This is a personal issue on this employee from pre-existing condition. EE was preparing to gather EE's tools when EE slipped and felt a pop in EE's groin area.

Hot material flushed from dust collector hopper causing burns on left and right ankles.

At approximately 10:55AM two employees reported to their immediate supervisor that they were stuck in the K3 elevator between the 5th & 6th floors. Executive Elevator was notified at 11:08AM and arrived onsite at 1:00PM. MSHA notified at 11:25AM (ref # 04688756). At approx. 1:28PM employees were released from the K3 elevator. MSHA was contacted again at 1:30PM (ref # 04689063).

EE was attempting to close passenger elevator door, in the process of doing so EE smashed middle finger. Injury was treated as a first aid. On 03/10/25 EE requested medical evaluation which resulted in EE's duties being restricted. EE was also given splint to immobilize finger.

Technician was climbing a ladder while holding a hammer on one hand. When the EE stepped on the third step of the ladder, the ladder shifted, and as a result EE fell and landed on EE's side on top of the ladder. Breaking 3 ribs and fracturing 1.

EE was working as a tool handler when EE felt dust in eyes. EE visited a clinic on the same day to receive treatment. The doctor confirmed that it was dust that had caused the irritation in the eye. The doctor washed the eye and prescribed some eye drops. As per doctor's order EE resumed work after the treatment on the same day.

Employee was using a jackhammer to break up hardened material. While operating the jackhammer the bit slipped off a large chunk being broken at the time and penetrated the employees work boot just behind the steel toe causing the bit to hit the employee's toe. Employee was placed on modified duty.

elevator unexpectedly stopped between the 3rd and 4th floor of k2 tower. Several attempts to get the elevator unstuck were made and elevator maintenance contractor was called. The elevator stopped due to dust build up on the track of the elevator causing the governor to engage stopping the elevator. The track was cleaned and r inspection before putting back into service.

The step switch oil tank for raw mill caught fire likely due to the motor continuing to run after the command from the PLC was given to stop. The wound rotor leads flowed current to the high resistance contacts of the step switch which would generate considerable heat and eventually cause the oil in the tank to ignite.

The employee pinched two fingers in a Coal mill rotor while checking for tight spots. This resulted in lacerations on two fingers requiring stitches. The employee was released to work with modifications of light gripping.

Worker was removing a rock from an airlock when the rock slipped and smashed their left middle finger between the rock and the housing. This caused a fracture and laceration to the employee's finger. Employee was placed on modified duty.

A contractor employee working during plant outage attempted to use the elevator when it became stuck between the floor 3 & 4. Attempts to get the elevator to restart were unsuccessful. Elevator contractor was called and found the fingers on the door sensor were bent from people slamming and banging on the elevator gate and door.

EE removed the bolts on the torque arm for the airlock which was believed to be plugged. When the last bolt was removed the torque arm moved pinning the impact gun the EE was holding against the wall which was next to the airlock. This caused the EEs hand to be wedged between the wall and the impact gun causing a sprain to the wrist. EE was given modified duty for 5 working days.

Employee was assisting in unchoking a plugged airline for the ckd system. The employee was attempting to re-install the nipple on the airlock and inadvertent contact with the airlock causing the employee to cut the tip of EE's right pointer finger and tip of right middle finger.

EE was descending a 10 ft ladder. The EE was wearing Bulky wet and muddy leather gloves. When EE began the descent, EE lost grip and fell straight down. EE first impacted their feet, then hit their hip and finally impacting the left elbow. The employee felt pain in the left elbow and immediately reported it. EE was taken to urgent care and x-rays were taken.

Miners were taking the K2 elevator to 7th floor. accidently pressing th Estop button. The elevator stop and was stuck. After it reset itself the car began moving releasing the trapped miners. No one was hurt.

Hot clinker dumped from the pan conveyor onto the rubber conveyor belt causing the belt to heat up and burn. This was caused due to a programming error programmed by the manufacturer of the plant when a detection of hot clinker is identified.

