EE placing tool bag in back of truck strained upper left back muscle.
Elevator got stuck with EE inside at 5:41. Service contractor notified. MSHA called at 6:21 ref. 04978221. Service contractor arrived at 7:05. Stuck EE out at 7:33. MSHA notified. Service contractor cleaned dirty ground floor switch.
EE was lifting a mill bearing cap and the chain hoist hook stretched causing the cap to fall and graze EE's hard had and right cheek bone.
EE was lifting vactor hose and felt back pain.
EE was inspector bucket elevator and slipped on mill ball on ground.
EE was replacing drive belts and caught left ring finger, resulting in the loss of nail.
EE felt right knee pain while ascending stairs on the shuttle wagon.
EE was attempting to clear a dust collector vent line and in the process of doing so they got a fragment of material in eye. Although they were wearing safety glasses the material got into left eye. The material was removed at the clinic and the EE was given an eye patch and placed on restricted duty for 2 days.
During manual concrete sounding inspection of a storage bam wall, the inspector impacted the left thumb with a 20 oz hammer, causing a fracture of the digit.
Contractor was taking the elevator down to the ground with an oversized ladder and it shifted in the elevator causing the elevator car to rock which locked the governor. This is a safety feature of the elevator car.
Worker was placing hand brake on rail car. Worker felt a strain in shoulder while doing this but did not think any medical attention was needed. Worker did not seek medical attention until 07/17/23, Dr modified worker's duty on this date of no overhead work.
Elevator went into earthquake mode due to excessive movement in elevator. One employee in the elevator stated They tripped over their broom causing the excessive movement which tripped a safety feature on the elevator engaging elevator brakes.
Temp Laborer was cleaning material off Group 3 scale and while attempting to step off the scale EE rolled EE's ankle. The step off the scale is approximately 8". The temp. laborer was taken for medical eval. and given restrictions due to sprained ankle.
EE was removing plywood form material with a pry bar. When pulling down to separate the materials, the bar slipped pinching the tip of the EE's right ring causing a laceration.
An EE was standing on a step ladder using a hammer to unchoke an air slide at the Plant. For unknown reasons they slumped over and slid down the ladder landing on their left side. EE was non responsive and workers provided first aid including use of an AED and CPR until EMS arrived and took over. EE was transported to the hospital, did not recover consciousness before they died.
EE was attempting to open a clinker rail car hopper door to dump the railcar using a prybar. The EE put their leg under the hopper door and when the door opened the clinker flushed onto EE's leg causing a burn to their right leg. The EE was taken for medical evaluation and placed on modified duty. The burn was cleaned and bandaged.
EE was walking down steps to access mens shower room when EE lost balance. EE landed flat footed on right foot and felt a sharp pain. EE had fractured the same foot 4 months prior. X-ray on 9-30-22 showed foot had been fractured once again in same area. EE was given restrictions and walking boot.
Train mechanic was using a sledge hammer on locomotive when they hit themselves in the lip with the hammer, causing a laceration requiring one stitch to close. Employee was given modified duty.
Employee had just arrived and clocked in for work when EE went to get into EE's service truck. As EE was attempting to open the door with exterior door handle EE stated EE felt a pain in EE's middle finger and finger and hand immediately swelled. EE was diagnosed with a sprained finger from this action.
An employee was attempting to clear a plugged air slide. When EE opened the inspection cover while it was running, material shot out of the air slide causing material to get in EE's eyes. EE was taken for medical evaluation and placed on modified duty.
EE was performing routine maintenance of a bucket elevator when a speck of sand blasting material that was used on the separator got in eye. The EE flushed the eye and felt confident the material had come out but later that evening felt discomfort in the eye. Taken for exam and object was removed by optometrist. EE given antibiotics to fight possible infection.
While unloading gypsum employee slipped on loose material while walking over uneven ground to open and close butterfly valves on trailers due to material sticking in unloading hole.
