EE was washing out the raw mill posimetric chute with a water lance. The water hose became entangled with the door, causing the door to begin closing. The door struck the employee's right right wrist resulting in a contusion. The injury became reportable on 11/18/25 when the doctor performed professional medical treatment during a follow up visit.

EE was helping a coworker install a snub roller scraper on A20 belt. EE lifted the scraper with both hands and felt a pain in right bicep. The scraper is approx. 60" long and weighs approx. 50 lbs. This became reportable when the doctor placed EE on restrictions on 12/24/24.

EE was diagnosing problem with a blower. They locked/tagged/tried out the blower and was wearing gloves. They tried to maneuver the pulley manually. As they did the tip of their left thumb was pinched between the belt and pulley. They received a small fracture at the very tip of their thumb and was given a splint for the thumb.

Two EEs were attempting to install new troughing roller on the A19 belt. EEs lifted the roller with comealongs to get it into place. While lifting the roller got caught on the hand rail. EE's pulled back on the roller to get it over the hand rail. Roller swung forward pinching the EE's left hand ring finger between roller and a roller frame under the belt causing a laceration.

EE was installing a takeup pulley on clinker feeder belt. It was placed on upper part of mobile work platform. When they & partner were attempting to slide it on platform to get it in place it started to slide off platform. EE reacted by trying stop the pulley and got a finger between pulley and grating of the platform causing a small fracture at the very tip right ring finger.

Employee was using a come along to help close and secure doors on the spray tower screw. The hook slipped and contacted employee's prescription safety glasses. The hard side shield on safety glasses caused a 1.3cm laceration in right eyebrow.

Employee was working on a flat platform for easy access to door closure. They felt the door was loose and did not relieve pressure from the unit. The pivot arm swung once they released the connecting pin. The arm lacerated the employee's ear.

While performing maintenance on the #5 finish mill discharge elevator, the employee used a safety utility knife to cut a rubber door seal. While cutting, the blade went through the rubber seal and through the leather glove EE was wearing. This caused a laceration to EE's left index finger.

Employee was removing a fuel filter from a 988K loader. While trying to loosen the filter EE overexerted self pulling on the filter wrench. This caused a strain in the lower abdominal area.

The employee was attempting to remove a metal valve seal from a valve at the bottom of a rail car. As employee pulled on the metal seal, in attempt to break the metal seal, employee cut the inside of left middle finger that required professional medical treatment.

Employee was in the drum process room opening and processing drums and received a cut on their right forearm from the drum lid.

Employee was kneeling to replace a bearing cap. When employee stood up, employee felt a "pop" in knee. Medical examination found a hairpin meniscus tear.

Employee states that EE needed to get down on EE's hands and knees to hand a co-worker a grease fitting below. Employee states that when EE got down on EE's hands and knees EE felt a pull in EE's abdomen. The doctor diagnosed EE with a work related hernia on 6/23/22, related to this incident.

The employee states that EE was working on a double dump that was sitting on a pallet in front of the maintenance shop. EE flipped the double dump so EE could get to the bolts on the other side. When EE did EE felt a pain the EE's left forearm area. Employee was diagnosed with an elbow strain. This became reportable on 11/22/22 when professional medical treatment was needed.

Employee states: while walking along a concrete walkway leading to W3801 tunnel EE slipped on a patch of ice. EE didn't fall to the ground. EE caught self with EE's left hand holding onto the handrail hurting EE's left elbow. Doctor diagnosed EE with an elbow strain. This became reportable on 6/16/22 when professional medical treatment was needed.

Employee stated that they were greasing the bearing on the lower valve of KC0742 double dump while standing on the walkway of KC0924-1 wheelabrator screw. Employee said they pinched their arm between the counterweight and the framework of the walkway above.

An employee was using a sledge hammer to drive off the cap from the rotor slot. A small piece of metal from the rotor cap chipped off and struck the forearm of an employee standing aside causing a laceration.

The employee was screwing a cap on a glass vial when the glass vial broke in employee's hand causing a laceration to the employee's finger on left hand.

Employee was working in truck bay when they felt something get in eye and start irritating it. Employee rubbed the eye but it was still irritated. Employee then flushed it with eye wash solution but this didn't help much. The employee then went to the doctor.

The employee states that EE was getting ready to pump a truck when EE went to the pump and valve to backflush a truck. While operating the valve EE bent down to pull the valve open with EE's left arm and EE felt a strain in EE's left shoulder. The employee went to the doctor on 4/20/2021 and it became reportable.

Employee was pushing the auger bit into the chute out of the walkway. It was not noticed immediately until exiting the manlift later in the day.

Employee twisted Right knee while operating chain fall. Employee on 7 days of restricted duty

The employee took EE's goggles off to put EE's safety glasses on and some dust that was on EE's goggles got into EE's left eye.