While fighting a fire in the clinker barn, a Fire Fighter sustained 1st and 2nd degree burns while fighting a conveyor belt fire in the clinker barn. The fire fighter was treated for the injury and taken off work for 2 weeks.

Elevator became stuck between floor 4 and 5 of the the preheater tower. The elevator co had to be called out to get the contractor out of the elevator. The contractor was out of the elevator at 12:45 am. No injuries occurred during this event. The elevator co inspected and replaced guide rails and gate lock keeper.

Odor of smoldering coal was reported and we began to immediately look for source. Once source was detected we began to dig and drag material to extinguish it. The loader became stuck in coal pile and slowed the progress of extinguishing it. once unstuck we continued to extinguish we had continued putting it out it was extinguished at 3:20am

4 miners became trapped in an elevator when the elevator over traveled 4in and stopped. Miners communicated via radio to control room that they were stuck. Elevator tech were contacted and responded. Miners were released after 2hrs 26min Techs freed miners after being in site for 3min.

FBC 2 conveyance system that transports coke to the mill had a return idler that has ceased and was no longer turning. Due to this condition the belt when running created friction against the return idler. When the belt stopped running the heat due to the friction ignited the fine coke on the return side of the belt causing the belt to burn. The fire last 65 minutes.

Multifuel media was smelled smoldering. We immediately began searching for source. Equip. was removed from service. Found source in dust collection equip. washed out bin with water. Called as soon as we were confident it was out.

EE was installing fike canisters weighting 133-148lbs on 12-30-19. EE developed pain in back over the next 2 days reported injury on Jan 2, did not want medical evaluation until Jan 7. Dr sent EE back to work with regular duty due to pain subsiding. EE reported still painful on 1-24-2020 taken to Dr. and given work restrictions and scheduled for chiropractor visit.

Coal was observed to start smoldering on the coal pad. We immediately began looking for the source. We began digging the large pile and compacting it to smother any smoldering. It took approx. 8hrs to dig out the source that was centrally located at the base of the pile. Called in as soon as we were confident there was no smoldering

CO sensor at 50 ton coal bin quit working and unknown to the operator. The operator ran the coal bin empty in order to perform repairs at the drive sprocket for the screw. There was no ventilation in the system and the agitator was not running causing material accumulation at the bottom of the bin. Co monitor failed giving a false reading pre and post explosion. No injuries.

EE reported to work on 11-2-18 complaining of possible flash burn to EE's L eye from welding. The EE was taken for a medical evaluation and it was found EE had a fine piece of metal in eye. The EE was sent to an Optomologist to remove the metal. EE was released with no restrictions but had follow up appointments to monitor healing progress and antibiotics to prevent infection

Smoke was detected at our multi-fuel tent. Within minutes the tent burst into flames. We applied water to the pile of tire fluff for about 25 minutes. At this time there were no flames. We then use a loader to start back dragging the material to uncover any hot spots. We had 1 flare up at 5:45pm where flame were visible. At 6:01pm the fire was out.

An employee moving a set of rail cars to a spur track, stepped off of a train car from the built in ladder designed for the car and felt a pain in ankle. EE did break both the bones in lower leg. The contributing factors included housekeeping on the surface that EE was stepping down onto and failure to complete a pre-job hazard analysis.

Over the road truck driver was making a delivery at mine site when employee stopped at our control room thinking was having a heart attack. Called 911, EMT did EKG, neg. on heart attack, but they did take employee to hospital for follow up. Truck driver was released to full duty at 4:30pm same day.

Maintenance Mechanic was replacing the seal in an air pipe under the K2 Cooler, while removing the pipe, it turned and made the front end of the pipe go up and pinch left ring finger between the pipe and cooler beam.

Employee was in the process of connecting a second come a long to the bucket elevator when the first come a long hook slip off striking the employee in the face knocking out a tooth.

October 10th Employee was using two wrenches to tighten DBH1 damper cylinder bolts when the lower wrench employee was holding with left hand slipped and employee's left wrist bent and hit the cylinder. On October 12th employee was taken to Urgent care doctor said employee had a hair line fracture to wrist.

