An unintentional roof fall occurred above the anchorage point of permanent roof support in the active workings of the # 1 Section, at the intersection of the #2 entry. (Survey Station 6847)

Employee stated employee was traveling the in #6 entry just outby the section power center. Employee stated employee walked over a small ledge in the mine floor, and hyperextended right leg, causing employee to fall to the ground injuring right knee.

Employee was installing a fly board in the #3 Entry on 1 section. Employee stated they twisted their body away from the machine, and when they took a step, they felt pain in the left calf.

An unintentional fall of the mine roof was found at 223 BRK (survey station: 5326) on 08-25-2019 at 08:35 PM. A pre-shift examiner traveling the #4 (track) entry when EE found the fall. The exact time of the fall is unknown. The fall was reported 18' W x 34' L x 8' thick, from the intersection of the #4 entry, continuing over into the crosscut between the #4 and #3 entries.

Employee stated EE was pulling C/M cable in the #7 entry when EE felt a sharp burning pain in EE's lower back. Employee continued to work the remainder of the shift. On EE's way outside, the employee stated the pain got worse. EE went to the bathhouse after getting outside, and then returned to the mine office to report the incident before leaving mine property.

A fall was discovered by EE on the evening shift on 07/30/2019. The fall was located at brk 28 #6 entry 100' inby spad 1231 on Beltway mains. The fall was cribbed and dangered off called into the hot line. The fall was 0 to 2' thick 14' wide and 40' long.

Footing got stuck in mud carrying a drill reamer. Fell due to feet stuck in mud and injured lower back.

Employee stated was tramming a bolter when the right rear tire slipped off the track causing the operator to be thrown up into the operator's deck canopy injuring neck and back. This section is currently moving across the mine. Employee sought medical treatment and was taken off work due to neck and back strain.

Employee stated they were operating the #507 outby scoop while cleaning the belt line, EE hit the rib with the operators' deck and the rib broke free and rolled onto them, striking them in the back of the head and upper back. The rib measured 30" x 62". The scoop the employee was operating does not have a canopy.

Employee stated ee was traveling across the section when ee stepped on a slick spot where the shuttle cars were traveling, causing ee to slip and twist left knee. Employee sought medical treatment and was placed off work due to a strain.

Employee stated ee was observing the miner cut when a rib rolled out and struck the back of left leg, causing ee to turn and twist quickly to get away. In the process of avoiding the rib ee twisted right ankle. Employee sought medical treatment and was placed off work under doctors care.

Employee was dismounting a bolter when their feet became tangled and fell to the mine floor, injuring their back and right shoulder. Employee sought medical treatment on 4/10/2019 and was taken off work under doctors care.

On the evening shift of 3/30/2019 a roof fall was discovered in the #1 and #2 entries. The #2 entries serves as the return travel entry. Area was breakered off and made safe. The stopping line was set over.

Employee stated EE was crawling on top belt running a shiv rope when the belt slid down between rollers causing EE to fall to the mine floor. Employee sought medical treatment and was diagnosed with a strained back. Employee was given days away from work. Note: Belt was locked & Tag

A roof fall was discovered at 220 crosscut in the #1 entry 1 crosscut inby spad 5281. This was a continuation of a previous fall.

Employee stated EE was getting ready to lift a piece of sheet metal when the end of an Aqua water line busted and the water struck EE causing EE to fall backwards landing on a I Beam. Employee injured EE's back during the fall and was diagnosed with a fractured vertebrae. Employee was taken off work.

A fall was discovered on the day shift of January 18th, 2019. The fall area was located at left return, #2 entry, 254cc to #255cc. The fall was cribbed and dangered off.

Employee stated EE was in the process of inserting a stick of glue when a rock 3'L x 2'W x 2" thick fell out from between bolts behind EE and struck EE's right shoulder and middle back. Employee sought medical treatment and was placed off work.

Employee stated was riding in a mantrip when the mantrip hit a kink in the rail then wrecked causing the injured employee to hit head on the seat. Employee complained of head and neck pain. Employee was taken to the hospital and taken off work under doctors care.

Employee was loading a hydraulic cylinder into a service scoop bucket and felt a sharp pain in the RT Shoulder. The employee finished the shift but the pain worsened through the night and EE drove self to the hospital after working.

