Mostrando los 26Operator went to grab falling stack of pallets (~20) that were loaded on forks. When EE reached through the roof of the forklift the pallets lacerated EE's 3 middle fingers and EE received 13 stitches.
In the process of dislodging wedged forklift forks from underneath a bulk bagger, an operator struck the steering wheel of the forklift with chin, causing a chin laceration with 2 stitches.
During their shift, employee got something in their eye. After the shift, they went to medical facility and they removed foreign material from between their eye and eyelid. They received medicine for the eye and reported to work for their next shift.

Employee was using a Kenworth water truck to provide dust suppression and ran over an unseen rock causing the truck to swerve and the operator felt pain in EE's neck from the movement. The operator went to the company clinic and was placed on modified duty for 1 week till re-evaluation
Forklift operator was loading a contract van truck and they had a miscommunication and the driver started to reposition the truck by pulling forward at the same time the operator was returning to start loading. The forklift did not fully enter the truck and the rear tires of the forklift did not make it into the trailer causing a jolt to the operator and a sore back.
Water truck rolled over while descending from quarry pit causing the driver a head laceration requiring three stitches/staples.
Individual was driving a fork truck down a narrow pathway. Individual caught sleeve on protruding screw head. Individual pinched arm between fork truck and a wall.
Employee was assisting the mechanic with relocating a inoperable truck, the mechanic drove the truck over the rear wheel chock causing the chock to strike the employee's right lower leg causing a laceration.
Employee drove haul truck during his shift, felt pain in left shoulder/neck area after work, claims pain due to lifting haul truck dump lever
The employee operated the loader over the service road. The rough road may have caused a strain on his back and left hip. He reported a pain to his back & hip after operating the loader. This did not get reported until 8-15-2012. The employee went to the doctor on his own before reporting this injury/pain to his manager.
Employee stepped into area to place bulk bag straps onto forks so bag could be lifted. Forklift operator drove onto his right foot.
Driver was going to spoil dump and the driver hit a rough spot and it snatched him around. The driver was wearing his seatbelt.

THE EMPLOYEE WAS ATTEMPTING TO LUBRICATE THE ROLLER ON THE INCLINE CONVEYOR BELT, BUT FAILED TO LOCKOUT THE SYSTEM. THE EE WAS USING HIS RT. HAND TO LUBRICATE THE BEARINGS. HIS LEFT ARM & HAND WERE RESTING ON THE TOP OF THE GUARD & EE SLIPPED CAUSING HIS LEFT HAND TO GO BETWEEN THE ROLLER & THE CONVEYOR BELT.
While uncoupling rail car, the employee pulled up on the release handle and strained lower back.
Employee leaned over a conveyor to examine a broken drive chain. While leaning over, he placed his hands onto the conveyor rollers. A photoelectric sensor sensed a presence and activated the rollers, pinching the employee's left fingers & hand between the rollers (compression injury & laceration - 8 stitches).
EE was operating a forklift & loading material onto an empty trailer. Upon leaving the dock & entering the trailer, the trailer "dropped" below the level of the dock plate. The automatic dock plate, sensing no trailer was present, retracted & returned to home position. Unknowingly, EE backed the forklift out of the trailer & into the retracted dock plate.
EMPLOYEE WAS CLEARING SNOW WITH A BOBCAT LOADER. THE BUCKET HIT THE RAILROAD TRACKS STOPPING THE BOBCAT LOADER INSTANTLY. THE EMPLOYEE WAS WEARING HIS SEAT BELT BUT HIS ABDOMEN HIT THE LAP SAFETY BAR. SEE ATTACHED LETTER.
Employee was talking with forklift driver, driver backed up and ran over his foot.
Employee slipped onto energized roller, hand got pulled in between two rollers. Full pallet proceeded down line and pinched leg into roller bracket.
EMPLOYEE AND A FORKLIFT MADE EYE CONTACT AND THE EMPLOYEE WAS WARNED BY ANOTHER EMPLOYEE TO MOVE OUT OF THE WAY. THE FORKLIFT DRIVER TURNED THE CORNER AND MISJUDGED HIS CLEARANCE FROM THE EMPL OYEE AND THE BACK OF THE FORKLIFT SWUNG AROUND AND FORKILFT DRIVER RAN OVER THE BACK OF THE EMPLOYEE'S RIGHT ANKLE.
WHILE BACKING OUT OF ROTOPACKER BLDG W/FORKLIFT, EE BACKED FORKLIFT INTO BLDG SUPPORT CROSS BEAMS. THE IMPACT JARRED HIS NECK & HE HIT HIS HEAD ON STEERING WHEEL. HE DID HAVE HARD HAT ON BUT T HE IMPACT CAUSED THE ADJUSTMENT KNOB ON HAT TO BREAK & HAT FLEW OFF. HE HAD A LACERATION ON THE TOP OF HIS HEAD THAT REQUIRED STITCHES.
EMPLOYEE WAS CLEARING CHUTE OF FROZEN STONE AND REACHED FOR SMALL STONE, WHEN CHUTE DISLODGED STONE, AND PINCHED END OF FINGER, BROKE END OF RIGHT INDEX FINGER.
