A miner was struck in the ribs with a drill steel that was hidden under the muck in a horizontal position. When the backhoe operator raised EE's outrigger it grabbed the steel causing it to twist up out of the muck striking the miner (right side ribs).
Injured was helping 2 other miners re-rail a 555 scoop at the 230 foot mark of the slope track. Two airbags were placed to lift the scoop, blocking was being installed to support the scoop to deflate the bags when a crib block shot out from underneath an airbag and struck the injured employees hard hat, resulting in a small contusion to EEs forehead and a sprain/strain to EEs neck.
Employee was exiting a mantrip when EE's right foot was stuck under the seat. When attempting to pull it free EE injured EE's right knee.
The rope gripper wear pads on Metz elevator wore down causing a pad wear microswitch to engage that causes the rope grippers to set, resulting in an entrapment of 2 miners approximately 350' down the shaft. The manual rope gripper release pump failed. The miners were removed from the elevator at 5:45 pm. No problems with equipment or compliance issues, training or PPE.
Injured was cutting down roof screen to unload "bagged" material at 3 block on 3 belt with a grinder and some loose rock that was in the screen fell out causing a laceration to the posterior side of EE's left forearm that required sutures to close. No problems with equipment or compliance issues, training or PPE.
While mining inby crosscut 23 of the #1 entry in 8 West injured was standing by rib bolting controls when a rock fell out from in between the roof straps and struck them in the back causing a closed fracture of spinous process of thoracic vertebra. No problems with equipment or compliance issues, training or PPE.
Using FCT to pull cable slack with loops of cable tied to a drill steel placed on the FCT while in the 8 West belt entry. Injured was holding drill steel to keep the rope from pulling off, but the rope pulled off anyways and the drill steel came back and struck them in upper left leg resulting in a contusion. No problems with equipment or compliance issues, training or PPE.
The mine received a Self-Insured Employer Decision on 3/16/26 from a medical report dated 12/18/25. The report identifies a 10% permanent partial disability possibly from occupational pneumoconiosis. Specific date and time of actual disease contraction is unknown.
Injured was striking a panline rack bar to get the panline to line up together on the 7W Longwall setup face when a piece of their pick hammer broke off and struck them in the chest, embedding itself in the soft tissue under the left pectoral. No problems with equipment or compliance issues, training or PPE.
Injured was using 28 center bolter in St Leo Mains 3-2 at 148 block. They had a drill steel laying on the tray above the drill controls and it was struck by the boom, flipping up and hitting them in chest resulting in a contusion. No problems with equipment or compliance issues, training or PPE.
Unplanned roof fall discovered reportable in the old 11 belt entry, which is now common with 9 South track, at 82 1/2 block. Fall was approx. 10' high, 16' wide and 30' long. The fall will not be cleaned up or re-supported. No problems with equipment or compliance issues, training or PPE.
The mine received a Self-Insured Employer Decision on 3/5/26 from a medical report dated 10/17/25. The report identifies a 12.55% permanent partial disability (hearing loss) possibly from occupational noise exposure. Employee has been retired since 4/16/25.
Injured stopped EE's motor outby the 9 North 27 block switch underneath a 6" aqualine (water discharge line). When EE stood up, EE struck EE's head on the airline causing pain in EE's lower neck (previously injured 4/28/25). No problems with equipment or compliance issues, training or PPE.
Injured was attempting to dismount Volvo rock truck with wet muddy boots and EE's foot slipped while climbing down steps causing an unexpected load on EE's left shoulder resulting in a sprain/strain type injury. Injured started missing work on 3/13/26 and was put off work pending an MRI. No problems with equipment or compliance issues, training or PPE.
Injured loading top belt structure into a belt tub in the Sugar Run Supply yard with the rest of belt crew. When EE placed the structure down the other employee pushed it over before injured's gloved finger was out of way and pinched it between structure and tub resulting in a chipped bone. Injured holding up airline and cables at 97 blk 5 belt while another employee was using grinder to cut of belt structure nuts when the grinder caught and kicked striking EE's left index finger and causing a laceration that required 5 sutures. Injured was carrying tools past 2 parked mantrips in 8W belt spur. While walking between mantrips EE stepped on an uneven surface causing EE to fall. Injured started missing work on 2-18-26. No problems with equipment or compliance issues, training or PPE.
