM.
The mantrip that the employee was sitting in was struck by another mantrip traveling in the opposite direction. The injured employee braced themself and felt a pain in the right shoulder from the sudden movement. The employee worked all scheduled shifts until employee was taken off of work on 11/16/2022 to surgically repair the right shoulder.

While assisting a pan line bolter installing bolts, the injured employee's left pinky finger was caught in a pinch point when the bolter operator dropped the mast down. Injury was not the result of a training or compliance issue.
Employee was walking out of the 12J section, left foot stepped in a shuttle car rut causing employee to slip and fall. Employee felt pain in left ankle. Employee was subsequently diagnosed with a fractured bone in the ankle.
An unplanned roof fall occurred in the 6 south area at 49 1/2 wall in #4 entry. Adverse geological conditions caused the roof to separate and break. The roof fell at the crosscut connection of #3-4 crosscut and outby entry. It fell 15' into the 4-5# crosscut and inby entry.
The Dry Ridge Elevator was taken out of service for an extended period of time to address equipment fault alarms. The Main Drive computer as well as all encoders were changed to correct the issue
While removing a rock from the handrail on the 9L longwall face. The rock shifted and EE's middle and ring finger were caught between the rock and the shield causing a laceration. EE was assessed at Ruby Hospital where EE received 6 stitches.
Employee stepped on the rail, EE's foot slipped, and EE felt a pain in EE's left foot. EE was examined at Wheeling Med Express and was diagnosed with a fractured 5th metatarsal.
Employee was bolting on the miner and EE's left hand was caught between the ATRS jack and the frame of the machine. The ATRS pad slipped off a ledge when employee reached for a drill steel. Employee was treated for a fractured little finger and received 3 stitches, release to return to work.
Employee was cutting a flapper on the belt and the knife accidentally slipped out of their hand and came in contact with their forearm causing injury.

Employee was sitting on the cable trough waiting for tools to service the longwall shearer in 9L section. A small piece of rock fell from between the shields, bounced off of the head gate drum and struck the employee on the right leg. This caused a laceration that used stitches to close. Employee did not miss any scheduled work as a result of the injury.
The Crabapple elevator was taken out of service for a period of time due to drum warmth. Industrial Elevator (repair contractor) found that the brake rods were not fully releasing. They adjusted the rods so that there was no longer contact during operation.