EE was clearing debris using a hammer and chisel. EE felt something hit leg and felt a small bit of blood on EE's leg. EE reported it to us on 12/16/19, EE was taken to the Dr. who ordered x-rays. The x-ray was read on 12/19/19 and it was found that there was a .3cm fragment. They attempted to remove it and did not find it. This caused EE to receive a single stitch on 12/19/19.
There was an abrupt motion inside the car caused by one of the miners while traveling down that activated motion switch that caused the elevator to stop therefore causing the miners to be trapped. Elevator service co. responded and miners were released within 58 min. The elev. was inspected and returned to service.

On 5/31/2017 Employee reported an injury that happened while loading mill balls. Employee got hit on the lip with a piece of metal. EE was released back to work full duty same day. On 6/17/2017 employee felt something in EE's lip. We took EE to the clinic. The doctor sent EE for a new x-ray that showed something in EE's lip. On 6/22/2017 they cut it out and put in 1 stitch.
Employee was using a utility knife to cut a slit in the skirt rubber, the knife slipped cutting the employee's left index finger. Employee was not wearing gloves at the time of this incident.
Employee felt dizzy and confused after climbing up and down stairway to KE16. Employee was taken to St. Mary's hospital where ee was given an IV
Employee was trouble shooting the air system on the vactor truck when they reached into a pinch point area and a butterfly valve shut on their left index finger.
Employee stated he was called back to work after his shift and he climbed 15 flights of stairs to work on an elevator and he felt pain and popping in his left knee. Employee did not report this incident until 8/14/14.
His left foot was on the stair and right foot was stepping down when his left heel was caught on bottom edge of stair. When his foot landed he twisted it.
Was removing the mist off his safety glasses and a small piece of metal struck his cheek.
EE stood up and struck his hard hat with a conduit. Causing neck pain.
Injured employee was talking to another employee and did not see where he was walking. Stepped into a pot hole and twisted his ankle.
Employee stepped off the vactor truck and did not see the large rock. He stepped on the rock and twisted his ankle.
Employee was utilizing a 36" aluminum pipe wrench o remove a Train Line Hose from the end of a railcar. The connection on the Train Line Hose broke loose causing the employee to twist and strain his back, due to improper posture/positioning while exerting force.
EE was cleaning a bearing cover in the solvent tank, it slipped and he pinched his right middle finger between the cover and the edge of the tank. Superficial laceration requiring a steri-strip. Medical treatment was not documented on the medical work status report. The clinic was called today for clarification of medical treatment (12-28-12), and this is reportable per definition.
Employee was using a chain hoist to raise counterweight on JBC2 conveyor when he felt something pull in his lower right back.
Employee was changing a roller on a conveyor belt. He was removing a clip that retains the cover, when some clinker dust was dislodged and blown into the employee's left eye by the wind. The employee was wearing safety glasses at the time of incident.
Employee's hearing was tested on 12/14/2011 which revealed a mild hearing loss. He was scheduled for a re-test but became ill and could not be re-tested until he recovered. Hearing loss from re-test showed a 25 decibel shift in the right ear at the 3000 Hz level. This became reportable at the time the company received the final re-test report on 2/2/2012.
Employee had a mild stroke and was transported via ambulance to the hospital. He did not have any accident or property or any type of incident on the mine property. This was determined to be non-work related.
Employee was diagnosed in 1990 with bilateral carpal tunnel syndrome. She only chose to have surgery on one hand. On 11-11-11, she requested to have the surgery on her left hand. She had surgery on 4-26-12 at which time this became reportable by definition. She returned to work on her next scheduled workday 5-01-12 on full regular work duty.
Employee was climbing up a ladder onto a clinker rail car when he went to sit down on the rail car catwalk and twisted his right knee.
Employee was inspecting the v-belts for the MBH9F fan. He was manually rolling the sheaves and the v-belts and his fingers got caught in the pinch point between the sheave and the v-belt. This equipment was locked out.
Driver was climbing out of his truck backwards and stepped down with his left foot. He twisted his left ankle and he felt a sharp pain. Driver fractured his left foot and ankle in 2 places.