Employee stated while walking escapeway drill employee twisted ankle stepping on a rock. Employee sought medical treatment and was diagnosed with a fractured foot. Employee was not placed off work or restrictions. Note: Employee did not notify the operator of the incident until the day shift of 12/14/2018, 10 days after the alleged incident.

Employee stated was hanging curtain when EE slipped and fell into the rib cutting hand on a pie pan. Employee received stitches.

In reference to citation 9161924. A roof fall was first discovered 11/8/2018, the fall area was on the 2 Left off 3 north panel. The fall occurred 16 brk between 5 1/2 and 7 1/2 entries. The fall is approximately 100' Long 18'-20' Wide.

Employee stated was using a sledge hammer to knock a final drive out of 406 roof bolter. Upon striking the sprocket a sliver of metal broke off and embedded in the employee's left thigh. Employee sought medical treatment to remove fragment.

Employee stated was dismounting a school bus used to transport miners when the employee slipped and fell going down the steps. In the process of the fall employee hyper extended knee. Employee was taken off work under doctors orders.

Employee stated EE tripped on a wash down hose and stepped on a cable causing EE to twist the right ankle. Employee sought medical attention and was diagnosed with a sprain ankle. Employee was taken off work under doctors care.

Employee stated they got off the bolter to attach curtain together where it overlapped, when a rib roll 35"L X 20"W X 13" thick rolled out and struck employee's right ankle. This caused the employee to fall forward and injure their right knee. Employee was placed off work under doctors orders.

Employee stated EE abruptly exited a scoop deck and bumped left side against the scoop deck frame and injured left side rib cage. Employee sought medical treatment after shift and was diagnosed with a fractured rib. Employee was taken off work.

Employee stated they were jacking up rail when the jack kicked forward and the bar struck them over the right eyebrow resulting in 3 stitches.

An unintentional roof fall occurred at break 11 between the #4 and #5 entries leading into the #5 heading of the #1 section.

Employee stated ee was helping hang miner cable when ee felt a sharp pain in right shoulder. Employee sought medical treatment and was taken off work under doctors orders.

Employee stated was bolting in the #2 entry when a piece of rock fell and struck left forearm. The piece of rock fell from the side of a strapped kettle bottom. Employee sought medical treatment and was placed off work due to a fractured forearm.

Employee stated was stepping into a motor with the right foot in the deck of the motor and the left in mud. As EE stepped up, EE twisted left knee. EE continued to work until 6/28/2018 when EE was placed under doctors care.

Employee was drilling a test hole with stack steel, when removed the bottom piece of steel the top piece fell out of hole striking ee left ring finger resulting in a laceration that required 10 sutures. No lost work days.

Employee stated EE was shoveling at a belt head when EE felt a pain in EE's lower back resulting in a sprain/strain injury and lost work days.

Employee was walking on section when ee slipped and felt a sharp pain in left knee. Employee sought medical treatment and was given a full release. On 5/2/2018 Employee began missing work due to knee surgery at which time this became a reportable incident.

Employee stated EE was putting a 40lb bag of rock dust into a pull behind duster when EE felt a strain in EE's lower back. Employee sought medical attention on 2/12/2018 and was diagnosed with a strain and given a full release. On 2/19/2018 employee followed up with family doctor and was placed on restricted duty.

An unintentional roof fall was found on 1/1/2018 at approximately 12:40am. The fall was in the # 2 entry inby spad #1473 and outby spad #1463. The approximate dimensions of the fall are 20' in length X 20' in width X 8' in thickness.

Employee was on a man trip and stepped off the ride and tripped over a bag of rock dust bruising left ribs.

Employee stated stepped off the back of the roof bolt machine and twisted right ankle. This resulted in a sprain and lost work days.

Employee stated bent over to move the continuous miner cable when felt pain in lower back. This resulted in a strain and lost work days.

Employee stated EE was operating a shuttle car on the #2 section when EE hit an irregularity in the mine bottom, which caused EE to be jarred in the operators compartment. This resulted in back/neck strains and lost work days.

An unintentional roof fall was found on 8/26/2017 at approximately 04:15pm. The fall was in the #3 entry at spad #5327. The approximate dimensions of the fall are 18' in width x 30' in length x 7.5' in thickness.

Employee stated that EE was stepping out of a shuttle car when EE slipped on the ground hyper-extending right leg resulting in a sprain/strain and lost work days.

