Using #39 walk through bolter T bar to push roof bolts out of the way and roof bolt after the place change miner had taken a cut out of the roof at 164-165 #1 St Leo. While extending the T bar oil shot out of a broken fitting and got into EE's left eye causing irritation. No problems with equipment or compliance issues, training or PPE.

Unplanned roof fall discovered reportable on 2 belt at 8 block at approximately 6:15 am on 08/06/2026. Fall was approximately 30' long, 16' wide, and 9' high. Area was mined near December of 1965. No problems with equipment or compliance issues, training or PPE.

Injured was changing a cradle at 143 block 6 belt and while pulling the old cradle out it rolled over and bent their right thumb around the back of their hand resulting in an unspecified sprain. No problems with equipment or compliance issues, training or PPE.

While mining towards the head on the 7W longwall face EE stepped over a relay bar and EE's foot slid on a piece of slick slate causing EE to fall and strike EE's left knee on a relay bar resulting in a contusion. No problems with equipment or compliance issues, training or PPE

Schindler changed out the four roller guide assemblies on the Miracle Run elevator, upon returning to service after the install there was enough side movement for the safety dogs to grab and set, resulting in an entrapment of 3 miners at the top of the shaft for approximately 35 minutes until the safety dogs could be reset.

Unplanned roof fall discovered reportable on 4 belt at 8 block at approximately 6:24 pm on 08/02/2026. Fall primarily occurred in an adjacent crosscut and came out into the belt on the off side walkway. Fall was approximately 12' long, 18' wide, and 6' high. Area was mined near June of 1967. No problems with equipment or compliance issues, training or PPE.

While walking down 10 belt at 75 block observing 2 employees carrying an air drill a rock fell from the top and struck EE, knocking EE off balance. EE fell to the ground on top of EE's left shoulder, resulting in a contusion to EE's shoulder. No problems with equipment or compliance issues, training or PPE.

Injured was loading supplies on CM (8W #2 51 blk) standing on the miner passing supplies to a miner on the offside. After miner was fully loaded they stepped down off miner and contacted a stored rib strap with left armpit and received a laceration that required 4 sutures to close. No problems with equipment or compliance issues, training or PPE.

Unplanned roof fall discovered reportable at the intersection approaching the #2 seal of the Sugar Run Seal Set at approximately 5:40 am on 07/15/2026. Fall was approximately 16' long, 14' wide, and 6' high. Area was mined near February of 1957. No problems with equipment or compliance issues, training or PPE.

Injured stuck head out from the passenger compartment of a moving mantrip near 144 block Main West (to look for a block number sign) and struck the side of head on an I-Beam that was bolted to the roof resulting in a laceration that required 7 sutures to close. No problems with equipment or compliance issues, training or PPE.

Injured parked scoop between airlock doors 2-3 at 53 block 7W and stepped on a block of coal while dismounting scoop, twisting EE's right knee. No problems with equipment or compliance issues, training or PPE.

Injured was walking and shoveling spillage along 6 belt at 120 block when a rock dislodged from the top and hit them in the back near the left shoulder blade area, resulting in a contusion. No problems with equipment or compliance issues, training or PPE.

Injured stopped trip to see if trucks were on track near 28 block 9 North and while standing next to de-railed block car a cube of block slid off and landed on their left foot, causing a contusion. No problems with equipment or compliance issues, training or PPE.

Injured was using a bar to assist in throwing the Sugar Run Crossover switch and felt pain in the middle of their back from a muscle strain. No problems with equipment or compliance issues, training or PPE.

Unplanned inundation of methane occurred in 8W #3 entry while mining through the west leg of SC-6 CBM well. No problems with equipment or compliance issues, training or PPE.

Injured exiting 126 mantrip at 37 block Main West with seat in raised position, the seat fell and caught their foot causing them to fall and injure neck. Went to doctor on 06/30/26 and started missing work. No problems with equipment or compliance issues, training or PPE.

Walking outby from #2 face of 8W at 41 block intersection the rib corner "popped" and they put a hand up to cover their face. A rock came out of the upper rib corner and struck their hand causing an open fracture and dislocation to the distal phalange of their right index finger. No problems with equipment or compliance issues, training or PPE.