Employee was diagnosed with carpal tunnel syndrome in 1994 but did not elect to have surgery. He filed a new claim on 11-12-10. He has been doing full work duty without any restrictions unti l the date of his surgery on 5-24-11, at which time this case became reportable.
Employee alleges he aggravated a pre-existing hernia while using a 16 lb. sledge hammer to loosen bolts from the #11 Mill Bull Gear. Employee alleges he felt pain and a tearing sensation to h is right side.
Employee was on the stairway between silo 14 and silo 15. He turned around on the steps to walk down, missed a step and overextended his left knee.
Employee was carrying a light bucket of tools upstairs with his left hand so he could hold the handrail with his right hand. The bucket weighed approximately 20 lbs. Employee strained his left knee walking up the stairs.
Employee was cutting a piece of hose to use as a patch for another hose on #12 Mill. His razor knife quickly went through the hose and lacerated his right thigh, which required seven sutures. He returned to full work status the same day.
Employee walked into an airslide, hit his head which caused him to jam his neck. He has been under medical observation only, but on 6-11-2010 the doctor felt he needed physical therapy, which makes this accident reportable on this date 6-11-2010.
While closing one of the double doors on the compressor room when a steel bar came down and struck him on the left hand ring finger. Hand was on the door knob when the bar came into contact with his finger.
Employee was taken off of work for a recurring occupational illness "carpal tunnel syndrome". Please see attached cover letter.
Employee alleges he strained his right groin area as he slipped while sweeping clinker and did a half spllit. He failed to report this accident until 12-15-09 at 8:50 AM. He was sent to the Industrial Clinic and was placed on restricted work duty. This alleged incident is being investigated as to whether or not it is work related or an incident that may have happened at home.
Employee was using a pry bar to hit an air activated rail car door that did not open. He was crouched down so he could pry it open, but there wasn't anywhere he could pry it. As he was crouched with the pry bar in his hand, the door automatically opened, striking the pry bar which hit the employee on the forehead.
Employee did not have an accident, but came in the following morning and said his leg began hurting at night. He is alleging that his left leg was strained from doing his regular job tasks.
Employee was injured on 6-26-09, but did not report this injury to his Supervisor until 6-29-09. Employee was turning the bottom cap of an airslide in the basement of Group 4 Silos, 26, 28, 3 0 (airslide to ME5 elevator). As he was turning the cap, it stopped while he was still applying pressure, and his thumb bent backwards.
Employee was operating Mule with leg outside of cab and hit rock with ankle.
EE was packing on B-line and #4 spout bag blewup, causing cement to go into left eye.
Employee was lifting 'frogs' from loader to rail unloading station when he strained his right shoulder.
Employee working on inverted cone. Employee slipped/tripped causing a minor injury to his right hand. Minor fracture to thumb/finger.
Employee was hooking up water blaster gun next to riser on preheat tower when an air cannon discharged blowing air and material through an open port causing a mild burn on the face and getting material in the eye which had to be removed at the medical clinic.
Employee was walking down stairs in K3 tower and pulled back muscle
Employee was cleaning vent lines on the roll press structure using a 5 lb sledge hammer. He missed the line and over-extended his elbow.
Employee was pulling large section of vactor hose from HPC 4 pit area and strained his back.
Employee was preparing to move stacker from the west pile when he fell about 8 feet from the lower platform.
Employee was lifting a wire feed welder with another employee when he felt a groin strain
EE was removing a guard from JBC1 belt when EE slipped on a loose clinker. He fell backwards with guard hitting him in the mouth and then fell against the wall and handrail causing a fracture to right hand.
EE was packing cement when a bag burst when cement dust went into EE eyes after flushing his eyes a partical was still in his eyes. Perscription eye drops were given and employee returned to work.
EE was replacing a start motor on a apt733 when he lifted the motor with his arms and felt a strain in his abdomen. EE did not seek medical attention until 08/31/06.
EE was working in the coal reclaim area when he started feeling ill. He felt light headed and couldn't see.