Employee was trying to turn bottom PLM plug on power center with a wrench when the wrench slipped causing employee's had to smash against the top plug resulting in a laceration that required sutures.

Employee stated was fire-bossing when EE encountered a rib roll that had knocked over 3 heisman jacks. Employee lifted and set the first two jacks and when EE tried to lift the third EE felt a burning pain in stomach resulting in a hernia and lost work days.

An unintentional roof fall was found on 8/9/2017 at approximately 6:50pm. The fall was in the #3 entry at spad #5290. The approximate dimensions of the fall are 18' in width x 25' in length x 6-7' in thickness.

Employee stated EE was installing roof support when the bolt would not go in the top. Employee stated EE placed the bolt onto the side of the pot to assist the bolt into the hole. The bolt jammed and slipped off the pot, pinching EE's right middle finger, resulting in a laceration that required stitches. No lost work days.

Employee stated that ee was installing roof support when the canopy jack on the roof bolt machine broke causing the canopy to fall and strike ee. This resulted in a strain to the lower back and lost work days.

An unintentional roof fall was found on 8/1/2017 at approximately 04:30am. The fall was in the conveyor entry at spad #5329. The approximate dimensions of the fall are 18' in width x 30' in length x 10' in thickness.

An unintentional roof fall was found on 7/31/2017 at approximately 12:28am. The fall was in the 2LT intersection in the last open line, spad 5584. The approximate dimensions of the fall are 20'X18'X7'

Employee stated was carrying a bag of rock dust to the face area when foot got stuck in the mud. As employee pulled foot out of the mud felt pain in left knee, hip, and ankle. This resulted in a strain and lost work days.

Employee stated was operating the scoop on the #2 Section in the #3 Entry when the scoop went over an irregularity in the bottom, causing EE to be jarred in the operator's compartment of the scoop. This resulted in a left shoulder and neck strain and lost work days.

Employee stated was walking through a return mandoor at 6HD and when EE stepped through the door, EE felt pain in the right knee. This resulted in a strain and lost work days.

Employee stated they and another employee were carrying a 20' joint of water line into the shop when the employee felt pain in the left shoulder. This resulted in a shoulder strain and lost work days.

An unintentional roof fall was found on 6/9/2017 at approximately 11:30am. The fall was in the high voltage entry at break 224 approximately 1 break outby spad 5357. The approximate dimensions of the fall are 20'X 20'X 7'

An unintentional roof fall was found on 6/3/2017 at approximately 5:02pm. The fall was in the belt entry on #10 belt between 226xc and 227xc approximately 15' inby spad # 5360. The fall was approximately 60' in length and approximately 4' to 8' thick. Timbers were set and both approaches to the fall were dangered off. No injuries occurred as a result to this accident.

Employee stated was operating the shuttle car on the #1 Section when the shuttle car went over an irregularity in the roadway, jarring employee. This resulted in a back strain and lost work days.

Employee stated employee was attempting to dismount the operators compartment of the scoop. As employee was stepping out of the scoop employee's foot slipped, and employee's right shin struck against the frame of the scoop. This resulted in a contusion and lost work days.

Employee stated they were hanging a reflector in the #1 entry on the #2 section. Employee stated they reached out to hang a reflector and felt a pop in the right side of the rib cage. This incident resulted in a lost time accident.

An unintentional roof fall was found on 3/8/2017 approx. 10:00am. The fall was in the right return between 166xc and 167xc, just inby survey station 4345. The fall was approx. 50' long and 6' thick. Timbers were set and both approaches to the fall were dangered off. No injuries occurred as a result to this accident.

Employee stated EE was rock dusting the #6 entry on the #2 section. Employee was walking backwards and slinging rock dust in front of self so the dust would float away from EE's body. EE then tripped on a rock approximately 13"L X 11"W X 10" thick, causing EE to twist left ankle. This resulted in a fracture and lost work days.

Employee stated was operating the RBM on the #1 Section, and as employee was walking through the center on the roof bolt machine, employee struck head against the mine roof. This resulted in a neck strain and restricted work days.

Employee stated was walking through a mandoor and struck right elbow against the door latch. This resulted in a contusion and restricted work days.

Employee stated EE was supplying the roof bolt machine. As EE walked toward the front of the bolt machine EE slipped and fell backwards. As EE fell EE extended EE's right arm to catch self. This caused EE to jam EE's right elbow.

