No 4 Mine Coal

Employee was running the miner on #6 Section in the #4 LOXC, when a rock EE loaded on the car rolled off the coal in the shuttle car and struck EE's right foot causing a fracture to the top of EE's foot.

Employee was running the miner in the #1 entry on #4 Section when a piece of blue creek seam (rib) rolled out and struck them and knocked them to the ground causing a fractured left hip/leg initially, but after further medical observations other injuries were determined.

(REPORTABLE DUE TO SUTURES) Employee was working on a 4" air line at 4-7 Fan compressor station when a 4" VIC coupling / bull hose blew off and struck EE causing a laceration to EE's left upper leg which required twelve (12) sutures.

Employee was standing in a crosscut while a group of miners were pulling belt together. During the pulling process, a rope accidentally broke and an object struck employee in the hand causing injury.

Employee was changing bits in the miner, using a pick hammer to knock a bit loose, when a piece of metal flew off and struck them in the left arm/elbow causing the metal to insert the skin resulting in a laceration and surgery to remove the piece of metal. (Started losing time 10/28/2025)

Employee was cutting in the #1 Entry on #8 Section, EE went to the offside of the miner (CM130) to slide the tubing out and when attempting to walk back to the operator's side EE was struck by a shuttle car (SC125) due to limited visibility causing fractures in EE's right lower leg (fibula bone).

Employee was traveling on a man bus on C-5 Track, XC #29, and slid into another man bus when they struck lower legs on the seat that was flipped in the down position in front of them causing a contusion to left lower leg (unable to apply weight to stand/walk) and an abrasion to right lower leg. *STARTED LOSING TIME 8/25/2025*

Employee had finished filling a tank for the rotary breaker oilers in the Prep Plant, when EE stepped off the platform and EE's right knee gave out and EE felt a pop causing a sprain to EE's right knee which later resulted in meniscus tears to the knee. (STARTED LOSING TIME ON 10/15/2025 DUE TO SURGERY)

Employee was hanging blowing curtain in the #1 Entry on #4 Section when they stepped into an uneven area (rut) with some loose rock and twisted right knee and felt a pop causing a tibial plateau fracture in right knee.

Employee was walking outby on 4 West #1 Belt checking for splices around XC #112 when they tripped over a rock in the walkway going over the old overcast and fell into the walk side of the belt structure and top side roller and caught their right arm between the belt and the roller causing a fracture/abrasion to their right lower arm and a contusion to the right elbow.

Intersection #112 in the #5 entry of the 4 West N-Panel seal line, was an old fault area that had a mixture of 4' resin bolts and 8' cable bolts with mesh in some areas along with some straps. It fell out just above the anchorage point of the resin pins with some of the cable bolts still hanging showing they were still secured in good roof.

Employee was putting connecting links in face chain and hit EE's thumb with a hammer causing injury

The crew was in the process of forming up walls to pump concrete around the steel sets. Employee was evaluating the roof conditions around the steel set area and unexpectedly, a rock fell from the top area striking employee and causing injury.

Employee was trying to exit an air lock door on C-1 H/G, and when EE started to push on the outer door, EE felt a discomforting pain/buckled feeling in EE's left knee causing a sprain. After MRI and receiving results on 6/5/2025, EE had a torn meniscus and MPFL and EE started losing time on 6/6/2025.

Employee was performing a pre-op inspection on the F250 pickup truck, and when they lifted the fire extinguisher out of the bed of the truck, they felt a pop in left shoulder causing a fracture to their left clavicle (collarbone).

Employee was laying track on the end of C-4 (L/W Setup) and while knocking track ties together, the rail slipped off the track jack the was underneath the rail they were working on, and as it was falling, it shifted over and caught the employee's right foot between the rail and the ground when it landed causing a contusion to EE's right foot.

Employee was de-trashing on North A Track XC #13, when attempting to uncouple the pipe car from the locomotive, EE flagged the other motorman to come towards EE to get slack in the draw head, and the pipe car struck the tip of EE's right foot while in motion causing a fracture and a laceration, which required 5 sutures, to EE's 3rd right toe.

Multiple layers of laminated slate rock were seen to have fallen out above anchorage and above the required depth of the test holes. A small fault can be seen on the outby corner of the fall and is the reason the entry was dropped off the mining cycle as the area was being developed. The fall is roughly 20 ft. by 20 ft, and ranges from 10-14 ft. thick.

Employee was helping unload a pipe car on the end of C-4 Track (#4 Section), XC #43, and while pulling pipe off the pipe car the pile of pipe broke loose and a 6" metal pipe fell and struck employee on the left ankle/lower leg causing a fracture to the left ankle.

Employee was roof bolting on roofbolter c/n RB130 in the #3 entry on #4 Section, when the platform broke, EE fell about 3' and landed on the ground, and when EE landed the platform struck EE on the left upper leg/thigh causing a contusion/traumatic hematoma to the upper leg/thigh.

