A roof fall of unknown length x 20' x 3' to 5' thick occured in the #1 return entry of 3 East Mains, 90' inby spad 3251 and 1150' from the active section. All access to the fall area has been dangered off.

A roof fall 30' x 20' x 5' thick occured in the #2 return entry of 3 East Mains, one break from spad 3263 and 1050' from active section. Timbers were set in the area prior to the fall. The return travel way has been re-established.

A roof fall 120' x 20' x 6' thick occurred 200' inby spad # 4326 in the bleeder system. The area was bolted with 4' fully grouted roof bolts. The area is taking pressure due to undermining.

A roof fall occurred at spad #2178 and measured 60'x18'x4' to 5' thick. The area was roof bolted with 4' long fully grouted roof bolts approximatelly 8 years ago. The fall will not be cleaned up and no injuries occurred.

EE was digging miner cable out of gob with a pick hammer and hit cable causing a flash and received burns to his upper extremities.

EE was using a lifting jack to raise the boom on roof bolter. The jack knocked out and struck his right knee.

Employee was walking on the section; a rock (approximately 1-2" thick by 24" round) fell from the roof hitting his hard hat and right shoulder.

Employee was walking in a low top area; he "ALLEGEDLY" turned his back when he raised up in a higher top area. Mine management was not aware of the injury until he was scheduled to return to work after the vacation period on 7/8/08.

The fall occured in the #3 entry approximately 1600 feet from the surface on 1-A belt roadway. Fall is about 40' x 20' x 2 to 4 feet thick in a remote neutral area and will be timbered off.

Injury "ALLEDGED" to have injured his back when he hit his head on the mine roof while loading 8 foot roof bolts onto the roof bolting machine. He continued to work until his first day missed on 6/14/08.

On Sept 24, 2007 a roof fall was found near spad #739 along #7 belt in the #7 entry. The fall measured approx. 74' long x 20' wide x 7' thick. The fall does not impede airways or travel and will be timbered off to prevent travel.

Fall occured in the intake entry #2, 14000 feet away from active face. Will clean up walkway and set jacks for escapeway. Fall was 12 ft. by 16 ft. by 3 to 4 ft. thick.

Employee stepped in a hole while mounting scoop deck and twisted left ankle.

Employee was walking by #2 Schuttle Car anchor, cable was taking up and the anchor hit him in the face and he received a cut above his left eye.

Roof fall occured in entry #4, section 2-A, 12,000 ft. from section. Area was breakered off. Timbers and jacks were set to establish walkway to seals. Fall was 3 ft. long, 12 ft. wide by 4 ft. high.

Roof fall occur in #4 entry on #3 belt 2000 feet from section. Area was breakered off and dangered off in all locationa and will not be rehabilated.

Roof fell 11,000 feet from active face at 2-A Belt crosscut right, #3 entry.

Employee was closing air-lock door on slope belt, slipped in mud and door closed on right hand.

Employee was taking lid off shuttle car, lid slipped out of helper's hand and all the wieght of panel went to employee, injuring his back. Employee quit 2/20/07.

Roof fall was found at 2:00 pm, 7000 ft. off section at 2-B Belt cross-cut between #3 & #4 entry outby spad #3270. Fall was 18 ft. wide, 45 ft. long, 2 to 4 ft. thick. This area was developed in April 2004.

Employee was walking beside roof bolter to move machine, rock fell from top and hit him on shoulder, neck and left arm. A rock approx 20"x 30"x 4" thick fell from between the rib bolt and the center bolt. First day missed due to accident was 1/24/06.

Employee was changing scoop battery at Mains battery station. He walked around front of scoop bucket and rib roll caught foot against scoop bucket. He returned to restricted duty work on 1/18/06.

EE. filed an ALLEGED claim for Occupational Pneumoniosis on 12/29/2005. Received notification on 5/4/2006 of Claim No. 2006022866. His last day worked was 12/15/2004 and retired on 7/1/2005.

Employee injured back while driving slope belt drive bull gear onto shaft with sledge hammer. Note: This accident was not ruled compensable until 3/7/06, received notification from Brick Street on 3/9/06. Originally beleived accident was not work related since first day missed was 1/26/06.

EE stepped in a hole while pulling roof bolter cable slack and ALLEDGEDLY injured his back. His first Doctor vist was on 4/18/06, missed work on 4/19/06 and returned to work on 4/25/06. We do not feel this a a legitimate lost time injury.

EE was prying a rock out of belt conveyor head piece, slate bar slipped and he fell straining his back.

Employee was standing on bolter cable while it was tramming. Bolter turned and jerked cable and flipped him, hitting head. He continued working that shift and all scheduled shifts until he reported to work on 9/8/05 and told supervisor his head hurt and was dizzy and went home before shift started. This injury is being CONTESTED since he worked 2 weeks before having a problem.

EE was moving and relocating roof bolter and canopy hit his left shoulder and neck. Worked remainder of shift and 2 additional days; last day worked 7/25/05.

Employee was drilling hole for roof bolt using 2 piece steel. Finished drilling with first piece and dropped head down and added second piece. Started drill pot up to connect steel and drill pot got hung or stuck leaning steel out towards operator, then came loose and steel smacked right thumb while holding first piece of steel.

Employee hit left knee on belt head motor plate while working on #4B belt head.

EE injured left leg and ankle when left rib rolled while operating Roof Bolter. He worked his next 4 scheduled days and missed work on 1/12/05, his first scheduled day not worked.

At 8:00am a roof fall was found at #1 East Main, # 5 entry one crosscut inby. Distance from the active face is approximately 4000 feet in the intake. Size of fall is approximately 50 feet long by 20 feet wide and 5 feet thick. The fall was breakered off. These works were developed in 1994.

Ee caught left hand on rope hanger with metal hook. Hanger was holding water line, water line slipped and hanger cut left thumb.

EE called the mine office on 03/16/04 alledging that he had twisted his right knee while shoveling material from under the belt conveyor on 03/13/04. He did not return to work on 03-19-04, his next scheduled work day. **EE was released to return to work on 5/20/04. However, he has not returned due to other medical problems.**

ELBOW & LOWER BACK STRAIN. MEDICAL RECEIVED 7-1-02 SHOWS NEG. EVIDENCE FOR KNEE/ELBOW INJURY. ALSO SHOWS NEG. BACK STRAIN. HOWEVER SHOWS SCLEROSIS, OSTEOPHYTES (SPURS) AND NARROWING OF THE LUMBAR SPINE.

EE STATED HE WAS STRUCK ON THE LOWER BACK WITH A PIECE OF COAL RIB WHILE STANDING BY THE CONT. MINER. HE CONTINUED TO WORK FOR TWO HOURS AFTER BUILDING A STOPPING. MEDICAL RECEIVED 6-21-02 ON INDICATES DEG SPINNING DEGENEATIVE MULTI LEVEL. HE HAS HAD OTHER BACK CLAIMS IN THE PAST FROM MEDICAL RECORDS.

EMPLOYEE STATED HE WAS OPERATING THE FLETCHER RRII ROOF BOLTER AND INSTALLING EYE BOLTS FOR THE NEXT BELT MOVE SET UP WHEN A TOP PIECE OF COAL RIB BROKE LOOSE UNDER PRESSURE AND STRUCK HIS LE FT SIDE, LEFT ELBOW, AND LEFT WRIST.