Employee stated that employee was carrying a pump at #6 Belt Head. Employee tripped and fell; pump struck employee on the L ankle area resulting in a fracture and lost work days.

An unintentional roof fall was found on 1/3/17 at approximately 6:00pm. The fall was in the #1 Entry on the West Form Mains, approximately 100' inby survey station 5219. The fall was approximately 30'L x 20'W x 10'T. Timbers were set and the fall was dangered off from all approaches.

Employee stated EE was attempting to cut a piece of belt when the knife slipped and EE cut left forearm. This resulted in a laceration and sutures to close the wound.

Employee stated that EE was in the process of removing the cat pads from the 226CM when a grease plug failed. This cause part of the grease plug to strike EE in the face (left cheek). This resulted in lost work days and a laceration, which required sutures to close the wound.

Employee stated EE and another employee were using a slate bar to pry a rock off the skirt board of the tailpiece. When the rock came free, the slate bar slipped, and struck the employee on the inner part of right knee. This resulted in a contusion. The incident became reportable when the employee began missing work on 10/21/16.

Employee stated was operating the roof bolt machine, drilling a 5ft hole, when a piece a draw rock fell from the top striking on the right forearm. This resulted in a laceration, which required sutures to close the wound.

Employee stated EE was lifting two joints of water line to clamp them together, and as EE lifted, EE felt pain in left shoulder and neck. This resulted in a strain and lost work days.

Employee stated ee was stepping into the deck of shuttle car when ee felt a sharp pain in right inner thigh. This resulted in a strain and lost work days.

Employee stated EE was installing a bolt into the rib. EE drilled the hole and was inserting a 6' bolt into the hole when the bolt hit a crack and came to a sudden stop. The employee stated that EE's left shoulder was strained. The incident resulted in a strain and lost work days.

An unintentional roof fall occurred in the intake at break no. 28 in the no. 7 entry. There were no injuries as a result of this incident.

Employee stated they were hanging the roof bolt machine cable on the #1 Section when they felt pain in their lower back. The employee continued to work, but was placed on restricted duty on 9/25/16 for a back strain.

Employee was in the process of building a stopping. EE mounted a ladder while another employee held the ladder in place. EE was standing on the ladder when they suddenly fell backward onto the mine floor. The employee did not slip, nor did the ladder tip over. This resulted in a concussion, multiple strains, and lost work days.

Employee stated EE was sitting on the back of the roof bolt machine in the #6 Entry on the #1 Section when a piece of draw rock approximately 9" wide x 35" long x 1.5" thick fell from between bolts from the mine roof and struck EE on the right hand and right knee. This resulted in a contusion, and three lost work days.

Employee stated was removing a broken bit from the bit lug. EE was using a punch and hammer to remove the broken bit. EE missed the punch and struck left index and middle finger with the hammer. This resulted in a fracture and two lost work days.

Employee stated EE was using a chain hoist to pull slack in the belt. The hook was attached to the belt rail. When tension was applied, the hook came loose, and the body of the chain hoist struck EE in the mouth. This resulted in the lose of EE's front tooth.

Employee was working as a roof bolter operator on the 012-0 MMU. Stated was sitting on the RBM with leg in the middle isle, a piece of rock approximately 20" long x 14" wide x 8" thick fell from the top and struck right lower leg. This resulted in a laceration, which required sutures to repair.

Employee was shoveling at the #6 belt tail. Employee stood up and stepped back to reposition. Employee stepped on a 6" waterline, causing a twist to right knee. This resulted in a strain and lost work days.

Employee stated he was walking in the #5 face when his foot slipped on a ledge in the bottom, twisting his left knee. Employee was diagnosed with a left knee strain.

Employee stated he was walking to a return mandoor when his foot slipped in mud, twisting his right knee. Employee was diagnosed with a right knee strain.

Employee stated that EE was walking in the face area of the 2 North section and slipped in a hole, and felt legs go numb.

Employee stated that he was stepping through the belt regulator on the 1 West Section when he tripped over a stopping block and felt pain in his back.

Employee stated that as he was operating the S/C Machine he trammed over a hole in the roadway which caused him to jar in his seat and strike his left shoulder on the canopy post in the deck of the S/C.

Employee stated that as he was riding on the mantrip going to the section the mantrip wrecked which caused pain in his lower back.