An employee was examining the work area at the top of the ramp when, while descending, employee stepped on a rock, causing employee to fall and injure leg.

Employee was sliding on EE's back to exit shield #145 after finishing repairs when EE's feet got caught on the pontoon and the weight of EE's body while sliding pushed EE's knees under EE causing a sprain to EE's right knee. After further evaluation, EE had a torn posterior horn medial meniscus which required surgery on 9/30/2024. (Started losing time 9/30/2024)

A roof fall occurred when a big chunk of the left outby corner and inby brow fell out above anchorage (approx. 8' thick and 60' long) on top of the overcast at XC #63 on 2 West Belt. The fall broke the belt in two and damaged about 80' of belt structure.

Employee wrecked their motorcycle on road headed to job site, which was located on mine property.

REPORTABLE DUE TO SUTURES/DAMAGE TO PERMANENT TEETH: Employee was operating the roof bolter c/n RB130 on #8 Section when the drill steel started clogging up. When EE hit the drill steel with the bolt wrench, the wrench bounced back and struck EE in the mouth causing lacerations to EE's lower lip/chin which required sutures and a dental injury to EE's top front teeth.

Employee was shoveling the belt at 2 North tailpiece on #6 Section and when EE went to bump the belt, the tailpiece was pulled out and EE was struck by belt structure causing a laceration to the head which required staples, a laceration to EE's left lower leg which required sutures, and a contusion to EE's left shoulder.

(REPORTABLE DUE TO FRACTURE) Employee was bolting on #6 Section, #3 Entry and walking back around the drill pot, when the steel hit the roof and the bolter at the same time throwing EE off balance. EE's left ring finger was caught between the steel and the back bar on the operator's deck causing a contusion to EE's fingertip. Didn't receive medical treatment until 2/16/2024.

(REPORTABLE DUE TO DAMAGE TO PERMANENT TEETH) Employee was at the bottom of 4-17 holding the net on the service cage waiting on everyone to load up, when another employee bumped into EE with force. The bar on the net struck EE in the mouth causing dental injury to EE's bottom teeth. Dental appointment was scheduled on 2/19/2024.

Employee was carrying some empty oil cans to the scoop, when EE slipped/fell and hit EE's left foot and rolled/twisted EE's left ankle on the scoop bucket causing a sprain/contusion to EE's left foot/ankle which required EE to be placed in an air walker boot losing time from work starting 12/20/2023.

(REPORTABLE DUE TO SUTURES) Employee was pinning the right rib in the 2-1 slant entry on #4 Section when a piece of scale rock became hung up on EE's drill steel, when EE was attempting to remove the rock along with another employee, a larger rock rolled from the top of the middle man (rib) and struck EE's left lower leg causing a laceration which required several sutures.

Employee was pulling off roof bolter cable when the employee got shocked.

(REPORTABLE DUE TO SUTURES) Employee was struck in the face (forehead) with a 2" VIC 90 pipe-fitting when it blew off the fire outlet that EE was getting water out of, resulting in a laceration to the face (forehead) which required five sutures.

REPORTABLE DUE TO FRACTURE/SUTURES: Employee was using a pry bar to remove/change a stuck top center roller on 2 West belt when the bar slipped and EE's right-hand pinky finger was caught between the bar and the whole frame crushing the tip of the finger causing a fracture and a laceration which required sutures. (Reported back to work for EE's next shift on 5/8/23)

(REPORTABLE DUE TO SUTURES) Employee was pinning top on #8 Section when the pin EE was pushing got hung up and as EE applied more pressure to the pin the bolter wrench broke off at the chuck, flew back and struck EE in the face (chin area) and neck causing a laceration which required multiple sutures and on the right shoulder causing a contusion.

REPORTABLE DUE TO SUTURES: Employee was removing the flinger duster from the scoop on #6 Section when the chain and the binder that was holding it caught their left thumb between the chain and the bucket causing an abrasion and a laceration which required eight sutures.

REPORTABLE DUE TO FRACTURE: Employee was using a Hilti drill while bolting in the top to install a belt hanger in the service entry at XC-#79 on the Longwall, when the drill spun and EE lost control of the drill and it struck EE's right-hand ring finger causing a fracture to the finger. (Started losing time on 3/3/2023)

Employee was prepping to do some concrete work and had to move some rail. During the process, the rail dropped, and EE caught EE's finger between the rail and the track tie causing injury.

Employee was re-railing a wrecked shield car loaded with a shield back on the track, once they placed the car on the rails, they let both jacks down and one of the jacks released quick, kicking the jack bar out of the jack they were operating and struck them on the right side of face causing a laceration to the right earlobe requiring sutures and a fracture to the right cheek bone.

Employee was walking down the steps at the Slope Belt carrying a roll of rubber when EE slipped off the bottom step, lost EE's balance and rolled EE's left ankle causing a sprain to the left ankle. (Started losing time on 1/17/2023)

Roof fall occurred in the intersection of an idled entry with no damage to any equipment or any ventilation controls. Roof fall occurred at XC-25 on East C in the #2 entry intersection with no disruption to travel or air flow.

