Employee was standing in front of scoop bucket dusting, as they finished and went to put the hose in scoop bucket they stepped back and had a foot under scoop bucket as it was lowered, catching their foot, causing fracture.

Employee was riding in manbus when EE's foot caught a rock, twisting EE's left knee and ankle resulting in sprain to left knee.

Injured was removing Shim Pack from Ventilation Fan, the Shim Pack unexpectedly shifted striking the injured on the left wrist.

The Aux Fan 217 trailing cable went phase to phase creating Fault/Arc in the cathead resulting in a flash/ignition from a small bleeder in the mine floor at 10 section.

Employee was pushing a whole frame back on a car where it had almost slid off, as they were pushing the frame they felt a pop in their right Bicep causing injury. Employee has been working since date of injury up until 1/20/26 when they had surgery on right Bicep.

Employee was standing on a 4 foot ladder taking slack out of chain when they lifted the ventilation tubing, they lost balance and fell off the ladder injuring right knee and right ankle.

Employee was operating the tailgate shear when a rock came off the drum striking the employee in the face/nose resulting in fracture to nose.

Employee was at the NW Track walking through a man door, as EE stepped through the door EE felt a pop in EE's right knee resulting in injury.

Employee was entering the belt line and went to open large 4' man door, when lost their grip on the door causing it to close on right ring finger resulting in fracture to finger.

Employee was riding in manbus toward the end of the track when the manbus lost traction and collided with parked bus at the end of the track. Employee felt pain in back. Employee has been working since the date of accident until taken off work as of 8/6/25.

Employee was lifting a cut 10"x7-1/2' long prop to set it as they were attempting to put it down they felt pain in lower back.

Employee was unloading tie down plates when EE lost EE's footing and fell, causing injury to EE's left knee and left ankle.

Roof Fall - Occurred at the Northeast track, #5 entry and crosscut #31 intersection. The inby brow fell out above the anchorage bolts and the fall measured 15' length, 20' in width and 10' in height.

A Unplanned Roof Fall Occurred On NE-9 (#8 Section) At Crosscut Number 49 In The Long Crosscut Between The #2 Track Entry And The #3 Belt Entry. The Mine Roof Fell Above The Anchorage Point Of The Primary Roof Support. The Roof Fall Occurred Due To Undetected Separation With Laminations Above The Anchorage Point Of The Primary Roof Support Installed During Development.

Employee was helping install tubing inby the trunk line in the #1 entry of 12 section, when EE got EE's finger caught between 2 pieces of vent tubing resulting in stitches in L Hand finger.

Roof Fall - Occurred on the East Panel Track (East Main #4 entry) at x-cut 77, the old E6 track/belt overcast. The roof fall area measured approx. 15' long x 15' wide and fell above the anchorage bolt.

Employee was assisting with installing a bull hose on a 8" Aqua pipe for electric pump on NE-9, when employee lost balance and fell into the water and the 8" Aqua pipe fell on their left foot/ankle causing it to become caught between the mine floor and pipe, resulting in a sprain to their left foot/ankle.

Employee was riding in a manbus to the bottom, when a piece of draw rock fell out of the top striking employee on the left side of face/mouth, resulting in a broken tooth.

Employee was lifting 10" aquamine pipe over the E22 return overcast to put the top piece on overcast when the pipe broke causing employee to fall approx. 8' to the mine floor resulting in fracture to right foot.

Employee was helping move the tail roller back into place to remount after changing bearing, when moving back into place the roller moved and EE's hand become caught between the bearing housing and upright I-Beam causing fracture to left hand.

Roof Fall - Occurred in the North West Mains track entry (#3 entry) at x-cut 143. The roof fall area measured 40' long x 40' wide x 8'10" in height and fell above the anchorage bolt.

Employee went to hang the scoop charger cable and as they raised the cable up they felt pain in right hip causing strain to hip.

Employee was lifting and pushing on ventilation tubing on the section, when EE felt a pop in EE's shoulder causing injury to left shoulder.  Employee has been referred out to orthopedic.

Employee was uncoupling a single end car from locomotive, when their left foot slipped causing contact with single end car resulting in fracture to left ankle.

Employee was building brattice and was on a 4' ladder to cap the top of brattice when a block fell striking employee in the lower right leg/foot causing contusion.