Employee stated he was attempting to hang a set of fly pads and needed a ladder. After retrieving a ladder in #2 entry while walking back to the feeder area he stepped in a hole and felt pain in his lower back.

A roof fall occurred on a pillar section where material fell out around the MRS units in the face of #2 entry on the 1 West Section.

Employee stated that he was attempting to place a 10' timber on the emergency sled when he lost his footing and fell onto his back. This caused pain in his lower back and legs.

Employee stated that he was handling the CM cable when he stepped on a rock and twisted his left knee.

Employee was walking between a scoop and timbers when one of the timbers dislodged and struck him in the middle of the back between the shoulder blades.

Employee stated that he ran over a rock while operating a shuttle car causing him to feel pain in his back.

Employee stated that as he was walking in the intersection at the 1 West Section Feeder, he stepped in gob and twisted his Left Knee.

Employee alleged that as he was attempting to exit the deck of the scoop he fell out of the scoop and was suspended over the door of the scoop until assisted by coworkers who could not confirm employee's statement. Mine management is submitting this under protest to abate WV State Citation No.300-0126-2015-0002. Mine management reserves the right to withdraw this submittal.

Employee stated that as he was attempting to move a fly board when he felt a pain in his back. Employee continued to work his normal shift until 1/7/2015.

Strata Contract laborer working at Black Oak Mine stated that he and 2 other workers were attempting to remove a section of draw rock from the track when it dropped on his left foot.

While assisting 2 other employees unload a Shuttle Car tire, the tire rolled onto the employee's right foot causing pain in his foot.

Employee stated that while trying to remove a stuck roller from a beltline, the roller shifted and caught his finger between the belt rail and the roller causing a laceration to his left pinky finger.

Injured employee and co-worker was to load 6" waterline on scoop. It was reported that as they moved the pipe across the track toward the scoop he stumbled over the rail and fell to the ground twisting his back.

Employee stated that as he was operating the S/C, he trammed over material in the roadway which jarred his lower back causing pain in his lower back and leg.

Employee stated that while lifting on a cable in an attempt to hang it from the mine roof he felt a pop in his right shoulder. Employee continued to work in his normal job class until January 6, 2015 when he was taken off work by a physician for surgery on the right shoulder.

Employee alleged that as he was tramming the scoop he encountered an offset in the mine floor which caused him to bump his head against the canopy of the scoop. Mine management is submitting this under protest in order to abate a WV State Citation No.: 300-0126-2015-0001. Mine management reserves the right to contest or with draw this submittal completely at a later date.

Employee stated that he was walking around the 4-wheeler when he lost his footing and slipped down twisting his lower back.

Employee stated that she was assisting in building a stopping. She was handing blocks to a co-worker who was stacking the blocks in place. When she had her back to the half completed stopping, the stopping collapsed knocking her to the mine floor and injuring the back of her lower right leg.

Employee was stating that he was walking through a set of fly pads, tripped on a rock and fell to the mine floor striking his right knee and right elbow on the mine floor.

Employee stated that he was attempting to move a P40 pump by himself when he felt a pain in his lower back and into his left leg. Employee did not begin to miss work until 11/12/14.

On 8/6/2014 mine management was notified of the employee's diagnosis of occupational pneumoconiosis. Employe has been retired since 4/1/2012.

Mine management was notified on 8/1/2014 of employee diagnosis pertaining to occupational Pneumoconiosis. Employee retired 5/1/2013.

Operator received notice on 7/17/2014 that Carpal Tunnel Syndrome claim had been filed for by employee.

Employee stated that while setting up the CM in the face of the #3 Entry, his right leg gave out causing him to fall and strike his right shoulder on the mine floor which caused pain in his right shoulder, back, right hip, and right leg.

Employee stated that he was operating the continuous miner in the face of the #5 entry. He had taken the 1st sump in the cut sequence and had positioned the off standard continuous miner for the second lift when a section of draw rock fell from the top between the roof bolts and the rib, striking him in the back of the neck and left shoulder blade area.

On 6/3/2014 mine management was notified that the employee had suffered an occupational illness in that he had hearing loss. Employee is retired.

Employee was standing in the intersection helping move the roof bolter cable when the portion of the cable that was hung across the intersection pulled out of the hangers and struck the Employee on the left side of his neck.

