EE and a co-worker came to my office to report an injury that happened. EE stated that EE woke up this morning with back hurting, take ibuprofen 3x/day as needed. Apply ice pack 2-3x/day as instructed. EE stated EE was shoveling limestone dust off the top of Silo #4 for 1.5 to 2 hours, when EE felt a sharp pain in EE's mid back. EE said it was hard to breathe because of the pain.

Employee was climbing a ladder when EE felt a pop in the right wrist. EE experienced no pain until the following day.

Was at scales loading a truck. Loaded truck and began to return to control room Walked across scale and started to walk down a hill (about 1' high) the ground was slick. EE stepped down with right foot and it slipped from under them and they fell back on their buttocks Their right knee started to have a burning sensation

Employee was coming down the stairs (jumped) at the dryer platform and when EE reached ground level, EE's ankle rolled and EE fell to the ground, injuring EE's ankle and EE's elbow. Employee was terminated for cause on 07/22/2022 (unrelated to the injury).

Employee had been working up on top of the #6 elevator for a few hours prior to lunch. After coming down from the silo, they ate lunch. After lunch, they began experiencing nausea and dizziness and was taken to the urgent care where they were diagnosed with heat exhaustion.

A contractor maintainer was using a 2-pound hammer. An approximately 1.3 cm wedge shaped shard of the hammer head separated causing projectiles that struck the maintainer through the face shield into the lower lip and chipping a tooth. A second shard struck the contractors right hand causing a laceration. The lip required sutures.

Employee while operating loader asked for medical assistance and 911 was called. Employee was removed from loader short of breath and in distress. Employee became unresponsive during first aid assistance and was provided CPR and AED while awaiting arrival of external emergency responders. Reported to MSHA immediately without knowing personal medical yet.

Employee connected the hook of the SRL to a positioning D-Ring on EE's harness the hook was not properly seated on the D-Ring, but rather the D-Ring was pinched between the tip of the hook and the hook gate (not seated in the hook throat).

Employee was getting a piece of metal from metal rack when the metal fell on EE's right foot.

employee was cleaning out build up material from hopper with a bar when EE hit EE's elbow on metal hopper causing lateral epicondylitis and contusion, Employee was not put on restrictions until doctors visit on 03/23/2018.

Employee suffered minor sprain to neck while operating FEL

Employee suffered a minor stroke while unloading a tanker of grinding agent.

Employee was walking from the crusher area and slipped in mud and wet grass spraining his left knee.

While walking to dust scale EE slipped and fell, injuries unknown

Employee lost footing and fell in parking area on his way to his personal vehicle.

While performing normal duties 03/19/11 employee bumped left index finger on a piece of metal. Finger got infected and was taken for medical treatment on 03/29/2011.

While repairing screw conveyor employee followed company lockout/tagout procedures but failed to realize that the screw shaft was free wheeling inside of collar when he stuck his finger into the bolt hole of the screw drive shaft.

Employee was tied off with lanyard on top of rail cars when he stepped off side of car, falling between silo column and railcar. Lanyard prevented him from falling to ground.

Employee was using his finger to remove a nut that was stuck in the socket of an impact wrench when he accidently turned the impact wrench on. This drove the nut onto his left index finger.

Sprained his neck while working on a fuel filter.

While working in low headroom area employee hit his head on a steel beam.

WHILE CLEANING ON TOP OF BALL MILL SEPARATOR ROOF I FELT MY BACK PULL. I WAS USING A PICK AND SHOVEL.

EE WAS WORKING ON REPLACING THE WEAR LINING IN THE CRUSHER FEEDER. HE BECAME OVERHEATED.

EE WAS USING THE BOBCAT TO REPLACE SECO FEEDER BELT. IT WAS RAINING AND UPON EXITING THE BOBCAT HE STEPPED ONTO THE BUCKET AND HIS FOOT SLIPPED CAUSING HIM TO FALL ON HIS LEFT SIDE INJURING HI S KNEE, HIP AND SHOULDER.

EE WAS STANDING IN THE PLANT SHOP TALKING TO PLANT PERSONNEL WHEN HE FAINTED. HE FELL TO THE FLOOR INFLICTING A CUT ON HIS FOREHEAD. SUTURES WERE REQUIRED.

PULLED BACK MUSCLE WHILE CLIMBING A LADDER.

EMPLOYEE WAS MOVING A 55 GALLON DRUM OF OIL FROMA PALLET ON A FORKLIFT INTO THE OIL SHED. DRUM TIPPED SLIGHTLY AND EMPLOYEE REACTED TO PREVENT DRUM FROM TIPPING OVER. THIS ACTION CAUSED HIM TO SCRAPE HIS RIGHT ARM AGAINST THE DOOR FRAME, RESULTING IN A LACERATION ABOUT 2.5" LONG.

EMPLOYEE WAS DESCENDING A VERTICAL LADDER BETWEEN CONTROL ROOM LEVELS; HIS FOOT SLIPPED FROM THE3RD RUNG FROM THE BOTTOM CAUSING HIM TO LAND WITH FORCE ON HIS RIGHT LEG.

INJURED WAS USING A LONG BAR IN INSTALLING TRACKS ON A DOZER WHEN HE STRAINED HIS LOWER BACK.

EE WAS CLOSING BOTTOM DOOR ON A RAIL CAR; THE DOOR STUCK. EE PULLED EXTRA HARD ON A 5 FT. PRY BAR HE WAS USING. THIS RESULTED IN HIM BEING IN AN AWKWARD POSITITON CAUSING HIM TO TWIST HIS RIGH T ANKLE.

