The highwall miner has just been recommissioned to Little Pete's Branch and is being assembled. EE was tightening the bolts on the super beam at the front of the HWM with a 3/4" drive impact gun with a universal swivel extension. The extension failed and flew apart, pieces of it struck EE on the left side of EE's face causing injury to the left eye.

Mostrando los 16The T-Bar on the right rear track lifting cylinder broke causing the highwall miner to drop very fast and causing the operator to be jolted by the impact. The operator has a bruise on hip and a finger laceration. Operator was treated and released at a hospital.
Apprentice miner & certified welder, EE was helping repair the cutter head. EE lit the cutting torch, as EE laid the striker down EE's left hand passed in front of the lit torch causing a burn on the back of hand. EE was treated by onsite EMTs and finished the shift. On 7/18/19 the blister had broken and it was decided EE should be checked.
Employee was pulling pins during retraction. When the unlatching jacks raised the latched stuck causing the push beam to raise appx. 4 inches. When the latches came loose the beam slammed down.
EE was on the ground as a spotter while the highwall miner was being trammed forward to the next entry when a sliver of metal sheered off the track from between the track and roller and struck ee in the right forearm
Employee was drilling an anchor hole with hand-held auger. The auger was bent and caused the employee's hand to be caught between drill and anchor pin, injuring his left middle finger.
Employee's shirt got caught on a bolt on the kelly bar of the auger- thus injuring his back.
Employee was injured while attempting to lift an auger flight with a hoist. The flight hung up and when the employee freed it, one end of the flight fell onto the employees left hand.
WHILE MOVING IN EQUIPMENT, EE'S RIGHT LEG GOT PINNED AGAINST CONVEYOR.
EE WAS LIFTING AUGER FLIGHT WHEN THE KELLY BAR PINCHED L THUMB AT 1ST JOINT.
THE CREW WAS PULLING THE CONVEYOR WITH A WINCH WHEN THE CONVEYOR CAUGHT ON A ROCK AND KICKED THUS CATCHING EE'S LEG BETWEEN THE SKID AND CONVEYOR.
A SMALL PIECE OF ROCK FELL FROM HIGHWALL & STRUCK EE'S KNEE WHILE HE WAS PULLING STEEL.
ROCK CAME OFF THE HIGHWALL AND STRUCK EMPLOYEE.
AUGER FELL OFF RACK AND STRUCK HIM IN HIS FOOT. THIS ACCIDENT WAS NOT REPORTED TO MINE MANAGEMENT UNTIL 4-15-95.
STEEL SLIVERS BECAME LODGED IN EE KNOCKERS
PUTTING PIN IN KELLY BAR GOT HAND PINCHED.