This is being submitted in conjunction with citation #9632689 being issued. It was some ice in the shaft, but no damage to the hoist or any controls of the hoist. The hoist would still function if needed but chose not to use the hoist from 12-23-22 through 1-2-23 due to mine being idled and only a few people being underground.

REPORTABLE DUE TO SUTURES: Employee was drilling a hole for a roof bolt when a piece of rock broke loose and struck them in the face between the chin and the lower lip causing a laceration to the area requiring sutures.

REPORTABLE DUE TO SUTURES: Employee was cutting a rubber mat with left hand when the knife slipped and struck right hand cutting the top of the hand behind the thumb causing a laceration which required eight (8) sutures.

Employee was re-hanging high voltage cable that had fallen from the overcast on #8 Section. They had almost finished hanging the cable when it fell again striking the right side of EE's hard hat, safety glasses and EE's right leg causing a laceration to EE's right ear that required 32 sutures, a contusion to the right side of the forehead, and a contusion to the right upper leg.

REPORTABLE DUE TO FRACTURE - Employee was replacing a top roller when the roller rolled off the belt and smashed EE's right thumb between the two rollers causing a fracture to EE's thumb. (Employee did not receive medical treatment until 5/2/2022)

Employee was struck by slider rail when the slider rail that was hung from a red hanger off the ground came loose and employee tried to catch the rail causing a contusion/hyperextension to right shoulder. (Started losing time on 3/8/2022)

Employee was walking on the screen side of the feeder (FD-127) to get an impact when EE's right foot slipped on a rock and EE's right foot went under the hopper on the feeder causing a fracture to 2nd and 3rd metatarsal bones on EE's right foot and a crush injury to the right foot/ankle/toes. Started losing time on 1/24/2022.

Employee was hauling a crossover strapped down on a duck-bill manbus when the crossover became lodged in the roof and got caught on some overhead cables and flipped up and struck employee causing a laceration to the left ear which required sutures.

While walking to mantrip, IE tripped on track switch and struck left shin on switch throw, resulting in laceration. Injury not the result of a compliance or training issue.

A roof fall occurred in the #6 entry of the North A Mains, in an intersection along the Northeast seal route at crosscut 27. The fall fell above anchorage and was approximately 7' high. No one was injured, had no effect on ventilation, and wasn't in a travel route.

Employee was standing in the last open x-cut while assisting the scoop operator service the #2 entry of the #6 Section, after telling the operator that EE was clear, the operator proceeded to scoop, a 4X4 curtain timber had fallen and the scoop bucket caused the timber to pivot and strike the lower left leg causing an abrasion (deep muscle bruise).

At 12:28 am, a fire was discovered at the 7 West belt drive. The belt had started running off and was rubbing the frame of the 7 West drive. The material under the drive caught on fire and two men that discovered the fire put the fire out with two fire extinguishers.

An ignition occurred at shield #58 on the E-7 Longwall panel face when the shearer was attempting to cut through a sandstone intrusion that had dropped down in into the coal seam from the top. When investigated by MSHA and company personnel, only one worn bit was found that might have caused the ignition to occur.

Removing a track pad from a Cat D11 dozer. Picked up track pad and pivoted catching pinky finger on left hand against adjacent track pad. Impact caused a small laceration requiring stitches

Employee was on 6 Section drilling a rib pin and was having trouble with suction and went to pull the steel out and lowered the pot and left hand index was caught between the mast and the pot of the roof bolter causing a severe fracture to the finger.

Employee was pulling on a piece of pipe. It broke loose and hit employee in the mouth causing injury.

Bolters were bolting crosscut from #3 entry towards the #2 entry but crosscut had not been cut through to the #2 entry yet on the #4 Section. As the bolter started to install the outside roof bolt, an ignition occurred and the flame was put out by fire extinguisher in less than one minute.

An unintentional roof fall occurred in the left return outby the #6 Section in the #4 entry of old headings. The roof fall was approximately 40' long, 16' wide, and 7' high.

CM Operator had cut their first 10' lift in a fresh cut. CM operator was going to set over after loading the car, and as the CM cutting head touched the top, the ignition occurred. All proper steps were taken immediately. Upon investigation, the Company & MSHA investigators found that the operator was following the approved plans & no citations were issued for this ignition.

(REPORTABLE DUE TO SUTURES) Employee was pulling a chain to unhook from the supply car when they slipped/fell forward hitting the right side of face on the draw head of the car causing a laceration which required seven sutures and a contusion.

Employee was operating LM86 and was coming down the North B Track when the high velocity air caught the windshield of the locomotive, flipping the windshield up causing the windshield to smash left hand ring finger causing a crush injury resulting in a fracture and lacerations requiring sutures to the finger.