Injured was walking away after throwing switch at Sugar Run slope back switch when EE slipped on the rail, falling onto the point rail. Injured started missing work on 2-19-26. No problems with equipment or compliance issues, training or PPE.
Injured was rib pinning in sump at St Leo Mains at 129 block, while walking around the bolter, EE stepped in a hole and twisted EE's left knee. Injured started missing work on 1/27/2026. no problems with equipment or compliance issues, training or PPE.
Employee was on crew changing rollers, and EE's hand was pinned between a roller and a locked out belt while replacing a roller in a cradle. Accident was not the result of a training or compliance issue.
Injured picked up approximately 15-pound rock off of catwalk to the Sugar Run Metso Pump and felt pain in lower right abdomen. Initially treated for appendicitis, but no evidence of this was found at follow up appointment, so employee was diagnosed with an unspecified abdominal muscle strain. No problems with equipment or compliance issues, training or PPE.
Injured installing rib bolt while cutting turn 3-2 at 115 block 9 North when a piece of the rock brow fell out and struck EE's left shin resulting in a laceration that required 9 sutures to close. No problems with equipment or compliance issues, training or PPE.
Employee pulling a power car in 7 West 3 left 3 block intersection by hooking a chain up between the power car and a scoop. They had scoop back up to take slack out of the chain and it failed immediately, throwing a link that struck them in the forehead resulting in 3 lacerations that required 6 sutures total to close. No problems with equipment issues, training or PPE.
At approximately 8:03am the bracket holding the top counter weight wheel failed and a piece of the bracket fell and damaged a sensor on the bottom of the sugar run elevator causing 3 miners to become entrapped on the elevator. The miners were safely removed from the elevator at 9:33am. No problems with equipment or compliance issues, training or PPE.
Injured employee was picking up a slope chain and placing it on the slope car when they felt a pain in their back. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.
Injured was hanging a run through curtain up in #2 entry of St Leo Mains 131-132 block, standing on the third rung of a 4' ladder leaning off the ladder to install a nail through the curtain when EE fell off the ladder and bruised EE's back when landing on the mine floor. No problems with equipment or compliance issues, training or PPE.
Injured picked up a lump of coal off the mine floor near 150 shield (6W) to throw into pan, but EE was closer to the pan than EE thought and struck EE's hand on the edge of the pan, pinching right ring finger between pan and coal, resulting in a fracture and open wound. While guiding roof bolt sections (#6 bolter Sugar Run Rehab) together (with bolt head in chuck) the drill head rose slightly when the other drill removed up pressure causing their left ring finger to get pinched between bolt sections resulting in an open wound and fracture to distal phalange. While walking near the mantrip station in St Leo Mains the injured felt pain in EE's right knee (did not trip/slip/misstep, no walkway obstruction). Reported injury and started missing work on 12/4/25. No problems with equipment or compliance issues, training or PPE.
D35 traveling toward Metz with clearance and collided with D84 (traveling towards St Leo without clearance) at 0 block 9 South causing injured to be thrown into the cab of the mantrip passenger compartment, resulting in 3 fractured ribs and 2 fractured vertebrae. No problem with equipment issues, training or PPE.

Injured was uncoupling a supply car incorrectly at the slope chute track and got EE's finger caught between the release lever and the coupler shank causing a crushing injury to EE's left middle finger that resulted in an open wound which required sutures to close.
At approximately 2:39 pm the governor rope for came off of the tail sheave and lodged in the governor wheel of the Sugar Run elevator causing one miner to become entrapped on the elevator approximately 20' from the bottom of the shaft. The miner was safely removed from the elevator at 4:12 pm. No problems with equipment or compliance issues, training or PPE
While removing fire suppression from the scoop charger during a power move in St Leo Mains (121 blk 2-3) the clamp slipped and when EE went to catch it EE ran EE's gloved hand down the edge of a roof strap resulting in a laceration that required sutures. No problems with equipment or compliance issues, training or PPE.
Injured was rolling a shuttle car tire from the scoop bucket to the side of the shuttle car to install the tire and felt pain in EE's right shoulder (7 West 1 left entry, 0 to 1 block). No problems with equipment or compliance issues, training or PPE.
