A 23 yr old was driving on the road toward the entrance. Was driving under the influence of alcohol. He sustained head injuries, multiple body contusions and abrasions. Was air lifted to medical facility. Prior to being transported was placed under arrest, CHP Report# 985017113. Was not an ee had no business at the facility. He thought he was driving in a different city.

Mostrando los 11A fatal accident occurred on mine property. Three off-road motorcycle drivers trespassed onto private property. One of the drivers lost control of his motorcycle and went over an embankment. The motorcycle driver was pronounced dead at the scene. The motorcycle drivers were exceeding the posted speed limit.
Employee driving truck somehow dust got into the truck and got under the safety glasses.

Supervisor was observing employees align pan conveyor. Of necessity, a guard was off. Supervisor placed hand on pan track to support himself as he leaned over to observe mechanic. Wheels from pan conveyor rolled over 2 fingers, crushing them.
EE was on BB1 conveyor belt looking at blockage when a second EE turned on the belt causing first EE's body to be projected towards the chute. His left leg went under the belt when left leg wa s caught by the head pulley severing the left leg, freeing the employee.
EE was on 416 locomotive when backing up on the mainline couple with the clinker train. The 416 coupled hard causing a jerk of the train, which caused the employee to be jerked hard. EE did feel any injury at the time however days later experienced pain in his lower back.
Internal audit found need to report. This was a vehicle accident involving 2 employees. EE1 was traveling down the hill from Quarry Office. EE2 was coming up the hill. The vehicles collided.