Employee went to K3 DTV to investigate a problem. The problem on the K3 DTV system lower gate was slamming hard when it closed. To fix the problem employee dialed the regulator down and gave it back to the CRO. When the gate cycled the exhaust on the regulator blew air and debris flew into employee eye.

While forklift operator was wedging piping into vessel using wood block, the wood block came loose striking finger of right hand against vessel. EE stated - was fine; however, EE later goes to doctor allegedly breaking right finger from the job.

Employee injured left calf muscle while performing routine duties. While holding grease-gun in EE's right hand, EE proceeded to step onto the lower step of the inspection platform. As EE mounted the step, while pushing up with left leg EE felt a pop and immediately experienced a burning sensation in left calf muscle.

While shoveling loose materials from the floor a machinery guard tipped over and struck the employee on the upper back and head. The guard had been disassembled by maintenance personnel just previous to striking the employee.

EE was experiencing numbness and tingling in for arm as a result of using water blaster. ***Per phone call, this occurred over the course of only one shift***

Employee opened a port without PPE and the heat caused a burn on his arm.

Two employees were traveling upward in the K3 elevator when it stopped between the second and third floor. Another employee tried to open an outer door while the elevator was in motion, triggering the elevator emergency stops.

Coal was smoldering and was spotted at approximately 10:40 AM. It was not extinguished within 30 minutes and was called into MSHA at approximately 11:09 AM. This coal fire was extinguished on September 18, 2014 at 2:59 PM.

EE was stepping up onto a forklift and as she was pulling herself up with both hands and arms her left foot was pushing her body upward causing pain and popping in her left foot.

Wood chips were stored in the back of the plant across from the bone yard that were going to be used for reclamation. The wood chip pile spontaneously caught on fire. It was completely extinguished by 8:30 PM.

EE was drilling holes closest to the edge of the 3775 bench. The bench had a contour, EE misjudged his clearance and drove the drill off of the bench. The drill went over sideways to the next level 3760 bench. EE was ejected out of the window and fell to the ground approximately 12 feet.

EE was clocked out and was leaving the plant to go home. He lost control of his vehicle and it rolled. He was seen at St. Mary's Emergency Room as a precautionary measure.

Employee was unplugging the DTV chute with a bar. The tire was stuck so he applied his body weight. When the bar slipped his right hand was in-between the bar and a handrail.

6 CEMEX Engineering project contractors were riding the K3 elevator and it stopped between the second and third floor. Executive Elevator Company was called out to troubleshoot and reset the elevator. The employee exited the elevator at 6:33 PM. There were no injuries as a result of this incident.

Employee was using a water blaster in the K2 Tower and felt pain in both hands and wrists from the repetitive motion of pulling the trigger.

Working in cooler replacing grates & beams, when shimming on beam a fellow worker below cut the beam on the opposite side making the beam shift & smashing left index finger. Employee had surgery on 3/18/14 released to modified duty.

A Labor Finder Laborer bent down to pick up a bucket full of dirt and some of the dirt splashed into his right eye.

Employee was walking down a stairway in the coal mill area when his foot hit the edge of the stairs which was smooth and worn down (slippery). His left foot stayed in place and his right foot slid down; he slipped and when he fell most of his body weight landed on his left leg causing a fractured fibula.

A Cemex Laborer and a temporary Labor Finder Laborer were traveling in the elevator and it stopped between the third and the fourth floor. Executive elevator was called to troubleshoot and repair the elevator. The men exited the elevator at approximately 03:20 AM. There were no injuries as a result of this incident.

EE requested medical treatment on 2/14/14 and said he injured his back on 2/10/14. Doctor determined injury to be non-work related. EE went to his personal doctor and his injury was determined to be work-related. EE was sent to a third doctor on 2/25/14 and his injury was determined to be work related. EE was placed on restricted work duty and this became reportable on 2/25/14

EE strained wrist while removing refractory

A Labor Finder Laborer took the water hose off of the pressure washer. He failed to release the pressure from the hose, and steam from the water line burned his right hand and wrist.

