Employee stepped on grease with EE's left foot and slipped. Employee did not fall but caused pain and discomfort in EE's left hip. Employee received medical treatment from chiropractor on 3/22/24 due to continued discomfort and pain.

The employee was setting a new roll of shrink wrap onto the bracket. The employee did not move hand out of the way when EE laid the metal pipe (that goes through the middle of the roll) down onto the bracket. The employee's middle finger got caught in this pinch point which caused a laceration to the middle finger. The employee received several stitches to close the wound.

The employee was using a pipe wrench to take a knuckle valve apart. The knuckle valve was secured in a vice. While rotating the pipe wrench, the pipe wrench slipped off the knuckle valve causing the employee to lacerate a finger on metal ducting that was on the work bench. The laceration required six stitches.

A new employee was being trained on how to seal and stack 50 lb bags. During this time the employee's left forearm and wrist became sore. On 9-30-2018 the employee reported that the employee's forearm/wrist area was swollen and it was hard to close the employee's hand. On 10-01-2018 the employee saw a physician and was diagnosed with possible tendinitis and put on restrictive duty.

EE was assisting in a four person team lift of an electric motor. During the course of the second lift, he indicated that he felt his shoulder pop. Two hours later he was taken to the hospital where an examination was performed and it was recommended by the physician that the EE be given restricted duty.

EE was closing the access door on the bag house when he adjusted his hands causing the right hand to slip and bringing the door down upon his left hand. The middle finger of the left hand was caught between the door and the seal edge severing the tip of his middle finger.

EE was cutting away cardboard strapping around a roll of shim stock. The roll unraveled quickly cutting EE's right middle finger. EE received two stitches in finger.

EE reached around a guard to clear material conveyor without following proper lock out tag out procedures. Hand was caught between the tail pulley and idler conveyor. He was able to pull a stop cord stopping the conveyor, but remained with his hand caught. He had to wait assistance to be removed from the conveyor. His hand received abrasions as a result of this incident.

EE was in an awkward position while standing on plug-mud product chute. He slipped while adjusting his stance. EE reached up and grabbed a hold of a piece of pipe overhead to catch himself, doing so he heard a pop in his right shoulder. The EE had a small muscle tear in his shoulder. He was put on restricted duty. EE was equipped with and utilizing fall protection.

While freeing the last bolt in a joint of two 8-inch suction lines the unsupported end of one pipe dropped roughly 4 inches. When the pipe dropped it pinched his index and middle fingers in between the flange on the pipe and a metal support bracket. Instinctively he tried to jerk his hand out of harms way. The nail on the index finger was lost and the tip of the finger was broken.

Employee mis-stepped while climbing a fixed ladder from the load-out shed into the load-out shack. Employee caught himself with his knee causing a small cut that required 3 stitches.

Helping maintenance with replacement of lid over the hammer mill, employee used a sledge hammer to align the lid. Metal fragment about 1/4"long and 1/8" thick broke off and penetrated his left arm just below the elbow. The metal fragment was not visible and had to be removed at the hospital requiring a couple of stitches to close the wound.

Employee was removing lids in dryer baghouse. With limited headroom and uneven footing employee strained lower back.

Employee reached around the conveyor guard and grabbed a plug mud bag between the conveyor belt pinch point and the bag roller. Upon doing this his hand was pulled into the pinch point between the conveyor belt and the bag roller.

Extension ladder was being used upside down in slick/wet conditions and it slipped and fell causing injury to employee's wrist.

Employee was reaching to pull a bag off a conveyor when his glove got caught between the conveyor and the deck, pinching his left hand and wrist. Employee suffered a bruised left hand and a wrist abrasion.

While cleaning the loading shed the employee was bitten by an unidentified insect. The bite resulted in swelling and discomfort. Employee was put on antibiotics.

While performing cleaning duties on a catwalk, employee stepped on a tool, causing him to fall and bruise his knee and shoulder.

Extension ladder used to elevate employee was not locked in securely. When the employee applied his weight to the ladder it fell causing him to lacerate his lip and bruise his inner arm. 3 sutures were required for his lower lip.

Employee exceeded safe speed limitation for fork lift while making a sharp turn causing the fork lift to turn on its side. Weather and slippery conditions were not a factor. Employee was wearing his seat belt.

Employee was pushing on pipewrench. Pipewrench popped off of bar. Employee lost his balance and struck the side of the car with the left side of his head.

While operating packaging equipment, employee began experiencing numbness in right hand, arm, and shoulder.

Employee was using a shovel to clean off the roof of the grizzly pit. As he went in for a scoop, the head of the shovel made contact with a nail. This reportedly caused the employee to jam his right shoulder.

Employee was lifting a small tank and pulled lower back muscles.

EMPLOYEE WAS LIFTING A GEARBOX FROM THE BACK OF A PICKUP WHEN HE FELT PAIN IN HIS BICEP.

WHILE OPERATING THE PACKING SYSTEM, EE'S RIGHT WRIST BECAME SWOLLEN AND SORE.

EMPLOYEE FELL WHILE STEPPING TO LADDER AND STRUCK HIS BACK AND HEAD.

EE STRUCK HAND ON A PIECE OF METAL WHILE LIFTING A SCREEN.

EE WAS LIFTING A PALLET FULL OF MUD WHEN HE INJURED HIS RIGHT SHOULDER.
