Employee was working near the doorway at the alkali spray tower dump box when wind blew dust under EE's safety glasses. This caused EE's left eye to become irritated. This did not become reportable until the employee went to the doctor on 1/9/2026, when the doctor prescribed eyedrops.

While walking across the parking lot, wind blew material into the miners eye.

While working on 3FM starter, employee had to get down on their knees and lay down to work on the contacts on the side of the starter approximately 10" above grade and 10"-12" inside the cabinet behind a bracket. During normal activity, when returning to a standing position, they lurched and had momentary difficulty standing, they then were fine

EE straddled the finish mill 1 pinion shaft bearing drive line guard to access the oil level plug located on the opposite end of the platform. They had their left leg on the platform and as they swung their right leg behind them they felt a pop in their right knee as they planted their foot on the platform. There is ladder access on the opposite end to access the oil plugs.

Employee was assembling the drive chain on a feed elevator hunched over. Once EE got the chain in position and connected, EE was tried to get back to a standing position, but was unable to straighten EE's back. EE felt it lock into position. When EE forced self up, EE felt pain in EE's lower back.

Employee was kneeling to replace a bearing cap. When employee stood up, employee felt a "pop" in knee. Medical examination found a hairpin meniscus tear.

Employee sat on a chair in the wash plant control room and started itching

While working at the water pump area, employee received an insect bite.

Employee was cleaning a plugged material line. Wind blew some of the material around glasses and into left eye. A prescription was written making this a reportable incident.

Laborer felt irritation in L eye while walking in the plant. Didn't know the source of the irritation & didn't try to treat eye until arrived home after shift. When they woke up next morning eye was still irritated & swollen so they decided to report condition when they arrived at work. 07/23/20, we sent them to doctor which prescribed antibiotics for eye.

Employee climbed down a ladder to access the crane in the Mill building. Employee felt a twinge in their back as they stepped from the ladder to the floor of the crane cab. Placed on lifting restrictions.

While working on kiln pier 3, wind blew dust up under the employee's safety glasses and got into left eye.

While operating a dozer employee was sting multiple times by bees

Employee had multiple stings on EE's shoulder from unknown insect.

Material was blown into worker's face.

Employee was attempting to trap a raccoon and it bit employee on the hand.

Employee was working in #4 Blend Silo repairing air slides. EE was standing in an air line slot, with both feet wedged in, repetitively turning at the waist while repairing air slide. EE said when turned at the waist, to left, knee shifted in the direction EE turned causing right knee pain.

On January 15, while pulling conveyor belt, employee stepped into crevice on ground and stumbled and reported pains. The pains went away and reoccurred on March 2. EE reported pain in knee after standing up from kneeling position. EE was taken to the doctor evaluated and was referred to specialist. EE was not place on restrictions until knee surgery on April 12.

At the end of the shift, employee was gathering welding equipment for storage & felt a wasp sting on their back. Employee experienced difficulty breathing and eyes began to swell. Was transported to hospital & was administered an injection and an IV by hospital staff.

EE was standing in line near Taco Stand located near parking lot. EE felt something crawling when suddenly he was stung by a bee. Fifteen to twenty min later the ee experiences radiating pain up his left elbow. EE was transported by company vehicle to Sweatwater hosp. EE received an injection by doctor and was told to rest for day. EE retuned back to full duty next day.

Was working outside, got hot, came in for lunch, drank a cold drink, causing gastric reaction and chest pain. Went to hosp, released on 10/1/15, employee instructed to contact cardiologist for a full release before returning to work, called MSHA when we believed to be heart attack, diagnosed with heartburn. Denver office advised us to make notes.

Employee was delivering water when she felt something bite her right arm. Turned out to be a spider which required two prescriptions.

Cleaning air slide at cement load out. Cement blew under safety glasses into employee's eyes.

Employee was using a 5' long air lance through a 2x2' door to remove the build up from the bag filters. A gust of wind blew a foreign object into his right eye.

Employee working on #3 kiln clinker breaker hammers, wind gusting in the door & blew foreign particle into right eye.

The employee was working on the 6th floor of preheater tower building scaffolds. The employee stated that he was experiencing pain in his legs and thought nothing of it. Employee stated, when he got home his wife gave him some Tylenol. He stated that when he woke up his whole right leg was numb.

Employee noticed injury to his eye when he woke up the morning of Thursday 11/21/13. He was unaware of any incident that lead to this injury. When notified safety officer and foreman noticed a spot on his right eye. He was driven to the Mayes County Medical Center for further examination. He was then sent to Claremore-Pryor Eye Clinic and given antibiotic eye drops.

Employee was working from his knees. As the employee stood up, he felt a sharp pain in his right knee.

Employee operator tractor was cutting grass at the North West corner of river silos when he drove over a in ground nest of yellow jackets. The worker stopped to repair his bush hog and was swarmed by yellow jackets. The employee was stung in the lip and right arm.

While working on a load out structure,EE got into an altercation/fight w/his brother. Both employees were terminated & escorted off the mine property. Prior to be escorted offsite, ee was provider 1st aid treatment for a minor head laceration. It's our understanding ee went to the local ER and received stitches after being removed from the mine property.

Delivery driver was closing doors on truck when a wind gust knocked him down. Driver was knocked out for 2 to 5 minutes.

Employee was walking in the cooler room and dust got in left eye.

Employee alleged that on 08/02/2011 he was squatting down in a small area and put all his weight on his right foot breaking his right big toe. Employee did not report the accident until 08/08/2011.

Employee was standing looking at a fuel tank that was sitting on the ground. He turned to walk away when he then felt a sharp pain in his knee. No work activity was involved.

Employee says dust blew off from the brim of his hardhat and got in behind his face shield and safety glasses. Causing scratches to his eye and eyelid.

Cleaning on the feed shelf on the south east corner top door. When finished, he started to climb down off the work platform when something blew in his left eye. Not sure if it came from his face shield or just blew up from the floor. At the time, he had his safety glasses and face shield on.

Employee was complaining of ringing and hearing loss.

EE was bending over to pick up a 6" C-Channel & felt a sharp pain in his back.

EE laid down in his vehicle during a break and didn't wake up. He suffered a non work related heart attack.

Employee stated that he had a tingling sensation in the fingers of his right hand which radiated up to his wrist. Employee performs truck bulk loading operations, which requires writing, typing and manipulation of switches.

Wind was gusty and blew dust in his eyes.

FOR RECORD ONLY. Employee of Uvalde Concrete Company picked up a load of cement in transport truck. While on our property he said that he was having trouble breathing and did not feel good. EMS was called and the driver was taken to a hospital and admitted for heart surgery. There was no accident and the above was NOT our employee.

Employee was working outside at the plant with very extreme windy weather conditions. Some foreign particles of cement dust and steel fibers were blowing around. A very small piece of steel material was lodged in his left eye causing irritation and ongoing discomfort since Friday.

Employee stated his knee popped when he stood up after he had been cleaning the infeed journal.

While walking in the plant, he realized foreign material somehow entered his right eye.

Employee was inside FB furnance #2 cleaning out the fused sand. As he was carrying out chunks of fused sand, some sand blew up under his safety glasses into his eyes.

Bent down to attach west drag cable to motor; stood up and experienced back pain. No specific cause.

Employee claims he reached down to pick up small piece of angle iron off the floor and felt pain in his lower back.

Descending steps from P & H and Cleveland crane respectively when he felt pain, pop of left knee.
