Employee while operating loader asked for medical assistance and 911 was called. Employee was removed from loader short of breath and in distress. Employee became unresponsive during first aid assistance and was provided CPR and AED while awaiting arrival of external emergency responders. Reported to MSHA immediately without knowing personal medical yet.

Mostrando los 8Employee was cleaning the windows of the front door to the main office. When the employee was coming back into the building the employee grabbed the side of the door when a wind gust suddenly blew the exterior door which swings outward shut, pinching finger and causing the skin to be removed from the tip of the right hand ring finger.
Employee suffered a minor stroke while unloading a tanker of grinding agent.
Employee was dumping empty bags into an outside trash dumper when the wind blew some residue from the bags into his face. He got some material in his left eye. He flushed the eye and when he returned to work the next morning it was red and irritated. He was carried to a walk-in clinic where a doctor examined his eye.
Employee was sitting in the breakroom and stood up. When he started walking he felt pain in the upper part of his right leg. As he walked the pain worsened. He was taken to the emergency room and diagnosed with a strain/sprain to the muscle in his leg.

EE COLLAPSED WHILE STANDING IN THE MILL CONTROL ROOM AS HIS MORNING SHIFT WAS BEGINNING. HE WAS CONVERSING AND GAVE NO INDICATION OF DISTRESS WHEN HE COLLAPSED. ATTEMPTS TO REVIVE WERE UNSUCCE SSFUL AND HE WAS PRONOUNCED DEAD AT THE HAMILTON COUNTY EMERGENCY ROOM. CAUSE OF DEATH WAS A BLOOD CLOT IN HIS LUNGS.
WAS STRUCK BY ANOTHER EE-FELL DOWN STRIKING HEAD.