EE was emptying a Trax Car when the train bar slipped from his hand hitting the top of his left foot.
EE was cleaning out drainage system. When disposing of the debris he cut his right ring finger on a pipe.
EE was assisting lowering of the boom on the American Crane when a sudden movement startled him causing him to step back. As he stepped back he tripped on a permanently mounted water jug falling to the ground below approx. 4 1/2 feet.
EE was working 10FM separator when fugitive cement dust got into his eyes. This caused a corneal abrasion requiring prescription eye drops.
EE was cleaning walkways near/around JBC8. While using a jackhammer EE smashed his hand in between Jackhammer and metal shoot. Causing a dislocation of pinky.
EE was cleaning K3 Feed shelve when hot material fell between gloves and jacket causing second degree burns to left hand
EE was spiking rails when a piece of metal hit him in the throat. Surgery was needed to remove the object.
EE was cleaning K3 riser on the north side of 2nd mezzanine. EE was using a jackhammer when the bite got stuck in a chunk of cement build up. EE used a spud bar to loosen the bit when the spud bar slipped off the jackhammer and hitting the employee on the lower leg.
EE was trying to fix a stuck pallet in the pallatizer when he got his arm stuck between the rollers and the pallet came loose and arm was caught. The result was crushing of his forearm and wrist resulting in damage to muscles and possible ligaments.
EE was assisting the Vactor crew with a clog in the hose by lowing it over the guarding when the second employee lost grip of the hose causing it to drop down jerking the EE forward jerking his shoulder and head which caused strain to his back. EE did not seek medical attention until 05/22/06.
EE was doing maintenance on air cannons on K3 Tower. EE was using a faceshield however when he looked up he got something in his left eye and had to get prescription eye drops.
EE called in on 5-04-06 said he hurt, complained of whole body sore. Was informed could not return to work - neeeded doctors release. Did not mention an on job injury. 05-05-06 EE reported at 3:30 PM a fractured toe from rebar received on 5-03-06. No doctors report.
EE was cleaning north side of tower with an airlance when hot feed blew onto his jacket on right side burning a hole through to his right forearm and wrist causing 2nd degree burns.
EE was on BB1 conveyor belt looking at blockage when a second EE turned on the belt causing first EE's body to be projected towards the chute. His left leg went under the belt when left leg wa s caught by the head pulley severing the left leg, freeing the employee.
Gas Fire on North side of Kiln 3 preheater tower. Gas was disabled immediately. The fire caused damage to cannons, lighting and air hoses. The connection fitting from the riser to the hard pipe failed, causing natural gas and oxygen to come into contact with ignition source and ignite. No injuries took place and MSHA was notified locally.
EE has standing near a second EE who was shaking off a dirty rag when something flew into the first EE's eye. Requiring perscription eye drops.
EE was using a grinder on a flange on Silo 14 Blade. When the blade was pinched causing the grinder to come out of his grip it swung around and cut the top of the EE's right hand.
Employee was doing clean up at the plant when he bent over to pick up a wire from the ground. After pulling on the wire he felt a pop in his lower back.
EE was on 416 locomotive when backing up on the mainline couple with the clinker train. The 416 coupled hard causing a jerk of the train, which caused the employee to be jerked hard. EE did feel any injury at the time however days later experienced pain in his lower back.
EE WAS REMOVING BOLTS FROM THE CUTTING EDGE OF THE 988G LOADER BUCKET IN DOING SO A SHARP EDGE ON ONE OF THE BOLTS CUT THROUGH THE EMPLOYEE?S GLOVE AND INTO HIS LEFT INDEX FINGER. THIS REQUIRED THAT THE EMPLOYEE HAVE SIX STICHES PUT IN HIS INDEX FINGER .
EE was closing the door on the southwest side of the third mezz K3 Preheater as he was stepping down his right foot came in contact with a squar tubing up against the toe plate and EE knee twisted.
EE was preparing for an up coming job while lifting elevator buckets on to a chain he strained his shoulders. EE was swing the buckets onto the chain.