(REPORTABLE DUE TO SUTURES) - Employee was hammering a hammer when a piece of the metal shard from the hammer shot off going into the skin of employee's left hand thumb finger causing a laceration that required two sutures.

Employee was using a 4 pound shop hammer to drive a nail into the fly pad when they missed the nail and struck the tip of right hand thumb finger causing a laceration and crush injury which required sutures. Started losing time due to this injury on 9/5/2020.

(REPORTABLE DUE TO SUTURES) - Employee was operating a man bus (PT-143) while traveling outby on the East C track when a piece of rib roll (a rock) slid down the side of the bus and struck employee's left knee causing a laceration and a contusion to the left knee receiving sutures to the laceration.

Employee was holding up the hood cover of a locomotive (LM66) with one hand looking for a chain in the engine compartment, then EE looked back for something and the hood cover slipped out of EE's right hand and struck EE's left hand causing a fracture/laceration to the tips of EE's left hand middle finger and EE's left hand ring finger.

An ignition of methane gas occurred on the Section #6, No. 2 Face entry on 8/7/2020 at approximately 0740 am due to the methane check in the No. 2 Face found 1.2% to 1.8% methane present and there were bad bits in the cutter head drums and sand stone intrusion.

Employee was at the 1 West dust hole when EE was struck by a 4" bull hose from the dust tank after filling the dust tank with dust causing a fracture to the lower back, a contusion to the right ankle, and a contusion to EE's left wrist.

Employee was helping to put a rock in the pan when the rock shifted, fell, and struck EE's left foot pinning the foot against the relay bar causing a contusion to the left foot.

Employee was carrying a bottom roller when EE slipped losing footing (balance) and pinched left pinky between the roller and the edge of the manbus causing a laceration to the tip of the finger. (Laceration caused for 4 sutures)

Employee was walking down the hill in loose gob on the LW setup when EE slipped/fell twisting EE's left knee causing a sprain to the left knee which resulted in torn cartilage and a small tear in the medial. (EE went off for surgery due to this injury on 8/21/2020.)

REPORTABLE DUE TO A FRACTURE - Employee was placing oxygen bottles in the bottle rack when one of the oxygen bottles fell over and struck right hand index finger causing a fracture.

REPORTABLE DUE TO SUTURES - Employee was setting beams on top of trailing shields when something fell onto one of the beams causing the beam to strike EE's right arm causing a laceration that required EE to receive five sutures.

Employee was jacking up the power center on the E9 track, when a jack bar fell striking left hand causing a fracture/laceration to left hand middle finger.

Employee was attempting to put a gob side dog bone on the tailgate drive and was pinned between the winch rope and the pan door causing a fracture to EE's right arm.

Employee was unloading a tail roller at #91 kickback on the West track, once employee got it unchained, employee stepped to the side of the car and it rocked over causing the roller to slide off striking lower left leg.

Employee was at the bottom of the West cage and was throwing a switch, when EE stepped on it to make it close all the way EE slipped off rolling EE's left ankle causing a bad sprain to EE's left ankle/foot.

An ignition occurred on the #7 miner section in the face of the #2 top left side of face 15 ft across face.

Employee was running the shearer on the tailgate side of the mid face when EE pushed a rock off of the handrail into the chain and EE's foot slipped causing EE to fall forward against the handrail over extending the elbow causing hyperextension/strain to EE's right elbow.

An ignition occurred on the #7 miner section in the face area of the #4 entry while the continuous miner was producing coal.

Employee was riding in Manbus #144 from #8 Section to the Bottom when they struck a Locomotive sitting at crosscut #90 on the West Track causing a strain to EE's neck. (Started losing time on 2/22/2020)

Employee was troubleshooting a cutter motor on #131 miner on the #1 entry, kneeled down on the operator side of the miner by the screen read out and a piece of rock rolled out and struck lower back causing contusion to the back area and chest wall.

Employee was working at the ROM when EE slipped/fell down the stairs on the 3rd floor of the Prep Plant landing on the left hip causing a contusion and affecting herniated disc. (Started losing time on 2/29/2020)

Employee was making a splice on ram car jumpers and when EE hit the connect EE was putting on, EE's hammer glanced off to the side striking EE's left thumb causing a laceration resulting in needing sutures.

Employee was helping to reinstall hinge pin on shaft collar doors when they placed a foot in the wrong location, allowing the cylinder base to come down on top of their left foot.

Employee was carrying monorail down the belt when they slipped/fell on some mud fracturing left ankle.

Employee was carrying metal for side boards through double doors and one end caught the edge of the door and slipped from hand and struck foot causing four fractured/broken bones to the metatarsal of left foot. (REPORTABLE DUE TO FRACTURE AND LOST TIME)

Employee was repairing an aqua pipe when ratchet slipped off bolt causing them to lose balance and twist right knee resulting in a sprain/torn ACL. (Became a LTA starting 11-27-19)

Employee was using a Lo-Trac to clean on the old E3-1 tailpiece when the Lo-Trac broke loose from the gob they were scooping and sent them into the chute catching them between the Lo-Trac and the the bottom part of the chute causing contusion/dislocated right shoulder and chest area complications.

