Mostrando los 11While using the impact wrench to take off a bolt that was holding the cutting edge in place on a loader he was holding the bolt up in the slot with his left index finger to keep the bolt from turning and it slipped down out of the slot while using the impact wrench letting the bolt spin cutting through his glove and also the pad of his left index finger.
EE caught safety vest in spinning drill auger and was entangled in auger. EE received bruising to ribs and right bicep.
Employee was working on installation of dust collection on pug sealer and while using a hand grinder a metal particle went in the employee's eye. Employee was wearing safety glasses and a face shield at the time of the incident.
Released the latch on the winch, the handle came around and hit his hand.

Employee has 5 broken ribs on the right side. Fractured right shoulder. Lacerated Liver. Fractured vertabrae #8 thoracic. Employee is in ICU at Deaconess Hospital in Billings, MT.
Employee stated that as she was sealing bags of plug mud, her right arm began to hurt just above the wrist.
EMPLOYEE WAS DRILLING CLAY WHEN AN AUGER FELL OUT OF THE STORAGE RACK AND STRUCK THE EE ON THE HEAD, DAMAGING THE HARD HAT & CAUSING A LACERATION ON THE EE'S HEAD.
EMPLOYEE WAS CHECKING THE SOCKS ON THE SOUTH MILL BAGHOUSE WHEN HE SLIPPED AND PUT HIS HAND DOWN TO CATCH HIMSELF. HIS FINGERS WERE THEN CAUGHT IN THE ROTARY VALVE, SEVERING THE TIP OF THE MID DLE FINGER AND CAUSING OPEN FRACTURES OF THE INDEX AND RING FINGERS.
EMPLOYEE WAS BENT OVER A PIECE OF METAL CUTTING A HOLE IN IT WHEN HE FELT A MUSCLE SPASM IN HIS LOWER RIGHT BACK.