While drilling hole for cable bolt in St Leo #3 126-128 block the steel got stuck in top, miscounted how many steels were in roof (removed 2 of 3 steels) and last steel fell out striking back of left hand causing open wound that required 11 sutures to close. No problems with equipment or compliance issues, training or PPE.
Injured was changing winch rope on duckbill scoop in 7W (75 blk 1-2 crosscut). They had their left hand resting on the mast of the scoop and threw the winch hook onto their own left thumb, causing an avulsion to the thumb nail. No problems with equipment or compliance issues, training or PPE.
Injured removed 70" roller and set it along the rib near the St Leo mantrip station, when the roller fell over and struck their right middle finger pinching it between the roller and a flat car resulting in an open wound that required 5 sutures. No problems with equipment or compliance issues, training or PPE.
Unplanned roof fall discovered reportable on 3 belt just outby the 3 block intersection at approximately 6:46 am on 10/31/2025. Fall occurred in an adjacent brow left from a previous roof fall that occurred 8/2/2020. Fall was approximately 8' long, 16' wide, and 6' high. Area was mined near December of 1988. No problems with equipment or compliance issues, training or PPE.
DC drive failed on the Metz elevator which interfered with its use for more than 30 minutes. No miners were on the elevator at the time of the fault. Drive replaced and elevator returned to service by 12:15 pm. No problems with equipment or compliance issues, training or PPE.
10 miners entrapped on Metz Elevator until 8:38 pm when a VFD failed on the elevator. The miners were exiting the mine and were still approximately 700' down the shaft. No problems with equipment or compliance issues, training or PPE.
Lowering empty trash car down Sugar Run Slope that de-railed around the 1492 mark, allowing rope slack to build up and coil behind the brake car. When tension was re-applied the rope caught on itself resulting in a kink in the rope approx. 700' away from the brake car. Hoist taken out of service until rope replaced. No problems with equipment or compliance issues, training or PPE.
Unplanned roof fall discovered reportable at 21 block 3 belt. Fall approximately 14' wide, 14' long and 5' high. 21 block overcast was damaged and compromised from the roof fall. No problems with equipment or compliance issues, training or ppe.
Injured operating fletcher center bolter in motor barn rehab area between #3 and #4 seal installing a bolt in a bacon strap and had left hand resting on drill mast. As mast went up it pinched the tip of EE's middle finger in the mast resulting in a crushing injury that required 6 sutures to close the wound. No problems with equipment or compliance issues, training or PPE.
While applying a chain around a recently coupled supply car to a motor when the motor on the other side of the trip tried to uncouple, but the couplers got stuck and ended up pulling the supply cars and motor a short distance causing a strain to their shoulder and upper back. Started missing work 9/20/25. No problems with equipment or compliance issues, training or PPE.
The Miracle Run elevator faulted around 3:40 pm, faults reset and then tested when it faulted again with slightly different results. Through investigation, in cooperation with Schindler elevator service it was eventually discovered that the bottom safety shoe had turned and resulted in broken pins. No problems with equipment or compliance issues, training or PPE.
Injured standing in first crosscut outby 8 west equipment doors near the end of loose take up rails while a scoop was unloading a double stack of take up sections on top of the loose rails. Duckbill drug across a loose rail, kicking it up when it got to the end of the rail striking injured, knocking EE down and landing on EE resulting in a fractured vertebrate.
Injured was bolting off side of miner in St Leo 113 blk 3-2 with new section of roof mesh, not bolted to top yet, on the miner when a piece of coal (3'x3'x5") dislodged, slid down the screen, bounced off the rib and struck EE, resulting in a fracture to the L1 vertebrae. No problems with equipment or compliance issues, training or PPE.
Module on the back of the touch screen/display went bad which blew a control fuse and caused rope grippers to engage, entrapping 1 miner on the Sugar Run Elevator until 9:25 pm. No problems with compliance issues, training or PPE.
Injured was cutting a zip tie off an oil soaked hydraulic hose in the Metz Motor Barn washdown bay when EE's left hand holding the hose slipped off and EE struck EE's left index finger on the razor knife blade resulting in a laceration that required 4 sutures to close. No problems with equipment or compliance issues, training or PPE.