Employee stated that he was attempting to lift a timber out of a scoop bucket when he felt a sharp pain in his lower back and left leg.

Employee stated that he stepped off of a rail flatcar backwards. As he stepped down to the ground he took one step backwards and twisted his left knee.

Employee stated that as he was backing the Continuous Miner out of a lift, he was handling the cable and waterline at the same time. When he stepped backward, he stumbled on a rock and fell striking his lower back. Employee continued to work until 8/15/2014 when he started missing work due to this injury.

On April 9, 2014 Black Oak Safety Dept. was notified that employee was diagnosed with Carpal Tunnel Syndrome and was taken off work on April 4,2014 and would be having surgery on April 9,2014.

On 3/13/2014 a roof fall occurred at 45 break on the 1 Right 1 West in the #3 Entry. This report is being filed due to citation 9112033. The roof fall was called in to the MSHA Hotline on 12/5/2016 to abate the citation. The current operator was not the operator of the mine when the fall occurred. Eastern Associated Coal Company, LLC was the operator at the time of the fall.

DURING A WEEKLY EXAM THE FIRE BOSS DISCOVERED A ROOF FALL IN THE 1 RIGHT OFF OF 2 EAST PANEL. THE FALL IS LOCATED IN # 4 ENTRY THREE BRKS. INBY SPAD # 2543.

Employee has been diagnosed with Occupational Pneumoconiosis.

During a weekly escape way examination the third shift fire boss discovered a roof fall in the 1Rt panel off of 2 East section just inby spad #2561. This is an area that has been mined out and no longer in producing status.

At approximately 2:00 p.m. there was a rib roll discovered at brk. 31(SPAD #1460) in the North Mains intake escapeway entry. The rib roll knocked three cribs which were previously built and blocked the escapeway. The men were withdrew from the mine until escapway was restored.

Employee stated that he was stepping from the bolter to the drill platform and slipped and fell appox. 24"-30" between the drill platform and the ATRS.

Employee received surgery on his wrist due to carpal tunell syndrome.

EE was trimming a piece of belt off the bottom of a door when he was finishing the cut. The knife came through the belt and into his left arm causing the injury. EE will return to work when the results of a drug screen are available.

Due to repetitive motion the employee has been diagnosed with carpel tunnel syndrome.

Eastern Associated Coal was notified 3/24/2014 that employee was diagnosed with hearing loss on 12/03/2013.

Employee stated while hooking up a pull duster behind a battery powered, rubber tired ride the duster rolled forward and caught the employee's hand between the duster and the ride.

While changing batteries on a scoop battery lock pin was stuck in battery lift arm. Employee was driving lock pin out with a hammer and screw pin when lock pin came out. Battery fork dropped and caught his finger between screw pin and battery.

Employee was installing a roof bolt in the rib on the 1 West section and as he was inserting the bolt into the rib he hit the top of his left hand on the drill guide. Employee felt pain and a knot raised up on his left hand. Employee worked fifteen days following the incident was off three days came back and worked seven days before being taken off work on Nov. 21,2013.

Roof fall occurred in front of the MRS jacks on the 2 East section covering them up and making them unable to retrieve. Management made the decision to leave the MRS jacks behind.

Employee stated while turning a left hand break in the #4 entry on the 1 West section a rib rolled out striking him on the legs knocking him down. Another employee had to help him get the rock off of his leg. When the rib hit him it caused him to twist his RT knee which caused pain between his RT knee and ankle.

Employee has been diagnosed with hearing loss.

EE was working as a section foreman and was in the process of hanging a set of fly pads between #5 and #6 entry located beside spad#3625. While hanging the fly pads EE fell from the ladder striking the back of his head resulting in a concussion.

The employee traveled up to the section and placed his hand on the back of the scoop batteries to talk to the miners loading bolt supplies on the roof bolt machine. One of the other employees tossed the cable cross-overs on the back of the scoop and struck his finger causing a laceration requiring 5 stitches.

On 8-21-13, the mine site was notified that the OP Board found sufficient evidence to justify a diagnosis of OP with no more than the 15% pulmonary function impairment attributable to this disease.

Employee was operating the roof bolting machine and was in the process of moving his drill steel from the bolt tray to the drill pot, when a small piece of rock fell and struck his forearm. He was diagnosed with a contusion and soft tissue damage to his arm and placed off of work until July 7 or further examination.