Employee was helping someone slide a refrigerator out from the wall to clean behind it in the lunchroom. While doing this the employee felt EE's left shoulder come out of place.

Employee states: picked up a 2.5 gallon container of oil and was pouring the oil into the discharge elevator on the #5 mill chain guard. Employee felt a spasm in lower left abdomen. The doctor diagnosed employee with a work related hernia on 4/20/21, related to this incident.

Employee was cutting some vanaflex on the clinker feeder belt. EE was cutting the NW section of the belt with EE's left hand when the breakaway knife snapped causing the knife to go forward striking the frame. This caused the hand holding the knife to ricochet over and cut EE's right wrist.

While working on kiln pier 3, wind blew dust up under the employee's safety glasses and got into left eye.

EE removed check valve from compressor set it up on elevated platform (~2 feet off of ground). The check valve fell off and hit EE's left foot above the steel toe of the boot.

EE was checking on the mill scale carryover screw conveyor for a choke and put EE's right hand up on the drive guard that was over the v-belt and sheaves. As EE's hand was on the guard the sheave rubbed the guard causing the guard to push back causing what the EE described as a jammed finger to EE's right ring finger.

Employee pinched EE's right index finger between a hammer and a drift causing a laceration.

Employee was helping another employee move an empty filing cabinet from the outside hoist cage to the office hallway. While they were moving it the employee cut EE's finger on the bottom lip of the filing cabinet.

Employee was standing off to the side while the witness was driving the liner out of place. When the witness struck the liner it broke a piece off striking the injured employee.

Employee was installing belts on limestone crusher when glove got caught between the belt and pulley. This caused fingers to be pulled in.

Employee was stacking bricks in the kiln and a brick fell and struck EE's right middle finger causing a closed fracture. EE was put in a splint and returned to work the next day.

EE was emptying contents of 55 gallon trash drum into the bobcat bucket, EE's foot hit a piece of steel liner material that was leaning up against a wooden cabinet. This caused the piece of steel to fall forward, striking the employee on the left side of the head/ear, knocking the hard hat off of head.

Employee had cut right arm on a burr located on stem at double dump.

EMPLOYEE WAS CLEANING SENSOR FOR MIX TANK. SWEAT CAUSED STREAKING ON RESPIRATOR MASK AND LOST SIGHT OF WHERE TO STEP. MISSTEPPED AND FOOT/ANKLE POPPED.

Employee had thrown track switch for rail cars and felt pain in left arm.

Employee got thumb pinched on door of primary crusher while trying to open it.

Employee had gotten off of forklift to move steel plate back on forks that had fell off and felt pain in right shoulder and bicep area.

Employee was in feed chute removing an eye bolt from a plate with a wrench. The wrench slipped and struck employee on right side of face.

The employee was assisting forklift operator set a 1-1/4" x 12' metal plate on a tie block so the operator could readjust the metal plate. The metal plate slid off of tie block onto injured employee's foot.

Employee was cutting plate with a grinder ans while taking off a little to fit into place the grinder came off the steel and struck employee in the left thumb.

Employee was removing scaffold board in 4th stage cyclone of the preheater vessel, when EE tripped over hose while carrying scaffold board, injuring EE's left knee.

Employee was getting into position to cut bar grating and while getting into position the employee cut their leg on a protruding piece of steel.

Employee was inspecting fire extinguishers. Employee walked inside of train locomotive and stepped into floor hatch, injuring left leg. The floor hatch was left open by employees to allow heat to get to air lines. Employee did not use barricade or signage.

Employee was patching holes on pipe with tin, when employee stretched outside of building to install fastener overexerting right shoulder and causing it to pop.

Employee had gone to 3801 belt tunnel to winterize heaters. Employee was servicing south heater from an eight foot ladder and somehow fell from ladder. The injured employee was discovered unconscious by electrical maintenance worker that was looking for injured employee.

The employee alleged that while walking down steps employee's left foot slipped behind step into the opening between the step causing pain in left knee.

Employee was drilling holes in screw conveyor covers overhead when the drill bit caught and twisted right arm.

The fire detection system alerted that a fire was detected in the burner building coal fines storage bin area. The shift supervisor and some crew members went to the area to determine if there was an actual fire. The supervisor determined that it was not safe to fight the fire and evacuated the area. The supervisor requested that the fire department is called.

EMPLOYEE WAS PROCESSING A TRUCK BY ADDING CHEMICAL WASH. THEY THEN USED THE AGITATOR TO BLEND SOLIDS WHEN A SPILL OCCURED. WHEN EMPLOYEE WALKED OUT OF THE TRUCK BAY AREA THEY FAINTED FROM EXPOSURE TO FUMES.

Employee and witness were loading argon cylinder onto back of a pickup truck. While the employee was lifting the bottle with left hand, EE felt a sharp pain in the front of elbow. The employee immediately dropped the bottle.