Labor Finder Laborer ws riding the K3 elevator and it stopped between the first and second floor. Executive Elevator Co. was called out to troubleshoot and reset the elevator. The employee exited the elevator at approximately 3:27 PM. There were no injuries as a result of this incident.

A fire occurred in a compartment of the GCC Cooler as a result of a hydraulic line break, when the kiln flushed.

Employee was adjusting the primary air on the burner barrel. One of the air cannons discharged and a hot piece of clinker was projected out of the hood door and struck the employee on the left side of his neck. This resulted in first and second degree burns.

Two contractor employees were in the K3 elevator travelling upward. One of the employees accidentally hit the inside door with a broom causing the elevator safety features to enable stopping the elevator between the second and third floors. The men exited the elevator at approximately 3:49 PM. There were no injuries as a result of this incident.

Coal was smoldering and was spotted at approximately 2:00 AM. It was reported to MSHA at approximately 2:30 AM. Steps were taken to extinguish the smoldering coal. The smoldering coal was extinguished at approximately 8:30 AM.

Employee was blowing-off his clothing and dust particulates blew into his right eye. He was wearing his safety glasses.

The K3 elevator was descending to the ground level and stopped between the third and second floors. Executive elevator was called to troubleshoot and reset the elevator and get the contractor employees out.

While installing a 9 inch screw into a shaft. The screw rolled onto his left pinky finger.

Employee was placing 20 lb. wooden blocks next to the railroad tracks in an awkward position with his hands extended out in front of him to re-rail the front trucks of the locomotive. He overextended and when he stood up felt pain in his abdomen. The employee is on restricted work duty.

A Cat 992G Loader was loading a truck and a loader tooth fell into the truck. As the truck dumped the rock into the primary crusher, the loader tooth got caught between the mantle and the crusher liner stopping the drive. The primary crusher motor did not stop and high friction on the pulley caused the belts to start to smoke.

Emp. was using a water blaster gun. He pulled the water blaster gun out of the K3 Calciner port, and his glove got caught in the trigger and it kept spraying water, but the force pushed him backwards causing him to become off balance and he rolled his right ankle. He refused medical treatment until 4/11/2013 at which time this accident became reportable.

Employee was in the elevator traveling down from the second floor of the K3 tower to the first floor K3 tower, and the elevator stopped. He immediately called the Control Room for assistance. Executive Elevator Company was called out to troubleshoot and reset the elevator. The employee was released from the elevator at approximately 8:25 AM

Temp. Agency Employee alleges his back began to hurt on 12-17-12, but he did not know if he did something at home or at work. This was not reported to Cemex until 12-28-12. He went to his doctor on 12-26-12. at which time the doctor put him on restricted work. This became reportable on 12-26-12. Cause of injury unknown.

Coal was smoldering and was spotted at 8:42 AM. Steps were taken to extinguish the smoldering coal. The coal fire was extinguished at approximately 9:40 AM.

Employee stepped outside the coal shaker building and twisted his left ankle.

Three contractor employees were using the elevator travelling from the 4th floor down to the 1st floor. They were transporting a brick saw and a pallet jack. The elevator passed the second floor and jerked twice and stopped. The elevator operator used the radio inside the elevator to call for help. The Elevator Company was called and got the men out at approximately 12:40 PM.

Coal was smoldering and was spotted at approximately 2:30 AM. Steps were taken to extinguish the smoldering coal. The smoldering coal was extinguished at approximately 5:50 AM.

Employee was changing a v-belt. He used a utility knife to cut the belt and cut toward his body instead of away. He lacerated the underside of his left arm by the elbow. This was a superficial laceration, too shallow for sutures, but two steri-strips were used. He did not need medication or a follow-up medical appointment, but this is reportable by definition.

EE was climbing a ladder and heard his right knee pop. His knee began to swell. He was seen by the physician but was able to continue his full work duty. On 8/30/2012 (his regularly scheduled day off) he had surgery on his knee and this injury became reportable. His next regularly scheduled shift began on 9/5/2012, at which time this accident became a lost time injury.