EE was doing his vehicle inspection on the water truck when he lifted the hood and felt a sharp pain in his middle to lower back. After finishing the inspection he closed the hood feeling a sharp pain in his lower back. He could not move.
EE was using the jackhammer while working under JBH3F as he repostioned the jackhammer he made a quick turn and hit his tooth on some conduit that was hanging down from the baghouse. This chipped his right front tooth.
EE was cutting blasting cord with a knife when he cut through and cut hid left forearm needing stitches.
Emp picked up 90# jackhammer. It slipped out of his hands. Shovel bit end landed on his Left foot smashing and breaking his big toe. Employee was wearing new boots without steel safety toes.
EE was cleaning built up material under CBC2 using a spud bar when he hit smashed his left hand lacerating his left index finger which needed stitches.
EE was cleaning Kiln #3 feed shelf with a waterblaster when from the port of tower a puff of hot kiln dust came out. The dust went under face shield and burned his chin and lips.
Employee working in silo cleaning rock fell and hit his knee.
EE was working inside silo 18 cone in a sitting position when he got up he felt a sharp pain in his back.
While EE packing cement on B-line a bag broke and cement dust got into eyes causing irration and a scratch on cornea of right eye.
EE was packing bags of cement on A-line when the top of the bag of cement broke shooting dust into the air and EE got dust into his eyes. EE washed eye out however left eye was irritated. EE received a prescription eye drops.
EE had prior injury to knee and is claiming that it did not heal states that it has been bothering him and wanted to place new claim.
EE calims to have been exiting Loader 370 (992G) on back steps. EE claims to have fell from steps hitting left side of arm and back on steps.
Employee was loosening bolts under truck when material fell in left eye
EE was lighting an acetylene torch when troch tip was partially plugged with slag causing torch to flash back and burnt employee hand and arm.
EE was tending the K3 elevator when he tried to use his weight to force a pallet into the elevator. EE didnot feel pain right away notified supervisor 2 day later that he thought he may have hernia.
EE was working at BBC1 (crusher) with rubber boots when some clinker dust got into the boots. After working and sweating in the boots, EE had a rash.
EE was removing a section of airslide on EAC19. A gust of wind blew dust into his left eye, causing irritation.
At 7:50 a.m. burning coal from the coal stockpile got on to the conveyor and into the coal mill. The coal ignited the coal dust causing the internal system to catch fire. The CO2 internal fire supression sys failed to extinguish. fire isolated and discharged to ground. extinguished at 9:40 a.m.
Mine fire discovered in coal stockpile. Smoldering in several locations. Have attempted to seperate burning coal from fire. Now attempting to drench with water.
EE was closing elevator door on K2 (kiln) elevator. While door was closing EE attempted to push strap to the outside of door and door slammmed on left hand.
EE has developed dermititis from the cement contact with skin.
EE was using a vise and drill press failed to clamp the vise to table. EE had his hand between the vise drill press post when drill hole the bit caught causing the vise to spin which cause his hand to be pined between the two tools.
EE was putting pin un self dumping hooper. While placing pin in the hopper tipped forward EE was holding on with right hand the weight caused the EE to be come forward causing him right shoulder pain.
Employee was dismounting from AHU395, lost his footing on the ladder and felt a pop in his right shoulder. Next day shoulder felt worse he and wanted to seek medical attention.
EE was closing elevator door on GPH2 7th floor when his finger got caught and cut right index finger.
EE was attempting to shoot build up in clinker cooler on K3 when from the port his was doing the task a puff of hot dustt came out. EE got hot dust into his right glove which cause a burn.
EE was cleaning vent lines on top of the rollpress with a 4 lb sledge hammer. When EE hit the vent line and missed causing him to over extend his arms straining both arms.
employee and partner were removing a bearing from the G cooler. The parter was attempting to remove the bearing with a 5' pry bar. the employee asked the partner to wait and then grabbed the bearing. The partner, not hearing the employee pulled on the pry bar one more time injuring the employee.