Employee was walking backwards and tripped, falling backwards onto hand, dislocating left index finger. Employee was working inside cooler.

EMPLOYEE WAS LIFTING A HEAVY POWER CORD AND WHILE BACKING UP, EE TRIPPED ON CORD BEHIND EE AND FELL BACKWARD ONTO FLOOR.

EMPLOYEE ATTEMPTED TO PICK UP THE TOP SET OF BLADES THAT HAD FALLEN AND BECOME LODGED BELOW THE MAN WAY OF THE TRUCK AND PULLED A MUSCLE IN EE'S ARM.

Employee injured EE's finger requiring 3 stitches to the index finger. The maintenance mechanic attempted to remove metal shaving from drill press while the drill press was on and drill bit slowly turning. The metal shaving penetrated through glove cutting right hand index finger.

Supervisor was lifting "starter" from emergency power generator for pumps located at the Sorau Pond building. Complained of back pains after lifting starter. Employee reported injury but requested to delay treatment to determine if back would improve. Employee did not improve and was taken to occupational doctor.

EMPLOYEE CUT THE LID OFF OF A DRUM. EE GRABBED THE LID WITH RIGHT HAND AND PULLED ON IT WHEN THE LID PIVOTED BACKWARDS STRIKING AND CUTTING EE ON RIGHT FOREARM.

Employee was changing a 4" drill bit to a 4.5" drill bit on a Drill mobile equipment. The employee was changing it in the vertical position, when the drill rod came loose, dropping 2" smashing employees finger. Severe cut and broken finger to the ring finger and contusion to the index finger.

Employee was working in #4 Blend Silo repairing air slides. EE was standing in an air line slot, with both feet wedged in, repetitively turning at the waist while repairing air slide. EE said when turned at the waist, to left, knee shifted in the direction EE turned causing right knee pain.

On January 15, while pulling conveyor belt, employee stepped into crevice on ground and stumbled and reported pains. The pains went away and reoccurred on March 2. EE reported pain in knee after standing up from kneeling position. EE was taken to the doctor evaluated and was referred to specialist. EE was not place on restrictions until knee surgery on April 12.

EE reported eye injury to EE's supervisor on 2/18/2015 stated that EE had gotten foreign body into EE's eye when chipping away slag. EE said the slag pop and enter from the side of safety glasses into EE's eye. The employee tried to flush EE's eye with water to remove it. EE was taken for treatment 2/19/2016. Diagnosed with rust ring and foreign body removed from EE's(L)eye.

On February 15, 2016 employee reported to EE's supervisor that EE had slipped on grizzly plates in the primary crusher as EE carried material. EE said EE later felt pain in EE's right leg as EE stood up from the kneeling position of where EE was removing mud from the edge of plates that were to be welded. EE was seen by company doctor on February 17, 2016.

Employee did not report to work on 2/13/2016. Supervisor called EE at the beginning of shift to check on EE and employee stated to supervisor that EE went to the doctor the evening of 2/12/2016 for eye pains and EE was diagnosed with welder flash. The employee reported to work on 2/15/2016 with no symptoms. EE was taken to company doctor on 2/16/2016 for evaluation.

Employee had stepped up on stack of plywood and over on stack of 2x10s to pull down 2x4s. Employee misgauged EE's step while stepping backwards and fell down 29" landing on EE's feet and slipping falling on stack of plywood. EE worked the following day. EE was taken to see company doctor and place on restrictions January 25.

Employee was striking wheel chain with a 1lb ball peen hammer when a metal fragment from the chain came off striking him in the forehead. The metal fragment became airborne and lodged underneath the skin of employee forehead. The employee returned to work with no restrictions.

EMPLOYEE EXPERIENCED HEAT EXHAUSTION WHILE WORKING AT THE 7TH FLOOR PREHEATER TOWER. EMPLOYEE WAS WORKING ON HIS KNEES, STOOD UP AND FELT DIZZY, AND FAINTED TO THE GROUND. HE REGAINED CONSCIOUSNESS AND WAS ASSISTED DOWN THE STAIRS AND TO THE OFFICE. HE WAS THEN TRANSPORTED TO THE ER.

EE was unloading trucks. It was a hot day. EE body started cramping. Another ee called 911 to get him medical attention. The hospital gave him 6 bags of IV fluids and ran some test on him to make sure nothing else was wrong. He was released from the hospital that night. He is very sore.

The employee had thrown railcar switch East of Engineering Building. He walked back to railcar to pull himself back up on the step of railcar and felt pain in his lower back. He said that his back had locked up and he cannot straighten up.

Supervisor said he must have cut his inner forearm on the filter cartridge flange while wrapping rope around it so that employees located on the stairway leading to railcar bin, could pull it up. He did not feel any pain at the time and noticed that he was bleeding when he raised his right arm.