EE was water blasting material buildup in the riser when a chunk of material flew out and hit his face shield of his aluminized hood, causing him to stumble backwards. He hit his head on a vertical beam and the face shield popped off and the open frame of the face shield pushed his goggles down past the bridge of his nose. Another chunk of material flew out and hit his right eye.

EE. was struck in the face with a crowbar, and lost teeth on 4/17/2012 EE received sutures during reconstruction on 4/10/2012.

Two men got in the K2 elevator on the sixth floor. The elevator stopped between the sixth and fifth floor. The radio inside the elevator was used to call the control room operator. A utility man was sent to the elevator and he pushed the elevator buttons which released the elevator. Elecutive Elevator Co. was called out to check the elevator.

Employee was hearing impaired on his baseline audiogram at the time of hire. He has experienced a shift in his hearing since his baseline.

Employee was tightening a fitting on a fuel pump with a pipe wrench, pushing the pipe wrench forward. As he pushed the pipe wrench, he heard a low popping noise in his right chest wall, felt pain and stopped the task.

Employee lifted the right fork lift (on a Terex forklift) in an attempt to reposition it to gain clearance to the compressor enclosure for the purpose of compressor motor replacement. Vertical portion of fork crushed tip of employees' s finger when fork release to running position. EE misjudged position of hand relative to fork.

Employee was exiting the south clinker silo on the east side and stepped onto several clinker balls causing him to sprain his left ankle. He was wearing the correct type of steel toed boots. Employee was placed on restricted work duty.

Coal was smoldering and was spotted at approximately 3:00 AM. Steps were taken to extinguish the smoldering coal. The smoldering coal was extinguished at 8:00 AM.

The K3 elevator stopped between the second and third floor and two contractor laborers were inside; one laborer from Labor Finders and the other from Southern Refractories. Executive Elevator was called to reset the elevator.

Employee was using a six foot spud bar to engage the inching drive for the raw mill (DRM2)when the bar broke loose causing the employee to fall backwards, hitting his side and back on the handrail. The employee sustained a contusion to his chest wall and back. The employee requested to go to the doctor on 6-21-11 at which time he was placed on restricted work duty.

Employee was removing twist lock venturis from the 12 foot dust collector bags. While moving the bags from one pile to another, he began to feel pain in his lower back.

Contract Laborer (Labor Finders) was pushing 6" rock off of BBC1 conveyor with a shovel and twisted his back. He strained the lower right side of his back.

ARSI employee leaned up against the inner service elevator door and caused the K3 elevator to stop while it was traveling from the 4th floor. Executive Elevator Co was called. Employee got out at 1:30 AM. Signage inside the elevator reads "Do not attempt to stop elevator by opening car gates. Wait until elevator comes to a complete stop before opening car gates".

Employee was trying to clear a tire jam on the DTV machine. As he pushed the tire through the bottom gate, an air cannon went off blowing material up the chute and into his face. He sustained foreign bodies in both eyes requiring prescription eye drops. He was wearing safety glasses.

Employee was moving a forklift fork with his right foot. The fork moved suddenly and employee strained his right knee. He has been on full work status with no restrictions since the accident. On 5-23-11, employee decided to have surgery on his right knee at which time this became reportable.

Temporary Contractor Laborer was operating the TIM System. His left thumb was smashed between a tire and a handrail while handling the tires. His left thumb required stitches.

A Contract temporary laborer was observing a driver off-load a truck of Ammonium Hydroxide 19%. As he and the truck driver were talking, the ee collapsed. CPR was administered at the scene. Ambulance took the ee to the hospital where he was pronounced dead. Cause of death is unknown at this time.

Contractor laborer clocked out for the day and was on his way home. He was traveling on the Black Mountain Quarry Road, lost control of his vehicle and it rolled. He immediately opened the door and got out. This is a non-work related incident, but is being reported since this road in on mine property. The employee was not scheduled for any future work shifts.

Employee has a pre-existing heart condition. He was having lunch, felt chest discomfort for approx two minutes and then he was fine. He elected to have the ambulance take him to the hospital because of his medical history. Once checked out at the hospital, there were no significant findings. He was released for work.