Weekend warrior slipped while walking down stairs from the plant feed belt, re-injuring lower back. Sought medical attention 6/28, missed 6/29, returned 7/4 missing 1 day of work due to a muscle strain of the lower back. No problems with equipment or compliance issues, training or PPE.

Injured using emulsion drill to drill hole for rib bolt (7W belt, 50' outby tail) when the drill steel got stuck and spun out of their hands and struck them in the lower forehead in between eyes resulting in a laceration that required sutures to close. No problems with equipment or compliance issues, training or PPE.

IE was working with another employee to offload a steel set parts crate from an irwin car. They were using lever bars to move the crate. The other employee's bar slipped and all of the weight was suddenly on IE's bar. This caused IE's finger to be caught between the bar and the elevated portion of the car's deck. Injury was not the result of a training or compliance issue.

IE was part of a crew building cribs on top of a steel set. A post was being hoisted up on top and struck a piece of v-deck that IE was standing on, causing EE to fall through. Injury was not the result of a training or compliance issue.

The injured employee took it upon self to enter and operate a parked loading machine in 9W in the 2-3 crosscut of 10 block. EE attempted to move a trailing cable out of the way with EE's foot and inadvertently bumped a control lever, pinching EE's left foot between the frame of the equipment and the rib. No issues with equipment or PPE.

While hosing down a recent spill at the clean coal sump on the second floor of the prep plant EE stepped down from the concrete floor to the grating and twisted EE's right ankle, resulting in a sprain. No problems with equipment or compliance issues, training or PPE.

A miner was struck in the ribs with a drill steel that was hidden under the muck in a horizontal position. When the backhoe operator raised EE's outrigger it grabbed the steel causing it to twist up out of the muck striking the miner (right side ribs).

Injured was helping 2 other miners re-rail a 555 scoop at the 230 foot mark of the slope track. Two airbags were placed to lift the scoop, blocking was being installed to support the scoop to deflate the bags when a crib block shot out from underneath an airbag and struck the injured employees hard hat, resulting in a small contusion to EEs forehead and a sprain/strain to EEs neck.

Employee was exiting a mantrip when EE's right foot was stuck under the seat. When attempting to pull it free EE injured EE's right knee.

The rope gripper wear pads on Metz elevator wore down causing a pad wear microswitch to engage that causes the rope grippers to set, resulting in an entrapment of 2 miners approximately 350' down the shaft. The manual rope gripper release pump failed. The miners were removed from the elevator at 5:45 pm. No problems with equipment or compliance issues, training or PPE.

Injured was cutting down roof screen to unload "bagged" material at 3 block on 3 belt with a grinder and some loose rock that was in the screen fell out causing a laceration to the posterior side of EE's left forearm that required sutures to close. No problems with equipment or compliance issues, training or PPE.

While mining inby crosscut 23 of the #1 entry in 8 West injured was standing by rib bolting controls when a rock fell out from in between the roof straps and struck them in the back causing a closed fracture of spinous process of thoracic vertebra. No problems with equipment or compliance issues, training or PPE.

Using FCT to pull cable slack with loops of cable tied to a drill steel placed on the FCT while in the 8 West belt entry. Injured was holding drill steel to keep the rope from pulling off, but the rope pulled off anyways and the drill steel came back and struck them in upper left leg resulting in a contusion. No problems with equipment or compliance issues, training or PPE.

The mine received a Self-Insured Employer Decision on 3/16/26 from a medical report dated 12/18/25. The report identifies a 10% permanent partial disability possibly from occupational pneumoconiosis. Specific date and time of actual disease contraction is unknown.

Injured was using 28 center bolter in St Leo Mains 3-2 at 148 block. They had a drill steel laying on the tray above the drill controls and it was struck by the boom, flipping up and hitting them in chest resulting in a contusion. No problems with equipment or compliance issues, training or PPE.

Injured was striking a panline rack bar to get the panline to line up together on the 7W Longwall setup face when a piece of their pick hammer broke off and struck them in the chest, embedding itself in the soft tissue under the left pectoral. No problems with equipment or compliance issues, training or PPE.