The employee was trying to dislodge balls that were stuck in the bottom of hopper with a short bar when he struck his hand on the side rail of discharge shoot.

Employee was grinding stainless steel flange when a piece of metal blew into his eye. The employee was wearing eye protection: safety glasses and face shield. The employee flushed his eyes to remove the metal but he scratched his cornea before the metal was removed.

The employee was using a rivet buster to remove castable around the 2nd stage thimble. He was working on top of a scaffold. The employee said that he felt a pop n his left arm (forearm area) and pain. The supervisor contacted the Safety and Health Manager on Saturday morning 2/07/2015, to report that the employee is needing medical attention for his (L) arm.

The injured employee (supervisor) stated that Control Room Attendent was opening small man door on kiln hood. The supervisor was standing behind CRA when the door started to open falling off the hinge towards CRA. The supervisor grabbed the door pulling it away from CRA, straining his right shoulder.

Employee said when removing clean-out port, he stood up and raised his face shield to the up position and dust fell into his right eye. He irrigated right eye several times but eye became irritated. He was taken to the ER for observation.

The injured employee was working on the head pulley of 3803 conveyor attempting to install a head pulley scraper, when the diverter gate located below the employee hand actuated. The sudden actuation of the diverter gate caught the employee right hand middle finger between the diverter gate and scraper, crushing and partially amputating finger.

EMPLOYEE WAS CUTTING THE TOP OFF A CLOSED TOP DRUM. WHILE REMOVING THE TOP HE CUT HIS RIGHT HAND ON THE EDGE OF THE LID.

Employee was working from low level scaffolding and while descending his shoe laces got tangled causing him to trip falling forward. He extended his hands to catch himself grabbing the scaffolding in front of him during the fall bent his left thumb back on impact.

The employee was working on the 6th floor of preheater tower building scaffolds. The employee stated that he was experiencing pain in his legs and thought nothing of it. Employee stated, when he got home his wife gave him some Tylenol. He stated that when he woke up his whole right leg was numb.

Employee had been using an 18" long pry bar removing ductwork. Employee complained of right arm/shoulder pain approximately 30 minutes after completing this task.

Employee received a contusion to his left wrist while trying to catch a ladder he was lowering down from an above work surface.

EMPLOYEE WE USING AN ALLEN WRENCH TO LOOSEN THE BOLT ON A GUARD OF THE TANK AGITATOR WHEN THE ALLEN WRENCH SLIPPED OUT OF THE BOLT HOLE. THIS CAUSED HIM TO STRIKE HIS RIGHT HAND INDEX FINGER AGAINST THE CORNER OF THE TANK AGITATOR FRAME.

Employee was working on R -3801 with other employees attempting to correct a belt that was folded under and pinned against conveyor rollers. The employee stated he was lifting up on the belt, when he felt pain in his right arm bicep region. The employee was taken to St. Francis Occupational Medicine for treatment

EMPLOYEE WAS HOOKING UP THE HOSE TO A TRUCK WHEN HE INJURED HIS WRIST TRYING TO CONNECT THE HOSE TO THE TRUCK ELBOW-ADAPTER. EMPLOYEE STATED THAT HE WAS TRYING TO HOLD THE ELBOW AND CONNECT THE HOSE WHEN HE FELT A POP IN HIS WRIST.

Employee operator tractor was cutting grass at the North West corner of river silos when he drove over a in ground nest of yellow jackets. The worker stopped to repair his bush hog and was swarmed by yellow jackets. The employee was stung in the lip and right arm.

EMPLOYEE WAS REMOVING A LID FROM A DRUM AND HIS HAND WAS CAUGHT BETWEEN THE LID & DRUM. DRUMS ON THE CONVEYOR BACKED UP AND PINCHED HIS FINGER BETWEEN LID & DRUM - CUTTING HIS FINGER.

WHILE LOADING OVER PACK DRUM CONTAINERS ONTO THE 1-TON TRUCK, EMPLOYEE FELT THE DRUM WAS GOING TO HIT THE TRUCK BUMPER SO HE SWUNG THE DRUM OUT AWAY FROM HIM TO GET IT IN THE TRUCK. HE FELT A POP ON HIS INSIDE RIGHT ELBOW.

BACKED TRUCK UP TO AN OVER PACK THAT SHOULD HAVE BEEN EMPTY; IT IN FACT WAS NOT. KNOWING THE NORMAL AVERAGE WEIGHT, EMPLOYEE WENT TO LIFT THE DRUM AND CAUSED STRESS TO BACK AND SHOULDER.

Employee was ascending a stairway when employee's boot became caught within a stair tread thus causing employee to fall forward. Employee tried to catch himself with his arms thus causing injury to his left wrist in the process.

