EE was backing the miner out of the cut of coal when the cable went under the miner. He went up to remove cable from under the miner and accidentally bumped the remote control box against the cable stand-off, activating the tram lever and then the panic bar. He was caught between the coal rib and the stand-off of the machine.

EE was operating roof the bolter on the operator side, standing on ground. The operator man-up platform, which was chained up, fell striking his left knee.

EE was cutting timber with a hydraulic chain saw. His hand slipped and landed on the chain requiring stitches.

EE "ALLEGEDLY" stated that while drilling the first of two holes for a 5 foot rib strap, he experienced pain in his lower back. He continued working the remainder of the shift and worked thru 9/10/10 with first day missed 9/11/10 and first Doctor visit was 9/11/10.

EE was installing a roof bolt and lowered the roof bolter drill pot down on his right foot.

EE was using a lifting jack to raise the boom on roof bolter. The jack knocked out and struck his right knee.

EE was inserting a piece of drill steel in the coal drill, lever was bumped causing the drill head to advance, mashing his right index finger.

EE started to drill a roof bolt hole and the drill steel kicked out and struck his left hand.

EE twisted his back while bolting roof.

Employee was bolting top when drill steel broke, striking his right forearm.

Employee was hanging the roof bolter cable and the operator pulled the lever to take up the slack. The cable took his feet out from under him and he landed on his back.

EE was moving the roof bolter, reached up to move an old roof bolt that was hanging down and caught his right ring finger between the roof bolter canopy and the roof bolt plate.

Employee was inserting a five foot roof bolt with plate. The plate slid down the bolt striking his left hand and thumb, possible fracture.

Employee's glove got caught on the roof bolter drill steel, twisting his right hand. His last day worked was 4/15/08 and started missing work on 4/16/08 for hand surgery.

Employee was shearing rib for belt line with miner, rock came out of conveyor chain hitting him in bottom of chin.

Employee was standing in front of miner cable, miner pulled forward and rt. foot got tangled up in cable.

EE was setting drill steel when he hit wrong lever, closing drill steel holder clamp onto his finger.

Employee was troubleshooting hydraulic circuit (cable reel valve) on a mobile roof support. While holding a rag and thumb over a port on the valve, oil pressure was applied and the oil pierced his left thumb and injected tissue with oil.

Employee was setting a roof jack and strained lower back.

Employee was putting up a roof bolt when it bent and mashed his left little finger against the pot.

Roof Bolter Operator was assisting maintenance crew in changing out the miner cutter head. While operating a portable hand grinder, the rotating cylinder disk cut his left forearm.

EE was taking down the roof bolter cable that was hung. The offside roof bolt operator started the bolter and the cable reel was left on. The cable hit under his right arm pit, lifting him into the air, jamming his right hand against the roof.

Employee was standing on bolter cable while it was tramming. Bolter turned and jerked cable and flipped him, hitting head. He continued working that shift and all scheduled shifts until he reported to work on 9/8/05 and told supervisor his head hurt and was dizzy and went home before shift started. This injury is being CONTESTED since he worked 2 weeks before having a problem.

EE was moving and relocating roof bolter and canopy hit his left shoulder and neck. Worked remainder of shift and 2 additional days; last day worked 7/25/05.

Employee was drilling hole for roof bolt using 2 piece steel. Finished drilling with first piece and dropped head down and added second piece. Started drill pot up to connect steel and drill pot got hung or stuck leaning steel out towards operator, then came loose and steel smacked right thumb while holding first piece of steel.

EE injured left leg and ankle when left rib rolled while operating Roof Bolter. He worked his next 4 scheduled days and missed work on 1/12/05, his first scheduled day not worked.

Employee let roof bolter boom down, boom hit piece of drill steel laying in tray, tray flipped up and hit left shoulder and neck.

EE was operating remote control of MRS while pillaring. A large piece of rock fell between rib and roof bolt hitting back and legs.

THE DAY SHIFT ROOF BOLTER OPERATOR WAS DRILLING A TEST HOLE IN THE INTERSECTION BETWEEN NO. 6 & NO. 7 ENTRIES NEAR SPAD # 1105 WHEN DRILL STEEL STRUCK HIS LEFT THUMB.

USING SPRAY MACHINE TO SPRAY PLASTER MATERIAL ON STOPPING HOSE NOZZLE STOPPED UP AND WHILE UNSTOPPING NOZZLE PLASTER MATERIAL SPRAYED IN EYES.

WHILE OPERATING CONT MINER RIB ROLLED OUT STRIKING LEFT FOOT AND LEG. FIRST DAY MISSED WAS 4-23-03 DUE TO EE WORK SCHEDULE. BECAME REPORTABLE 4/3/03.

