For unknown reason, haul truck veered over the berm and into quarry lake.
During a pipe breaking operation by the float crew on the #5 Matrix line near the #8 Lift Pump, the Leaderman was struck by a pipe and suffered fatal injuries. The investigation of the accident by MSHA and the company is ongoing. Autopsy results are also pending.
An employee was operating a CAT 390F excavator in the South Quarry Pit. The machine went into the water and sank to a depth of approximately 25 feet. Consistent with 103K order modifications approved by MSHA, the operator was recovered the following day.
EE entered feeder when gate would not close. While cutting out a piece of metal that was blocking the slide gate, material sloughed off and engulfed the EE.
Employee was underneath Conveyor Belt and became entangled.
During regular duties the operator of the dozer stopped, exited the cab, and the dozer rolled back crushing the operator.
The victim was operating an excavator near a canal when the excavator tipped on its side and went in the water. The victim was able to get out of the cab and transported to a hospital where he died at 5:45 p.m.
CEMEX loader opr was moving flyash from mat'l storage bldg to hopper. Raw mat'l del driver, ind. o/o contracted w/ Tri-State Carriers(TSC), entered mat'l storage area & exited his tractor, preparing to unload. Loader reversed to get another load & impacted the tractor, striking the driver. MSHA issued Contr ID B0350 to TSC under protest as it is not a contr on CEMEX mine site.
Employee was atop of cement silo when the roof collapsed engulfing him.

EARLY MORNING AND THE SUN WASN'T COMPLETELY UP HOWEVER IT WAS LIGHT ENOUGH FOR GOOD VISIBILITY WITH OR WITHOUT LIGHTS BEING ON. BACK UP ALARM WAS FOUND TO BE IN GOOD WORKING CONDITION. The victim was struck by a road grader when it backed up. The accident occurred in the staging area where the victim and other coworkers were checking their equipment for the shift.
The senior blaster received fatal injuries when he was conducting a post-blast examination. The accident occurred when the ground on which he was walking caved in. The circumstances surrounding the accident and the cause of the accident are still under investigation.
Sand bank gave way.
(Under investigation) (Per Supervisor) Employee was assigned task of pushing overburden wall down with bulldozer. This was being done to install a water jack (floating pump) to pump out old mine cut. A short period of time later bulldozer and employee were discovered under water in mine cut by another employee.
While employee was driving the haul truck, it appears he lost control of the vehicle and went over a 7 - 9 foot high berm into the lake that was approximately 70 - 80 feet deep.
EMPLOYEE STATES HE WAS USING BAND SAW TO CUT NOTCH IN WOOD AND PLACED HIS HAND UNDER BLADE CAUSING SEVERE CUT TO HAND. (LT).
EE WAS WELDING A PLATE ON TOPOF THE 3-DECK SCREEN. AT ABOUT 1855 HE SCREAMED. COWORKER WAS WORKING BELOW HIM AND HE RAN TO SEE WHAT WAS WRONG. HE IN TURN YELLED FOR THE SUPERVISOR. THEY FOUND EE LYING ON THE SCREEN WITH THE WELDING STINGER ON TOP OF HIM. COWORKER THREW THE STINGER OFF, RAN AND TURNED THE WELDER OFF. HE CAME BACK, CHECKED AND FOUND A PULSE. HE THEN RAN AND CALLED 91
DRIVER OF BULLDOZER WAS WORKING AT EDGE OF LAKE "PADDING" THE SLIP FACE FOR THE DRAGLINE USED BY RINKER, WHEN BULLDOZER FELL INTO LAKE RESULTING IN DEATH TO DRIVER.
