Mostrando los 28Traveling down haulroad in loaded truck, driver thought he was in the wrong gear and got truck out of gear. Driver applied brakes. Truck wouldn't slow down enough to get it in gear. Truck made left hand turn. The truck tipped to the right, the driver over compensated and the truck turned over on left side.
While dumping load from truck bed, the bed fell off when raised to full extension of the first jack.
EE stated, while his truck was being loaded, a rock rolled & struck the inside of the bed of his truck & caused a severe jolt to his neck & then when he went to dump the load, felt pain in neck. When he returned to the loader, he contacted his foreman & requested an injury report be filled out; he completed his shift & drove himself home.
Employee stated he was traveling uphill when his truck powered out, causing him to drift backwards. He cut toward the bank which caused the trailer to flip onto its side and the tractor vertical.
Employee was a passenger in a pickup truck being driven by a fellow employee. They were returning from the loadout to the plant after loading a train. The pickup struck a berm and rolled on its right side. Employee received lacerations of the right arm from the accident.
Employee attempting to shift gears going up grade with truck. Got it out of gear. The truck rolled backwards into a ditch and turned on its side. Employee got out, noticed a fire starting in engine compartment. Used fire exinguisher but did not get fire undercontrol.
Employee was drivng truck down mine road. Lost control and jumped from truck. Vehicle rolled over onto side. Employee did not see Dr. until 5-28-07. 3 days restricted duty. Returned to full duty on 6-1-07.
A rock slid & struck the inside of the bed. EE had on his seat belt & the belt pulled on the stitches from non-work related hernia repair some 7 weeks earlier. He was taken to hospital for evaluation. Dr saw no significant damage to repair but suggested follow-up with initial Dr. Only time lost was due to wait to see Dr. The back complaint is new. Citation will be contested.
Employee stated that roads were ice and snow covered. He stated that as he was hauling from pit to cleaning plant. He approached a grader as he was moving through a curve. He stated that as he de-accelerating, the truck started sliding and hit the grader.
EE was driving back to pit to get another load and ran into the ditch.
Employee stated that while driving up a hill, the truck stalled. The truck then drifted back into a ditch, turning over on its side.
Employee was driving truck up hill. Truck started rolling backward and turned over onto it's side. Employee treated at ER - prescribed light duty.
Employee stated that he fell asleep, ran truck in ditch; truck tipped over on side.
EE stated that truck was climbing the hill. The truck stalled and then the truck rolled backward into the berm and turned over on its side.
EMPLOYEE ALLEGES AN INJURY AS A RESULT OF A MOTOR VEHICLE ACCIDENT, AFTER THE INCIDENT EE SHOWED NO EMOTIONAL OR PHYSICAL SIGNS OF PAIN OR DISCOMFORT; NOR DID HE REPORT THE INJURY. HE CONTINUED TO WORK FOR (4)HOURS. EE REPORTED AN OFF-THE-JOB INJURY OCCURRING ON OR NEAR 10/25/05, TO HIS BACK & SHOULDER WHEN HE FELL FROM A ROOF. EE WAS ABSENT (4)DAYS DUE TO THIS INJURY.
EE ALLEGES AN INJURY TO HIS BACK WHILE DRIVING A COAL TRUCK. HE STATED TO HIS SUPERVISOR THAT HE HURT HIS BACK WHEN HE HIT A HOLE IN ROADWAY. EXAMINATON FOUND NO HOLE IN THE ROADWAY. EE TOLD OFFICE PERSONNEL THAT HE WAS INJURED IN A MOTORCYCLE ACCIDENT AND THAT THE BOUNCING OF THE TRUCK HURT HIM BECAUSE HE WAS SO SORE. KANAWHA TRUCKING DISPUTES THE ALLEGED INCIDENT.
EMPLOYEE STATED THAT HE WAS UNDER A TRUCK ATTEMPTING TH CHECK AN AIR LINE LEAK. HE ASKED THE DRIVER TO START THE TRUCK TO CHECK THE LEAK. DRIVER THEN STARTED THE TRUCK IN GEAR AND IT DRIFTED PINNING EE RIGHT KNEE BETWEEN AIR TANK AND GROUND
Driver stated that the transmission jumped out of 3rd gear, he lost control. Truck turned over onto driver's side.
EE FELT PAIN/SORENESS IN LUMBAR AREA OF BACK AT END OF SHIFT AFTER DRIVING TRUCK FOR TEN HOUR SHIFT
Lost control of truck, ran in ditch & turned over. Laceration to right ear. Broke fender, air filter, door & mirror & damaged corner of truck bed.
PASSENGER IN VHICLE DEPARTING PREP PLANT OF ACCESS ROAD, BEHICLE HIT A LARGE HOLE IN ROAD CAUSING DRIVER TO BOUNCE & HIT ACCELERATOR WHEREUPON THE VEHICLE LURCHED FORWARD HITTING A SECOND HOLE . THE PASSENGER HIT HIS HEAD ON THE ROOF AND ON DASH. HE WAS UNRESTRAINED.
DRIVER OF VEHICLE DEPARTED THE PREP PLANT VIA ACCESS ROAD WHEN HE HIT A LARGE HOLE AND BOUNCED VIOLENTLY CAUSING HIM TO HIT THE ACCELERATOR. THE VEHICLE LURCHED FORWARD STRIKING ANOTHER HOLE T HROWING HIM AGAINST THE STEERING WHEEL AND HITTING HIS HEAD. HE WAS UNRESTRAIEND AT THE TIME.

EE WAS PASSENGER RIDING WITH CO-WORKER WHEN HE WENT TO SLEEP AND DROVE UNDER A ANFOEMULSION BULK TRUCK. EE WAS TAKEN TO CAME GENERAL, CHECKED OUT AND RELEASED 12/13/00. HE HAS A FOLLOW UP 12/1 5/00 AT CAME GENERAL.
EE WAS COMING TO WORK AS HE STARTED TO ENTER CO-WORKER'S AREA HE FELL ASLEEP AND RUN UNDER THE BACK OF A ANFOLEMULSION BULT TRUCK (MACK 61707). EE WAS TAKEN TO CAME GENERAL CHECKED OUT AND REL EASED. EE WAS DRIVING HIS PERSONAL VEHICLE ( A MITSUBUHI ECLIPSE 1995).
EMPLOYEE WAS CHOPPING STOCKTON SECOND PLIT IN THE L7 NORTH PIT. HE STATES THAT THE S2 WAS NOT SLIDING ON TOP OF THE S3 BINDER AS IT NORMALLY DOES WHEN CHOPPING THIS SEAM. THEREFORE, HE WAS HAV ING TO CHOP DOWN HARDER THAN NORMAL TO BREAK THES2 AWAY FROM THE BINDER. IN PROCESS OF CHOPPING, HE ENCOUNTERED AN AREA WHERE THE COAL BROKE SUDDENLY, JOLTING EMPLOYEE, CAUSED BACK PAIN.
EE WAS DRIVING COAL TRUCK INTO PIT TO BE LOADED. HE ALLEGED HIS NECK WAS STRAINED WHEN LOADER BACKED INTO HIS TRUCK.
