During an altercation with another EE, Inj EE was struck in nose. Both sent home until incident investigated. Inj EE did not seek med attention for 11/2 wks. Safety Dept was notified EE had missed 1 day to see doctor & 1 day for x-rays. All negative.

While walking from 1 bldg., to another, EE fell while turning the corner & bruised his left knee & wrist, causing some swelling.

Employee was operating a roof bolter. He was drilling a hole when a small piece of draw rock fell, striking his left hand and fingers.

While trimming conveyor belt at the tailpiece employee's razor knife slipped, cutting his lower thigh.

Employee stated that he slipped and fell, striking his back, as he was moving the tie off for his shuttle car cable. This alleged accident occurred on 2/8/07 but was not reported until 2/12/07.

Employee stated he was crossing the tailpiece and fell, hitting the starting box.

Employee stated that due to repetitive motion, surgery was required to repair the rotator cuff. Employee continued to work from date of injury, 2-6-07, until 8-2-07, his surgery date.

Scoop was stuck 2 breaks inside. The conditions were wet and had frozen into ice.

EE was conducting preshift exam, when he slipped and fell. The bottom was wet and slick. EE seeked medical attention and advised to wait and watch. Became a reportable accident when EE had surgery on 6/8/07.

Ee dislodged a piece of draw rock as he was installing a ventilation curtain. The rock measured 1'x5"x1". As he drove a nail into the curtain, the rock came out on an angle and struck him in the head, requiring 4 stitches.

EE was walking on cross over, foot slipped, he fell and hit his ankle on the concrete block.

While removing a spill board during a belt back, EE caught his finger between the spill board and the tailpiece.

Ee was repositioning the roof bolter when he caught his hand between the rib and the canopy of the roof bolter.

Employee was assisting in making repairs to the continuous miner when the rub rail struck him on foot. He consulted a doctor and was told he has a fractured foot.

EE stated that he was changing conveyor belt structure out when he felt pain in his lower back. Ee went to the E.R.Dr where it was determined that he had suffered a lower back strain.

EE was taking the rubrail off the continuous miner when it fell on his right middle finger. He consulted the ER Dr at the hospital who determined that his finger was broken. He did not lose any time with this injury.

EE stated that he was walking and pulling miner cable when he struck him right ear on a strap. He went to the harh hospital where he received 2 stitches to the head and ear.

EE stated that he was roof bolting when a piece of rock fell from the roof and struck his side and ankle on the left side. EE was x-rayed and x-rays are negative.

EE stated that he was pulling a service hose from the reel of the service truck when he felt pain in his lower back. EE went home and consulted a Dr. who odered x-rays which were nega- tive. The Dr ordered an MRI and EE is currently losing time.

Employee was working on a hose clamp when it broke and cut his hand. He was transported to the hospital and received three stitches to his hand. He returned to work and has not lost any time.

EE was swinging the head of the roof bolter machine to the next position. He had his right hand on the top of the roof bolter when a piece of rock fell striking his little finger causing crushing injuries. He was treated at Harlan hospital and told to come back 6/21/06 for amputation.

EE stated that he was removing a torque shaft, when oil blew out of clutches and burnt his hands. EE was transported to the hospital, where he was treated for burns. EE returned to work on 6/12/06, his next regular sheduled shift.

EE stated that he was walking at the back of the bolt machine when he stumbled and fell striking his left hand on a piece of coal. This caused a laceration requiring 4 stitches between the index finger and middle finger.

Employee was using a cut off saw when the blade broke & struck him on the left forearm. He was transported to the hospital where he received 5 stitches. He did not lose any time and returned to regular duty.

EE stated he was roof bolting when a piece of rock, approx. 4" thick by 12" in diameter, fell from the roof and struck his right ring finger. The X-ray showed that his finger was fractured at the tip.

EE was attempting to straighten a piece of roof bolter drill steel that was bent, by lowering the head of the bolt machine on it. The drill steel sprang out and struck EE in the face. EE was X-rayed and had received a fractured cheek bone and jaw.

EE was riding to the section in the mantrip with the rest of the crew, when the operator went through a dip in the track and applied the brakes. The rail runner skidded into the stop blcok at the end of the track. EE was sitting in the end of the rail runner with his side against the frame. The impact resulted in 2 broken ribs. No one else was injured.

Ee stated he was using a shovel to muck the belt when he shoved his shovel up his thumb struck the mounting bar of the scraper. Ee went to the Dr., xray and tests were negative however, ee was put on light duty. Ee has not missed any time from work but is still on light duty.

EE stated that he was carrying a box of glue while loading the roof bolter when he tripped and fell striking the scoop bucket with his lower back.

EE stated he was putting a rope on the cable of the miner as the tail was being swung over. EE was struck by the tail on the back and left arm.

Employee was roof bolting when a piece of rock fell from the roof and struck him on the left hand.

EE stated that he was rolling a piece of rock off the water line when he felt pain in his right shoulder and neck.