Employee was bolting on #7 Section and had the mast tilted to put pin in but couldn't reach to install pin, so EE climbed up and had EE's foot of the mast, let the mast down and caught EE's left foot between the mast and the drill pot causing a fracture to EE's left great (big) toe.

Employee was standing in outby end of safety ride at the end of the track when the night shift bus came in too fast and hit safety ride knocking employee backwards and employee was caught between the two rides.

Roof fall on the North A belt line at crosscut 31 from the edge of the offside belt into the return in the crosscut.

ROOF FALL: There was a roof fall on the North A Belt at crosscut 5 which was 16 feet wide, 1 foot to 3 feet high, and 25 feet long on the belt.

Employee was climbing down off the dozer, not operating, they stepped down the last step and rolled the right ankle causing a sprain to right ankle. (Losing time as of 7/10/2019 due to this injury)

ROOF FALL - #1 to #2 entry big block roof fell out for 75 ft rib to rib, 20 feet wide, 6 to 8 feet high due to bolt spacing on 5 foot centers using 4 feet resign bolts and 8 feet cable bolts.

Employee was jacking a man bus on the track when employee pushed down on the bar the jack slipped and caused employee to give away injuring lower back.

There was a roof fall at 14 crosscut on West side of the left intake which was 40 feet long, 18 feet wide, and 6 to 8 feet high.

Employee was working with rail. The rail slipped off forks and hit employee's foot causing injury.

Employee was running airline in #4 entry when EE stepped in thick mud, as EE went to walk EE's foot stayed back causing EE to twist EE's knee resulting in a partial tear in EE's quadriceps tendon in EE's right upper leg.

Employee was roof bolting and accidentally hit the ATRS causing the drill steel to bend and sprung back and struck EE's left hand. (Losing time as of 5/8/2019 due to this injury)

REPORTABLE ONLY - Employee was stepping off the motor when left ankle twisted causing EE to fall on back against the foot wall resulting in a sprain to left ankle and small fracture in EE's back.

REPORTABLE ONLY DUE TO SUTURES - Employee was opening EE's tool box and had reached in to the box when another employee walked by closing the tool box lid catching EE's left ring finger causing fracture to tip of finger requiring sutures.

Employee was assisting in building brattice wall and was capping the left side, the brattice wall fell hitting their left leg causing fracture to the left lower leg requiring surgery.

Employee was caught in miner cable slack, waterline, when the employee slipped/fell while trying to take down slider curtain causing the employee to sprain right ankle.

Employee was walking through fly pads when check curtain flapping in the wind on the bottom caught employee's foot causing employee to slip/fall resulting in a re-tear to right upper arm (bicep) and right shoulder (rotator cuff).

Employee was lifting a rock to put on the EC belt when employee felt a strain in left shoulder causing employee a lot of pain resulting in damage/tears to left rotator cuff.

Employee was lifting the drop-off back on the flat before it fell off the car when employee felt a strain in left upper arm (bicep) resulting in a tear in left upper arm (bicep).

Employee left hand middle finger was smashed in a pinch point between belt cover door while opening the door causing a laceration requiring six (6) or more sutures.

Employee was working on the flappers on the East C drive roller when employee felt self getting hot. Employee walked toward the track entry to get some fresh air but passed out, falling and hitting head/face area on the belts toolbox causing a laceration under right eye (received 3 stitches) and a contusion to head above right eyebrow.

Employee pants leg was caught on the dozer door when employee was attempting to get off of the dozer causing employee to slip/fall to the ground resulting in a contusion to right hip, right pectoral, and employee striking their head on the dozer.

REPORTABLE DUE TO SUTURES - Employee was pushing on a rock when EE's hand slipped off the rock and the edge of the rock struck EE's right forearm (lower arm) causing a laceration that required 8 stitches.

REPORTABLE DUE TO ROOF FALL - 1 West belt line at crosscut 6 top fell above support for approximately 20 feet wide 20 feet long on belt line and 2 feet above roof bolts. Entry was dangered off at all crosscuts and double header row set in crosscuts, no airway blocked, no escapeway blocked, no injured employees.

Employee was trying to tie up throttle with welding rod to make stay in position, in the process they rested the right hand between the step and the bucket of the Bobcat. Employee hit joy stick with body and slammed bucket down on their right hand causing fracture to the right hand thumb finger.

Employee was cleaning up rock in front of 163 shield. A rock was hanging from end of drive, when EE went to pick a rock up from under the one hanging, it fell and struck EE's left hand thumb finger smashing it against the edge of another rock causing laceration.

Employee was cutting bog balls with utility knife when the knife slipped and cut left hand causing laceration.

(REPORTABLE ONLY DUE TO SUTURES) - While loading two inch water line, Employee caught right index finger between the end of the pipe and the forks on the Lo trac requiring ee to receive about eleven (11) stitches.