Employee was lifting Shield pontoon up using crib blocks on the panline, employee reached under Shield boom to retrieve chain when the cribs slipped causing boom to strike employee on the back of the head causing injury to neck. EMPLOYEE HAS BEEN WORKING SINCE DATE OF INJURY UNTIL 03/21/25 WHEN EMPLOYEE WENT OFF WORK DUE TO SURGERY ON NECK.

Employee was putting track rail together. EE pushed the jack bar down and the jack bar came up hitting EE in the right eye causing laceration, resulting in sutures to EE's right eye.

Employee was helping carry tubing to face. EE stepped on a rock causing EE to twist their ankle resulting in a sprain to left ankle.

Employee was roof bolting when water started coming out of bolt hole. As EE was trying to get the bolt in, EE lost balance falling backwards, knocking hard hat off. The bolt hit EE on top of the head causing laceration requiring sutures.

Employee was working on longwall monorail taking off rollers and got EE's finger caught between the rail and the roller causing injury

Employee was helping carry a 6X6 timber, when employee tripped on a rock in walkway causing employee to fall forward, injuring right knee and resulting in fracture to left rib. Employee did not start losing time until 8/12/24.

Employee was operating roof bolter while installing a T-Board to support a pot when EE caught EE's right index finger between the T-Board and the mine roof, causing laceration to tip of finger.

Employee was changing starter on skid steer and had another skid steer they were using the bucket of it to keep tools in. Employee went to change position and slipped and fell back in bucket of skid steer hitting left arm/elbow causing puncture wound/laceration requiring sutures.

Employee was carrying sand jack in the #3 entry of NE1T/G when employee stepped on uneven ground, twisting left ankle causing sprain to ankle.

Employee was carrying oxygen bottle when they tripped on a piece of rail, twisting their right ankle causing injury.

Employee was on the section removing tubing with the Lo-Trac when the tubing chain that was hanging down caught their nose causing laceration requiring sutures.

Employee was operating Locomotive when it derailed hitting overcast, causing block/material to hit EE's left ring finger causing fracture and requiring sutures.

While cutting out a bolt from Longwall Crusher (recovery), Employee was hit in the face with the head of a bolt resulting in laceration to face requiring sutures and fractures resulting in tooth loss.

Employee was moving a rock off the handrail at Shield #175 when EE's hand was caught with another rock, resulting in a fracture to left middle finger and sutures to left hand.

Employee was connecting lifeline to the refuge chamber when the draw head fell and struck employee on EE's right lower leg/ankle, resulting in a fracture to EE's right ankle.

REPORTABLE ONLY DUE TO SUTURES - Employee was standing on the back side of the Lo Trac, Lowering the engine lid with right hand and unlatching it with left hand, when the engine lid fell hitting left arm causing laceration requiring sutures.

ROOF FALL - Occurred on the Northwest travelway (#5 track entry) at crosscut 141-1/2. The fall was approximately 20 feet wide by 30 feet long by 20 feet above the mine roof.

ROOF FALL - Occurred on #2 Longwall, falling from shield tips on longwall headgate shields outby for approx. 30 feet (#1 entry, crosscut 36-36-1/2). The fall was from rib to rib and fell 7 feet above the anchorage bolts.

Employee was working on the #1 Longwall when EE's Left Lower Leg became pinched between the Pan Line and a part that was laying on the #7 Shield Pontoon resulting in a contusion and swelling to left lower leg.

ROOF FALL - Occurred at cross cut 126 in the #5 track entry in the North Mains. The roof fall was approximately 16 to 20 feet long and 18 feet wide and 1 foot above the anchorage bolts.

ROOF FALL - Occurred at crosscut 38 in the #6 entry intersection on East B Mains. The East power hole cable was damaged.

Employee was buffing a belt for splicing while EE moved EE was struck by the buffer when the cord got stuck. Resulting in 4 stitches.

Employee was washing down the slope when they tripped over a rock falling, injuring their left shoulder. Employee has been working since date of accident until they were taken off work on 12/13/23 due to surgery.

Employee was in the track entry to move a pump. As EE picked the pump up, EE's foot slipped causing EE to fall on their back resulting in injury to lower back. Employee has been working since date of accident until was taken off work on 12/20/23 due to surgery.

Crew loaded a piece of plywood on the outby end of the mantrip. Once they got on the main line, the air current started to lift the plywood up and down. While traveling the air current got under the plywood, flipped it back and hit the employee in the chest. EE was in the inby end of the mantrip.

Employee was installing rib pin on left rib when EE felt pain in left shoulder, was placed in sling due to rotator cuff injury.

