Farley Eagle Mine Coal

He was breaking rock & coal off the roof bolter cable. He was using a short roof bolt & a hammer. While doing so he missed the bolt striking his hand. He finished the shift and went to the Dr. the next day. He got a return to work slip for 6/18/13.

Employee left the section to travel to the no.4 head to perform permissibility when the steering knob slipped from her hand and spun around striking the top of her hand.

EE was performing checks on the dust sampling device being worn by the miner operator. When he walked by the turn table of the machine a rock meas. 74" x 39" x 8" thick fell and struck EE on the left side of his body, resulting in a chipped distal femur, fractured knee, tibia, fibula and multiple lacerations to the face and head.

EE was driving the mantrip to the section when a piece of rock fell from the roof striking his right arm knocking it against the steering wheel.

EE was opening a can of smokeless tobacco, cut palm of right hand with a utility knife, requiring sutures.

EE filed an ALLEGED claim for Occupational Pneumoconiosis on 10/28/11. Received notice on 11/16/11 for Claim No. 2011016807.

EE was making a cable splice, knife slipped and cut palm of left hand, requiring sutures.

Employee filed an ALLEGED occupational pneumoconiosis claim on 1/12/2011. Received notice on 3/17/11 of Claim No. 2011001986.

EE was taking cable down as roof bolter was backing out. The operator turned the cable reel on and the cable jerked off the hangers, striking him and driving him to the ground. He continued working his regular schedule through 1/3/11, then Doctor ordered a 20 pound maximum lift.

EE was loading pizza pans on the roof bolter, felt pain in groin area on Saturday, went to Doctor on Monday and returned to work on Tuesday.

EE stepped off the roof bolter and twisted his right knee.

Employee "ALLEDGELY" strained his back while lifting roof support pizza pans.

EE was removing the tail piece guard, stepped back and twisted his right ankle.

EE slipped off the roof bolter and fell to the ground, pulled muscle in lower back.

EE was riding in a mantrip that was pulling a broken down mantrip. He was sitting on his side when the chain pulled tight. He ALLEDGEDLY hit his hip on a piece of metal on the back of the seat. This injury was not approved by Brick Street that occurred in the course of and as a result of covered employment until August 13, 2010.

EE was installing belt on #9 Belt Conveyor. Both hands got caught under cable, cutting and mashing fingers on both hands.

EE was moving a feeder reducer from one side to another, pulled muscle on right side of back with right leg pain.

EE was changing a hydraulic hose on the roof bolter t-bar when he slipped and twisted his left knee.

While restoring old mine works for advance mining, EE was lifting on a rail to remove a chain and pulled a back muscle.

EE was inserting a piece of drill steel in the coal drill, lever was bumped causing the drill head to advance, mashing his right index finger.

Putting BOP on wellhead. BOP was started and was not threaded on far enough when it fell to the ground striking employees foot.

EE was assisting in making a belt move when the tailpiece kicked sideways. The side board knocked him into the rib, cutting his head and mashing his left thumb.

EE was moving shuttle car inby feeder, hit hole in roadway and struck head on canopy.

EE was on his way to the outside, when a rock fell from roof, striking his left knee.

Employee was hitting a crib block with a hammer, pulled hammer back and hit himself in the forehead.

Employee was bolting top when drill steel broke, striking his right forearm.

Employee was operating a scoop in roadway when a rock fell out between bolts striking his head, neck & shoulder.

Employee was bolting roof and a rock fell out between roof bolts striking his left shoulder and neck.

Employee was hit on the right side of the face by a rock that fell from the rib and came inside the scoop canopy.

EE was cutting jacket on shuttle car cable, knife slipped and cut his left little finger.

Injury hit his head on the shuttle car canopy when he hit a hole in the road way bruising his neck and back.

Employee was unloading belt structure, structure hung up and jerked his left shoulder. His first day missed was 9/10/09 for surgery to left shoulder.

EE was hit in the head by a piece of rock that fell near the rib. The rock was approximately 22" x 29" x 1" thick. He finished working a 10 hour shift.

EE filed an ALLEDGED claim for OP on 3/10/09. Received notification on 5/14/09, Claim No. 209006782. This claim was filed on the same day that an injury was reported on E-Document # 829395 and he returned to work on 5/13/09 for that injury. Previous Claim No. 206010933 (E-Document # 253251 filed 10/17/05) was filed on 9/22/05 and no award was granted.

EE was moving the roof bolter, reached up to move an old roof bolt that was hanging down and caught his right ring finger between the roof bolter canopy and the roof bolt plate.

Employee filed an ALLEGED occupational pneumoconiosis claim on 10/24/08. Received notice on 11/4/08 of Claim No. 2008033384. He was awarded 10% on Claim No. 97035477 on 11/10/97.

Employee filed an ALLEGED claim for Occupational Pneumoconiosis on 9/26/08. Received notice on 10/21/08 of Claim No. 2008030403.

Employee was lifting locker in change room; locker slid thru hands cutting palm of left hand.

Employee was standing in front of miner cable, miner pulled forward and rt. foot got tangled up in cable.

Employee injured his left foot and ankle when his foot tangled in the miner cable while running from a fall on the pillar section.

EE was pulling belt back, stepped in a hole and twisted right knee

Employee filed an "ALLEDGED" occupational pneumoconiosis claim on 1/19/07. Received notice on 3/1/07, Claim No. 2007002049. His last day worked was 5/6/06 due to injury, e# 338228.

Employee filed an "ALLEGED" Occupational Pneumoniosis claim No. 2008007063. Received notification on 3/10/08.

Employee was pulling Mobile Roof Support Cable, fell backwards and strained back.

Employee filed an "ALLEDGED" occupational pneumoconiosis claim on 9/25/06, Claim No. 2006056582. This is his second filing, First Claim No. 98057615.

Employee filed an "ALLEGED" occupational pneumoniosis claim on 9/22/06. Received notification on 10/16/06, Claim No. 2006055598.

EE "ALLEGED" that while operating a shuttle car, he hit a hole in the roadway causing a back injury. His first day absent from work was 8/2/06.

Roof fell in #1 entry, one break inby spad 6196 and continued for approximately 40 feet thru crosscut. Fall was approximately 90feet long, 19 feet wide and 4 feet thick.

Roof fell in #6 Entry inby spad #6189. Fall was approximately 120 feet long and 4 to 6 feet thick.

Using a slate bar to pry rock off miner, resulted in a herniated groin muscle. It is felt that this injury should not be considered an occupational injury.

Employee was installing roof bolts on the operator's side of the roof bolter when the left rib rolled off, pinning both legs under the debris. It bruised and caused swelling of the left leg.

EE was taking down the roof bolter cable that was hung. The offside roof bolt operator started the bolter and the cable reel was left on. The cable hit under his right arm pit, lifting him into the air, jamming his right hand against the roof.

Loose coal mashed employee's foot against the dust box of the roof bolter, breaking his left ankle.

Employee filed an ALLEGED claim for Occupational Pneumoconiosis on 09/22/05. Received notification on 10/11/05. Claim No. 2006010933-OP. On 3/9/06 the OP Medical Board findings of 1/12/06 did not make a diagnosis of OP. No award was granted and is not entitled to any medical benefits. Therefore ee should be removed from the OP list.

EE was setting a steel roof jack under a steel I-beam at the fan housing. Picked up jack & twisted his left knee & strained his shoulder. He returned to work on 8/22/05 but was unable to return to work on 8/23/05. EE HAD ROTATOR CUFF & KNEE SURGERY.