WAS STRUCK BY ANOTHER EE-FELL DOWN STRIKING HEAD.

INJURED WAS SWINGING A SLEDGE HAMMER AND CAUGHT FINGER BETWEEN SLEDGE HANDLE AND STEEL STRUCTURE HE WAS WORKING IN A CONFINED AREA.

INJURED WAS USING A CHAIN HOIST TO LIFT AN MCC CABIENT INTO POSITION. HE APPARENTLY STRAINED HIMSELF WHILE PULLING ON THE CHAIN HOIST HANDLE AND CAUSED A HERNIA.

YINJURED, WHO IS A CRUSHER OPERATOR, WAS PERFORMING REGULAR DOWN-DAY CLEANING AND SERVICING DUTIES ALONG WITH A FELLOW EMPLOYEE. HE WAS DESCENDING AHSORT STAIR (APPROX 4') WHICH LEADS FROM A CONVEYOR CATWALK TO THE GROUND. HE LOST HIS FOOTING AND FELL TO THE GROUND. IN THE FALL, HIS LEFT FOOT APPRENTLY BECAME CAUGHT ON A STAIR STEP OR ON A HANDRAIL POST CAUSING A COMPOUND FRACTU

EE AND OTHER WERE TRIMMING OVERHANGING LIMBS AND VINES FROM THE PIT HAUL ROAD. OTHER WAS CUTTING WITH A CHAIN SAW AND EE WAS RUNNING THE MANLIFT. JUST AS OTHER WAS CUTTING SOME VINES, EE REACH ED FOR THE VINE BEING CUT. HIS LEFT HAND WAS CUT HIS LEFT HAND WAS CUT IN THREE PLACES.

EE WAS WELDING ON LOWER SECTION OF ELEVATOR. DUE TO ELEVATOR BEING OUT OF SERVICE FOR REPAIRS, THE PLANT OPERATOR REDIRECTED THE MATERIAL FLOW TO BY-PASS THIS ELEVATOR BY MOVING AGATE. MOVE MENT OF THIS GATE CAUSED SOME MATERIAL TO FALL LANDING ON PEOPLE WORKING BELOW CAUSING EE TO GET SOME AMTERIAL IN HIS EYE.

EE WAS USING A BAR TO HOLD THE FRICTION BAND ON HTE DRAG DRUM. THE FRICTION BAND SLIPPED, CATCHING EE'S LEFT THUMB BETWEEN THE BAR AND THE FRICTION BAND. THE THUMB WAS MASHED AND IT TOOK 8 STI TCHES TO SEW UP THE CUT.

EMPLOYEE HAD STOPPED HIS TRUCK ON A GRADE AT THE CRUSHER & OPENED THE LEFT DOOR THE DOOR SWUNG OPEN & EE CAUGHT HIS LEFT HAND IN DOOR LATCH IT BENT HIS FINGER BACK & BROKE THE BONE IN HIS LEFT HAND JUST ABOVE HIS RING FINGER

EMPLOYEE WAS STRIPING INSULATION FROM AN ELECTRICAL WIRE WHEN THE KNIFE SLIPPED

EE WAS INVOLVED IN HORSEPLAY DURING WHICH TIME HE ATTEMPTED TO BREAK A 2X12 BOARD WITH HIS BARE FIST

55015D OTHER WERE MOVING THE SUMP PUMP TO THE GRANULAR SCALES. RIGHT BEFORE THE SCALES THERE IS A SLIGHT DOWNHILL SLOPE. OTHER LEG GO. EE HAD HIS HAND UNDER THE TONGUE OF THE TRAILER HITCH AND 08920HAND WAS SMASHED INTO THE GROUND.

FELL HEAD FIRST DOWN LADDER STRIKING HEAD ON LADDER

CUTTING CONV KNIFE CUT END OF BELT STUCK IN LEG

EMPLOYEE WAS OPERATING THE FORKLIFT.INJURED GRABBED THE MAST ON THE FORKLIFT TO PULL HIMSELF UP TO GRAB A BULK BAG STRAP.FORKLIFT OPERATOR SIDE SHIFTED THE FORKS AND SCRAPED INJUREDS HAND.

THE MATERIAL DISCHARGE CHUTE BELOW THE 66 MILL CYCLONE HAD BECOME PLUGGED OR BLOCKED OFF,THE ASST OPERATOR INSPECTED THE CHUTE TO CORRECT THE BLOCKAGE BY REMOVING THE SAMPLE PORT AND STICKING HIS HAND AND ARM INTO THE SAMPLE PORT.THIS WAS DONE WITHOUT SHUTTING OFF THE ROTARY VALVE.TO CONTACT THE ROTARY VALVE THE ASST OPER MUST HAVE REACHED UPWARD CAUSING HIS FINGERS TO BE SEVERED B

EMPL LIFTING RAIL CAR LID -IT DIDNT WANT TO MOVE. DIDNT FEEL PAIN AT THE TIME. Y

EMPL ON PORTABLE PLATFORM REACHING FOR 50 LB BAG,SLIPPED THROUGH GAP BETWEEN PLATFORM AND MAIN LOADING DOCK CUT RIGHT HAND WHILE TRYING TO CATCH HIMSELF.

EE GOT OFF LOADER RIGHT FOOT WENT ONE WAY RIGHT LEG WENT ANOTHER.