ROOF FALL - Occurred on NE-6, 10 section in the #2 entry of the bleeder system, one crosscut inby spad #32236 crosscut #1. The Mine Roof fell above the anchorage bolts due to adverse roof conditions.

Employee was installing liner plates in the south skip While working out of a Spider basket from an elevated position, employee reported an electrical arc and instructed them to cut the power, the rope broke supporting the man basket causing the basket to fall approx. 30 feet to the ground causing fractures to employee back and ribs.

ROOF FALL - A roof fall was found during the on-shift belt examination of the North B Conveyor belt. The fall occurred at cross cut 138.5 in the #4 entry. The roof fall was approximately 21 feet wide x 35 feet in length and fell above the anchorage bolts.

Working on a Caterpillar D-11 putting a pin back in the push arm when the pin fell striking the employee in the finger cutting their finger

Employee was using a come-along to help pull face chain together. The chain on the come-along accidentally broke loose and the hook on the chain came in contact with employee's arm causing injury.

Employee was building a crib on top of a shield and smashed a finger between two crib blocks.

Employee was helping install ventilation tubing and as EE was rotating the tube for it to fit together, a bolt caught EE's left thumb causing laceration requiring sutures.

Employee was helping install a top overcast panel when the end of the panel came loose falling and striking employee, causing contusion to left hip and thigh.

Employee was walking at x-cut 59 on the 12 section, when a piece of brow fell out striking employee on the left foot causing laceration to their foot resulting in sutures.

Employee was walking the NE6 belt when employee tripped on a rock causing employee to fall, coming in contact with the belt resulting in abrasion to right shoulder and elbow. Employee has continued to work since date of injury until 6/28/23 when employee went off work for medical procedure.

Employee was working with a group to recover water line and was holding on end of the line. When the coupler was removed, the water line shifted and accidentally sprung up and hit employee in the ear causing injury.

During the disassembly of a feeder, the duck bill portion fell on the individual's feet resulting in crushing injuries to the 5 toes on the left foot and the big toe and adjacent toe on right foot.

Employee was opening the door to the Dozer when the handle broke causing EE to fall hitting EE's head resulting in laceration to the back of EE's head requiring sutures.

Individual stated was helping change a scoop battery and EE's foot was caught between the scoop battery and charger. Investigation revealed conflicting information regarding the movement of the scoop. Injured stated it moved and operator stated it was stationary. At the time of investigation there was adequate clearance to prevent a caught between incident.

Employee was using roofbolter to install cable bolt, when bolt came out of pusher the employee reached up and as they did, they inadvertently hit the rotation lever causing the plate to come around hitting their right lower arm causing laceration and fracture.

While bending over to lift several fly pads to put back on the forks of the Lo-Trac, employee felt severe pain in EE's abdomen, resulting in an umbilical hernia requiring surgery.

Employee was manually pushing a longwall shield out as the tip jack started to leak down, they put their hand down on handrail and rock rolled off canopy striking right ring finger resulting in laceration requiring sutures.

Employee was helping build a regulator on the section. As EE was handing blocks to another employee, some of the blocks fell striking EE's right knee and ankle causing contusions and abrasions to ankle.

REPORT ONLY - After inspection of the 7-3 hoist, a broke cushion spring was found and the the hoist was taken out of service until the repairs were made.

During the descent of the mine cage, ice fell from the shaft wall and struck the protective cover (metal top) of the cage. Abnormally extended cold temperatures resulted in the ice forming in the shaft, in spite of the shaft heaters being used.

Employee was pulling drill steel out of the hole when EE twisted, hitting the spin lever, causing EE's right hand to twist around resulting in fracture to wrist and finger.

Employee was helping to change the scoop battery when they climbed on top of scoop to plug jumper cable into battery, the battery slightly tilted forward catching their left foot between the plug end of battery and brace bar causing contusion to the foot.

Employee was attempting to change out a valve on a longwall shield and accidentally got hit in the face by the valve when the employee removed the valve pin causing injury.

Employee was at the 7-2 Bunker cars approximately 30' on the southwest walkway, shoveling when EE caught right foot on rail causing EE to fall, twisting right ankle.

Employee was driving an 18 wheeler from prep plant to barge facility and truck overturned. EE was taken to hospital with injuries to left leg, ribs and shoulder.

Employee had exited the manbus and was walking to the cage when EE stepped wrong rolling EE's right ankle, causing EE to fall, resulting in a fracture to EE's ankle.