Unplanned roof fall discovered reportable in the old 11 belt entry, which is now common with 9 South track, at 82 1/2 block. Fall was approx. 10' high, 16' wide and 30' long. The fall will not be cleaned up or re-supported. No problems with equipment or compliance issues, training or PPE.

The mine received a Self-Insured Employer Decision on 3/5/26 from a medical report dated 10/17/25. The report identifies a 12.55% permanent partial disability (hearing loss) possibly from occupational noise exposure. Employee has been retired since 4/16/25.

Injured stopped EE's motor outby the 9 North 27 block switch underneath a 6" aqualine (water discharge line). When EE stood up, EE struck EE's head on the airline causing pain in EE's lower neck (previously injured 4/28/25). No problems with equipment or compliance issues, training or PPE.

Injured was attempting to dismount Volvo rock truck with wet muddy boots and EE's foot slipped while climbing down steps causing an unexpected load on EE's left shoulder resulting in a sprain/strain type injury. Injured started missing work on 3/13/26 and was put off work pending an MRI. No problems with equipment or compliance issues, training or PPE.

Injured loading top belt structure into a belt tub in the Sugar Run Supply yard with the rest of belt crew. When EE placed the structure down the other employee pushed it over before injured's gloved finger was out of way and pinched it between structure and tub resulting in a chipped bone. No problems with equipment or compliance issues, training or PPE.

Injured holding up airline and cables at 97 blk 5 belt while another employee was using grinder to cut of belt structure nuts when the grinder caught and kicked striking EE's left index finger and causing a laceration that required 5 sutures. No problems with equipment or compliance issues, training or PPE.

Injured was carrying tools past 2 parked mantrips in 8W belt spur. While walking between mantrips EE stepped on an uneven surface causing EE to fall. Injured started missing work on 2-18-26. No problems with equipment or compliance issues, training or PPE.

Injured was walking away after throwing switch at Sugar Run slope back switch when EE slipped on the rail, falling onto the point rail. Injured started missing work on 2-19-26. No problems with equipment or compliance issues, training or PPE.

Injured was rib pinning in sump at St Leo Mains at 129 block, while walking around the bolter, EE stepped in a hole and twisted EE's left knee. Injured started missing work on 1/27/2026. no problems with equipment or compliance issues, training or PPE.

Employee was on crew changing rollers, and EE's hand was pinned between a roller and a locked out belt while replacing a roller in a cradle. Accident was not the result of a training or compliance issue.

Injured picked up approximately 15-pound rock off of catwalk to the Sugar Run Metso Pump and felt pain in lower right abdomen. Initially treated for appendicitis, but no evidence of this was found at follow up appointment, so employee was diagnosed with an unspecified abdominal muscle strain. No problems with equipment or compliance issues, training or PPE.

Injured installing rib bolt while cutting turn 3-2 at 115 block 9 North when a piece of the rock brow fell out and struck EE's left shin resulting in a laceration that required 9 sutures to close. No problems with equipment or compliance issues, training or PPE.

Employee pulling a power car in 7 West 3 left 3 block intersection by hooking a chain up between the power car and a scoop. They had scoop back up to take slack out of the chain and it failed immediately, throwing a link that struck them in the forehead resulting in 3 lacerations that required 6 sutures total to close. No problems with equipment issues, training or PPE.

At approximately 8:03am the bracket holding the top counter weight wheel failed and a piece of the bracket fell and damaged a sensor on the bottom of the sugar run elevator causing 3 miners to become entrapped on the elevator. The miners were safely removed from the elevator at 9:33am. No problems with equipment or compliance issues, training or PPE.

Injured employee was picking up a slope chain and placing it on the slope car when they felt a pain in their back. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.

Injured was hanging a run through curtain up in #2 entry of St Leo Mains 131-132 block, standing on the third rung of a 4' ladder leaning off the ladder to install a nail through the curtain when EE fell off the ladder and bruised EE's back when landing on the mine floor. No problems with equipment or compliance issues, training or PPE.

Injured picked up a lump of coal off the mine floor near 150 shield (6W) to throw into pan, but EE was closer to the pan than EE thought and struck EE's hand on the edge of the pan, pinching right ring finger between pan and coal, resulting in a fracture and open wound. No problems with equipment or compliance issues, training or PPE.