He was using a hammer to remove a bolt from a screw and struck his thumb

Employee received non life threatening injury (abrasion, leg strain) to his (L) leg while operating an excavator, when the bank sloughed off causing the excavator to travel down hill bank, striking the right rear end of haulage truck in its path. The excavator cab was damaged in the incident.

Employee was improperly using a 4-1/2" grinder (whereby the guard thus became ineffective) to grind welding slag off of a metal ring. While holding the grinder with his left hand improperly, the grinder became out of control striking the tip of his right thumb. Employee was wearing all required PPE.

Employees had been removing some existing ductwork from a series of dust collectors. Two employees were manually loading this scrap steel into a scrap bin whenever one of the scrap pieces caught the inner left forearm thus causing a laceration.

For some unknown reason, employee alleges that he climbed onto a flat-bed trailer in order to look for some rigging that his foreman had left to retrieve, employee alleges that he hurt his bicep while descending from the flat-bed trailer.

Employee was cleaning roof on very windy day when material flew into his eye. Doctor issued a prescription.

WHILE REMOVING COUPLER FROM RECIRC PUMP #4, THE PUMP SLID OFF PAD AND ONTO FOOT.

Employee was standing on rebar grid and was tying rebar. He was bent over and his right foot slipped off of the rebar that he was standing on. As his right foot (heal) slipped off he hyper-extended his right knee. He said he felt immediate pain and discomfort but not enough to stop working. Later in the afternoon after standing on it he became sore and stiff.

EMPLOYEE HAD UNHOOKED A RINECO 6" HOSE AND WAS GOING TO PUT THE CAP BACK ON AND HURT SOMETHING IN HIS BACK.

Employee stated he strained his back while shoveling mill scale along W3801 belt.

The employee was throwing a track switch in the rail yard. He felt a pain in his ribs. He was diagnosed with a collapsed lung.

The employee's hand was on the cable of an electric winch that was lifting a bearing cap. His hand was pinched as it was drawn into the winch.

Employee was removing and installing bags in the Dust Collector. This is a repetitive lifting type work.

The employee stated he felt his back tighten up while moving approximately 40 pound cooler grates from the fabrication table to a pallet. He will receive PT for a strained muscle.

An employee was climbing a ladder. He was about 14 feet above the floor when his hand missed the rail and he lost his balance and fell backwards to the floor. He was wearing gloves but did not have anything in his hands.

Employee was climbing down from the bed of a pickup and his foot slid off the bumper. He landed with his weight on his right leg and sprained/twisted his knee.

Employee was "barring" an engine in the diesel shop which means he was turning the engine by hand with a wrench.

Employee was cleaning a duct and product came down from above. The product pushed the employee into a metal turning vane. It was hot enough to cause 1st and 2nd degree burns to his arms and waist. An operations problem led to an unknown material buildup in another part of the duct.

Employee was operating a hand held grinder. It "kicked back" and cut his forearm.

The employee bumped his knee on a piece of steel while working on the Raw Mill.

While helping electricians pull a cable, the cable broke and the pulley swung and hit the employee in the leg.

Tightening a fitting with a large pipe wrench.

Employee was lifting a metal cover and felt a twinge in his back.

The employee slipped in the parking lot on the snow and fell on his right shoulder.

EMPLOYEE WAS ON TOP OF THE MIX TANK INSTALLING DISPERSER MOTOR, STANDING BETWEEN DISPERSER AND SCRAPER LIFTING AND MANEUVERING MOTOR TO FIX PROPERLY WITH THE COUPLER. HE FELT TWINGE/POP IN LOWER LEFT BACK.

Employee complained of headache and feeling sick after welding in the kiln while bobcats were mucking the kiln.

Employee was walking on the Raw Mill Deck with a grease gun in left hand. He started to fall and grabbed the handle of an empty tool cart with his right hand. It tipped over letting him fall on his right knee.

WHILE CARRYING A PUMP, EMPLOYEE TRIPPED OVER HOSES AND FELL. THE PUMP LANDED ON HIS FOOT.

The employee got dust in his eye as he was finishing cleaning a port at Module #3 on Bypass at the bottom of the hopper. A prescription was written and filled.

WHILE TRYING TO HOOK UP HOSE TO TRUCK, HOSE WOULD NOT GO ON PROPERLY AND EMPLOYEE HAD TO PUSH/PULL HOSE TO GET IT ON PROPERLY

Employee was unblocking a feeder to allow product to flow. When the product started flowing the employee got it in his eyes and on his arms and hands.

EMPLOYEE WAS PULLING LID & RING OFF A DRUM WHEN HE FELT A "POP" AND PAIN IN HIS LEFT ELBOW.

The employee pulled on a strand of metal shavings at the metal lathe. The sharp edge of the strand cut through his glove and into his finger. The employee required stitches.