REPORTABLE ONLY DUE TO SUTURES - Employee was washing down on the offside of the slope belt, when EE picked up a rock to move out of the way and caught the right index finger between the rock and rib causing laceration that required sutures.

ROOF FALL - Occurred on East A belt entry at x-cut 28 and fell onto the East A belt line. The roof fall was approx. 25 feet in length, 30 feet wide and approx. 6 to 8 feet high.

Employee was walking down belt line when employee slipped in mud, falling and twisting left ankle causing injury to ankle.

The injured worker was driving an 18 wheeler loaded with coal that overturned after exiting the prep plant. The worker was treated at a local ER and released that same night and is awaiting an appointment with an orthopedic doctor to determine extent of injury and potential time off.

Removing and replacing Hoses/Hydraulic lines. Hydraulic fluid sprayed in Left eye

Employee stepped on a curve rail at the track switch on East A track, twisting left ankle. EE sprained ankle and was put in a walking boot.

REPORTABLE ONLY DUE TO SUTURES: Employee was pushing the slider tubing back when chain came loose causing the employee to fall forward cutting nose resulting in sutures to tip of nose.

Employee was riding in manbus coming from the section going to the bottom, when a two inch bull hose caught on the rail of the manbus striking employee in face causing laceration and fracture to EE's nose resulting in sutures.

Employee was lowering the cover down over the engine compartment of a Lo-Trac when EE felt a sharp pain in EE's left shoulder. EMPLOYEE HAS BEEN WORKING SINCE THE DATE OF ACCIDENT UNTIL 1/31/22, WHEN EE WAS TAKEN OFF WORK DUE TO CONTINUED PAIN IN EE'S LEFT SHOULDER.

Employee was using a slide hammer while pulling a pin from a trapping shoe, when EE's left index finger was caught between the end of the slide hammer and the slide weight causing fracture to tip of finger and laceration requiring sutures.

REPORTABLE ONLY DUE TO SUTURES - Employee was working on East C belt header. When EE went to remove dirt from receiving end of bracket, the dirt became dislodged and the wiper fell catching EE's left middle finger between the ITC blade and drip pan causing laceration and fracture to tip of finger, requiring sutures.

Employee was pulling rock dust hose through the man door, when EE tripped on the dust hose while going through the door, falling and spraining EE's left leg/ankle.

ROOF FALL - A roof fall was found by examiner conducting 7 day exam at x-cut 142-1/2 in the North Main #6 entry air course. The fall was approx. 30 feet long, rib to rib and 10 foot high. No methane at location and proper air flow.

Employee was cleaning garbage out of mantrip and tripped when stepping out and fell causing injury to both arms.

REPORTABLE ONLY DUE TO SUTURES - Employee was on East C Track on the locomotive taking an empty single end car back to put in E3, when one of the sander lids came off hitting employee in the mouth causing laceration to lip requiring sutures and chipping front tooth.

Individual reported they slipped while stepping off of a step ladder twisting right knee. Based on medical information received on 11-3-21 the alleged injury is being submitted as Lost time accident.

Rock fall found by Cody Kinney while walking East B belt at cross cut #96. The fall was about 10 feet long and rib to rib.

An employee was on the 7-10 Service Cage headed down into the mine, when at about 170 feet from the top, the switch gear at the mine 7 east sub-station went phase to phase causing the cage to stop from loss of power. The employee was not injured and was entrapped on the cage from 4:30 p.m. until 6:50 p.m. when power was restored and the employee was hoisted to the surface.

Employee was walking toward the section when employee stepped off side of bridge board in roadway spraining left foot and was placed in a boot.

Employee was walking to the man bus when they stopped and shined their light into a cross cut to check oil supply and took a step backwards tripping over track rail falling backwards, landing on left elbow causing injury to elbow.

IGNITION - Occurred on 14 section in the No. 1 entry while continuous miner was cutting bottom. Ignition lasted 1 to 2 seconds and went out on its own with no water or fire extinguisher being applied.

Employee was attempting to move tubing over to align with existing tubing, when the handle employee was pulling on broke free from tubing and employee tried to catch self, fracturing left wrist. EMPLOYEE HAS BEEN WORKING SINCE DATE OF INJURY UNTIL 3/3/21 WHEN EMPLOYEE WAS TAKEN OFF WORK.

REPORTABLE ONLY DUE TO SUTURES - Employee was installing bit inserts using porta power and chisel, porta power pushed chisel out striking left hand causing small laceration requiring sutures.