Employee was helping operator change bits on the tailgate side of the shearer when employee was struck by middleman that rolled out pinning employee's leg to coal in face chain causing employee to turn left ankle.

Employee stepped down onto a rock turning EE's right ankle while getting off Mega Track resulting in a very bad sprain.

Employee stepped on rock in roadway twisting the left ankle resulting in a very bad sprain.

Employee was operating doghouse between shift, while shear was going to tailgate roof bolt was hung in the drum. The bolt came out of the drum on the chain and struck EE in the face causing lacerations on top lip and chin and knocking out tooth.

Employee was walking while moving power and stepped on a rock covered in rock dust in the slant causing ee to sprain right ankle.

Employee was checking hydraulic fluid in sight glass. Employee had 3 point of contact moving towards tank when feet slipped on wet track, EE fell and landed on EE's right side resulting in dislocation to EE's left shoulder.

Employee was riding in mantrip. A chunk of coal fell into the mantrip and caused injury to employee's finger.

Quick connect coupling was loose and went to tighten and blew the coupling. The Employee was struck in the left side of the chest by a bull hose and water under pressure, pushed them back and they fell on the track rail.

(REPORTABLE DUE TO SUTURES) - Employee was taking bolter cable slack out of CAB hook, when rib rolled and rock struck right forearm causing a laceration on right lower arm.

Performing planned maintenance on the braking system of the cage, the cage fell to the shaft bottom.

Employee was lifting a box of glue off of the scoop to pinner and as they turned their body they felt pain in their lower back causing them to fall forward resulting in a strain to the lower back.

Employee was pulling rock dust hose slack down the belt line when a rock came off the belt and struck the top of right foot.

REPORTABLE DUE TO SUTURES - Employee stated employee was hanging blower curtain when employee slipped/fell and struck employee's elbow on a sharp rock causing a laceration to employee's right elbow.

Employee slipped and fell from the slope due to high pressure water hose, loose rock, coal and debris resulting in EE fracturing lower right arm.

Employee was working on East B Belt and stepped on a piece of angle iron and punctured boot and left foot.

Employee was pulling on hydraulic hose that was caught under a flinger duster when employee's upper arm started hurting resulting in a torn distal tendon.

Injured was building a brattice with mandoor in it. The employee was on their knees cutting a solid block that would be used to tie in the outby end of the stopping to the mine roof when the wall fell. Some of the block fell on the employee smashing right 5th finger (little finger) causing lacerations and fractures.

REPORTABLE ONLY DUE TO FRACTURE - Employee was unloading belt structure from a car, when the whole frames shifted catching employee's thumb between the frames causing fracture to thumb.

Employee was attempting to set power on No. 2 circuit breaker when the power center went phase to phase causing burns to employees left hand, arm and neck.

REPORTABLE ONLY DUE TO SUTURES - Employee was operating shields when a piece of rock came over the handrail hitting the lower right leg causing laceration requiring sutures.

Employee was stepping out of man bus to throw track switch when employee's leg got caught on man bus causing employee to trip and fall on employee's left arm resulting in tear to bicep tendon.

Employee was riding in a Shuttle Car on rough road when they felt pain in lower back. EMPLOYEE HAS BEEN WORKING SINCE DATE OF ACCIDENT AND WAS TAKEN OFF WORK ON 4/20/17 FOR A PROCEDURE AND THERAPY.

Employee was lifting rock out of walkway to put in panline, when ee lifted rock onto handrail ee felt pain in groin area. EMPLOYEE HAS BEEN WORKING SINCE THE DATE OF THE ACCIDENT AND DID NOT START LOOSING TIME UNTIL 2/9/17 WHEN EE HAD SURGERY TO REPAIR HERNIA.

REPORTABLE ONLY DUE TO SUTURES - While working on continuous miner section employee reached hand up to grab a string, hanging from the roof, that another employee was going to cut, missed the string and cut hand. Employee did receive stitches.

Employee was building brattice and blocks started to fall, the employee turned to move out of the way ee stepped on a drop board causing sprain to right foot.

Employee was trying to get a bolt out of a chain that was holding the cowl arm, when the chain came lose it caught the end of EE's finger causing crush injury to tip of finger requiring surgery.

Employee was operating a roofbolter in the No. 1 entry, hand was on the drill steel as employee was attempting to raise up the offside of the machine, while doing so, mistakingly hit the close clamp lever catching finger causing fracture and laceration to left hand middle finger.

Employee was checking the fire suppression on the miner, when the water inlet hose came loose hitting employee in the eye causing injury to right eye.

REPORTABLE ONLY DUE TO SUTURES - Prep Plant employee had removed the 4" air hose from the surge bin of the air cannon to clean rock out and unstop the hose, as they were cleaning it out the hose came up and the bolt on the end of hose cut employees arm requiring sutures.