Notification of hearing loss claim

Employee suffered from carpal tunnel syndrome in his wrist and forearm due to repetitive motion.

Employee stated that he was helping install a bolt canopy on the machine as the canopy was raised it slid over pinching the side of his hand between the canopy and the bolt tray.

At approximately 10:00 am a roof fall was discovered near survey spad 2962 blocking the approaches to DPP 52.

The employee stated that he and two other employees were pulling a 2" waterline with the scoop. They were utilizing a 1/4" cable to pull the waterline and the cable broke and recoiled back striking the employee. When the cable struck the employee, he fell into the belt structure striking his chin and received 7 stitches from the injury.

Employee stated that he was leaning timbers against the rib to prepare to install them on the mine roof. One of the timbers slid off of the rib and struck him in the side and ribs. The employee was in an automotive accident prior to the incident and had broken his ribs. This incident re-injured his ribs. Employee began missing work on 10-25-2012

Employee was pulling discharge water-line with a scoop and parked beside the line to re-hook it. He stepped on the discharge line to reach the chain and slipped off of the line falling to the ground. He has swollen muscles in his back and a possible pinched nerve. He did not begin missing work until 10-22-2012.

Employee has chronic back pain and stated that his back goes out occasionally and seeks treatment. Employee was diagnosed with degenerative disc disease on October 16, 2012 and began missing work on 10-17-2012

The mine site received notification on September 18, 2012 that the employee received 5 sutures. The employee stated that he was loading timbers in the scoop bucket when he tripped over a block of wood landing on his right knee.

On 4/24/2012 operator was notified employee was diagnosed with carpal tunnel.

On 3/9/2012 at approximately 6:05 pm a roof fall was discovered in an in-active area of the old 2 East Mains. This area is not required to be traveled. The fall was breakered off on all approaches. The fall did not effect travel of ventilation.Continuation 4/27/12 fall extends to Entry#7 and now meas. approx. 20' W. X 130' L. x 6' thick. Has been breakered off on all approaches.

Employee stated he cut his hand while installing a kennedy stopping, 4 sutures were required to close the wound.

On 2/28/2012 at approximately 1:30 pm a roof fall was discovered on the old works of 1 East, this area is not required to be traveled, the fall was breakered off on all approaches. The fall did not effect travel, ventilation, and no equipment was involved.

Employee was installing a ram jack into a scoop, when his finger became caught between the jack and the edge of the scoop bucket, causing laceration and a fracture to the right pinky finger.

Employee has been diagnosed with hearing loss.

Employee stated while hanging curtain, he was struck by a falling rock, striking his left knee causing a contusion. Employee began missing work on 1/25/2012.

Employee stated that as he lifted a bottom roller out of a hanger bracket he felt a sharp pain in his right shoulder. Employee continued to work, without a change in job classification, until 5/5/2014 when he was taken off work for an MRI and scheduled for physical therapy.

No equipment running at the time, conditions good/setting spads before mining coal started.

Occupational Illness: Employee was diagnosed with Occupational Pneumoconiosis (black lung). Workers compensation notified the mine on 9/28/2011

Employee was walking when he tripped causing him to fall, the employee fractured his left wrist in attempt to catch his fall.

Employee was struck by rock that fell from the mine roof while placing the wash down hose on the continuous miner. The rock measured 3' wide by 4' long and up to 3" thick. Employee was seen on 8/23/2011 by a neurologist at which time he was diagnosed with a back strain and given 5 days away from work.

Employee was standing with his left hand resting on a rib bolt, a rock fell striking his left pinky finger causing a fracture and a laceration which required sutures.

Employee was walking and carrying roller hangers when he tripped and fell, causing his left upper torso to be struck against the hangers he was carrying.

Employee was struck on the left side of his face by a rock that fell from the mine roof causing a laceration above and below the left eye. Sutures were required to close the wound.

A roof fall was discovered at 10:15 a.m., the location was # 6 entry 16 break 2 east, spad 1877, 1 break inby

Employee was loading headers on 427 roof bolter and received a laceration to his left ring finger when his hand was caught between header board and frame of machine. 5 sutures were required.

Employee was struck on top of the right foot by a piece of rock that fall from the mine rib, fractures his first right metatarsal.