Employee was shimming up cat track on feeder, when a grease fitting came apart striking employee's left elbow causing laceration requiring requiring sutures.

Incident - East C Belt had a bottom roller at cross cut 126.5 to go bad on offside of belt, CO sensor down wind gave alert to rise in CO. A 5'x 9' area in the floor was found smoldering. Water was applied to area and material shoveled to ensure the smoldering area was out.

Employee was beside the roof bolter while the operator was trying to use the ATRS to loosen the chain holding the slider, when the chain broke letting the slider and two tubes fall striking employee causing EE to fall to the ground spraining right knee and right ankle.

Employee was doing chute work along the belt and was standing on a ladder. The ladder slipped causing the chute plate to fall and hit EE's finger causing injury.

Employee was operating shear when a rock came over the handrail striking employee's left lower leg resulting in fracture to fibula.

Employees were using the jack bar to help re-rail a manbus, as they were moving the bus back to the rail the bus came off the jack, hitting the jack bar which struck employee's left lower leg causing fracture to left lower leg.

Employee was operating the shearer and was walking along the shearer when at the haulage box EE turned to look at a mimic and was hit by an object causing injury to upper back.

Employee was prying a rock with a rock bar at the headgate drive, the rock moved unexpectedly causing employee to lose footing and fall backwards as the employee extended right arm out to catch self, when employee fell on right shoulder resulting in a torn tendon requiring surgery.

The Longwall mechanic was changing shear drum in service entry, had a hand on ranging arm when a small piece of scale rock fell hitting employees left hand causing laceration requiring sutures.

Employee was handling panel and injured finger while loading it onto a low track.

Employee was loading pipe on the Lo-Trac when EE's finger was caught between the pipe causing fracture to EE's right index finger requiring sutures.

Employee was hanging monorail cable on double hook and the hook that was attached to the chain straightened out causing cables to come down on EE's right shoulder causing contusion. EMPLOYEE DID NOT BECOME REPORTABLE UNTIL EE'S MRI ON 9/15/2020 SHOWED A COMPRESSION FRACTURE TO EE'S RIGHT SHOULDER.

Employee had finished cut in #1 entry and was standing talking to the day shift miner operator coming on shift, when a piece of coal fell off the rib striking employee hard hat and right shoulder causing abrasion and contusion. EMPLOYEE HAS BEEN WORKING SINCE DATE OF INJURY UNTIL 10/8/2020 WHEN EE WAS TAKEN OFF WORK DUE TO FRACTURE REQUIRING SURGERY.

Employee was trouble shooting a SC cable and individual stated they thought they felt a shock and stepped back on material (rock/coal) twisting a knee. Employee was diagnosed with a sprain/strain to the right knee and placed in a knee brace.

Employee was traveling from section to the bottom in a manbus when a piece of draw rock fell striking employees hard hat, pushing glasses causing laceration above and below right eye requiring sutures and a fracture below sinus.

Employee was kneeling down working on a breaker on the power center. When EE went to move over to the left, EE's knee popped causing immediate pain to the right knee.

Employee was attempting to install the first row of bolts when they noticed a piece of draw rock looked loose, they turned to get the rock bar when a piece of rock fell out of the top striking left heel causing laceration to heel requiring sutures and partial laceration of the Achilles Tendon.

Employee was traveling south on northwest track when the manbus EE was in, was struck by the water car the locomotive was hauling. Employee received laceration on both lower legs requiring sutures.

REPORTABLE ONLY DUE TO SUTURES - Employee was installing a roofbolt on 6 section, when EE pushed the bolt in with EE's hand, EE's knee hit the fast speed lever catching right index finger between the bolt and plate causing laceration requiring sutures.

Employee was running scoop charger cable when employee stepped on a rock causing employee to lose balance, employee tried to regain footing and employee's right knee buckled causing a torn ACL and torn medial meniscus to right knee.

REPORTABLE ONLY DUE TO SUTURES - Employee was helping remove a hyd. hose that had become wrapped up in drive roller. As EE was getting down from belt EE cut EE's right ear on the fire suppression nozzle causing laceration to right ear lobe resulting in sutures.

Employee was operating locomotive and was pulling out of the E12 Headgate switch when dust or debris blew off of the car into left eye causing injury to eye.

REPORTABLE ONLY DUE TO SUTURES - Employee was walking making a belt roller list when employee slipped and grabbed the frame to keep from falling, resulting in a laceration to right thumb requiring sutures.