REPORTABLE ONLY - Employee was working at the tailpiece of the slope belt when a rock came off the belt hitting employee in the face chipping several teeth.

REPORTABLE ONLY DUE TO SUTURES Employee was bent over to set a track tie under the rail as another employee was using track hammer to clip the latches on another track tie. During the downswing, the track hammer struck the bent over employee on back side of hard hat causing small laceration to scalp, requiring sutures.

Employee was attempting to open door on powder box and (he had is ring finger on his right hand in the crack) when he opened the door, he mashed his finger. Surgery will be on 12/4/15. No work.

Shift started at 11 PM on 10/21. While employee was attempting to load a barrel of chain onto the forks of the lotrac, the operator exited the lotrac to assist and then the lotrac rolled forward and rolled against employee's right foot/ankle.

After arriving at the unloading point the employee got off of his Locomotive and began disconnecting safety chains from between the four Locomotives. As he was walking by Locomotive 43, the sight glass for the coolant system popped off and sprayed the employee with coolant fluid, contacting him on the left shoulder, neck and face.

Employee was getting off back hoe and lost his footing. He caught himself grabbing onto a beam to hold on injuring his right arm. His first date of loss was 09/01/2015.

Employee was walking and slipped off side of track rolling his left ankle. This has resulted with a fracture.

Employee started his owl shift on 8/20/15 @ 11:00 PM. Employee went to area where a brattice wall was being built. He stepped on a block that was under water and rolled his right ankle. Crutches and boot was administered by doctor. Right ankle sprain.

Employee was setting a 30 h.p. pump and shoveling material over boots when the bunker washed out. Employee then felt burning on both of his legs. He is being treated for chemical burns.

Employee was finishing up scooping behind the cage. There was a piece of blue Aqua 2" water line about 7' long buried in the gob. When he filled the bucket and raised it up to take it to the car, the pipe got free and slid down the edge of the bucket striking him in the face. He received four sutures in upper lip.

Employee riding bus when bus struck 2" pvc pipe, which pushed the pipe coupling into the employee's left arm, causing a laceration requiring 20 sutures

Locomotive derailed and employee was helping jack locomotive back on to the track. After jacking approximately 4", the load shifted and the jack kicked out striking employee in the face. Employee received stitches and is awaiting surgery to fix his fractured nose.

Employee's shift started on 6/15/15 at 11:00 PM. Employee was pulling shield #42 up and as it set he moved to pull shield #41. Rocks fell between shields and cut the right arm right above the elbow. Sent out for treatment and received 15 stitches.

Employee pulled slider rail forward and it slid out of the bracket striking him on the right shoulder. Employee worked up to his surgery date which was 7/13/15.

Employee was lifting scaffold board up onto the pallet of blocks. His finger was caught between the end of the board and the edge of blocks. He was taken to the doctor and received two stitches. Returned to work on 6/6/2015.

Employee was blowing up a shield and the tip of the shield was under the previous shield. When the shield tip slid from under the other shield, a rock fell either off the shield or from the top striking employee on the right hand. Employee had surgery on 5/29/15.

Employee was washing at the bottom of the slope - standing on loose coal. The loose coal gave way and employee lost his footing falling to the footwall. The water hose he was using apparently whipped around striking him in the face/eye area. He was taken off work with the first date of loss being 5/15/2015.

Employee started work on 5/10/15 at 11:00 PM. Employee was walking down slope when he slipped and fell injuring his left foot. Employee stated that he looked to see what caused the accident and he had stepped in a hole and it was not visible due to water. Doctor took him off work since he had a fracture.

Co-worker cut bands on pallet on the outby end of a car while employee was standing on the inby end. When the bands got cut, the weight shifted and caused the banded rollers to fall off the car striking employee in left ankle causing a fracture. Started losing time 4/28/15.

Employee stated that he sprained his right ankle when he stepped off of the rock dust motor. He started his shift on 1/30/15 at 11:00 PM.

Employee was moving structure from back of cross cut and stacking by the track with a lo track. Employee was exiting lo track and stepped part way on a 4" cinder block and rolled his left ankle. The block was buried part way in rock dust and was not visible. Started losing time on 1/29/2015.

Motor 57 came off top of hill and slid into 183 man bus. Employee was not able to get off before motor hit. He bumped his knee on edge of bus and twisted his knee. His shift started on 1/25/2015 at 11:00 PM and his injury occurred on 1/26/15 at 6:05 AM.

Employee was going through a brattice man door at cross cut 621. She opened the door, started through the door and the air caught the door. She was trying to hold the door and it caused her to strain her right shoulder and lower left leg trying to protect herself. We could no longer accommodate work restrictions starting on 2/19/2015.

While employee was recovering timbers, he had lifted 4x4 timber on his right shoulder and was coming around the scoop to load it and he tripped over the rock on the footwall and landed on the 4x4 timber which landed on the side of the scoop and the footwall striking him across the chest and right ribs. Treatment on 12/31/14 where he found out he had a fracture to his rib.