Employee cut his finger with a utility knife and required stitches.

Employee was adjusting a guard to allow him to change out some drive belts. The belts began freewheeling and the employees gloved hand was pulled into the sheave.

WHILE SAMPLING CKD, EMPLOYEE OPENED THE VALVE AND NOTHING CAME OUT OF THE HOSE. HE TAPPED HOSE ON CONCRETE BLOCK AND STILL NOTHING CAME OUT. WITH THE VALVE OPEN HE TAPPED VALVE BODY WITH CHIPPING HAMMER THAT WAS THERE. GASKET IN THE CHICAGO FITTING AFTER THE VALVE BLEW OUT COVERING EMPLOYEE WITH DUST FROM THE WAIST UP.

Employee stated he felt a "pull" in his right shoulder as he was closing the door to the elevator. First day of physical therapy was 10/19/2009.

Employee was burned by hot product coming out of a port on the preheater tower. He received 2nd degree burns.

EMPLOYEE WAS PICKING UP THE UN-LOAD HOSE TO CONNECT TO TRUCK AND HURT HIS BACK

Employee got something in his eye. The ER doctor issued a prescription for medicine for the eye.

Employee stated he felt pain in his back as he closed the freight elevator door.

Employees were grouting next to an approx. 50 foot highwall. A small 1.5 inch rock fell and struck employee's hard hat. The impact caused the lining of the hard hat to create a small 1 inch laceration to the forehead.

Wind was gusty and blew dust in his eyes.

Employee was cleaning chunks out of hopper for kiln feed seal and dust got in his eye.

EMPLOYEE SLIPPED WHEN WALKING THROUGH RECEIVING ON A SPOT OF WASTER. HE TURNED LEFT ANKlE, LEFT KNEE AND FELT A SPASM IN LOWER BACK. HE FEEL INTO DRUMS WITH LEFT SHOULDER.

Employee fell while exiting the crusher and needed stitches to his elbow.

EMPLOYEE WAS CUTTING THE TOP OUT OF A 35 GALLON DRUM TO BE REPACKED WITH DRUM DE-HEADER AND SMASHED RIGHT RING FINGER.

The employee stated he was running the drill press and as he was pulling the bit back through a bored hole it hung. The metal piece he was drilling came loose from the vice and struck his wrist.

Employee has a history of back conditions. Employee was assisting another employee in lifting a 40 lb piece of job site material when during this movement he simply twisted and felt some onset of right low back pain.

Employee was working outside the plant on a drag chain at the raw mill when the plant employees were cleaning above creating dust, metal shavings and other debris to circulate below where ee was working thus causing a peice of metal shaving to enter his left eye behind his safety glasses.

EE slipped and fell on some dust and dirt while walking down a ramp and landed on the front side of his body. EE struck his chest area when he fell, therefore we EE to local hospital ER for check. Checked out with no apparrent injuries from the fall.

EMPLOYEE WAS PERFORMING DENSITY ON A SOLID SAMPLE WHICH REQUIRED ADDING THE SAMPLE TO A GLASS VIAL. WHEN CAPPING THE VIAL, THE VIAL BROKE AND CUT RIGHT HAND.

EE had welded a pick eye onto some steel in order to pull a chute (to be installed) w/a chain come-a-long. EE apparently placed too much tension on the come-a-long, which in turn placed too much tension on the pick eye. The weld pulled loose, causing the pick eye to strike EE in the hand, injuring the index finger on his left hand.

Several ee were welding. In an area whenever the stub of a hot used welding rod being disposed of struck ee on the back of the neck thus causing a burn to his neck.

Employee was working outside at the plant with very extreme windy weather conditions. Some foreign particles of cement dust and steel fibers were blowing around. A very small piece of steel material was lodged in his left eye causing irritation and ongoing discomfort since Friday.

Employee stated his knee popped when he stood up after he had been cleaning the infeed journal.

Employee was swinging a sledge hammer continuously throughout the night shift & lifted heavy liner plates during project demolition. He stated that he had a stiff back on his drive home from shift Thursday morning 1/10/08.

EMPLOYEE WAS STEAM CLEANING IN THE PROCESS ROOM WHEN HE STUMBLED AND RAN STEAM ACROSS RIGHT FOOT.

Employee claims he reached down to pick up small piece of angle iron off the floor and felt pain in his lower back.

Employee was cleaning a spill from a drum and got hazardous waste on his pants & shoes.

EMPLOYEE CUT RIGHT MIDDLE FINGER WHILE CUTTING LIDS OUT OF DRUMS.

During lunch the employee began having chest pains, his fellow employees notified the supervisor and safety manager immediately and the employee was transported to the local hospital. It was confirmed by the hospital that the employee was having a heart attack.