REPORTABLE ONLY DUE TO SUTURES - Employee was hanging roof bolter cable when EE lost balance and grabbed a rib pin to keep from falling, causing laceration to palm of right hand requiring sutures.

Employee was riding in a manbus that was stopped at a switch for the switch to be thrown so they could continue straight, when they were struck by another manbus from behind causing injury to lower back. EMPLOYEE HAS BEEN WORKING SINCE DATE OF ACCIDENT UNTIL 7/17/2020, WHEN EE WAS TAKEN OFF WORK DUE TO CONTINUED BACK PAIN.

Employee was operating a mantrip and accidentally lost control wrecked the bus. Employee attempted to jump out before impact which caused injury to ankle, leg, and foot.

IGNITION - Occurred on 9 section as continuous miner was cutting bottom resulting in a frictional ignition that lasted few seconds and went back out. One fire extinguisher was applied to area.

Employee was hanging water line, as EE reached out EE missed the hook and caught self with EE's right hand on the rib, when a piece of rock above EE's right hand came out striking EE's hand resulting in abrasions and contusions.

REPORTABLE ONLY - Employee was taking apart a 2" Aqua pipe, as they were removing coupling the pipe and coupler swung out striking employee in the mouth and breaking a front teeth.

NON-INJURY INCIDENT - Occurred when a damaged bottom belt roller, in bottom belt structure "offside" at cross cut 44 on East A belt, found with damaged bearings creating frictional heating resulting in embers that were extinguished with water.

REPORTABLE ONLY DUE TO SUTURES: Employee was working on by-pass belt when come-a-longs slipped off belt rail striking employee in the mouth resulting in laceration to EE's top lip requiring sutures.

Employee was helping to pull the head drive off the stageloader, when it came lose a foot became caught between the plate on the head drive causing fracture to left ankle.

Employee was attempting to unstop the drill steel by striking the steel with a wrench and in doing so EE struck hand against the drill steel resulting in fracture to tip of left ring finger.

NON-INJURY INCIDENT - The West Seals 1-14 were unable to be inspected on the seventh day weekly inspection due to water in the walkway. Management withdrew the miners except miners used to pump water.

Employee was at the East header when walking to reset the belt take up employee tripped on rope, falling injuring the right ankle resulting in a fracture to right ankle.

Employee was under the Ramcar trying to remove a drain plug to drain one of the tram gear cases when EE felt pain in EE's right shoulder while pulling on the pull bar. EMPLOYEE HAS BEEN WORKING SINCE DATE OF ACCIDENT UP UNTIL 2/21/2020 WHEN EE WAS TAKEN OFF WORK DUE TO A TEAR IN EE'S RIGHT SHOULDER.

REPORTABLE ONLY DUE TO SUTURES - Employee was shoveling on North Main when they tripped falling forward cutting left index finger on a piece of metal resulting in laceration requiring sutures.

Employee was preparing to service the #1 face, as EE removed a timber that was leaning against the rib, a piece of rock fell out striking employees right foot causing contusion to EE's foot.

Employee was operating shearer mining toward the headgate when they were struck by two rocks, one struck left shoulder causing contusion and one struck left hand that was holding the shearer remote causing contusion and laceration resulting in sutures to left ring finger.

REPORTABLE ONLY DUE TO SUTURES - Shearer drum was cutting toward headgate and was at 30 shield and the operator was at 24 shield when a piece of rock came up panline knocking employee down and hard hat off, when standing up employee hit head resulting in laceration to top of head requiring sutures.

Employee was walking back to the scoop from supply car as EE rounded the corner at the scoop employee tripped over blocks, landing on both elbows resulting in fracture to left elbow.

Employee was using a scoop to move a refuge chamber when the scoop operator side tire slipped off of a ledge, causing the scoop to come down hard, jarring the employee causing injury to EE's back.

REPORTABLE ONLY - Employee was helping splice belt when EE's left hand was hit by another employee with a hammer causing fracture and laceration to EE's left middle finger requiring sutures.

Employee was walking to the tool car when EE stepped in an area that was slick due to water and slipped and fell hyper-extending EE's right knee causing injury to knee.

Employee was on the longwall working on getting shield out of the top and pulled up with the others shields, when the pan line pushed the relay bar struck employee's right knee causing laceration requiring sutures.

REPORTABLE ONLY DUE TO SUTURES - Employee was installing ventilation tubing and attempting to pull out slider that was stuck with left hand, EE braced with right hand catching right ring finger in the O-ring causing laceration to tip of finger requiring sutures.