Employee shift started on 11/9/14 @ 11:00 PM. Employee was lifting 700+ lb. pump over a dam to set in sump. While pulling with come a longs, the pump rolled to the left where employee's hand was. His hand was caught between the grating on bottom of pump and a 4x4 on the decking coming off the block dam. Employee received sutures to his left thumb.

Employee was walking down slope belt and he put his left foot down to continue down slope and he slipped and fell down to the foot wall. He fell between 3 and 4 chain. Area was wet with loose coal fines.

Supply car came uncoupled from 62 locomotive. In the process the back locomotive was pushing the supply cars and pushed them into the front locomotive no knowing it was uncoupled. This injured the employee's left side rib cage. The result from treatment was a fractured rib.

Employee stepped in hole full of rock dust and sprained his right ankle.

Employee was stepping into man bus and his right knee gave away. At first the doctor thought it was related to his 2/2013 injury and after the MRI was done, he stated that this indeed was a new injury.

Employee was walking N3 longwall face distributing face phones and cables. Employee twisted his right ankle walking on uneven ground.

A roof fall occurred on 10/10/2014 at approximately 7:00 AM. No one sustained any injuries. The roof fall was located in the West Mains area.

Employee was cleaning muck at East C header and stepped in a hole and twisted his right ankle. Started losing time on 9/30/14.

Manbus #191 stopped for a block light at N-9. He stopped and the #4 section bus came down the track and hit bus #191 that the employee was driving and hurt his back.

Employee was getting an ax out of a tool box and the lid fell down onto his left hand mashing his thumb. Went for treatment and received stitches.

Employee was walking up steps at the bottom of the slope belt and slipped on the 2nd step up causing his left ankle to fall down between the steps and twist. Doctor took employee off work on 9/5/2014.

Employee had begun to board DZ 33 on left side of tractor. When he stepped up on the second step and reached for the hand hold, he lost his balance and fell backwards on the ground. He tried to catch himself with his hands and was diagnosed with a chip fracture his wrist.

A roof fall accident occurred on the N-4 longwall (MMU-034-0) in the tailgate entry at this operation on 8/18/2014 at 1:20 AM. No injuries occurred from this roof fall.

Employee stated that the piece of steel had to come down for this repair to take place. He then took a grinder so he could pull the metal down. He was grinding from 3 to 5 seconds (at eyebrow level) on top of the old weld when the grinder hit his safety glasses and caused a laceration under his left eye. He received 7 stitches.

Employee slipped and landed on his right knee with all his body weight. Conditions were 2" to 3" deep muddy bottom. Employee started losing time beginning on 8/20/2014.

Employee explained that left side drill steel was stopped up and he tried to assist left side pinner. When he entered the area to beat on the steel he went to duck under the light receptacle on the ATRS; he hit his head (hard hat) and it jarred his neck backward. Employee was under supported top at the time. Started losing time on 8/15/2014.

REPORTABLE ONLY DUE TO SUTURES - Employee was changing a wiper on West D header when the pin came out causing wiper to fall down hitting his left index finger causing laceration requiring sutures.

Employee was walking down the overcast stairs while stepping down he twisted his left knee causing pain behind knee cap.

Employee was moving feeder cable. He was pulling on cable and felt pain in his left lower back. His first date of loss is 7/16/2014.

A roof fall in the North Bypass track entry from cross cut 32 to cross cut 33 was found on the morning of 7/4/14 at 6:00 AM.

An ignition occurred on the longwall headgate drum as the shearer was advancing to the tailgate at shield #176. The headgate operator observed a blue streak around the turning drum. He stopped the shearer and grabbed the wash down hose and applied water to the headgate drum estimated time of flames was 10 seconds.

Springs broke in the forklift seat causing the employee to be bounced and jarred in his back when moving across the yard. This will be employee's first lost time for this accident. Date of loss started on 1/27/2015.

Rock fall on 1 West track at 12:37 AM. The rock was 10 feet wide and 7 feet high. No injuries occurred.

Employee was hooking up relay bar snake head and had relay bar lifted with ¾ ton come longs, hook broke on come a long and hit employee in left hand causing a laceration.

Employee was picking up belt structure by hand and felt a pain in lower back. He also felt pain in back from almost falling. He went for initial treatment on 5/5/14 and put him on restrictions that we could accommodate. Went back to doctor on 5/6/14 since he could not put on his boots because of pain and the doctor took him off work.

Employee had hooked a chain to the # 3 section power center while some other employees had double hooked a chain on the other side. It was high tension chain. The hook pin either came out or broke and struck employee in his left arm/elbow causing a laceration.

Employee was turning to leave the West B drive/take-up area and struck his head against the 2" water pipe running from the track entry to the drive area jamming his neck. He started losing time for this injury on 9/19/2014.

A non-injury accident (ignition) occurred at this operation on 3/31/2014 at approximately 1320 hours. This happened on 7 section (MMU-035-0) inby the loading point.
