During the downswing, the track hammer struck the bent over employee on back side of hard hat causing small laceration to scalp, requiring sutures.
Employee was setting a 30 h.p. pump and shoveling material over boots when the bunker washed out. Employee then felt burning on both of his legs. He is being treated for chemical burns.
Locomotive derailed and employee was helping jack locomotive back on to the track. After jacking approximately 4", the load shifted and the jack kicked out striking employee in the face. Employee received stitches and is awaiting surgery to fix his fractured nose.
Employee was using a spinner wrench to tighten the cathead on the power center, as he was pulling up on the wrench it slipped causing the wrench to hit him in the mouth breaking teeth.

Co-worker cut bands on pallet on the outby end of a car while employee was standing on the inby end. When the bands got cut, the weight shifted and caused the banded rollers to fall off the car striking employee in left ankle causing a fracture. Started losing time 4/28/15.
REPORTABLE ONLY DUE TO SUTURES - Employee was attempting to loosen a clevis pin with pliers when the pliers slipped off, his left arm recoiled back striking a yield tube on the adjacent supply car cutting his arm requiring sutures.
REPORTABLE ONLY DUE TO SUTURES - Employee was holding a 3/8 chain that was connected to a water pump, when the other employee moved the equipment to get slack in the chain, the chain tightened up and a broken link caught his finger causing fracture and laceration to end of finger resulting in sutures to his finger.
Employee was helping lift a 50HP motor in the prep plant, using boom truck and chain falls, from off conveyor belt to drive base when the motor hung on something and suddenly came free employee raised his arm to shield himself from motor which forced his arm into an electrical cabinet causing fracture to arm.

Employee and coworker were loosening panel bolts on the shearer with a ratchet and socket. They were assisting one another so that the socket did not slip off the bolt head. When the bolt broke loose, the ratchet head pinched employee's right finger between ratchet head and shearer frame. Employee received 5 stitches on 7/22/13.
REPORTABLE ONLY DUE TO SUTURES - Employee was cutting a piece of corrugated pipe when the knife slipped cutting employee hand.
Employee was installing support arm for canopy and while using come-a-longs the chain came out of the clevis allowing the canopy to fall hitting employee's finger causing the tip of finger to be cut off.
Employee was helping install the plow on stage loader. The plow was held on by 2 pair of comealongs and 2 bolts. As the was being lowered the 2 nuts on the bolts popped off allowing the plow to twist and hit the employee on the foot. Employee was standing behind the plow.
Employee was tightening up a screw jack with a sledge hammer and it hit his left hand with the wooden handle against metal screw jack arm. He received treatment at DCH where he received four stitches.
While trimming a corner of belt skirt on feeder, knife slipped out of employee's hand cutting right lower arm. Employee was sent for treatment and received stitches.
Employee stated that he was using a pry bar when the bar slipped and struck employee in the left side of his bottom lip. This resulted in a laceration that required sutures.

After employee sharpened axe, he noticed axe head was loose. He secured the axe handle in vice grip and inserted two screws into the end of handle to tighten the hand to the axe head. When he loosed vise to remove the axe it slipped and he went to grab axe in his left hand causing a laceration that required sutures.
Employee was helping jack wrecked loco up. He was holding jack while another co-worker was jacking. When co-worker pushed down on jack handle hard, employee's finger was under it causing it to pinch and cut his right index finger. This required stitches. No lost time.

While working on the lotrac, wrench slipped and struck employee's left wrist against the lotrac. Fracture ulna.