EE was lifting door on ES3 and strained back. There was difficulty opening the door due to draft suction.
EE was airarcing inside of GCC2 and got a foreign object in left eye.
during high winds dust blew off of clinker stacker belt and into employee's eye. He then rubbed his eye aggrivating the eye. employee was wearing safety glasses.
While EE was conducting routine maintenance on coal milling area when the wind blew up dust which entered his right eye.
employee was retrieving a vacuum system from LBH2 baghouse. As he climbed the ladder to retrieve the equipment he felt pain in his right side. As he reached the bottom with the equipment the pain became extreme.
EE was using Water Blaster at Reduction Zone. While moving from the 3rd level to the 4th level EE claims as he was pushing the Water Blaster up to the 4th level. This caused the gun to fire water into EE right hand causing injury.
When installing a bucket on the upper chain, the bucket fell forward as they were lining up holes. When the bucket fell foward the employee finger got smashed between the bucket and chain.
While trowing away trash in the trash hopper the employee tripped over a pallett on ground fell on left knee and left hand.
Employee was jackhammering for bulk of shift at Scale 3. THe following morning employee reported that she had upper back pain and numbness and tingling in both arms and hands.
Ee was fabricating conveyor belt skirting and was punching holes to accomodate skirt boards. ee swung hammer, missed the punch and struck his left hand.
Internal audit found need to report. EE was was bending over equipment while fixing it. While pulling out an air hose he felt a sharp pain in his back.
Employee was moving boxes when wrist made a popping noise. Sprained right wrist.

Employee was closing water valve on K3 water tank. The valve became stuck and the employee pulled hard to loosen valve, straining his back and shoulders.
The employee was attaching a bucket to a skid steer loader. When the latches wouldn't close the employee stomped on them with his foot to pound the closed and strained his right foot.
Employee was trying to open the doors on rail car #37. The door was difficult to open and employee gave extra push and popped his right shoulder.
EMPLOYEE WAS CLEANING HOT DUST WITH BOBCAT WHEN BOBCAT STALLED. EMPLOYEE EXITED EQUIPMENT AND STEPPED INTO HOT KILN DUST AND RECEIVED SECOND DEGREE BURNS OF THE FOOT.
Ee attempted to remove full oil drum from discharge end of mill #11. The edge of the drum was placed on the lip of the bobcat and when ee tried to lift the drum more it fell over and landed on his left foot.

EMPLOYEE WAS ATTEMPTING TO UNPLUG PREHEATER TOWER USING A CARDOX SYSTEM. SYSTEM DISCHARGED PREMATURELY BURNING EE ON THE CHEST, NECK, AND ARMS. HE SUSTAINED 1ST DEGREE BURNS TO THE MAJORITY OF THE AFFECTED AREA AND 6% WAS 2ND DEGREE BURNS.

EE MUST WEAR KNEE BRACE AND WILL BE ON RESTRICTED DUTY UNTIL COMPLETION OF PHYSICAL THERAPY.

HIS EYES WERE FLUSHED AND HE WAS GIVEN RESTRICTED DUTY. HE CALLED IN SICK.
EE was re-lamping Group 3 Silos. EE reported that he was sore from rolling stepladder from silo to silo.
TWO EMPLOYEES WERE MOVING HOSE ON VACTOR TRUCK. FIRST EMPLOYEE LOST GRIP ON HOSE. THE WEIGHT OF THE HOSE FELL ON THE SECOND EMPLOYEE STRIKING HIS SHOULDER.
EMPLOYEE WAS PLACING SPIKES INTO A RAIL ROAD TRACK. WHILE HITTING A SPIKE, THE METAL TO METAL CONTACT CAUSED A PIECE OF METAL TO SHOOT OFF AND STRUCK EE'S FOREARM (RIGHT) CAUSING LACERATION AND LODGING INTO THE FOREARM.
EE was replacing DNF12 feeder belt while puting belt on converyor rollers by hand he twisted his body causing strain tp back.