Employee has been treating with his personal physician since 8-30-10 for bilateral carpal syndrome. The doctor instructed him to report this as a work related case. The employee was sent to the company physician on 11-02-10. It has not been determined if this is work related. The employee was placed on restricted work duty.

Four contractors with Safway Scaffold were stuck in the K3 elevator. Executive Elevator was called to get them out.

Two employees were stuck in the K2 elevator between the second and third floors.

Employee was cleaning the back of his welding truck (parked by the Maintenance Shop)and as he turned he bumped his right elbow on a vice.

Coal was smoldering and was spotted at approximately 12:35 AM, steps were taken to extinguish the smoldering coal.

Employee was adjusting a guard, removed a small rock when his co-worker moved the guard causing employee's finger to get pinched. He fractured his finger, but did not require medical treatment. He returned to full work status. June 30, 2010, the doctor put a pin in the finger to help it heal quicker, at which time this became reportable. Employee is on restricted work status.

Employee alleges he was experiencing pain and numbness is both hands from using a water lance to clean the tower. He failed to report this to his Supervisor, went to his own doctor on 5-7-10. He brought a note from his doctor which stated the employee has carpal tunnel: work related. This became reportable on 5-13-10.

Cleaning chuck from K3 shelf with water blaster. Chuck fell hitting barrel causing the gun to twist. His hand was caught in the handle causing his hand, arm & shoulder to twist backwards. He strained his right shoulder.

Employee alleged he was injured on 4/28/10 but did not report until 4/29/10. He alleges he strained his back while using a vactor hose. He did not report this to his supervisor.

Employee was reaching up to grab an oversized tire out of a chute and he twisted his lower back.

Fire was reported to the control room at 7:02 AM. Employee access to the area was barricaded off. Flames were coming out of the inspection doors, second mezzanine level of the riser. Raw meal feed rate to the kiln was reduced, biosolids machine was started to reduce the flow of natural gas to the preheater. The fire was contained within the riser by 7:48 AM.

Employee was welding and he lifted his welding shield too soon and got flash burn to both eyes requirng a prescription.

Employee was carrying two buckets of material up a ramp, when he heard his left knee pop. He felt pain behind his left knee.

EE was shoveling and got clinker dust in his eyes. He was given first aid and was alright. That evening he went to the emergency room without notifying company representatives. Because of the patient privacy act, we were not able to obtain medical information until today 2/5/2010, at which time this became reportable.

Employee alleges he had a work related injury to his left elbow 12 years ago and he declined surgery at that time. He reported pain in his elbow and received first aid diagnostic treatment on 12-31-09. On 2/8/2010 he was placed on restrictions that prevented him from doing his regular job at which time this became reportable.

Employee was attempting to light a torch and it flashed back and he sustained burns to his face, ear, neck, part of his arms and part of his hands.

Employee was pulling bags from one of the baghouses and felt soreness in his left wrist and left lower arm.

Employee was working at the bottom of a cone crusher when a rock rolled down rendering him unconscious. Employee was air evacuated to a trauma center with head trauma/concussion.

A 23 yr old was driving on the road toward the entrance. Was driving under the influence of alcohol. He sustained head injuries, multiple body contusions and abrasions. Was air lifted to medical facility. Prior to being transported was placed under arrest, CHP Report# 985017113. Was not an ee had no business at the facility. He thought he was driving in a different city.

As employee was replacing a 2 inch water line to the GPH3TAL, another water line ruptured with hot water and steam and hit employee in the face. Employee had his safety glasses on but sustained a corneal abrasion to the left eye. A piece of metal was removed from his left eye.

Employee alleges he aggravated a pre-existing non-work related injury to his right shoulder on 7-17-09 while setting a return roller into a conveyor frame. Employee felt okay until 7-24-09 at which time he requested medical treatment. Employee was placed on restricted work duty for a strain to his right shoulder.