#6 ENTRY WHILE BOLTING ROCK FELL FROM MINE ROOF STRIKING THE TOP OF HEAD AND NECK.

EE'S RIGHT HAND WAS ON ROOF BOLTER, ROCK FELL FROM TOP STRIKING RT. HAND AND FINGER.

EE STATED HE WAS OPERATING A RRII ROOF BOLT MACHINE AND DRILLING A HOLE. AFTER DRILLING THE HOLE, HE LEFT THE DRILL STEEL HANGING INSIDE THE DRILLED HOLE WHILE HE LOWERED THE DRILL HEAD. THE D RILL STEEL DISLODGED AND FELL OUT FROM THE HOLE STRIKING HIS INDEX FINGER ON HIS LEFT HAND.

EMPLOYEE STATED HE WAS OPERATING THE FLETCHER RRII ROOF BOLTER AND INSTALLING EYE BOLTS FOR THE NEXT BELT MOVE SET UP WHEN A TOP PIECE OF COAL RIB BROKE LOOSE UNDER PRESSURE AND STRUCK HIS LE FT SIDE, LEFT ELBOW, AND LEFT WRIST.

ROCK FELL FROM TOP FROM ROOF BOLT TO RIB. HIT BACK AND LEG. 2 FT WIDE, 2-3 INCHES THICK, 4-5 FT LONG.

EVENING SHIFT ROOF BOLTER OPERATOR WAS DRILLING ROOF IN NO 1 ROOM NEAR SPAD NO. 483 WHEN A PIECE OF ROCK FELL FROM THE ROOF AND STRUCK HIS FOREHEAD.

ROOF BOLTER CREW WERE DRILLING IN BREAK BETWEEN NO. 4 AND NO. 5 ENTRIES (BETWEEN SPAD NO. 5 260 AND 263) WHEN A PIECE OF ROCK STRUCK INJURED ON THE HEAD AND DROVE HIM TO THE GROUND.

THIRD SHIFT ELECTRICIAN WAS CUTTING AND WELDING MINER BIT ON CONTINUOUS MINER WHEN HOT SLAG WENT INTO HIS BOOT AND BURNED HIS LEFT LEG AND FOOT.

BOLTING THRU BREAK WHEN A PIECE OF DRAW ROCK FELL ON CANOPY, STRIKING INJ CUTTING HIS LOWER BACK & HIP

OPERATING ROOF BOLTER A SMALL PIECE OF SLATE FELL STRIKING LEFT HAND MIDDLE FINGER.

DRILLING A HOLE TO SET TAIL PIECE. DRILL STEEL HUNG IN BOTTON, CAUSING DRILL TO TWIST IN HIS HAND. HANDLE OF DRILL STRUCK HIS RIGHT ARM.

EMPLOYEE WAS APPLYING STOPPING SEALANT WITH A SPRAY MACHINE WHEN APPLICATION HOSE SLIPPED THROUGH HIS HANDS AND SEALANT STRUCK HIS FACE, NOSE, LEFT EYE AND EAR.

EMPLOYEE WAS ADJUSTING HYD CHAIN SAW WHILE IT WAS RUNNING, AND HIS FINGER GOT IN CHAIN.

WHILE OPERATING A S&S FEEDER, EMPLOYEE STRUCK HIS RIGHT EYE WITH A PIECE OF THE WIRE.

EE WAS PERFORMING HIS REGULAR JOB AS ROOF BOLTER OPERATOR WHEN HE EXPERIENCED CHEST PAINS.

BOLTING TOP STRIKING LEFT ELBOW AGAINST BOLTER. UNAWARE THIS WAS AN ACCIDENT, EMPLOYEE SAID HE WAS OFF SICK.

EMPLOYEE WAS INSTALLING A ROOF BOLT AND AS HE BEGAN TO TIGHTEN THE BOLT, A PIECE OF ROCK FELL AND STRUCK HIS RIGHT CHEEK.

EMPLOYEE WAS ON A LADDER HANGING LINE CURTAIN BESIDE ROOF BOLTER. AS THE BOLTING MACHINE MOVED, AN S-HOOK ON ROPE CABLE HANGER CAUGHT LADDER ANDPULLED IT OVER. EMPLOYEE FELL AND INJURED RIGHT HAND AND GROIN.

INJURED WAS BOLTING ROOF. HE WAS WEARING RUBBER GLOVES AT THE TIME. HE STATED THE STEEL GOT HOT AND DRIED THE GLOVE, STICKING TO IT AND TWISTING HIS HAND WITH THE ROTATION.