Emp. reported that while shoveling material off the roof of KB2300 compressor roof, a wind picked up material and blew up into his face getting dust into the right eye. This a reportable only. He did recieve a prescription from the doctor for the eye.

This is a reportable illness, the employee was diagnised with Carpal Tunnel on 3/30/2007. Skin grafts from a previous injury contributes to the current condition. We were notified on 4/2/07 of the diagnosis

Emp. stated while driving elevator chain pin back to make clearance on chain link section, he felt pain in lower back. Pushing an approx. 30 lb. object with a twisting motion.

Employee stated, while climbing a ladder, he felt something pull in his left knee. First lost time day 4/12/2007.

Employee tripped while walking carrying material for the jobsite. Material fell with his body to the ground hitting his right hand injuring ring and pinky fingers. Employee went to the hospital for treatment, x-rays & employee had fractured both fingers and received stitches in the nail area of both fingers.

Emp stated he was lifting a piece of lumber from the 5th floor of the RSP Tower to the 5 1/2 floor. When he slid his hand down the board to get a better grip, a splinter from the wood punctured the left palm. Emp was wearing leather gloves at the time of injury. His first lost time day is 2/8/07.

Bilateral Carpal Tunnel due to repetitive motion (left and right wrist)

Employee reported he was walking and turned sharply and slipped on a piece of clinker causing him to fall on his left shoulder. First Lost time day is 4/3/2007.

The emp. stated he was servicing the compressor at KB2300, this required the air pressure to be bled off the tank. The emp. opened the valve, stepped outside and felt material enter his right eye.

Employee claims he was jackhammering and felt his muscle pull in lower abdomen. THIS EMP. HAS BEEN ON LIGHT DUTY SINCE 9/11/2006 AND IS HAVING SURGERY TODAY, ON 10/16/2006. TODAY IS THE 1ST LOST TIME DAY.

Employee claims he was getting off of the RSP Tower elevator on the 3rd Floor. When he closed the door, with a broom in his right hand, the broom caught in the door breaking his right thumb.

EE was loading priming stemming holes in the heat of the day.

Emp. stated - while cleaning out blockage in plastic transfer pipe; a gust of wind blew dust in his right eye. No lost time. Prescription eye drops prescribed by physician. Doctor visit only

Employee stated that he was working on the hydraulic rock breaker at the primary crusher and pulled a muscle in his lower back while working on the repair of the unit.

Missed a step on a ladder coming down off of a tanker trailer.

Injured was removing scrap from R3803, which was shut down. A kick out gate was opened, Employee placed hand in gate to remove scrap & gate closed on his right hand. Resulted in a fracture & lacaration to right hand fingers.

While loading a burn pump into the bucket of the bobcat, employee's hand got caught between the burn pump & the bobcat bucket.

The emp. alleges that while he and another employee were closing an approx. 130 lb. refractory lined door on the recoup duct when the emp. felt something pull in his back. 1st Lost Time Day 3/10/06

The Pre-Heater Tower was choked and it was found that the third stage east (6 1/2 floor) was blocked. A 4"x10" door was removed and hot material shot out of the port, striking the injured, who was in the stairway. Burns to face, head and both hands.

Employee was rolling drums on conveyor and strained left shoulder.

Employee was working in the ball mill removing plates from partition. Employee fell through exposed hatch in floor, falling 10 feet to concrete floor below.

EMPLOYEE WAS WORKING ON SHOT, THE FOREMAN OF THE QUARRY CAME BY & INFORMED THE CREW THAT HE WAS CANCELLING THE SHOT DUE TO WEATHER CONDITIONS. THE TRUCKS WERE PARKED IN AN AREA WHICH WAS MUDDY & SLIPPERY. AS THE EE GOT INTO THE PASSENGER SIDE OF UNIT 1178 HIS FOOT SLIPPED & HE FELL FORWARD HITTING HIS CHEST AGAINST THE TRUCK DOOR FRAME.

EE claims while changing lime slurry spray lance, he experienced burning/stabbing sensation lower abdomen/groin area. Diagnosis: Inguinal Hernia.

Emp. stated, while climbing a ladder his left knee popped and he felt a sharp pain. 1st Lost Time Day was 5/2/05

Emp. claims while mounting A-C Compressor on large loader it slipped off mounting and started to fall, he caught it and felt pain in left shoulder. 1st lost day began 6/1/05.

EE was chipping welding slag when a piece of slag entered his left eye. EE was wearing the required personal protection equipment at the time of the accident.

While walking through scrap lumber area, employee stepped on a nail causing a puncture wound to his left foot.

EE ALLEGED - GETTING OUT OF DISTRIBUTION BOX HE TWISTED HIS KNEE. FIRST LOST TIME DAY WAS 12/16/04.

EMPLOYEE STATED: HE WAS WALKING THROUGH UNIFORM ROOM AND SLIPPED ON A COAT HANGER. 1st LOST DAY 12/31/2004.