Kiln GK3 pushed due to lack of fuel to calciner. Drive belt for calciner coal feeder broke at 4:15 AM. Cntrl Rm Opr received an alarm for coal feeder speed 40 sec after the belt broke. Lack of fuel caused kiln to push (rush hot feed thru kiln) and hot material went to the clinker conveyor system. Diverter gate was opened too late to avoid burning belts.

Employee was picking up a 1 1/2 round stock and felt a pain in his lower back.

INJURY ALLEGES HE SUSTAINED AN INJURY TO RIGHT SHOULDER DUE TO POSSIBLE WORK RELATED ACTIVITY. HE HAD BEEN UNDER THE CARE OF A PERSONAL PHYS. FOR MONTHS TREATING FOR SHOULDER AND BACK INJURY. HE HAS BEEN PLACED ON LIGHT DUTY WORK. THE COMPANY IS INVESTIGATING THIS ALLEGATION BUT WILL REPORT AS PER REGULATORY REQUIRMENT TO AVOID PENALTY.

EE a Mine Engineer, was surveying the pit using a GPS system. He walked out 6' from the edge of the bench when the ground sloughed out from under him.

Grinding commutator on 700HP DC Motor. Glove got caught between commutator and brush holder. Pulled left thumb into gap between commutator and brush holder resulting in laceration/abbrasion to thumb

A fatal accident occurred on mine property. Three off-road motorcycle drivers trespassed onto private property. One of the drivers lost control of his motorcycle and went over an embankment. The motorcycle driver was pronounced dead at the scene. The motorcycle drivers were exceeding the posted speed limit.

A fire was reported at 9:02 a.m. on the coal rejects pile. The loader operator confirmed that the coal was smoldering on the NE corner of the pile. The loader operator dug out the smoldering coal, spread it on a flat area, sprayed water on it and compacted the material with the loader. Fire was out by 11:30 a.m.

At 4:27 p.m. the Control Room received a report that smoke was coming out of the "coal rejects" pile. The production supervisor on shift called the loader operator and the water truck to the site to spray water over the affected pile, to dig out the smoldering coal and to compact this coal. The incident was reported to MSHA at 4:55 p.m.

Fire was reported at 7:19 a.m. at the "coal rejects" pile. A loader and a water truck were sent to the site to spray the smoldering coal with water, dig it out and back drag it. Coal exposed to air for extended time periods may spontaneously ignite. Fire was reported to be out by 2:48 p.m., after removing the smoldering coal from the pile and have it compacted.

At 11:10 am on 12/23/08 smoke in coal pile was reported. Investigation discovered smoke coming out of the top of the coke pile. They set up the coal/coke reclaimer over the smoldering material and scraped it off. The operation was successful and the fire was out by 12:00 p.m. Since it took longer than 30 minutes, the CR Operator reported incident to MSHA at 11:40 am.

Fire in the coal pile discovered at 7:37 a.m. This incident was reported to MSHA at 8:00 a.m. using the 800 telephone number and it was completely put out by 8:18 a.m. No person was injured and no equipment damaged was reported.

Smoke discovered at 6:28 a.m. and reported to the control room operator. A loader operator and the water truck were called to the site. They dug out the portion of the coal that was smoldering, sprayed water on it and back dragged. This took 30 minutes. It was reported to MSHA at 6:58 a.m. due to the possibility of reappearance.

Smoke discovered in coal pile & reported to control room operator. Called two haul trucks to remove coal from area and create a separate pile with this coal. Loads were watered and back dragged in order to put out the fire (smoke). This took close to 7 hours. Control room operator contacted MSHA at 7:20 a.m. to inform about the incident. No signs of fire after 10 am on 10/21/08.

Smoke discovered in coal pile at 6:58 am and reported to control room. A utility leadman, utility man and water truck went to coal pile. Coal was dug out, watered and back dragged by loader. Supervisor didn't believe fire was going to be out within 30 minutes, so the control room operator called MSHA at 7:45 am to inform about the incident. Fire was extinguished by 9:50 am.

Employee driving truck somehow dust got into the truck and got under the safety glasses.