Employee was tripped by the roof bolter cable causing him to fall and land on his left arm fracturing his left elbow.

Employee stated that he was letting the bolt canopy down when he caught his finger between the second stage of the canopy jack and the bolter frame.

Employee was hanging the roof bolt machine cable while standing on a bucket. When he stepped down he twisted his right ankle.

Employee was walking at the #2 tailpiece when he slipped and sprained his left ankle.

A roof fall was discovered in the old 1 East panel in break #7 near spad 1308. Breaker posts were set to prevent travel to the fall. Travel and ventilation was not effected by the fall.

The roof bolt crew moved from #7 to #6 when the bolts in the tray snagged the line curtain and began pulling supplies off. The off side operator began freeing the curtain while the operator trammed forward to give slack in the curtain. The bolt machine tracks rolled over the tip of the employees right foot partially amputating the tip of his big toe.

Employee was hanging a reflector to mark an unbolted intersection. He was standing between the third and fourth row of bolts back from the intersection and a rock fell from between the bolts striking him in the head and back. The rock measured 35 inches wide by 80 inches long and was approximately 2 inches thick. He was diagnosed with two compression fractures in the lower back.

Employee was helping move the miner from the #5 right face to the #8 heading. Once the miner reached the face, the employee discovered that he needed more cable slack and was sheaving it around the corner using a roller hanger. The hanger came loose and the slack struck the employee throwing him to the ground. He was diagnosed with a mild concusion.

EE stated he was drilling a hole in the mine roof with a Hilti drill and after completing the hole, he handed the drill to another EE and began to descend the 2' step ladder that he was standing on. When he stepped down, he stumbled. He stated that the other EE grabbed his leg in an attempt to keep him from falling, but tripped him, knocking him to the ground and striking his hand.

While dewatering/rehabilitating the Black Oak Mine, the North Mains and East Mains Intersection was pumped down exposing a roof fall located near spad #1475. The fall is 1 break outby the butt-off of the North Mains and is approximatley 5' high in the intersection.

The employee stated he was tramming scoop and hit a hole in the roadway which jarred his back. He took the scoop inby one crosscut and got hung up. Crib blocks were placed under the wheels and when he trammed off the blocks it jarred his back.

The employee stated he was bolting top and was taking the steel out of a drill hole. When he took the wrench off the steel, the steel fell. He reached for it and felt a pain in his shoulder. He went to the doctor and x-rays were negative. He has been diagnosed with calcification tendonitis of the right shoulder.

The employee stated he was bolting top and had let the head down and was reaching for the wrench when a small piece of rock fell from the roof and struck his little finger.

The employee stated he was walking under the belt when tripped on a water line and fell on his knee. The employee continued to work until 1/24/09. He went to the doctor and was diagnosed with a nondisplaced fracture to the knee cap.

The employee stated he was bolting and was pushing the bolt wrench onto the bolt and had his hand on the bolt when he accidently hit the rotation on the machine. His glove was caught between the wrench and the roof bolt and this caused the glove to the pulled off his hand and hyperextended his fingers.

The EE has been diagnosed with occupational hearing loss.

The employee was assigned to build a stopping and after finishing the stopping she was assigned to build another stopping. She stated her back was hurting from building the first stopping. She was been diagnosed with degenerative changes in her back and neck.

The employee has been diagnosed with occupational hearing loss.

The employee stated he was operating a shuttle car and had parked in a cross cut to switch out with the other shuttle car. He started to pull into the entry when he felt pain in his left arm. He was not aware of what happened but did sustain a scratch and bruise on his forearm. He has been diagnosed with arthritis of the elbow.

The employee stated he was operating a bolter and taking the steel out of the drilled hole when he let the drill pot down his right foot was under the frame of the drill pot.

Employee filed an ALLEGED carpal tunnel syndrome claim on 6/18/08. Received notice on 6/23/08 of claim No. 2008019287.

Employee was pulling on conveyor belt when he injured his back.

Employee "ALLEDGED" to have fallen over the miner cable that was next to the roof bolter and injured his right shoulder, arm and wrist. This alledged incident is still under investigation.

Employee's glove got caught on the roof bolter drill steel, twisting his right hand. His last day worked was 4/15/08 and started missing work on 4/16/08 for hand surgery.

Employee was shearing rib for belt line with miner, rock came out of conveyor chain hitting him in bottom of chin.