EE WAS LOADING SUPPLIES ONTO THE SCOOP FROM THE FLATCAR WHEN HE GOT HIS LITTLE AND RING FINGERS CAUGHT BEHIND THE RAM OF THE SCOOP. THE RAM CRUSHED HIS FINGERTIPS, RESULTING IN THE AMPUTATION OF HIS RIGHT LITTLE FINGER TO THE MIDDLE PHALANGE AND HIS RIGHT RING FINGER TO THE DISTAL PHALANGE.

EMPLOYEE STATED THAT WHILE INSTALLING ROOF BOLTS A PIECE OF DRAW ROCK FELL FROM THE ROOF & STRUCK HIM ON THE LOWER BACK. EE WAS TREATED & RECEIVED 10 STITCHES TO HIS LOWER BACK.

An employee was moving an electric motor across the floor with a pallet jack when he felt pain in his upper back and left shoulder. He took off work on Jan. 8 & 9. X-ray tests were negative at the hospital.

Employee was operating the shuttle car when a piece of rock rolled out from the rib and struck him in the back. He continued to work the rest of the shift and consulted a doctor on 1/5/06.

EE was roof bolting when a piece of rock approx. 6' long x 2 1/2' wide x 4" thick fell from the roof between roof bolts and struck him on the right leg at the calf. He was transported tot he ER medical center

EE stated that he was roof bolting when a piece of draw rock fell and struck him on the right hand. He was xrayed and showed no fracture.

EE stated that he was putting oil in the shuttle car when the hadle broke on the can of hydraulic oil. The can struck his left hand and cut it requiring 5 stitches.

EE STATED THAT HE HAD CLEANED AROUND THE HEAD MOTOR OF THE CONTINUOUS MINER AND WAS STANDING NEAR THE RIB WHEN A PIECE OF ROCK ROLLED OUT OF THE RIB AND STRUCK HIM ON THE RIGHT FOOT. THE ROCK FRACTURED HIS LITTLE TOE AND REQUIRED 5 STITCHES.

EE WAS OPERATING THE ROOF BOLTER WHEN HE HAD PLACED HIS LEFT HAND UPON THE CANOPY OF THE ATRS. HIS FINGER WAS PINCHED BETWEEN THE MINE ROOF AND THE ATRS. HE WAS TRANSPORTED TO THE HOSPITAL WHERE HE RECEIVED 3 STITCHES. THE HAND WAS X-RAYED AND SHOWED NO FRACTURES.

EE WAS WALKING UP #4 ENTRY WHEN HE WALKED INTO A ROOF BOLT THAT WAS HANGING DOWN FROM THE ROOF. HE WAS TAKEN TO THE HOSPITAL WHERE HE RECEIVED STITCHES TO THE RIGHT EAR.

EE was shoveling coal onto the tailpiece when he felt pain in his lower back. EE continued to work the rest of the shift and consulted a dr after he got off work. He was x rayed and determined to have a muscle strain.

EE STATED THAT HE WAS WALKING ACROSS THE SECTION WHEN HE TRIPPED ON THE MINER CABLE. HE FELL ON HIS SHOULDER AND WAS TREATED AT THE HARBOR HOSPITAL. X-RAYS SHOWED NO FRACTURES, ONLY BRUISED. HE WAS OFF WORK ONE SHIFT AND RETURNED TO WORK FULL DUTY ON 9-19-05.

EE STATED THAT THE FEEDER SHEARED A PIN FOR THE CONVEYOR. EE STATED THAT HE ATTEMPTED TO CHANGE THE PIN WHILE THE FEEDER WAS RUNNING AND HIS RIGHT INDEX FINGER WAS AMPUTATED AT MIDPOINT OF THE NAIL. HE WAS TAKEN TO HARLAN HOSPITAL FOR TREATMENT. THE DR RELEASED HIM FOR LIGHT DUTY.

Employee stated that he was in a previously supported area spotting a roof bolt when a piece of drawrock approx 1' x 3' x 4" thick fell and struck him in the back. He was transported to the hospital where he was x-rayed and diagnosed with a bruised back.

Employee stated that he was roof bolting & caught his left index finger between the drill steel and the roof bolting wrench. He was transported to the hospital where it was determined that his finger was broken & excessive tissue required surgery to repair the damage.

EE stated that he was holding a shuttle car cable up so that a scoop could tram through under the cable. The cable caught on the scoop canopy and employee stepped forward & the scoop ran over his foot. EE was transported to the hospital and diagnosed with a fractured big toe.

EE STATED THAT HE WAS WORKING AT THE SECTION TAILPIECE WHEN HE WALKED INTO A ROOF BOLT THAT WAS HANGING DOWN. HE WAS TREATED FOR A LACERATION TO THE UPPER ARM. HE RECIEVED 24 STITCHES

Employee was walking across a belt crossing on the surface of the mine when he slipped and fell striking his right arm. He was transported to the hospital where it was determined that he had no fractures and was to return to work the next regular shift. Employee did not report for duty and now is claiming to have a back injury from the accident.