ROOF FALL - Occurred on E-12 No. 2 Longwall in No. 1 belt entry from cross cut 33 to 33-1/2. The rock fall was above roofbolt anchorage and was approx. 60 feet long, 20 feet wide and 10 to 14 feet in depth. No one was injured.

Employee was pulling curtain on the section, when EE felt pain in the left shoulder.

Employee had got off of locomotive and went to pick up a chain when they felt pain in their lower back.

Employee was lowering the canopy on 650 when the canopy guarding struck left elbow. EMPLOYEE HAS BEEN WORKING SINCE DATE OF ACCIDENT AND DID NOT START LOSING TIME UNTIL 3/19/2020 TO EVALUATE SHOULDER PAIN RELATING TO THIS INCIDENT. MRI IS PENDING TO CONFIRM CORRELATION.

Employee was walking the 10 inch discharge line in #12 sections return when the pipe came apart striking employee with debris and pipe resulting in sutures to arm and leg.

REPORTABLE ONLY DUE TO SUTURES - Employee was binding down the versa track duck bill when it shifted catching EE's right thumb between the chain and the equipment causing laceration requiring sutures.

REPORTABLE ONLY DUE TO PRESCRIPTION EYE DROPS - Employee was using a pump up sprayer to spray degreaser underneath the floor board of dozer, when the sprayer hose blew off the sprayer causing degreaser to get into both eyes.

Employee was shoveling coal at the feeder when EE twisted to the right and felt pain in lower back. Employee did not start losing time until 7/19/19.

REPORTABLE ONLY DUE TO SUTURES - Employee was operating the roofbolter on a section and was drilling hole, the bolter had become off balanced from being on a rock causing the roofbolt steel to strike employee in the head causing laceration requiring sutures.

REPORTABLE ONLY - Employee was welding in throat of feeder when the employee received a burn to right eye, resulting in a corneal flash burn.

Employee was hauling a shear jack on a pallet using a Lo-Trac when the jack fell off the pallet, the employee then tried to lift the shear jack back onto the pallet when they felt pain in right forearm. EMPLOYEE HAS BEEN WORKING SINCE DATE OF THE ACCIDENT UP UNTIL 10/16/19 DUE TO SURGERY ON RIGHT FOREARM.

REPORTABLE ONLY DUE TO SUTURES - Employee was standing in cross cut when a chain they were using to load a piece of pan line onto the 650 Versa Trac broke striking them in the mouth resulting in sutures to lip and broken teeth.

REPORTABLE ONLY DUE TO SUTURES - Employee went to sit down on tool box and caught EE's right index finger between the lid and box causing fracture to tip of finger requiring sutures.

Employee was helping Load a conveyor chute door onto a duck bill bus when EE's right middle finger became caught under door resulting in laceration to tip of finger requiring surgery.

Employee was standing in rear cargo area of delivery truck using lift gate to lower a Z-rail clothes cart. Z-rail cart shifted causing employee to fall from truck resulting in fractured wrist, knee and ribs.

Employee was washing down at the bottom of the slope, using a hose with a hole in it, the hose burst knocking the employee down causing a tear in the right achilles tendon requiring surgery.

Employee was walking through an airlock and as the employee closed one of the airlock doors the door suddenly shut with high velocity striking the employee's right wrist. Differences in air pressure across the airlock caused the door to slam shut.

Employee was in the process of changing drum liners at 7-18 hoist when employee was hit in the mouth by a bottom belt roller bracket which was being used to pull the wedge out of the drum. Employee required surgery to remove fractured teeth.

Employee was operating the shear on the longwall face, when a rock came over the handrail into the walkway striking employees right lower leg requiring sutures.

Employee was installing belt when EE tripped, started to fall and raised up hitting hard hat against the bar being used to support the roll of belt causing injury to neck.

REPORTABLE ONLY DUE TO SUTURES - Employee was helping to load blocks in a scoop, when EE saw a piece of wood in the way EE went to toss it out of the way and that piece of wood came back down on EE's left hand finger causing fracture to the tip and required sutures.

Employee was working around the scoop on a section while moving up the return, when the scoop hit a timber, the timber struck the employee causing a laceration to the forehead requiring sutures and a fracture above EE's left eye.

REPORTABLE ONLY DUE TO SUTURES - Employee was beside the feeder when the ram car entered the feeder to dump, causing bat wing to move and hit employees left lower arm resulting in laceration requiring sutures.