EMPLOYEE STATED - HE WAS MOVING REFRACTORY WHEN A PIECE HIT HIS HAND. (1ST LOST TIME WAS 12/16/04).

EE ALLEGES WHILE INSPECTING THE CLINKER COOLER DUST BLEW INTO LEFT EYE. DOCTOR VISIT ONLY

Employee claims, while removing rubber seals from drill press he turned to put rubber seal on pallet and twisted left knee on rubber cutting in floor.

EMPLOYEE STATED: WHILE WORKING INSIDE FEEDER BELT HOPPER, DUST FELL FROM ABOVE AND WENT INTO HIS LEFT EYE. (SAW DOCTOR ON 8/25/04; PRESCRIPTION DRUG ONLY; NO LOST TIME).

EE WAS USING A GLASS BOD BOTTLE IN THE LABRATORY. SHE CLAIMS THAT THE NECK OF THE BOTTLE BROKE AND CUT HER INDEX FINGER ON HER LEFT HAND. FIVE STITCHES WERE NEEDED. ***THIS INCIDENT DID NOT RESULT IN ANY LOST TIME***

EMPLOYEE WAS HELPING SET 4TH STAGE BEAM IN HOLDING RACK AND HAD FINGERS CUPPED UNDER TOP OF BEAM WHILE LETTING BEAM DOWN WITH THE HELP OF OTHER PEOPLE. EVERYONE ELSE LET OFF BEAM BUT HIM. HE B ROKE THE TIP OF HIS RIGHT RING & INDEX FINGERS.

EE WAS DIAGNOSED WITH TENDINTIUS IN HER RIGHT THUMB. THIS WAS NOT A SINGLE ACCIDENT THAT CAUSED AN INJURY. SHE WORKS IN THE LABORATORY, BUT THE CAUSE OF TENDINITUS IS UNKNOWN. THIS IS AN OCCUP ATIONAL ILLNESS OF UNKNOWN CAUSE AT THE CURRENT TIME.

EMPLOYEE STATED; WHEN DUMPING MIXER POT, THE HANDLE FOR THE MIXER POT CAUGHT HIS FINGER AGAINST HANDRAIL. STITCHES ONLY. NO LTA.

EE STATED HE WAS INSTALLING STEEL IN COOLER AND DOES NOT KNOW HOW SPECK GOT INTO HIS EYE. NO LTA ACCIDENT.

CUTTING BOLTS WITH A TORCH. HE REALIZED HE MISSED ONE. HE WENT BACK TO CUT IT AND LEANED AGAINST (NOT REALIZING). HE SMELLED THE SMOLDER BUT DIDN'T KNOW IT WAS HIMSELF. WHEN HE REALIZED HE PAN ICED & FORGOT TO DROP & ROLL. HE TRIED TO TAKE SUIT OFF CAUSING OXYGEN TO FEED THE FLAME.

EMPLOYEE STATED: HE WAS HAULING COAL FROM STOCK PILE TO DUMP HOPPER WITH A 988 FRONT END LOADER, HITTING A HOLE IN THE ROADWAY, JARRING HIS LOWER BACK. 1ST DAY LOST 1/20/04.

WHILE REMOVING THE SECURITY SEAL FROM THE TOP MANWAY OF A BULK TANKER TRUCK, EE CLAIMS TO HAVE CUT HIS MIDDLE FINGER ON HIS LEFT HAND. EE IMMEDIATELY STOPPED WHAT HE WAS DOING IN ORDER TO SEE K MEDICAL ATTN. EE UNDERWENT A SURGICAL PROCEDURE TO REPAIR A NERVE IN THIS FINGER ON 10/23/03.

EE CLAIMS HE WAS MOVING A 55 GALLON DRUM AND STEPPED BACKWARD ONTO THE EDGE OF A PALLET. HE CLAIMS HE INJURED HIS LEFT FOOT IN THE AREA OF THE HEEL.

AT COMPLETION OF MORNING WORK ACTIVITIES, EE WAS HEADING TO THE BREAKROOM FOR LUNCH, WHEN HE STARTED HAVING BREATHING DIFFICULTIES.

EMPLOYEE STATED HE WAS PLACING CRIBBING (WOODEN 6"X8" APPROX. 36") TO CENTER UNDER THE AXLE. THE CRIBBING HAD TO BE MOVED BACK ABOUT 3" TO CENTER UNDER THE AXLE. HE WAS SITTING ON A SHORT WORK STOOL WITH LEGS SPREAD, ELBOWS PLACED BEHIND HIS KNEES AND PUSHED THE CRIBBING WITH HIS HANDS AND FELT A POP AND BURNING SENSATION IN HIS LEFT DISTAL BICEP. FIRST LOST DAY 8/13/03.