Was using water blaster cleaning cement riser. Chunk fell from side of riser hitting the tip of the water blaster nozzle.

Cleaning K2 tower 3rd floor, Northwest corner, top door. Cleaning with 4' water lance shot into and hot dust puffed back hitting employee's right side of jacket. Heated jacket and caused burn to arm.

Positioning spools of size #1 wire on pallet. Strained right hand & right shoulder. Lifting spools in postion for a forklift.

While changing out one of the coal pods, the tube sheet was still wet from the ice that we cleaned out and I was on my knees pulling out collector bags and it soaked through my uniform on my right knee.

EE was walking up steps near K2 cooler when his foot slipped and he hit his face on the step, cutting his lip. The area was free of debris, water, or anything else that may have contributed.

Employee was climbing into haul truck at beginning of shift and strained left knee.

During preheat of Kiln 3 the burner flame extinguished. This went undiscovered by the CRO while the burner continued to send natural gas into kiln. After 3-4 minutes the gas found and ignition source and ignited causeing an explosion causing extensive damage to the heat exchanger and cooler.

EE strained his right calf while he was climbing in the rock breaker.

Employee was operating the Dingo to move clinker and pinched his leg between the Dingo and a concrete pillar.

After using a pry bar to open 4th stage tipping gate on K2, ee dropped bar and hit himself on left knee.

Supervisor was observing employees align pan conveyor. Of necessity, a guard was off. Supervisor placed hand on pan track to support himself as he leaned over to observe mechanic. Wheels from pan conveyor rolled over 2 fingers, crushing them.

Employee was lubricating gear box wearing safety glasses, turned his head and dust blew into face.

Primary crusher was overloaded and seized while 350 h.p. drive motor continued to run causing the drive belt to catch on fire. MSHA was called at 7:20 pm; fire was out by 7:25 p.m.

Smoke was discovered on the back side of the coal pile at 7:50 a.m. and reported to control room operator who sent a utility person to dig out the smoldering coal. The utility operator, using the loader to dig out and back drag the coal, extinguished the fire at 8:50 a.m.

Employee was cleaning K2 tower with spud bar. While knocking chunk loose employee smashed hand between bar and air cannon, breaking bone in #2 finger on right hand.

Smoke was discovered on the back side of the coal pile at 6:24 a.m. and reported to control room operator who sent a utility person to dig out the smoldering coal. The utility operator, using the loader to dig out and back drag the coal, extinguished the fire at 10:00 a.m.

A small area of smouldering coal was discovered on the back side of the primary coal pile. The smouldering coal was dug out and extinguished by 9:23 a.m. the same day.

Employee was climbing on a 988 loader when he slipped due to wet boots. Employee caught himself, preventing a fall but strained his back.

Employee was cleaning feed shelf on K2 tower when he felt a pinch in his lower back.

Employee was cleaning the K2 preheat tower with a spud bar. Material struck bar causing bar to hit the employee's face causing 2 small fractures.

The victim (contractor) and 2 Cemex employees were trying to clear a plug from the K2 preheat tower. As the plug broke free hot material discharged out the Cardox port and impinged on the victim causing burns.

Employee was standing on steel platform designed to catch slag. Platform broke loose from weld under weight of EE and other employee. Employee hit back on on roll press housing leading to bruises on back.

While setting the K2 burner pipe in position, employee used hand to align the adjustment pin. The burner pipe shifted and caught his middle and index finger of left hand.

Employee was cutting bolts overhead with a torch. Slag fell on to the employee. The employee stopped cutting and removed his goggles to clean the built up slag off of them and a piece of slag fell in to his right eye. Employee required prescription eye drops.

Employee was stepping off a forklift and lost footing on wet ground. The employee refused medical treatment on 2/3/07. Employee was transferred to another job site when on 2/6/07 he requested medical treatment. Employee was diagnosed with a strain of the left knee and was placed on restricted duty.

Employee was stepping off the pinion stand to connect the ground when he twisted his left ankle. He was placed on restricted work.

Employee was walking up stairs carrying a shovel and twisted his back while turning a corner.