REPORTABLE ONLY DUE TO SUTURES - Employee was preparing cable to re-enter on Shuttle Car, as employee was cutting away the outer jacket of the wire, when ee cut right index finger requiring sutures.

An air reservoir tank was being installed when it was dropped approximately 36" landing on the injured's right pinky finger causing several fractures. Treatment was given at a local hospital and referred to a surgeon for follow up. Surgery was performed on the broken finger on 1/15/19.

Employee was prying on the snake head trying to get an E-bolt started through the slot in the pan line, when EE reached down to try and start it by hand, the other employee did not see EE put EE's hand down, accidentally hitting EE's hand with a sledge hammer causing fracture and sutures to fingers.

Employee was repairing cable on a shuttle car when the employee stepped back into a hole, twisting left foot causing sprain to EE's foot. Employee was put in air walker boot.

REPORTABLE ONLY DUE TO SUTURES - Employee was walking when ee tripped over bags of rockdust falling and hitting safety chain between power centers causing laceration to bottom lip requiring sutures.

IGNITION - Occurred at East A head roller at the North Bunker, while burning a hole at the top of the North Bunker a piece of slag dropped down into bunker and ignited methane that was in the coal, in the Bunker. The ignition was extinguished with one fire extinguisher.

REPORTABLE ONLY DUE TO SUTURES - Employee went to turn Lo-Trac around and struck a pallet of Brattice Blocks, one of the blocks struck left thumb causing laceration resulting in sutures.

IGNITION - occurred on 9 section in the No. 1 entry while installing a rib pin on the third row of bolts. Ignition lasted 4 to 5 seconds and was extinguished with one fire extinguisher.

Employee had placed the skirt hold down plate across the forks of the Lo-Trac to burn holes in plate, the hose from the torch caught the plate causing it to slide pinching finger between the plate and mast causing laceration requiring sutures and fracturing tip of right hand finger.

Employee had the engine hood cover up as EE was servicing the Lo-Trac, when the engine was started the vibration caused the door cover latch come out allowing hood to fall on employees right hand causing fracture to hand.

Employee was riding on a man bus when the bus came off the track. Employees were trying to jack bus back up on track. Employee was using a jack and the bar slipped out of the jack causing employee to fall and fracture their right leg.

IGNITION - Occurred as the headgate drive was raised about 7" above the foot wall when a methane/frictional ignition occurred under the panline at headgate drive from the longwall face chain and was extinguished with water.

Employee was roof bolting in the No. 1 entry when the employee put hand on top of the canopy and the left ring finger became caught between canopy and roof mashing the finger requiring sutures.

Employee saw that a fork was lose on the Lo-Trac and went to try and put the fork back on mast, when the fork fell off the Lo-Trac landing on employees foot causing injury.

Employee was assisting with lifting a chute using three ton come-a-longs, when they went to unhook from the frame they unhooked from chute causing it to come down hitting their left arm causing contusion.

Employee slipped and fell as they were walking down a staircase at the Preparation Plant injuring their right knee and left upper arm. EMPLOYEE HAS BEEN WORKING SINCE DATE OF ACCIDENT UNTIL TAKEN OFF WORK ON 10/25/18 DUE TO SURGERY ON THE RIGHT KNEE.

Employee was helping hoist a gearbox in place when a chain slipped as the bolts were being installed to secure the reducer causing it to catch employee's hand pinching it, requiring sutures.

Employee was walking down the stairs checking the belt in the raw coal tunnel at the Preparation Plant when their foot slipped, causing them to catch themselves on the handrail and caused injury to right upper arm.

IGNITION - Occurred on No 2 longwall while the shearer was cutting coal and rock mining toward the tailgate. Ignition lasted approx. 15 to 20 seconds and was extinguished by fire extinguisher.

IGNITION - Occurred on No. 2 Longwall while the Joy shearer was cutting and came in contact with a pumpable crib, the wire became entangled around the drum causing a frictional ignition with the drum at shield 175, which was extinguished by water and fire extinguisher.

REPORTABLE ONLY DUE TO SUTURES - Employee was trying to take down 4" Aqua Mine pipe at the coupling when it struck their upper right leg requiring stitches.

REPORTABLE ONLY DUE TO SUTURES - Employee was helping with belt prep for belt move when EE heard structure falling, EE turned around and frame struck EE causing laceration to EE's lower leg requiring sutures.

Employee went to pull slack and received an electrical shock when handling the roofbolter electrical cable.

Employee was working on hanging a new power cable and it slipped off and landed on the employee's right thumb.
