A miner was struck in the ribs with a drill steel that was hidden under the muck in a horizontal position. When the backhoe operator raised EE's outrigger it grabbed the steel causing it to twist up out of the muck striking the miner (right side ribs).

Injured operating fletcher center bolter in motor barn rehab area between #3 and #4 seal installing a bolt in a bacon strap and had left hand resting on drill mast. As mast went up it pinched the tip of EE's middle finger in the mast resulting in a crushing injury that required 6 sutures to close the wound. No problems with equipment or compliance issues, training or PPE.
Injured was roof bolting on the 36 1/2 block main west roof fall with an air drill when some head coal around an adjacent roof bolt fell out and allowed a bowling ball sized rock to slide out and strike them in the upper back, resulting in a possible muscle strain/sprain (no bruising, x-ray and CT scan negative). No problems with equipment or compliance issues, training or PPE.
Injured was assisting panline bolter operator near headgate of 5W longwall recovery face, attempting to install a bolt wrench in the bolter head when their right foot slipped and went under bolter head while it was lowering, pinching their foot between bolter head and mine floor, resulting in a contusion. No problems with equipment or compliance issues, training or PPE.
Injured installing rib bolt with 3' metal rib strap off right side of miner 41 block 1-2 crosscut 7W when the strap rotated from vertical towards horizontal and while trying to catch it, it struck EE's right wrist resulting in a laceration. Injured cut piece out of 1" steel plate at welding table on 1st floor of plant, while observing the scrap 1" steel being lifted off table with chain and chain hoist the chain slipped causing the steel plate to hit welding table and bounce off striking their right index finger resulting in a fracture. Miner starting turn into 54 block 1-2 crosscut and had mined enough to install first roof strap. Injured climbed up on the rear bumper and reached over the conveyor tail boom to retrieve roof strap and got pinched between the tail swing jack and the roof bolt tray on the frame of the miner, resulting in two fractures to their pelvis.
While using center bolter in 6 West 1-2 crosscut at 39 block a rock fell from the top and struck left hand as they were reaching for a stick of resin that was in a glue box placed on the bolter tray above the bolter controls, resulting in a contusion. No problems with compliance or equipment issues, job skills, proficiency, training, or PPE issues.
EE was bolting on a twin boom bolter in 9 North Mains. The bolter lost suction, so EE began checking the machine. EE noticed a piece of rock or coal stuck in the pre-dump door. EE cleared the material with right hand, and when EE did the door closed on EE's thumb and opened back up. EE began missing 4/3/24.
The EE was in 9 North Mains Section, bolting in the ladders on the left side. The injured was installing 16' cable bolts. The hole was drilled, and the starter steel became stuck. The EE reached for another steel, and when EE turned around the starter steel fell from the hole and struck EE in the head. EE received 3 staples in the head, but didn't miss any work.
EE was bolting on 6 Belt at 109 block. While adding a steel, one steel fell down from the mine roof and struck the EE in the right hand. The EE was not wearing gloves at the time. Began missing work on 8/5/2023.
The injured EE was using a grinder to cut a piece of pipe on the 9th floor of the plant. The wheel got bound and swung back and struck them in the hand. The EE was wearing gloves, and the grinder had a guard. The EE did not miss any scheduled shifts.
The EE was operating the miner on the 5W section, and was retrieving a bolt from the bolt tray located on the miner when a piece of slate/rock fell from the mine roof striking the EE right index finger. The EE was wearing gloves at the time of the incident.
The EE was in the process of securing a belt scrapper with a yellow rope hanger. The hanger caught something on the EE pulling their right arm into the belt roller. The EE was wearing the appropriate PPE. The EE began missing on 5/16/23.
The employee was rib pinning on the return side of the miner on 5W Section. A piece of rock/coal 18"x18"x4" thick fell from the binder, behind the tube and struck the EE in the lower back. EE began missing work 1/9/23.
The EE was using a grinder cutting bolts from belt structure, when a small piece of shaved metal struct the EE in the left eye, causing a corneal abrasion. The EE was properly trained and was wearing all PPE. EE began missing work 11/16/22
While installing a couple bolts in the Sugar Run Rock Dust Chute, part of the drills became stuck in the mine roof. When the steel cooled off, the piece fell striking the EE in the left hand causing a contusion.
Injured was tramming a Fletcher center bolter in the 9 north mains #5 entry when two roof bolt straps fell off the body of the bolter and struck EE in the left knee causing a contusion. No problems with equipment or compliance issues, training or PPE.
While using oxygen & acetylene torch to cut a metal grating (at the prep plant 1st floor welding table) the metal popped (from excess moisture and rust) causing a piece of slag to pop out and contact right eye lid, resulting in a partial thickness burn to upper and lower eye lid. Injured was wearing welding glasses. No problems with equipment or compliance issues, training or PPE.
EE was rib bolting on the miner in 4 West Section. A rock fell from in between two roof straps and struck the EE's left middle finger. This caused a pinching injury requiring 6 stitches. Also, the EE began missing work on 9/7/2022.
The EE was using a breast drill to rib bolt on the 3W Section. The drill got stuck, and came around struck the EE in the right forearm. EE did not sustain any breaks, didn't return to work until 8/4/22.
The EE was bolting on the L/W set-up face in 3 West Section. As they were lowering the steels, they accidently hit the rotation lever causing the steels to spin around and strike them the face. This caused a laceration and fracture to the left side of their face. The EE had a follow-up that was scheduled on 5/25/22 with a specialist, and then returned to work.
The employee was bolting with an air bolter on 2 ML Belt. When the injured went to put the drill steel in the hole and shove up, EE felt a pop in the left shoulder. The injury did not become reportable until EE being given off work on 9/21/21.
EE had just cut out at the tail of the 2W L/W and was heading back towards the head. The EE looked at the tail operator, and when they turned back a rock flew off the drum and struck them on the left side of their face. EE received three layers of stitches, and a facial fracture.
The EE was rib bolting off of the miner. A piece of rock fell out of the top and struck the EE on right ring finger causing a laceration. The injured received 5 stitches. The EE was wearing gloves, and was properly trained. EE did not miss any time.
The injured was the utility man on the 2 West Section. When injured, EE was on the rear right side of the miner putting a strap across to the left side. The head of the miner was sumped in the miner boom pushed the employee into the frame of the machine. The EE sustained no fractures or internal injuries.
The EE was on 1 West HG Section recovering track rail. The injured was cutting a piece of rail with a set of torches, when some slag popped and a piece fell behind safety glasses. It contacted around the right eye. EE began missing on 11/13/20.
The EE was bolting on 9 North Mains Section, in crosscut 2-4. EE removed the steel from the rib to install a rib bolt. The rib broke loose at the same time, and the EE hit the rotation on the rib drill. EE's right hand got caught by the rib steel causing abrasions/lacerations to EE's right pinky finger. Received 9 stitches.
